Abstract

A descriptive study to identify the preparedness of pharmacist in community pharmacies to cope with globalization impact was conducted in 2009. This cross-sectional study was done in DKI Jakarta, Bali and Maluku. Informants were purposively determined involving pharmacists from schools of pharmacy, Indonesian Pharmacist Association (IAI), community pharmacies, Provincial and District Health Offices, as well as comunity pharmacy owners. Primary data were collected through in-depth interviews and observation using check-list in community pharmacy. Data were analyzed descriptively and qualitatively using triangulation method. Results of the study show that according to Health Offices and the Indonesian Pharmacist Association, pharmacists were not adequately prepared and pharmacists in stand alone community pharmacy are less prepared than those in a network or franchise pharmacy. Licensed pharmacists of network community pharmacy in the metropolis are going to prepare themselves to face the new patient-oriented paradigm and to meet the standard of pharmacy service, whereas stand-alone community pharmacy still prioritized fast service and lower drug price

Abstract

Self-efficacy and low adherence was significant problem on using antihypertension drugs. The study purpose was to evaluate the effectiveness of counseling and provision of leaflets againts self-efficacy and adherence as well as the blood pressure of hypertensive patients at two community health centers in Depok City. The study desain is a quasi-experimental with 37 patients in counseling group and 36 patients in the leaflets group during March to June 2013. The instrument determine self-efficacy is MUSE scale, MMAS-8 scale for adherence and the tensimeter for blood pressure. Data were analyzed uses the Wilcoxon and Kolmogorov-Smirnov test. The results showed that the largest percentage of hypertension patients in community health centers have mild hypertension, has been suffering from hypertension between 1-5 years, received a single drug, mostly captopril, and did not feel any side effects. The counseling and provision of leaflets can increase self-efficacy and medication adherence, and lower systolic blood pressure significantly. Provision of leaflets to patients can increase self-efficacy and adherence, and lowering the systolic and diastolic pressure significantly. There is no significant difference between the effects of counseling and the provision of leaflets to increase self-efficacy and adherence, as well as a decrease in blood pressure in hypertensive patients in community health centers in Depok City

Abstract

Diarrhea is a major health problem in developing countries. WHO data showed diarrhea caused the death of 2.5 million people, 80% are children under five years old. Riskesdas 2013 stated the prevalence of diarrhea in Indonesia is 7%, whereas 12.2% of it are children under five years old. Self-medication is common and access for the medicine is easy. This analysis aims to assess the rational use of diarrhea medication stored in households. The rational drug used is associated with the mother's education level and household ownership quintile. This study design is cross sectional. The data was taken from Riskesdas 2013 namely type of medicines (data from block VI), mother’s education level data (from block IV) and economic status presented as household ownership quintile level (data from block IX). The result showed that the highest percentage of diarrhea drugs stored at household is adsorbents (40.4%), followed by antibiotic (22.4%) and traditional medicine (18.5%). Rationality of diarrhea medications stored in households is 74.7% and irrational (25.3)%). Wealthy people (quintiles 4 and 5) are 2.019 times more rational compared with the poor (quintiles 1, 2 and 3), while mothers with a good education (high school and above) are 1.944 times more rational in using medicine for diarrhea treatment compared with mothers with low education (below high school).

Abstract

Hyperlipidemias are disorders which are manifested by elevation of blood lipid, such as total cholesterol and/or triglyceride above the normal level. Hyperlipidemia management is done by lifestyle modification (high-fiber diet, exercises) as well as using antidyslipidemias. On the other hand, non-conventional or traditional treatment of hyperlipidemias using herbals (jamu) is becoming ubiquitous, performed by jamu practitioner clinician (dokter praktik jamu). This study is a part of a web-site Jamu Registry Study, which is intended to figure herbal (jamu) dispensed by jamu practitioner clinician for hyperlipidemic patients. A descriptive, cross-sectional design is applied, using medical record data of ‘jamu’ patients from some health facilities, in 7 provinces in Indonesia. Results showed, out of 97 hyperlipidemic patients were mostly female (60.8%), aged 48-58 years (46.4%); most of them (62.9%) were treated merely with jamu for 1 – 2 weeks, and the rests were treated with jamu in combination with conventional and/or other alternative medication. The most jamu used were branded herbal capsules (55.7%) containing of Guazumae folium and Murrayae folium. Adverse events were reported in four cases i.e. diarrhea and diuresis

Abstract

Injectable contraceptives raise health problems risk than oral or implan contraceptive. This study aimed to compare the incidence of adverse drug reactions (ADRs) single injectable contraceptive use (Depot medroxyprogesterone acetat/DMPA) with a combination of injectable contraceptive (CICs) use (DMPA and Estradiol cypionate/E2C). The study design was a cross-sectional two population comparison. The sample consisted of 88 acceptors in each group. The ADRs were analyzed using Chi Square and logistic regression multivariate. The largest side effect in single injectable contraceptive group were menstrual disorders (86,4%) and mood dischange, while the side effects in CICs group were mood (64,8%) and bone pain (51,1%). Menstrual disorders in single injectable contraceptives usage was 10.0 folds than CICs. Headache in injectable contraceptives usage was 1.9 folds than in the use of CICs. Menstrual disorders in overweight acceptor 3,8 folds than not overweight.As the ADR observation, the use of CICs is relatively safer than single injectable contraceptive

Abstract

Due to complexity enhancement of health care services, pharmacists are required to provide their orientation to the patient.The objective of the study are to evaluate the pharmaceutical standard prosedures and measuring the effect of the pharmacist’s presence on the pharmacy services quality in the Community Health Centers (CHCs) of Banyumas districts. The study is an observational analytic categorical through cross sectional approach with colleting data technique is divided into two periods, through observation and structured interviews to the person in charge of pharmaceutical care unit. Based on the analysis, from 38 CHCs it’s found out that 24 (63,16%) CHCs have the pharmaceutical standard procedures and 14 (36,84%) CHCs doesn’t have it. However, of the 39 CHCs, there are 33 (84,61) CHCs that have pharmacist, 2 (6,06%) CHCs were categorized in a moderate quality and 31 (93,94%) of them categorized as having less quality. While the six (15,38%) CHCs that doesn’t have a qualified pharmacist categorized as having less quality. There are the presence of pharmacists towards the availability ofthe pharmaceutical standard prosedures, have p value of 0.363 (p>0.05) and the pharmaceutical services checklist with p value of 1.00 (p>0.05)at CHCs of Banyumas districts. Conclusionis pharmaceutical services in Banyumas district aren’t running optimally because presence of pharmacists hasn’t improve the quality

Abstract

Nanocolloid human serum albumin (HSA) labeled with technetium-99m is widely used in nuclear medicine for detection of breast cancer through examination of sentinel lymph node (SLN). SLN examination is used to detect the spread of cancer cells before and after surgery. Accumulation of a radiopharmaceutical in lymph nodes was detected by using a gamma cameraor SPECT. The aim of this study is to establish the protocol of HSA nanocolloid preparation with composition and specification in accordance with the Technical Leaflet Nanocoll Albumon which refers to The Society of Nuclear Medicine and Molecular Imaging (SNMMI) and European Association of Nuclear Medicine (EANM).This study includes the step of preparation and quality test. The method was carried out by means of protein denaturation of HAS at certain pH and heating at certain temperature. Quality tests were carried out using particle size analyzer (PSA) to determine the particle size, Transmission Electron Microscopy (TEM) to observe the morphology of the particles, and paper chromatography to measure radiolabeling yield. This study has obtained the particle size 80% of 95% and pH of 7.3–7.4. The result was a protocol of preparing HSA nanocolloid.

Abstract

Prescribing indicator is an easy core indicator to evaluate drug utilization as stated in WHO guideline. Rational prescribing pattern is important to improve rational use of drugs whereas irrational prescribing pattern can lead to irrational drug use. This study aimed to assess the prescribing pattern in public and private primary healthcare facilities which were included in the National Health Insurance Program in Indonesia. This study has been conducted using the cross sectional research design, quantitative and retrospective method in four provinces in Java island (West Java, Banten, DIY and Central Java provinces) from May to July 2014. The method of retrospective data collection were collected 757 prescriptions from 26 PHCs and 900 prescriptions from 30 clinics or General Practitioners (GPs). All data in the prescribing form were analyzed manually using Microsoft Excel 2007. The average number of item in public and private facilities was 3.31 and 3.06 respectively. The percentage of generic prescribing was 93.3% and 62.0%, the percentage of drugs in accordance with the 2013 national formulary was 89.0% and 64.5%, the percentage of antibiotic per total number of encounter was 42.8% and 39.4%, while the percentage of injection per total encounter was only 0.7% and 2.2%. Paracetamol and dexamethasone were the most frequently prescribed drugs while for antibiotic group were amoxicillin, ciprofloxacin, cotrimoxazole and cefadroxil.The average number of items per encounter and antibiotic were not met the WHO standard(≤3 for average number and less than 30% for antibiotic percentage). Suitable interventions are needed to improve the prescribing practices in Indonesia

Abstract

Star fruit (Averrhoa carambola L.) is potential as raw material for medicine, native in tropic areas, including Indonesia. According to other study report, star fruit leaves containing flavonoids apigenin and quercetin as potential anti-inflammatory and anticancer agents. The raw material for the drug in Indonesia mostly obtained through imports from other countries. In order to support the independence of traditional medicine raw materials, it is important to standardize the quality of traditional medicine raw materials, in this case is star fruit leaves by High Performance Liquid Chromatography (HPLC) method. The sample used is star fruit leaves extract obtained from maceration process using ethanol 70%; water fraction, ethyl acetate and hexane fractions obtained from fractionation process of the ethanolic extract. Physical parameters analyzed in sample include appearance, color, odor, taste, extract yield, water content, loss of drying, total ash content, residual solvent. Chemical parameters analyzed include apigenin and quercetin contents. The results shows that star fruit leaves used in this study meet the standards of Indonesian Herbal Pharmacopoeia with highest apigenin (6.37%) and quercetin (4.49%) content are in ethyl acetate fraction.

Abstract

Malaria infection is still a public health problem, especially in developing countries, including Indonesia. Malaria infection is usually accompanied by increased free radicals in the body of the patient. This situation will cause decreasing the immune system of the patient. Explorating of new drugs that have antimalarial and also antioxidant activity is very important. Therefore, this study aims to determine in vitro antimalarial and antioxidant activity of endophytic fungi extract origin turmeric from Sukabumi. This study begins with fermentation process of endophytic fungi on the broth medium and ethyl acetate was used to extract the filtrat to gain the test extracts. Heme polymerization inhibition and free radical DPPH scavenging method were used to antimalarial and antioxidant in vitro assay respectively. The results showed that 18 isolates of endophytic fungi have antimalarial activity and one isolate inactive and also all isolates have antioxidant activity. Smi.Cl.6F isolate was the most active isolate on the antimalarial and antioxidant assay with IC50 value of each assay were 1.93 mg/mL and 32.28 mg/L respectively. Therefore, Smi.Cl.6F isolates potentially be used as new antimalarial drugs
