Abstract

Bronchopneumonia is a lung infection that commonly occurs in children under 5 years old which causes inflammation of the bronchioles (small airways and alveoli air sacs in the lungs). The purpose of this study is to describe the application of nursing care to pediatric patients with bronchopneumonia. This method uses a case study conducted for 5 days on (8–12 February 2026) in the pediatric ward of Prof. Muhammad Yamin SH. Pariaman Regional Hospital. After the assessment was completed, 4 nursing diagnoses were found, namely ineffective airway clearance, ineffective breathing pattern, hyperthermia, and nutritional deficit. The implementation of nursing provided aims to monitor general conditions and vital signs, teach semi-Fowler's position to facilitate breathing, monitor respiratory status, provide warm compresses, and educate about fulfilling nutritional needs to the family. After 5 days of treatment, the patient's condition showed improvement, marked by improved breathing patterns, decreased body temperature and increased family understanding of nutritional needs being met. The results of this case study emphasize the importance of the role of parents in supporting the child's healing process, especially in recognizing early signs of shortness of breath and Parents should be able to respond quickly and appropriately if their child is short of breath. With proper knowledge and preparedness, parents can play an active role in ensuring their child's safety and health.

Abstract

Hallucinations are a form of perceptual disorder, a condition where a person experiences sensory stimulation without any real stimulus from the surrounding environment. Based on medical record data from RSJ Based on medical record data from RSJ Prof. HB. Saanin Padang, hallucinations have become the most common nursing diagnosis in the last 3 year (2023-2025) with a total of 17,591 clients Prof. HB. Saanin Padang. When hallucinations occur, individuals often feel very anxious, panicked, and have difficulty distinguishing between what they imagine and the reality they are experiencing. This study aims to describe the application of psychiatric nursing care to patients with auditory and visual hallucinations at Wisma Flamboyan. The method used was a case study with a nursing process approach that includes assessment, diagnosis, planning, implementation, and evaluation. The results of the assessment showed that the client experienced signs and symptoms of hallucinations in the form of hearing whispers and seeing shadows that were not there. The main nursing diagnosis was hallucinations with a co-diagnosis of self-care deficit. After being given nursing care, the client was able to control his hallucinations by implementing hallucination implementation strategies, namely SP 1-4. These results indicate that SP implementation is effective in helping clients control hallucinations. Self Care Deficit diagnoses can also be addressed with implementation strategies (SP), namely 1-4. This research is expected to increase public understanding and awareness of the importance of maintaining mental health to achieve optimal mental well-being.

Abstract

Hallucinations are sensory perception disturbances that occur without any real stimulus. One common type of hallucination is auditory hallucinations, which involve the perception of hearing voices or sounds that do not originate from external stimuli. Based on medical records at Prof. HB. Saanin Mental Hospital, Padang, the most common nursing problem in the past three years was hallucinations, with 12,419 patients. The purpose of this case study is to apply psychiatric nursing care to a client with auditory hallucinations at Prof. HB Saanin Mental Hospital, Padang. The method used in this scientific paper is a case study through a nursing process approach, which includes assessment, diagnosis, intervention, implementation, and evaluation. The assessment results indicate that the client experienced signs and symptoms of hallucinations, including hearing whispers. The primary diagnosis was hallucinations, with a co-diagnosis of self-care deficit. After providing this nursing care, the patient was able to independently control the hallucinations using implementation strategies (SP): SP 1: reprimanding, SP 2: taking medication regularly, SP 3: having conversations, and SP 4: carrying out scheduled daily activities. These results indicate that the implementation of implementation strategies is effective in helping the patient control hallucinations. It is hoped that through this research, everyone will understand the importance of psychiatric nursing care, thereby increasing awareness in maintaining and preserving mental health, and implementing behaviors that support optimal mental well-being in everyday life

Abstract

Stroke is the leading cause of disability in the world and ranks second as a cause of death after heart disease. Data shows that every year there are around 13.7 million new cases of stroke with a death rate reaching approximately 5.5 million people. Data obtained at the PSTW Sabai Nan Aluih Sicincin, Padang Pariaman Regency, shows that stroke ranks fourth with a total of 12 cases. The purpose of this researcher is to provide nursing care to patients with stroke at the Sabai Nan Aluih Sicincin Social Home for the Elderly, Padang Pariaman Regency. The research method used is a case study on one client with a nursing process approach that includes assessment, nursing diagnosis, intervention, implementation, and evaluation. The assessment was conducted on February 11–16, 2026. The results of the assessment showed that the client experienced impaired physical mobility, self-care deficits, and a risk of falling due to muscle weakness and balance disorders. Interventions provided included Range of Motion (ROM) exercises, assistance with daily activities, health education, and fall risk prevention. The evaluation results obtained after 5 days of implementation showed an increase in nursing problems, with muscle strength in the right lower extremity being 3%, an increase in self-care skills, and a decrease in the risk of falls. The conclusion of this study is that comprehensive geriatric nursing care can help improve the functional abilities of elderly people with stroke. It is recommended that clients and caregivers regularly exercise, maintain a healthy lifestyle, and undergo regular health checks to prevent complications and stroke recurrence.

Abstract

Pulmonary tuberculosis (TB) is a contagious infectious disease that remains a global and national health problem. It is caused by Mycobacterium tuberculosis and is transmitted through droplets. Based on data from the World Health Organization (WHO) in 2024,the number of TB cases reached approximately 10.7 million worldwide.In Indonesia, there were around 889,000 cases,while in West Sumatra Province there were approximately 25,000 tuberculosis cases. At RSUD Prof. H. Muhammad Yamin, S.H. Pariaman,the number of cases in the last three years was 56 cases in 2023, 69 cases in 2024, and 45 cases in 2025.The purpose of this case study is to provide nursing care for a pulmonary TB patient in the pulmonary inpatient ward at RSUD Prof. H.Muhammad Yamin, S.H. The method used is a case study with a duration of 5 days,conducted from February 5 to February 9, 2026, in the pulmonary ward of RSUD Prof. H.Muhammad Yamin,S.H.Pariaman.The approach used is the nursing process,which includes assessment,diagnosis, intervention,implementation, and evaluation stages,with one patient diagnosed with pulmonary TB as the subject.The results identified six nursing diagnoses: ineffective airway clearance,hyperthermia,disturbed sleep pattern,risk of nutritional deficit,risk of infection,and knowledge deficit.The nursing interventions provided included airway management,hyperthermia management,sleep support,nutritional management,infection prevention,and health education.The evaluation over five days showed that all nursing problems were resolved,and the patient was allowed to be discharged.Patients and their families are expected to adhere to the full course of treatment,adopt a healthy lifestyle, use masks, and increase the intake of nutritious food to support recovery and prevent transmission.

Abstract

Acute gastroenteritis (GEA) remains a common health problem with a high morbidity rate, both globally and nationally. Based on World Health Organization (WHO) data, gastroenteritis causes millions of cases each year and can lead to serious complications such as dehydration and electrolyte imbalances. In Indonesia, the prevalence of acute gastroenteritis is approximately 6.8%, while in West Sumatra it reaches 8.5%. Data from Aisyiyah Pariaman Hospital indicate that gastroenteritis is among the top ten diseases, with an increasing trend in cases from 43 cases in 2023 to 90 cases in 2025. This condition shows that gastroenteritis remains a significant health problem requiring optimal management, particularly related to hygiene, dietary patterns, and environmental factors.The purpose of this case study is to understand and apply comprehensive nursing care for patients with gastroenteritis. The method used is a case study approach based on the nursing process, including assessment, diagnosis, intervention, implementation, and evaluation of patients in the Muzdalifah Ward at Aisyiyah Pariaman Hospital.The results show that patients with gastroenteritis generally experience diarrhea, nausea, vomiting, abdominal pain, and signs of dehydration such as decreased skin turgor and dry mucous membranes. The main nursing diagnoses include diarrhea, hypovolemia, hyperthermia, and risk of electrolyte imbalance. Interventions implemented, such as diarrhea management, fluid monitoring, rehydration therapy, and health education, have been shown to improve the patient’s condition.In conclusion, appropriate and comprehensive nursing care is essential in managing patients with gastroenteritis to prevent complications. It is recommended that healthcare providers enhance education on hygiene and healthy dietary habits, as well as strengthen patient monitoring to reduce the incidence of gastroenteritis.

Abstract

Children are individuals undergoing a process of growth and development that encompasses physical, cognitive, psychological, and social aspects in stages from infancy through adolescence, in order to achieve maturity and realize their optimal potential as adults. Dengue Hemorrhagic Fever (DHF) is an infectious disease caused by the dengue virus transmitted through the bite of the Aedes mosquito. The objective of this study is to provide pediatric nursing care for patients with DHF. This study employed a case study method using the nursing process approach from March 8, 2026, to March 12, 2026, in the Marwa Ward of Aisyiyah Pariaman Hospital. Following an assessment, the identified nursing problems were hyperthermia, acute pain, hypovolemia, and risk of nutritional deficit. Nursing interventions included management of hyperthermia, pain management, hypovolemia management, and nutritional needs management. The results of the case study, obtained after five days of nursing care, showed an improvement in the patient’s condition. This was indicated by a return to normal body temperature, the absence of pain complaints, adequate fluid status, and an increased appetite. Furthermore, the planned and continuous nursing interventions proved effective in addressing the nursing problems that arose.

Abstract

According to data from the World Health Organization (WHO), the global prevalence of diabetes reached 11.11%, or approximately 589 million adults, in 2024 and is projected to increase to 853 million (12.96%) by 2050. In Indonesia, the prevalence of diabetes has risen from 5.7% in 2007 to 11.7% in 2023, with the number of cases reaching 20.4 million and expected to continue increasing. In West Sumatra, the prevalence based on physician diagnosis is 1.6%, with cases increasing from 43,464 in 2021 to more than 52,000 in 2023. The city of Pariaman has the highest prevalence in West Sumatra at 2.23%, with the number of cases continuing to rise annually. The purpose of this case study is to enable the researcher to provide nursing care for a patient with Type II Diabetes Mellitus. The research method used is a case study design, in which one patient was selected as the sample and managed through a nursing care approach. The assessment was conducted on February 9, 2026. Based on the assessment, the nursing diagnoses identified were: unstable blood glucose levels, impaired skin integrity, risk of infection, and risk of nutritional deficit. The interventions implemented included hyperglycemia management, wound care, medication administration, and nutritional management, all carried out in accordance with the established care plan. After five days of implementation, the outcomes showed a reduction in tingling in the right foot, increased comfort after dressing changes, a decrease in fever, and improved appetite. It can be concluded that appropriate nursing care can improve the patient’s health status. It is expected that the patient and family will be able to provide proper home care, anticipate potential complications, and offer support during the recovery process, maintain a healthy lifestyle—particularly in terms of diet—and utilize nearby healthcare facilities. These measures are important to ensure that the patient’s blood glucose levels remain controlled and to help prevent disease within the family.

Abstract

The risk of violent behavior is one of the common nursing problems found in patients with mental disorders and has the potential to harm themselves, others, and the environment. This study aims to describe psychiatric nursing care for patients at risk of violent behavior. The method used is a case study of patients, who was treated at Prof. Dr. HB Saanin Mental Hospital in Padang for 5 days, from February 2 to February 7, 2026. The assessment results showed three nursing diagnoses: risk of violent behavior, hallucinations, and self-care deficit. Nursing interventions were carried out using implementation strategies (SP) 1 to 4, which included physical exercises, medication adherence, verbal approaches, and spiritual approaches. The evaluation results indicated a decrease in the signs and symptoms of violent behavior risk; the patient became calmer, was able to control emotions, and showed improved ability to perform self-care independently. It can be concluded that the implementation strategies are effective in helping patients control violent behavior and improve their independence. Additionally, family and environmental support play an important role in maintaining the outcomes achieved by the patient.

Abstract

The postpartum period is a critical phase for mothers after normal delivery, characterized by uterine involution and psychological adaptation. According to the World Health Organization, the global maternal mortality rate in 2023 was estimated at 260,000 deaths. At Aisyiyah Pariaman Hospital, particularly in the PONEK ward, there were 178 cases of normal delivery, with postpartum cases ranking third among the ten most common conditions.The purpose of this study was to provide comprehensive nursing care for a primiparous postpartum mother with normal delivery to prevent complications and improve independence in self-care. This study used a case study design. The subject was a postpartum mother (Mrs. D) who was treated from February 14 to 18, 2026, in the PONEK ward. Data collection techniques included interviews, physical observation, and documentation review.The nursing problems identified included acute pain related to physiological injury agents as evidenced by complaints of perineal wound pain; ineffective breastfeeding related to inadequate milk production as evidenced by the absence of breast milk; self-care deficit related to physical weakness after delivery as evidenced by the patient not bathing and appearing unkempt; and risk of infection as evidenced by a first-degree perineal rupture. Evaluation results showed that all nursing problems were resolved.
