Abstract

One of the non-communicable diseases (NCDs) is Chronic Kidney Disease. CKD is a global health problem with an increasing prevalence and contributes significantly to high morbidity and mortality rates. According to the World Health Organization, CKD is one of the leading causes of death worldwide. In Indonesia, the prevalence of CKD based on the 2023 Indonesian Health Survey (SKI) was 0.18% or approximately 638,178 people, while in West Sumatra it reached 0.23% or 13,042 cases. At the local level, RSUD Prof. H. Muhammad Yamin, S.H Pariaman recorded an increase in CKD cases in the internal medicine ward, with 118 cases in 2023, 227 cases in 2024, and 327 cases in 2025. This study aimed to describe medical-surgical nursing care for patients with CKD. The method used was a case study with a descriptive approach involving one patient during five days of hospitalization. Data collection techniques included interviews, observations, physical examinations, and documentation studies. The results identified seven nursing diagnoses: impaired gas exchange, ineffective peripheral perfusion, hypervolemia, acute pain, sleep pattern disturbance, impaired skin integrity, and risk of decreased cardiac output. Nursing interventions provided included positioning, blood transfusion, circulation care, hypervolemia management, pain management, sleep support, skin integrity care, and cardiac care. The evaluation showed gradual improvement in the patient’s condition. It can be concluded that comprehensive, individualized, and continuous nursing care is effective in improving the condition of patients with CKD.

Abstract

Hypertension is often dubbed the "silent killer" as it is frequently asymptomatic yet carries a high risk of serious complications such as heart disease, stroke, and kidney failure. According to the WHO (2025), hypertension is the leading cause of premature death worldwide, with an estimated 1.4 billion adults suffering from the condition. Based on the 2023 Indonesian Health Survey (SKI), the prevalence of hypertension in Indonesia reached 30.8% among residents aged >18 years, while in West Sumatra, it was recorded at 25.16%. In Pariaman City (2024), there were 8,164 hypertension patients who received health services. Medical record data at Prof. H. M. Yamin, S.H. Hospital, Pariaman, showed that hypertension patients experiencing complications reached 99.01% (602 patients) of the total cases in 2025.The purpose of this case study is to provide comprehensive nursing care for Mrs. E with hypertension in the Internal Medicine Ward of Prof. H. M. Yamin, S.H. Hospital, Pariaman. The method used is a descriptive case study conducted over five days, from February 4th to 9th, 2026, involving one research subject.The results of the initial assessment revealed extreme blood pressure of 210/117 mmHg and a pain scale of 7. Based on data analysis, four nursing problems were established: acute pain, risk of ineffective cerebral perfusion, disturbed sleep patterns, and risk of nutritional deficits, with acute pain as the top priority. The evaluation over five days showed that all nursing problems were resolved, marked by a decrease in blood pressure to 165/97 mmHg, the disappearance of pain (scale 0), and improved sleep patterns and nutritional intake, allowing the patient to be discharged. It is expected that the patient and their family will ensure that they adhere to a healthy lifestyle, a low-salt diet, and regular health checks to prevent complications.

Abstract

Mental disorders are conditions in which individuals experience disturbances in thought patterns, behavior, and emotional responses that affect their ability to carry out daily activities. One of the most common mental disorders is schizophrenia with symptoms of auditory hallucinations. This study aimed to implement psychiatric nursing care for patients with auditory hallucinations at Cendrawasih Ward, Prof. Dr. HB Sa’anin Mental Hospital Padang in 2026. The method used was a case study approach through the nursing process, including assessment, nursing diagnosis, intervention, implementation, and evaluation. The case study was conducted over six days from February 4 to February 10, 2026, involving one patient with the primary problem of auditory hallucinations. The assessment results showed that the patient experienced auditory hallucinations characterized by talking and laughing alone, hearing voices without real stimuli, social withdrawal, and difficulty controlling the hallucinations. The main nursing diagnosis was auditory hallucinations with a secondary diagnosis of self-care deficit. It can be concluded that the implementation strategy (SP) was effective in helping the patient control auditory hallucinations. It is expected that the results of this study can serve as a guideline for improving the quality of psychiatric nursing care services, especially for patients with auditory hallucinations.

Abstract

Smoking habits are the main factor causing Chronic Obstructive Pulmonary Disease (COPD), both in active smokers and individuals exposed to cigarette smoke. Continuous exposure to cigarette smoke can trigger chronic inflammation in lung tissue, leading to respiratory function disorders. According to the World Health Organization (WHO) in 2024, COPD ranked as the fourth leading cause of death worldwide, with approximately 3.52 million sufferers. At Prof. H. Muhammad Yamin, SH Regional General Hospital, COPD is included among the ten most common diseases and ranks tenth overall. The purpose of this paper is to describe the implementation of nursing care for patients with COPD. The sample was Mr. Y, who was treated from February 6–10, 2026. Data were collected through interviews with the patient and family, observations, physical examinations, and diagnostic reports. The nursing problems identified included ineffective airway clearance, ineffective breathing patterns, activity intolerance, sleep pattern disturbances, and knowledge deficits. Evaluation results showed that ineffective airway clearance was not resolved, while ineffective breathing patterns, activity intolerance, sleep pattern disturbances, and knowledge deficits were resolved. Therefore, patients and families are advised to continue post-COPD care through chest physiotherapy (clapping and vibration), effective coughing techniques, and drinking warm water to help thin secretions.

Abstract

Type II Diabetes Mellitus is a chronic health disorder characterized by an increased blood glucose level due to the pancreas being unable to produce insulin hormone according to the body's needs. At RSUD M. Yamin Pariaman, Type II Diabetes Mellitus ranks third among the ten most common diseases. This study aims to provide nursing care to patients with Type II Diabetes Mellitus in the internal medicine ward at RSUD M. Yamin Pariaman. The case study subject is Mrs. R, who was admitted on February 1, 2026. The methods used include direct observation, physical examination, interviews, and documentation. The nursing problems identified include blood glucose level instability, nutritional deficit, and sleep pattern disturbances. Evaluation results showed that blood glucose level instability, nutritional deficit, and sleep pattern disturbances were fully resolved after 5 days of implementation, and the patient was discharged on February 6, 2026.

Abstract

Bronchial asthma is a chronic respiratory disease characterized by airway inflammation, bronchospasm, and increased mucus production, which can lead to breathing difficulties. The purpose of this study was to improve the quality of nursing care in managing pediatric patients with bronchial asthma in a comprehensive manner. This study employed a case study method conducted from March 3 to March 7, 2026, in the pediatric ward of RSUD Prof. H. Muhammad Yamin, SH Pariaman. The nursing care approach included assessment, diagnosis, intervention, implementation, and evaluation, with data collected through interviews, observation, physical examination, and documentation.The subject of this study was a pediatric patient (An. A) diagnosed with bronchial asthma who was hospitalized for five days. The results identified three nursing problems: ineffective airway clearance, ineffective breathing pattern, and activity intolerance. After implementing interventions such as airway management, respiratory monitoring, and energy management, the patient’s condition improved, as indicated by reduced shortness of breath, a more regular breathing pattern, and increased activity tolerance. These findings suggest that the nursing care provided was effective in improving the patient’s condition.It is recommended that patients and their families maintain proper home care and ensure a clean and healthy environment by regularly cleaning dust, avoiding exposure to cigarette smoke and air pollution, refraining from keeping furry pets indoors, and maintaining proper home ventilation to support good air circulation and prevent respiratory illnesses among family members.

Abstract

Pulmonary tuberculosis remains a health problem and is the second leading cause of death after Covid-19, with 10.6 million deaths reported globally in 2024. Indonesia ranks second with a pulmonary tuberculosis burden of 1,060,000 cases. In West Sumatra, 7,942 cases were recorded, while Pariaman City reported 62 cases. Locally, Rumah Sakit Aisyiyah Pariaman experienced an increase in the number of cases from 61 in 2023 to 104 in 2024.The purpose of this case study was to provide nursing care for patients with pulmonary tuberculosis in the Muzdalifah inpatient ward of Rumah Sakit Aisyiyah Pariaman. The method used was a case study conducted over five days, from February 10 to February 14, 2026, in the Muzdalifah ward of Rumah Sakit Aisyiyah Pariaman. The approach used was the nursing process, which included assessment, nursing diagnosis, intervention, implementation, and evaluation, with one patient diagnosed with pulmonary tuberculosis as the research subject.The results showed eight nursing diagnoses: ineffective airway clearance, impaired gas exchange, nutritional deficit, risk of infection, hyperthermia, acute pain, disturbed sleep pattern, and activity intolerance. Nursing implementations provided included airway management, oxygen therapy, nutritional management, infection prevention, hyperthermia management, pain management, sleep support, energy management, and health education.The evaluation conducted over five days showed that the nursing problems were resolved, and the patient was allowed to return home. Patients are advised to comply with treatment recommendations, maintain personal and environmental hygiene, avoid smoking, consume nutritious food, and routinely attend health facility check-ups to optimize the healing process and prevent transmission to others.

Abstract

Acute gastroenteritis is an infectious disease that remains a global and national health problem caused by pathogenic microorganisms such as bacteria, viruses, or parasites that enter the digestive tract. Based on data from the World Health Organization (WHO) in 2024, the number of acute gastroenteritis cases reaches 1.7 billion each year. According to the Indonesian Ministry of Health, gastroenteritis is the second most common disease in Indonesia with around 2.2 million cases, while in West Sumatra there were 43,706 recorded cases. In Pariaman City, 1,325 cases were treated out of a target of 2,544. Medical record data at RSUD Prof. H. Muhammad Yamin, S.H. Pariaman showed that the number of patients with gastroenteritis in 2025 reached 219 cases. The purpose of this case study is to describe the application of nursing care for patients with acute gastroenteritis. The method used was a case study through observation and interviews conducted for 5 days, from February 11 to 15, 2026, in the Internal Medicine Ward of Prof. H. Muhammad Yamin Regional Public Hospital, S.H Pariaman, involving one patient with acute gastroenteritis. The results of the study identified six nursing problems: diarrhea, hypovolemia, acute pain, risk for ineffective peripheral perfusion, risk of infection, and risk of nutritional deficit, with diarrhea as the primary priority. Evaluation over five days of care showed that all nursing problems gradually improved and were resolved, allowing the patient to be discharged in a stable condition. Patients is expected to maintain a proper diet, food and environmental hygiene, as well as a healthy lifestyle.

Abstract

COPD is a condition characterized by narrowing of the airways. Based on World Health Organization data (2024), Chronic Obstructive Pulmonary Disease (COPD) is the third leading cause of death globally, with mortality rates reaching 3.5 million. The Indonesian Ministry of Health (2023) states that COPD is a significant health issue in Indonesia with a prevalence of 16 million sufferers, while West Sumatra recorded 4,284 cases (West Sumatra Health Office, 2024). Medical record data at Prof. Dr. H. Muhammad Yamin, S.H. Pariaman Regional Hospital shows that the number of COPD patients in 2026 was recorded at 122 patients. The objective of this case study is to describe the application of nursing care for patients with COPD in the Pulmonary Ward of Prof. Dr. H. Muhammad Yamin Hospital, Pariaman. The method used is a case study conducted from February 6th to 10th, 2026, at the Pulmonary Ward of Prof. Dr. H. Muhammad Yamin, S.H. Pariaman Hospital on one subject, Mrs. N, with COPD. The results identified five nursing diagnoses: ineffective airway clearance, impaired gas exchange, ineffective breathing patterns, disturbed sleep patterns, and activity intolerance. Interventions included effective cough training, respiratory monitoring, airway management, sleep support, and activity management. Evaluation over five days showed that all nursing problems—ineffective airway clearance, impaired gas exchange, ineffective breathing patterns, disturbed sleep patterns, and activity intolerance—were resolved. It is expected that the patient will increase awareness in avoiding triggers of breathlessness and maintaining a healthy lifestyle to prevent recurrent exacerbations.

Abstract

Chronic kidney disease (CKD) is a global health problem with prevalence increasing every year. Data from the World Health Organization shows that the number of CKD sufferers in the world reaches hundreds of millions of people and is one of the diseases that contributes significantly to mortality. In 2023, CKD was ranked 9th as the biggest cause of death in the world with around 1.5 million deaths and is estimated to continue to increase until 2040. In addition, the number of patients undergoing hemodialysis therapy also continues to increase, indicating a high burden of disease and its impact on the health system and the quality of life of patients. The purpose of this study was to analyze and apply nursing care, using a case study method on one CKD patient in the Internal Inpatient Room of Dr. M. Djamil Padang General Hospital in 2026. The results of the 5-day study found nursing problems including Ineffective Airway Clearance, Ineffective Breathing Pattern, Hypervolemia, Ineffective Peripheral Perfusion, Instability of Blood Glucose Levels and Impaired Skin/Tissue Integrity. Nursing interventions and implementation included effective coughing exercises, airway management, hypervolemia management through fluid restriction and ice packs to help reduce thirst, blood transfusion, hyperglycemia management, and skin integrity maintenance. In conclusion, after nursing care, the patient's condition showed effective coughing, reduced shortness of breath, dizziness, and weakness, and edema remained. This case study is expected to be a source of information for patients and their families in understanding and managing CKD.
