Abstract

World Health Organization recorded that about 44% of infant deaths in 2012 happened in the first 28 daysafter birth, and 78.5% of deaths happen in the first week after births. It was estimated that 80% of infantdeaths happen at home (with little or no contact with health workers). Good quality care service for infantsespecially newborns at home is a serious challenge, thus an effort that could be performed is to optimizethe role of health workers in the community by doing homevisit. The goal of this researchto identify theself-efficacy of postpartum mothers before home visit and after home visit, to compare the self-efficacy ofintervention group and control group on newborn care by means of health workers role during home visit. This research use quantitative research with quasi-experimental non-equivalent control group design method. The sample of the research was health workers; each with responsibility of one fostered family of a motherin postpartum period. The research site for the intervention group was the working area of UleeKarengHealth Center, and for the control group was the working area of Baiturrahman Health Center. The result ofthe research revealed that there was a change in self-efficacy of postpartum mothers on newborn care afterthree home visits by the health workers. The difference of the average efficacy score was 12.5, with p valueof 0.0001. When the efficacy of postpartum mothers from the intervention group (who received homevisits) was compared to that of mothers from the control group, there was a significant difference found, with p value of 0.003. Home visit executed by the health workers can improve self-efficacy of thepostpartum mothers on newborn care. This can be seen from the difference between self-efficacy of thepostpartum mothers before and after home visit by the health workers.

Abstract

People with a level II leprosy disability have limited functions in society. In Nagan Raya District, there is a 41% leprosy endemic area with a 5% level II disability rate. The goal of this research was to prove the relationships of traditional medication, stigma, and leper’s family support to level II leprosy disability. This research used mixedmethodsand a case-control design with 84 respondents42 case respondents (level II leprosy disability) and 42 control respondents (level 0 leprosy disability)chosen by consecutive sampling under some inclusion and exclusion criteria. Data collection used the in-depth interview method. This study used bivariate data analysis with the chi-square test and multivariate data analysis with the logistic regression test and the Miles and Huberman’s model. The results of the chi-square analysis indicated that traditional medication had p value=0.014 (OR: 2.3; 95% CI: 0.2–7.7), stigma p=0.04 (OR:3.6;95% CI: 1.5–8.9), and family’s support p=0.192 (OR:2.0; 95% CI: 0.7–5.8). The results of the logistic regression analysis showed that stigma was a risk factor for leprosy level II disability with p value=0.006, (OR=4.9; 95% CI: 1.6–15.1) and probability of 99.96%. Based on the interview results, lepers were feeling uncomfortable with the society’s not acceptingthem in a good way. This was especially the case with khanduri(feast) when it came down to food in that the society assumed lepers would transmit the infectious, disgusting disease. The Nagan Raya District Health Office is expected to encourage the societyto accept lepers and provide information regarding the process of leprosy transmission so as to appease the community’s fearand to prevent further disability.

Abstract

The government health financing system paid for hospital using the INA-CBG’s method. Stroke is the third most common cause of death after heart disease and cancer and stroke rank first as a cause of disability. Thus, the treatment of strokes due to disability and recurrent strokes requires a fairly high maintenance cost. Results: The total cost of ischemic stroke in 345 hospitalized cases with a total real cost was IDR 2.800.210.362 and a total cost of INA-CBG's of IDR 2.830.254.600.This has a significant difference, as well as severity levels and comorbidities. There is a difference between the real hospital rates and INA-CBG’s rates with a value of P = 0,000. The cost of caring for ischemic stroke patients the most cost was accommodation (22%). Conclusion: There is a difference between the real hospital costs and the INA-CBG’s rates for ischemic stroke patients at X Hospital, this is because there is still a negative difference associated with severity level, type of comorbid, length of stay and class of care.

Abstract

Regional General Hospital Dr. Zainoel Abidin (RSUDZA) is one of the public service installations that provide direct health services to the community, especially outpatient and inpatient services. RSUD dr. Zainoel Abidin has implemented a package payment system (INA-CBGs) since January 2014. One of the management elements of implementing this system is planning. Planning (planning) is a process that is very important of all other management functions, which specifically includes the actions to be carried out in order to achieve goals. This study uses a mixed method that uses a combination of descriptive quantitative research and qualitative research with a phenomenological approach through in-depth interviews with informants (in-depth interview). The data used to assess the effectiveness of implementing package payment systems (INA-CBGs) is secondary data taken from hospital medical records. The statistical results assess the frequency distribution by comparing the effectiveness before and after the implementation of the package payment system (INA-CBGs) at RSUD dr. Zainoel Abidin Banda Aceh. The results showed the INA-CBG system was more effective than the Fee for Service (FFS) payment system. The results of this study indicate an increase in efficiency based on service indicators before / after the application of the INA-CBG system, namely (BOR: 75% / 72%, AvLOS: 5 days / 4 days, TOI: 2 days / 1 day, BTO: 46 times / 58 times, GDR: 6 per 1000 patients with discharge / 27 per 1000 patients with discharge, and NDR: 8 per 1000/21 per 1000). It can be concluded that the planning process (planning) of the implementation of the package payment system (INA-CBGs) which has been implemented well by the RSUD Dr. Zainoel Abidin Banda Aceh, including the provision of adequate Human Resources (HR), provision of adequate facilities (infrastructure and facilities), the process of formulating and forming strategies and the process of preparing the budget. The implication of implementing this system has been to be able to increase the efficiency of hospital service performance.
