Abstract

Hallucinations are sensory perception disorders characterized by the emergence of experiences felt without any real stimulation from the environment. Individuals with hallucinations can feel false sensations in the form of hearing, sight, smell, taste, or touch, even though there is no actual stimulus. This study aims to describe the application of psychiatric nursing care to patients experiencing auditory hallucinations at Prof. HB Saanin Mental Hospital Padang. The method used in this scientific paper is a case study with a nursing process approach that includes five stages, namely assessment, diagnosis, planning, implementation, and evaluation. The results of the assessment showed that the patient experienced signs and symptoms of hallucinations in the form of hearing whispering voices. The main nursing diagnosis was hallucinations with a co-diagnosis of self-care deficit (DPD). After nursing care was carried out, the patient was able to overcome self-care problems and was able to control hallucinations independently through the application of Implementation Strategies (SP), namely SP 1-4. These results indicate that the application of implementation strategies is effective in helping patients control hallucinations and improve self-care abilities. Therefore, it is hoped that this research can increase public understanding regarding the importance of mental health nursing care, thereby encouraging awareness in maintaining and preserving mental health and implementing behaviors that support the achievement of optimal psychological wellbeing in everyday life.

Abstract

Mental disorders are conditions characterized by disturbances in cognitive function, emotional regulation, and behavior, which affect an individual’s ability to perform daily activities (American Psychiatric Association, 2022). The World Health Organization (WHO) defines mental disorders as clinically significant disturbances accompanied by psychological distress and impaired functioning in important areas of life (WHO, 2025). Globally, approximately 970 million people experienced mental disorders in 2023, increasing to more than 1.1 billion in 2024. In Indonesia, the prevalence of mental disorders in West Sumatra Province is 1.3% (Indonesia Health Survey, 2023). One of the common problems found in severe mental disorders such as schizophrenia is the risk of violent behavior, which is a maladaptive response due to the inability to control aggressive impulses that may harm oneself, others, and the environment. Data from RSJ Prof. Dr. HB. Saanin Padang indicate that the risk of violent behavior is the second most common nursing problem after hallucinations. This study aims to describe psychiatric nursing care for patients with a risk of violent behavior in Wisma Nuri at RSJ Prof. Dr. HB. Saanin Padang in 2026. The method used is a case study with the implementation of the Nursing Intervention Strategy (SP), including physical exercises, medication adherence, verbal emotional control, and spiritual approaches over five days. The results showed an improvement in the patient’s ability to control emotions and behavior. It can be concluded that comprehensive psychiatric nursing care is effective in reducing the risk of violent behavior.

Abstract

Bronchial asthma is a chronic disease of the respiratory tract characterized by inflammation and narrowing of the airways, causing shortness of breath, chest tightness, coughing, and wheezing. Data from the pediatric inpatient ward at RSUD Prof. H. M. Yamin, S.H. Pariaman showed an increase in cases from 37 in 2023 to 117 in 2024, and in 2025 it ranked 7th among the top 10 most common diseases with 61 cases. This study aims to provide direct nursing care to patients with bronchial asthma through the stages of assessment, diagnosis, intervention, implementation, and evaluation. The study was conducted from February 11 to 15, 2026, in the pediatric inpatient ward using a case study approach on one patient, A.n S, aged 5 years. The assessment results identified four nursing diagnoses: ineffective airway clearance, ineffective breathing pattern, sleep pattern disturbance, and risk of nutritional deficit. Interventions included monitoring breathing patterns and respiratory rate, monitoring food intake, and identifying factors that disturb sleep. The evaluation showed that sleep pattern disturbance and ineffective breathing pattern were resolved on the fourth day, while ineffective airway clearance and risk of nutritional deficit were resolved on the fifth day.This paper is expected to serve as a reference for healthcare professionals to improve their skills in providing nursing care for children with bronchial asthma. Parents are expected to pay closer attention to their child’s health through proper home management, such as maintaining good ventilation, ensuring medication adherence, avoiding triggers like cigarette smoke, and recognizing early signs of recurrence.

Abstract

Congestive Heart Failure (CHF) is a condition in which the heart is unable to pump blood optimally to meet the body’s metabolic needs, resulting in impaired tissue perfusion and various complications. The World Health Organization states that cardiovascular disease is the leading cause of death worldwide, accounting for approximately 17.9 million deaths each year or about 32% of total global deaths. Based on data from the Kementerian Kesehatan Republik Indonesia in 2023, heart disease remains one of the major health problems in Indonesia with an increasing incidence rate. The purpose of this scientific paper is to provide nursing care for patients with Congestive Heart Failure (CHF), including assessment, nursing diagnoses, interventions, implementation, evaluation, and nursing documentation.The method used in this scientific paper was a case study approach using the nursing process on one patient, Mrs. N, in the cardiac inpatient ward at RSUP Dr. M. Djamil Padang from February 4–8, 2026. Data collection techniques included interviews, observation, physical examination, and documentation studies.The results of nursing care showed an improvement in the patient’s condition after five days of nursing interventions, indicated by improved blood pressure, reduced shortness of breath, decreased pain scale, and increased activity tolerance. It is expected that the patient will be able to perform optimal self-care through adherence to therapy, activity regulation, and the implementation of a healthy lifestyle in order to improve quality of life and prevent recurrence and further complications.

Abstract

Pneumonia is a lower respiratory tract infection characterized by cough, shortness of breath, and sputum accumulation. Globally (2024), lower respiratory tract infections caused 2.18 million deaths, the majority of which were caused by the bacterium Streptococcus pneumoniae infection. In Indonesia (2023), there were 56,600 deaths caused by lower respiratory infections (LRI), of which pneumonia among individuals aged ≥60 years accounted for 23,600 deaths. The number of pneumonia cases in West Sumatra in 2022 reached 5,591 cases, while in Pariaman in 2024 there were 947 cases. Meanwhile, at RSUD Prof. H. Muhammad Yamin SH Pariaman in 2025, pneumonia ranked ninth with 89 cases. The purpose of this case study was to provide nursing care for a patient diagnosed with pneumonia. The method used was a six-day case study conducted through interviews, observation, and physical examination from February 10–15, 2026. The results identified six nursing diagnoses: ineffective airway clearance, ineffective breathing pattern, activity intolerance, sleep pattern disturbance, knowledge deficit, and risk of infection. The interventions implemented included airway management, respiratory monitoring, energy management, sleep support, health education, and infection prevention. Evaluation on the fourth day showed that the knowledge deficit had been resolved; on the fifth day, the ineffective breathing pattern, activity intolerance, and sleep pattern disturbance were resolved; and on the sixth day, ineffective airway clearance and risk of infection were fully resolved. Patients are advised to adopt a healthy lifestyle by avoiding exposure to dust, cigarette smoke, and air pollution, as well as consuming a balanced nutritious diet consisting of high-protein foods, vegetables, and fruits, and adhering to prescribed treatment in order to support the healing process and prevent recurrence. Families are expected to play an active role in providing support, including reminding patients to take their medication, refrain from smoking, and helping to maintain a clean and healthy environment.

Abstract

Based on the International Diabetes Federation Data in 2021, more than 500 million people worldwide, precisely 537 million, suffer from diabetes. The percentage of the incidence of Diabetes Mellitus in Indonesia according to the Indonesian Health Survey is 1.7% with a total of 877,531 sufferers and the incidence of Diabetes Mellitus in West Sumatra shows 1.2% with a total of 18,138 sufferers. Diabetes Mellitus is a chronic disease caused by the body's inability to produce the hormone insulin or due to ineffective use of insulin production. This is characterized by high blood sugar levels. This case study aims to implement nursing care for Mr. E with Diabetes Mellitus at the Sabai Nan Aluih Sicincin Social Home for the Elderly, Padang Pariaman Regency. The method used is a nursing process approach consisting of assessment, nursing diagnosis, intervention, implementation, evaluation, and documentation. The research design used is descriptive with a case study approach. Data were obtained through interviews and observations. The results of the study are Mr. Mr. E, 73, experienced itching on his palms, with peeling skin, and reported taking Metformin 850 mg twice daily. Three diagnoses were diagnosed in Mr. E's case: unstable blood glucose levels, impaired skin/tissue integrity, and risk of falls. After a 5-day evaluation, the stability of blood glucose levels increased, skin integrity improved, and the fall rate decreased. It can be concluded that comprehensive geriatric nursing care can increase the independence of the elderly in caring for their physical condition independently.

Abstract

These disturbances can affect cognition, emotional control, and behavior, thereby influencing an individual’s functional abilities (American Psychiatric Association, 2022). The World Health Organization reported that approximately 970 million people worldwide experienced mental disorders in 2023, increasing to more than 1.1 billion people in 2024. In Indonesia, mental health remains a significant concern, with a prevalence of 1.3% in West Sumatra Province (Indonesia Health Survey, 2023). The Prof. HB. Saanin Mental Hospital in Padang is recorded as the hospital with the highest number of psychiatric patient visits in the region, reaching 38,332 visits in 2024 and remaining high in 2025. This study aims to describe psychiatric nursing care for patients with auditory hallucinations in Wisma Melati at Prof. HB. Saanin Mental Hospital, Padang, in 2026. Nursing care was provided through theimplementation of the Implementation Strategy (SP), which includes practicing rebuking the hallucinations, medication adherence, engaging in conversations, and performing scheduled activities. The five-day implementation showed an improvement in the patient’s ability to control auditory hallucinations. The results of this study emphasize the importance of the nurse’s role in providing comprehensive psychiatric nursing care to help patients control violent behavior and prevent risks to themselves and their environment.

Abstract

Data from Prof. Hb. Saanin Psychiatric Hospital in Padang indicate that hallucinations have been the most common nursing diagnosis over the past three years (2023–2025). with a total of 17,591 patients. The purpose of this study is to describe the implementation of psychiatric nursing care for Mrs. L with auditory hallucinations in Melati Ward. The method used is a case study with a nursing process approach through five stages: assessment, diagnosis, planning, implementation, and evaluation, conducted over six meetings. The assessment results showed that Mrs. L experienced signs and symptoms of hallucinations, such as hearing whispers and an inability to perform self-care. Case analysis indicates that implementation strategy interventions (SP), such as rebuking hallucinations, medication adherence, engaging in conversations, and scheduled activities, are effective in helping patients control hallucinations. The discussion demonstrates an improvement in the patient’s ability to manage hallucination symptoms and increased independence in self-care. It is expected that the results of this research will increase public understanding of psychiatric nursing care. This understanding will serve as a basis for building awareness, maintaining mental health, and adopting behaviors that support optimal mental well-being.

Abstract

The increasing number of elderly people causes an increase in the incidence of chronic diseases, one of which is rheumatoid arthritis which can reduce the quality of life. Data in the world shows that sufferers of joint disorders reach 81% of the population. In West Sumatra, it is recorded at around 8.50% of the population. Data obtained at PSTW Sabai Nan Aluih Sicincin, Padang Pariaman Regency, namely rheumatoid arthritis ranks first with a number of cases of 22 sufferers. The purpose of this case study is to be able to provide nursing care to Mr. M with Rheumatoid arthritis at PSTW Sabai Nan Aluih Sicincin, Padang Pariaman Regency. The research method used is a case study method with a nursing process approach that includes assessment, nursing diagnosis, intervention, implementation, and evaluation. The study was conducted at PSTW Sabai Nan Aluih Sicincin, Padang Pariaman Regency, the implementation time was from 11 to February 16, 2026. The results of the study obtained nursing problems, namely chronic pain, impaired physical mobility, and disturbed sleep patterns. Nursing interventions carried out included pain management, mobility support and sleep support. The implementation included providing non-pharmacological techniques to reduce pain, encouraging range of motion, and modifying the environment to increase comfort. Evaluations of chronic pain, impaired physical mobility, and disturbed sleep patterns were continued and accompanied by caregivers. The conclusion of this case study is that appropriate and continuous nursing care can help reduce pain, increase mobility, and improve the patient's sleep quality. Recommendations are aimed at patients to be able to independently perform non-pharmacological pain management techniques, perform simple mobilization independently, and take medication regularly as prescribed by the doctor. Caregivers are also advised to provide support, regular monitoring, and assistance in implementing optimal patient care.

Abstract

According to data from the World Health Organization (WHO), the number of Chronic Kidney Disease (CKD) cases in 2021 was recorded at more than 843.6 million. By 2025, this number is expected to reach around 850 million, with approximately 4 million individuals requiring renal replacement therapy. In Indonesia, the prevalence of CKD reaches 638,178 cases. Meanwhile, in West Sumatra, the prevalence is around 0.40% with a total of 13,834 cases. In Prof. H. Muhammad Yamin, SH Pariaman Regional General Hospital, CKD ranked first in 2025 with 335 cases. The purpose of this case study is to understand and apply nursing care to patients with Chronic Kidney Disease (CKD). The method used is a case study with a nursing process approach for one patient, Mr. E in the internal room of Prof. H. Muhammad Yamin, SH Pariaman Regional General Hospital on February 9-13, 2026. The results of the 5-day study showed nursing problems, namely: Hypervolemia, Ineffective Breathing Pattern, Ineffective Peripheral Perfusion, Acute Pain and Risk of Ineffective Renal Perfusion. Interventions and implementations carried out were hypervolemia management, airway management, circulation care, pain management, shock prevention anddisease process education. The author's suggestion is that patients can adopt a healthy lifestyle, such as limiting fluids, limiting foods high in sodium and potassium and routine hemodialysis to the hospital. In conclusion, after nursing care with CKD, the patient's condition showed improvement marked by reduced shortness of breath, edema, fatigue and pain.
