Abstract

Access to healthcare services is a crucial determinant in improving community health, particularly in rural areas that face geographic and social challenges. This study aims to describe the accessibility and utilization of healthcare services among residents of Desa Sama Subur, Kecamatan Motui, Kabupaten Konawe Utara. A descriptive quantitative design was employed, with the population comprising all active household heads in the village. A total of 45 household heads were selected using total sampling. Data were collected through face-to-face interviews using structured questionnaires during the 2025 Field Learning Experience (Pengalaman Belajar Lapangan, PBL). Descriptive analysis was conducted and results were presented in frequency distributions and percentages. The findings indicate that the majority of respondents possess health insurance (88.9%); however, the utilization of healthcare facilities remains suboptimal, with only 48.9% having ever accessed services. Perceptions of healthcare accessibility varied, with 51.1% considering it easy and 46.7% reporting difficulty. Motorcycles were the most commonly used mode of transportation (77.8%), and community health centers (Puskesmas) were the most frequently utilized facilities (60.0%). Regarding service satisfaction, nursing staff behavior received the highest satisfaction rating (28.9%), while waiting time remained the main concern. In conclusion, healthcare access in rural communities is multidimensional, influenced not only by health insurance ownership but also by geographic factors, transportation availability, and service experience. Enhancing the utilization of healthcare services requires strengthening primary care and improving physical access to healthcare facilities.

Abstract

Indonesia ranks second in the world and Bandar Lampung City is the region with the highest TB incidence in Lampung Province with a treatment success rate that is still below the national target. This study aims to analyze the risk factors associated with the incidence of pulmonary TB in the Kedaton Community Health Center Working Area of Bandar Lampung City using a quantitative analytical study with a case-control design. The study sample consisted of 40 cases (smear-positive pulmonary TB patients) and 40 controls (closest neighbors without TB) taken using a consecutive sampling technique, where data were collected through interviews, observations, and measurements using questionnaires and physical measuring instruments at home. The results of the bivariate analysis showed that employment status (p = 0.024) and bedroom ventilation (p = 0.040) had a significant relationship with the incidence of pulmonary TB, while the results of the multivariate analysis using multiple logistic regression showed that gender (p = 0.015) and bedroom ventilation (p = 0.009) were significantly related variables. The conclusion of this study determined that bedroom ventilation was the most dominant variable associated with pulmonary TB incidence, with inadequate ventilation having a 3.91-fold increased risk (OR=3.91; 95%CI=1.41-10.94) compared to adequate ventilation after controlling for gender.

Abstract

Acute Respiratory Infections (ARI) remain one of the public health problems that contribute significantly to high morbidity rates, especially among vulnerable groups. Community health centers, as primary health care facilities, play an important role in recording, reporting, and monitoring cases through surveillance systems. This study aims to determine the description of ARI cases and evaluate the implementation of the ARI surveillance system at the Perumnas Community Health Center from August to October 2025. This study uses a descriptive evaluative approach. The surveillance system evaluation is based on the 5M components, namely man, money, material, method, and machine. Data were obtained through a review of surveillance documents and observation of the implementation of the ARI case recording and reporting system at the health center. The results of the study indicate that ARI is still one of the most common diseases found in the Perumnas Community Health Center's working area. The implementation of ARI surveillance is generally running well, but there are still several obstacles in terms of human resources, supporting facilities, data completeness, and optimization of reporting methods. These conditions affect the quality of the epidemiological information produced. Therefore, it is necessary to strengthen the surveillance system by increasing the capacity of officers, providing supporting facilities, and improving recording and reporting mechanisms so that the implementation of ARI surveillance is more effective and supports disease control efforts.

Abstract

Occupational health is an important aspect in maintaining worker productivity and safety, particularly in sectors with a high risk of chemical exposure such as gas stations (SPBU). Gas station workers are potentially exposed to hazardous chemical compounds such as benzene, toluene, and xylene which may affect the hematopoietic system and lead to decreased hemoglobin (Hb) levels. The use of Personal Protective Equipment (PPE) is one of the preventive measures to minimize exposure to these hazardous substances. This study aimed to determine the relationship between the use of Personal Protective Equipment (PPE) and hemoglobin levels among female workers exposed to fuel at gas stations in Kendari City. This study employed a quantitative research design with a descriptive analytic approach using a cross-sectional method. The population in this study consisted of all female fuel filling operators working at gas stations in Kendari City, totaling 30 workers, who were all included as research samples. Data on PPE usage were obtained through observation and questionnaires, while hemoglobin levels were measured using a Hematology Analyzer with venous blood samples collected in EDTA tubes. Data analysis was conducted using univariate and bivariate statistical tests to determine the relationship between PPE usage and hemoglobin levels. The results showed that out of 30 respondents, 21 (70%) had normal hemoglobin levels and 9 (30%) had abnormal hemoglobin levels. A total of 20 respondents (66.7%) used PPE according to the required standards, while 10 respondents (33.3%) did not meet the PPE requirements. Statistical analysis showed a p-value of 0.104 (p > 0.05), indicating that there was no significant relationship between PPE usage and hemoglobin levels among female gas station workers in Kendari City. The study concludes that the use of PPE does not have a significant relationship with hemoglobin levels among female gas station workers in Kendari City. However, the consistent use of PPE is still recommended as a preventive measure to protect workers from exposure to hazardous chemicals in the workplace.

Abstract

Preterm birth remains a significant public health concern due to its contribution to neonatal morbidity and mortality worldwide. Among the various risk factors, maternal infections—particularly sexually transmitted infections (STIs)—have been identified as preventable yet critical determinants of premature delivery. This study aimed to examine the relationship between STIs and preterm birth among mothers receiving care at Tomalou Primary Health Care Center. A cross-sectional analytical design was employed, involving 30 postpartum mothers whose medical records and antenatal care data were available. Data on STI status and birth outcomes were collected from health center records and laboratory results, while maternal demographic and obstetric characteristics were also documented. Results indicated that 40% of participants were diagnosed with STIs during pregnancy, and 33.3% experienced preterm birth. Chi-square analysis revealed a significant association between maternal STI status and preterm delivery (p = 0.003). Logistic regression demonstrated that mothers with STIs were sixteen times more likely to deliver prematurely compared to uninfected mothers (OR = 16.0; 95% CI: 2.45–104.3; p = 0.002). These findings suggest that STIs represent a substantial risk factor for preterm birth, even among mothers with adequate antenatal care attendance. Early detection, timely treatment, and comprehensive reproductive health education are recommended to reduce infection-related adverse outcomes. Strengthening STI screening and management within primary healthcare settings is essential to improve maternal and neonatal health outcomes. This study provides evidence for the importance of integrating infection prevention strategies into routine antenatal care to mitigate the risk of premature delivery.

Abstract

The elderly often experience a decline in physical function that can interfere with the ability to carry out daily activities. This condition is more common in the elderly who have had a stroke, which is always accompanied by a decrease in muscle strength. Inactivity of the joint over a long period of time can lead to contracture. One of the nursing interventions that can be done to prevent this condition while increasing muscle strength is  passive Range of Motion (ROM) exercises. The purpose of this case study is to apply nursing measures to the elderly with a decrease in muscle strength that is already research-based. This scientific paper uses a case study design  with passive ROM training interventions. The number of respondents in this study was three patients. The intervention was carried out for three days on each respondent, with a frequency of twice a day for 20 minutes in the Stroke Nursing Room of Charitas Hospital Palembang. The results of the application showed an increase in muscle strength based on Manual Muscle Testing (MMT) scores in all respondents. Mr. P experienced an increase in the muscle strength of the left hand and leg from 2 to 4, Mrs. L from 2 to 3 in the right hand and foot, and Mr. A from 0 to 4 in the right hand and foot. This application is expected to add information about the use of passive ROM in stroke seniors who have decreased muscle strength.
