Abstract

The cadres failure in the implementation of health center activities to monitor the growth of children under five will result in the wrong conclusion which will give incorrect information and precaution. Therefore, an analyze is required to determine the behavior of cadres in monitoring the growth of children under five  in Mandala health center in sub district Medan Tembung. This research is a descriptive cross sectional design research carried out in September 2013 to December 2014. The population in this study were all cadres in 39 integreted health posts in working area of public health center Mandala district of Medan Tembung that is 159 people. The sample in this study were 100 people who is obtained using cluster sampling. Existing data will be analyzed descriptively and presented in form of a frequency distribution table. The result of this research showed that 51,0% cadres aged < 37 years old; 61,0% of cadres education is primary education (Primary Junior High School); 90,0% has becomed cadres for > 2 years; unemployed caders is 83,0%; and the most frequent task is the registration (41,0%). Cadre behaviour in implementation of children under five year growth monitoring, 49,0% of cadre has good knowledge, but only 14,0% cadre has good manner and only 3,0% cadre with good action. It is suggested for the clinic to give education to the cadres in growth monitoring of children under five and to provide training such as plotting training, weighting children under five and calculating ages periodacally to the cadres. Abstract

ABSTRACT Guava (Psidium guajava L.) is one of medicine plant which leaf has all kind of purpose. Guava leaf contains alkaloid, flavonoid, tanin, saponin and etherial oils which effect to kill mosquito larvae. The purpose of this research is to know the effectiveness of guava leaf extract as an Aedes spp. mosquito larvacide in ovitrap and the value of LC50 within 24 hours. This study is a quasi experiment. The method of research using Completely Randomized Design which consist 6 concentrations of guava leaf extract (0 ppm, 500 ppm, 2.500 ppm, 4.500 ppm, 6.500 ppm and 8.500 ppm) by 4 times repetition. Seven hundred and twenty (720) mosquito larvaes are used in 100 ml solution of guava leaf extract. Observation times of Aedes spp. mosquito larvae mortality are 2 hours, 12 hours and 24 hours. Data was analized using Friedman and Kruskal Wallis test with 95% of credibility and probit analysis. According to statistics result indicates average difference of Aedes spp. mosquito larvae death in various guava leaf extract concentration and all time observation is showed by p-value < 0,05. The result of probit analysis showing that LC50 of guava leaf extract on concentration 2.502,67 ppm. As larvacide, the toxicity of guava leaf is safe for non-target organism. The conclusion of this research is guava leaf extract contains chemical substance which can be used as larvacide that most effective on concentration 8.500 ppm (93,33% death percentage) and the amount of the dead Aedes spp. mosquito larvae descended after 12 hours. Guava leaf is expected to be an alternative on Aedes spp. mosquito larvae restraint.

Abstract

ABSTRACT The impact of mosquitoes and mosquito repellent can cause health hazards to humans, as well as the expensive of mosquito trap is a problem at this time. The purpose of study on the fermentation of sugar as a mosquito attractant is to determine whether there is any difference in the number of mosquitoes trapped by the fermentation of sugar concentration variation and long of observation. This research was conducted at the center for Environmental health Engineering field. This study is quasi-experimental, and the method used is Completely Randomized Factorial Design (RAL) The first factor is fermented of sugar concentration which are; 0%, 5%, 15%, 25%, and 35%. The second factor is long observation on day-1, day-2, day-3, day-4, day-5, and day-6. This study was conducted from March until June 2015.. The sample used in this study is 300 adult mosquitoes for each treatment. Data were analyzed using Kruskal Wallis Statistical test and continued Duncan test. The result showed that there is a difference of five variations fermentation of sugar concentration on the number of mosquitoes trapped. There is no difference in the number of mosquitoes trapped with long observations. Fermentation of sugar concentration in 35% indicates as the effective concentration because there is a significant difference from five other variations of the fermentation of sugar concentration. The higher sugar material, the stronger the adhesion of the fermentation of sugar. Long observation of the number of mosquitoes trapped showed that there is no significant difference from day to day-2 until day-5, and start decline at day-6. Fermentation of sugar can be used as an alternative for vector control especially the mosquito as the safe attractant for the environment and humans. The fermented sugar as a mosquito attractant should be used more than one in a room.

Abstract

ABSTRACT Diarrhea was a public health problem in developing country like Indonesia. Jakarta Pusat was endemic diarrhea area. Climate was proponent factors of diarrhoea occurrence. Research sites in Jakarta Pusat was done by collecting data recorded from the years 2002-2013 from Department of Health in Jakarta Pusat and Central Bureau of Statistics in DKI Jakarta Province. The design of this research was time trend ecological study which the unit of analysis is per month and per year during ten years. This research aims to know the relationship among rainfall, temperature, humidity and wind’s speed with the diarrhea occurrence from the years of 2004-2013 in Jakarta Pusat. Data analysis was performed by using univariate and bivariate analysis using pearson correlation and simple linear regression. The results of this research, there was significant correlations of rainfall with diarrhea case per month (p = 0,031) and strong correlations (r = 0,621), had positive pattern, there was significant correlations of humidity with diarrhea (p = 0,006) and strong correlations (r = 0,739), had positive pattern, while temperature per month and wind’s speed per month did not correlate significantly. There was significant correlation of Wind’s speed with diarrhea per year (p = 0,025), and strong correlations (r =  0,697), had positive pattern. Based on the research results, it was concluded that the rainfall and humidity correlated significantly with diarrhea cases per month. While the wind’s speed correlated significantly with diarrhea cases per year. Need the existence of cooperation program among Department of Health in Jakarta Pusat, relevant agencies, and society in exploiting climate variation data to prevent of diarrhea case explosion (KLB) in the future. The people always be alerted diarrhea with taking care of personal hygiene and environment

Abstract

Abstract Tiganderket sub-district is one of the districts that were less than 10 km from Mount Sinabung that erupted since 2010 until now. The problem in this research is the increasing cases of Acute Respiratory Infection (ARI) in the Primary Health Care (PHC) Tiganderket sub-district in 2014. The purpose of this study was to determine the relation between the physical quality of houses with ARI incidents after eruption of Mount Sinabung in the PHC Tiganderket sub-district Karo Sumatera Utara in 2015. This study is observational analytic study with case control study design. The population is suffering from ARI and was treated at the PHC Tiganderket during 2014 as many as 3,504 people, with a sample of 124 peoples were obtained by proportional random sampling. Data were analyzed with Chi Square test statistic 95% CI (p = 0,05) and Odds Ratio value. Based on the results of research in the Puskesmas Tiganderket sub-district concluded respondent characteristics related to ARI is a job (p value = 0,010). Physical conditions related to the ARI is the condition of the floor (p value = 0,000, OR = 5,697). Occupant behavior characteristics related to ARI is to open a window every day (p value = 0,001, OR = 3,838) and smoking (p value = 0,000, OR = 7,091). There is no relation between age (p value = 1,000), sex (p value = 0,463), marital status (p value = 1,000), education (p value = 0,407), village address (p value = 1,000), spacious room ventilation (p value = 0,769), type of wall (p value = 0,402), room air humidity (p value = 0, 15), room temperature, room density (p value = 0,094), the occupants sweep the house every day (p value = 0,697 , OR = 0,737), mop the house every week (p value = 0,243) with ARI in the PHC Tiganderket sub-district. It is suggested to the community to keeping house clean, opening the windows only in the morning, avoiding smoking inside the house, and using mask when outside the house

Abstract

ABSTRACT The workers at PTPN IV Unit Usaha Pabatu directly confronted with the production process during operational activities, so that the Occupational Health and Safety (OHS) is important, therefore, it needed the good of OHS risk management to get zero accident. The company has implemented OHS risk management but there are still found work accidents and occupational problems. This study described the results of hazard identification, risk assessment and occupational accidents control by using HIRAC method 2015. This research was descriptive research that describes the application of risk management in the palm oil mill. Technique of collecting data used in this study was assessment documents. The implementation of risk management at PTPN IV Unit Usaha Pabatu 2015 implemented by the independent risk management team at seven stations is sortation/ loading ramp station, sterilizier station, thresher station, pressan station, klarification station, kernel station, and boiler station. The implementation of risk management based on the results of HIRAC 2015 showed that there are in each station, yet according with government regulations No.50 tahun 2012, because risk assessment not rated objectively. The company has implemented the risk control like the compliance of work instructions, installation of OHS signs, fire extinguisher provision and the usage of personal protective equipment (PPE). It is regulated in accordance with government regulations No.50 tahun 2012. Based on the results of the study, OHS management is expected to conduct OHS training and to improve the socialization of OHS to the workers.

Abstract

ABSTRACT Salted fish is one of fishery products that have an important position and also is one of the nine basic foods on a national scale.Curring aims that preserve the fish doesn’t relieves metal properties. Metal properties can not demolished. Metal will dissolved in the water and absourbed by microoganism then eaten by fish and finally that’s happen bioaccumulation and biomagnification so that fish which make processing, it doesn’t relieve metal properties that contained the fish. The method used in this research is descriptive survey. Samples were obtained from salted fish producers in kelurahan Bahari, Subdistrict of Medan Belawan, checked in Health Laboratory, Province of North Sumatra and Center for Research and Industry of Medan.The five samples of salted fish are ikan Lemuru (Sardinella Aurta), ikan Gelama (Pseudoceina amoyensis), ikan Kresek(Trissa mytax), ikan Gembung (Restreluger kenagona), dan ikan Cincaru (Eleutheronema aurta). To determine the content of mercury (Hg) and Cadmium (Cd) is performed by Spektrofotometri Serapan Atom(SSA) method. To result showed that the content of Mercury (Hg) in salted fish produced in Kelurahan Bahari has been qualified according to BPOM in 2009, which is 0.5 ppm. The content of Cadmium (Cd) contained in a salted fish which exceed the NAB set by BPOM, namely Lemuru fish 0.480 ppm.The four different salted that fish are qualify according to BPOM in 2009, which is 0.1 ppm. Society may consume salted fish produced in Kelurahan Bahari with the exposure limit and not constantly

Abstract

ABSTRACT Fresh fish is the type of fish which has the same characteristic as living fish, in shape, smell, taste and texture, hasn’t gone through preserving process or any other further processing. The fish which is taken mostly comes from Belawan Sea, with the size of 1 kg, 0.5 kg an 0.3 kg. One of the closest fish market with the Belawan Sea is KUB (Kelompok Usaha Bersama). The purpose of this research is to find out the content of cadmium (Cd), metal (Pb) and formaldehyde in the fresh fish which comes from KUB Belawan. The method is descriptive survey to analize the content of cadmium (Cd), lead (Pb) and formaldehyde in some fresh fish. The result showed that the level of cadmium (Cd) in tuna fish (Euthynnus affinis) with the size of 1 kg, 0,5 kg and 0,3 kg are 0,04 mg/kg, 0,01 mg/kg and 0,003 mg/kg and in white snapper fish (Lates calcarifer) are 0,01 mg/kg, < 0,003 mg/kg and < 0,003 mg/kg. The result showed that the level of lead (Pb) in tuna fish with the size of 1 kg, 0,5 kg and 0,3 kg are 0,167 mg/kg, 0,131 mg/kg and 0,101 mg/kg and in white snapper fish are 0,140 mg/kg, < 0,0025 mg/kg and < 0,0025 mg/kg. Formaldehyde showed is negative result. Based on SNI 7387:2009 regarding the maximum limit of heavy metal pollution in food product, which was drafted by taking into account the Ditjen POM Decision No.03725/B/SK/VII/1989, the level of cadmium and lead in tuna fish and white snapper are under the treshold limit value which are 0,1 mg/kg and 0,3 mg/kg. The highest level of cadmium and lead was found in fish with the bigger size and the value of formaldehyde in fishes that were taken from KUB Belawan is negative. The fish from Belawan Sea is safe to consume, but it’s better to consume the small size fish instead of the bigger ones.

Abstract

ABSTRACT The accident that happened to the company truck drivers besides harmed directly to the victims, also raised the big impact to the company. Therefore, a research about factors that influence to the potential of the work accident of the truck drivers at PT BerkatNugraha SinarLestari, Belawan in 2015. The research samples are 39 truck drivers. The characteristic of this research is analytic using cross sectional design. The data analysis used are univariate, bivariate using Exact Fisher test and multivariate using Binary Logistic Regression test with backward stepwise method. The result of the research shows that truck drivers who ever got work accident are 14 drivers (35.9%) and 25 drivers (64.1%) are never got work accident. The result of exact fisher test show that the condition of the vehicle, the condition of the road and the condition of the body physical have a corellation to the potential of the work accident. The result of binary logistic regression test with backward stepwise method show that the most significant variable influence is the condition of the body physical with value p=0,003 and Exp (/6)=0,065. According to the result of this research, it is suggested to PT BerkatNugraha SinarLestari to provide a seat bearing for truck driver in order to reduce the vibration when they go passing the broken road. After driving for 4 hours, the drivers must take a rest at least 30minutes. After that, drivers must stretch leg, hand, shoulder, and the whole body muscles in order to release muscle tension and drowsiness before continuing the trip. In addition, the truck drivers must provide mineral water during the trip in order to left out thirsty or drowsiness while driving and be careful when driving a truck.

Abstract

ABSTRACT Work accidents are mostly caused by unsafe behavior. Behavior Based Safety Program plays an important role to create zero accidents. The implementation of behavior-based safety program is focused to observe the behavior of a process to improve safe work behavior. This study used a descriptive research with cross sectional design approach which aimed to describe the application of behavior-based safety program and work accidents in PT. Inalum Kuala Tanjung year 2013-2014. The samples of this research were all employees which injured in PT.Inalum Kuala Tanjung year 2013 and 2014. The available data were analyzed descriptively. The results showed that the accident happened in 2013 mostly happened in the reduction factory as much as 2 times (40 %) and in 2014 mostly happened in the maintenance as much as 3 times (50%). Work accidents which occurred in 2013 and 2014 mostly caused by unsafe behavior. The application of behavior-based safety program is still new and needs to run a sustainable improvement especially inform improvements of Inalum Secure Card (ISC). The result of the evaluation Inalum Secure Card (IKA) conducted by safety promoter at each department or section each month. But not all safety promoter reported the result of the evaluation Inalum Secure Card (IKA) to section Inalum Internal Control (IIC). The application of Inalum Secure Card (IKA) have not given rewards and sanctions to the workers. Based on the results of the study, PT.Inalum Kuala Tanjung is expected to improve communication about Occupational Health and Safety (OHS), especially regarding the behavior-based safety program so that the workers could care and aware to behave a safety work to create Occupational Safety and Health (OHS) culture.
