Abstract

Introduction: Curry leaves (Murraya koenigii L.) are plants that naturally grow in the Indian subcontinent, except at the peak of the Himalayan mountains, which in their development have spread widely to various corners of the world. Traditionally, curry leaves have been used by people as medicine. However, it is now known that this plant contains carbazole alkaloid compounds and polyphenols that have antioxidant activity. This is very useful for controlling free radical levels in the human body so that they remain within reasonable limits. This is very useful for controlling the levels of free radicals in the human body to stay within reasonable limits. Free radicals are produced by normal cell metabolism which has recently been associated with the pathophysiology of various diseases. Methods: This in vitro research was to determine phenolic content and antioxidant potential in methanol extract of curry leaves. Total phenolic content testing used the Folin-Ciocalteu method and expressed by Gallic Acid Equivalent (GAE). Total antioxidant capacity testing used the DPPH method and Trolox as a comparison solution. Results: The total antioxidant capacity is expressed in the form of IC50. The total phenolic content of curry leaf methanol extract was 4007.78 μg/mL or equivalent to 133.59 mg GAE/g DW and the total antioxidant capacity of curry leaf methanol extract was 37.418 μg/mL. Conclusion: The total phenolic content of curry leaf methanol extract is high (>5 mg GAE/g). Correspondingly, the total antioxidant capacity of curry leaf methanol extract is also classified as very strong (<150 µg/mL).

Abstract

Introduction: Foods high in cholesterol increase the production of free radicals and inflammation, and may also trigger the formation of atherosclerotic lesions. Cnidosculus aconitifolius or tree spinach leaves were known to have antioxidant and anti-inflammatory activities. This study was to determine the effect of Cnidosculus aconitifolius on blood cholesterol level and coronary artery atherosclerotic lesions formation using a rat model. Methods: A randomized post-test-only control group of 30 male Wistar rats (Rattus norvegicus), aged 2-3 months, randomly divided into 3 groups: standard control was given regular food; negative control was given an atherogenic diet; and the treatment group was given an atherogenic diet + Cnidosculus aconitifolius leaf ethanol extracts. The treatment lasted 44 days. Data was analyzed with OneWay ANOVA and Kruskal-Wallis test. Results: The total cholesterol and LDL levels were significantly lower in the treatment group compared to the control group (p<0.001). A decrease of atherosclerotic lesion scores in the treatment group compared to the control (p=0.010) was also observed. Conclusion: Cnidosculus aconitifolius leaf ethanol extracts have an anti-hypercholesterolemic effect, hence the potential to prevent the formation of atherosclerotic lesions in the coronary arteries of hypercholesterolemic Wistar rats.

Abstract

Background: Alcoholic cardiomyopathy (ACM) is cardiomyopathy caused by consuming large amounts of alcohol. Diagnosis is determined based on alcohol consumption’s history, clinical, and supporting data. Case: Male, 29 years old, came with shortness of breath especially when walking, lying down, and waking-up at night. The patient has been consuming 4 bottles of “Flores wine”/day for the past 14 years. Physical examination showed BP 124/93, pulse 101 x/min, RR 24 x/min, left apex and parasternal area systolic murmur, basal rhonchi, and edema of both feet. Electrocardiography showed RAD, RAE, LBBB, and LVH. Chest x-ray showed cardiomegaly and minimal pleural effusion. Laboratory examination showed SGOT 986.2 U/L, SGPT 448.3 U/L, and electrolyte imbalance. TTE results showed global hypokinetics, all cardiac chambers dilatation, reduced LV systolic function (EF 21.6%), severe TR, and moderate-severe MR. Angiography results showed no stenosis in all coronary arteries. MRI result is similar to TTE without late gadolinium enhancement (LGE). Therapy given according to guided-directed medical therapies (GDMT). Conclusion: ACM is cardiomyopathy with highly variable objective findings. In-depth history regarding alcohol consumption is crucial and other possible causes must be excluded.

Abstract

Background: Asthma is a chronic inflammatory condition affecting the airways, marked by tissue changes and heightened bronchial sensitivity. Vitamin D, as an immunomodulator, may affect the inflammatory responses of the airways. Recent studies suggest that vitamin D supplementation has resulted in fewer exacerbations of asthma and improved lung function in both individuals with asthma and smokers. Other studies revealed a reduction of asthma exacerbations in corticosteroid-treated patients. This study sought to investigate whether vitamin D supplementation can decrease the frequency of asthma exacerbations. Methods: Literature research was done on three databases: PubMed, Cochrane Library, and ScienceDirect, related to the clinical question. Articles were chosen according to specific inclusion and exclusion criteria. All articles were critically appraised using the Oxford Center for Evidence-Based Medicine worksheet. Results: Three meta-analysis articles were critically appraised. The result is vitamin D supplementation can lower the incidence of asthma exacerbation, despite varying doses of vitamin D. Conclusion: Vitamin D supplementation was associated with a decrease in asthma exacerbations, despite varying doses of vitamin D. Vitamin D supplementation to corticosteroids can be considered potentially cost-effective and safe to prevent asthma exacerbations. Future studies should analyze the effect of vitamin D supplementation in long-term asthma therapy using the same dose.

Abstract

Lupus nephritis (LN) is one of the most common complications in systemic lupus erythematosus. If not detected quickly, LN will often cause reduced kidney function in five years necessitating kidney replacement. Diagnosis and classification of LN are made with a kidney biopsy. The drug of choice is based on the histopathology classification and needs to be tailored case by case. If complete remission is not achieved or if it relapses, the causes need to be investigated. More nephron cells can be saved in earlier therapy to prevent end stage kidney disease complications.

Abstract

Ophthalmia neonatorum or neonatal conjunctivitis is an acute conjunctivitis in newborns under the age of 1 month. The causes are grouped into aseptic and septic types. Diagnosis is made by clinical symptoms and signs as well as laboratory examination. Appropriate treatment based on etiology can prevent permanent eye damage. Without optimal treatment, complications such as corneal ulceration, perforation, and even blindness may occur.

Abstract

Untreated diabetes can lead to complications and increase the risk of death. Many patients with type 2 diabetes will require injectable medication within 5-10 years after diagnosis, following the failure of two or three oral antidiabetic therapies. Adding basal insulin can be well tolerated, more effective with lower risks of hypoglycemia and weight gain compared to other insulin regimens, but it carries a risk of hypoglycemia. Insulin glargine U300, with its special formulation, reduces the risk of hypoglycemia in diabetic patients.

Abstract

Introduction: This article aims to determine the role of plasma fibrinogen level as a prognostic factor for hepatocellular carcinoma patients. Method: A structured literature review will be conducted through PubMed, Google Scholar, and ScienceDirect, according to the selected clinical questions. Literature selection was based on inclusion and exclusion criteria. The three selected literature were then critically appraised using criteria that included validity, importance, and applicability. Discussion: The three kinds of literature in this article all state that high plasma fibrinogen levels correlate with poor overall survival in hepatocellular carcinoma patients. Conclusion: Plasma fibrinogen levels may act as a prognostic factor for overall survival in hepatocellular carcinoma patients.

Abstract

Down syndrome is congenital or genetic abnormality with extra chromosome 21 causing various clinical manifestations and health problem. Down syndrome ranked first as the most common cause of birth defect in Indonesia. Chromosomal abnormality cause Down syndrome children have a higher risk and more common health problems. Down syndrome can burden family or caregiver physically, socially, financially, and emotionally. Proper screening and diagnosis criteria need to be done to establish the diagnosis.

Abstract

Introduction: Emergency department (ED) mortality is a critical issue that has a major impact on individuals, society, and the entire health system and is one of the indicators of emergency department service performance. This research aims to provide input to hospital management regarding the impact of non-medical factors (number of visits, length of stay/LOS, and ED crowding) on patient mortality, related to improvement policy-making in ED patient services. Method: A retrospective study on 130 ED shifts from medical record data during 2022-2023. The independent variables were the number of visits, LOS, ED crowding measured by CEDOCS, and ED mortality as the dependent variable. Results: There were deaths in 38 out of 130 shifts in the ED, with a total mortality of 1.6%. Statistical tests showed a significant effect between crowding and ED mortality (p = 0.01), a non-significant effect between visits and ED mortality (p = 0.76), and a non-significant effect between LOS and ED mortality (p = 0.62). The results of the regression analysis were able to explain the data (p = 0.91; Hosmer-Lameshow test). The area under the curve (AUC) for prognosis in hospital treatment is 0.641 with a 95% confidence interval of 0.539-0.744 (medium category). Conclusion: The significant effect between crowding and patient mortality in the ED indicates that strategic management must be carried out to reduce ED crowding.
