Abstract

Graves’ disease is the primary cause (60%-80%) of hyperthyroidism. The risk factors include genetic, environmental, and immunological aspects, with a higher prevalence in women. The diagnosis is based on medical history and physical examination, supported by Wayne index, as well as appropriate diagnostic tests including thyroid function tests, antibody testing, thyroid scan, or color-doppler ultrasonography. Treatment options include anti-thyroid medication, radioactive iodine treatment (RAI), and surgery. Complications include Graves’ ophthalmopathy, thyroid dermopathy, cardiovascular effects, and thyroid crisis. The prognosis depends on the likelihood of remission and relapse, with remission being more common in adults than in children.

Abstract

Melasma is a chronic skin pigmentation disorder caused by sunlight exposure. Melasma is a multifactorial disease with various pathways that can induce hyperpigmentation. Exposure to ultraviolet (UV) light as a source of reactive oxygen species (ROS) and free radicals plays a role in the pathogenesis of melasma. Another mechanism is the role of ROS on melanogenesis and melanocyte cells as well as the signaling pathway for melanin synthesis. Concentration of antioxidants as a skin defense against ROS, namely superoxide dismutase (SOD), catalase, and glutathione peroxidase, also influences the occurrence of melasma.

Abstract

Adequate iodine intake during pregnancy and lactation is vital for thyroid hormone synthesis which iscrucial for fetal and neonatal development, especially with exclusive breastfeeding. This review explores the link between maternal iodine levels and breast milk iodine concentration (BMIC), focusing on factors infuencing maternal iodine and its impact on BMIC. This review synthesizes current findings on the determinants of maternal iodine status, including dietary intake, supplementation, and iodine metabolism, and their impact on BMIC underscores the necessity for healthcare providers to monitor and guide iodine intake in pregnant and lactating women. The fndings advocate for targeted public health interventions and the implementation of robust guidelines to ensure adequate iodine nutrition.

Abstract

Post-COVID-19 neurological disorders, also known as post-covid neurology syndrome (PCNS), are an integral part of long COVID and include various health problems that can last for several weeks, several months, or even years. These neurological disorders can vary, such as postCOVID fatigue syndrome, ognitive impairment, anosmia and ageusia, headaches, or diseases such as stroke and GBS, which still require further research on the mechanisms. Until now, there has been no definitive diagnostic examination, so the history and physical examination must be precise. Further management is given according to the complaint.

Abstract

Introduction: Cardiovascular diseases (CVDs) are the leading cause of death, accounting for 31% of global mortalities. This systematic review explores the role of electrophysiological profles in cardiovascular prevention, focusing on their integration into risk management protocols. Methods: A comprehensive review was conducted using the PubMed database, adhering to PRISMA guidelines. Of the 1,050 identifed studies, 10 met the inclusion criteria and were assessed for quality using the STROBE method. Results: Key electrophysiological markers were identifed as signifcant predictors of cardiovascular events. Abnormalities in Holter monitoring, such as a turbulence slope (TS) ≤25 ms/RRI, are strongly correlated with sudden cardiac death (HR 4.7). Prolonged QT intervals in ECG were linked to increased mortality from cardiac arrest (HR 2.4) and heart failure (HR 1.74). ST-segment depression during exercise stress testing was a strong predictor of coronary heart disease death (HR 2.47), especially during the recovery phase (HR 4.01). Discussion: Electrophysiological markers like Holter monitoring, ECG fndings and exercise stress testing are valuable for identifying high-risk individuals. Their integration into routine screening could improve early detection and prevention strategies, further research is needed to standardize protocols.

Abstract

Coats’ disease is an idiopathic condition characterized by non-hereditary exudative retinopathy caused by abnormal retinal vascular development. Patients often present with signs such as leukocoria, significant vision loss, and strabismus. Some characteristic features include telangiectasia, aneurysms, and exudative retinal detachment. Ultrasonography and computed tomography scan can detect masses and calcifications, which are useful in ruling out differential diagnosis of retinoblastoma. Coats’ disease has 5 stages, but it is not uncommon for patients to seek treatment in advanced stages or when it has progressed to a malignancy in children, such as retinoblastoma, therefore the patient will be referred for enucleation.

Abstract

Influenza viruses have posed a global threat since the 19th century, presenting serious challenges and threats to public health worldwide. Influenza viruses are among the major seasonal outbreaks impacting health and the economy yearly. Numerous new strains emerge over time due to the antigenic drift of influenza viruses, which undergo mutations. Therefore, vaccines and medical technologies continuously need innovative approaches to stimulate immune responses effectively. mRNA-based influenza vaccines are potentially more effective because of the inclusion of a broader range of antigens that can enhance cellular immunity or expand protection beyond just hemagglutinin (HA) and neuraminidase (NA), they can incorporate more than four antigens (HA) in a single formulation, offering advantages in effectiveness, safety, and rapid large-scale production. mRNA-based influenza vaccines hold significant potential and are expected to benefit children due to their various advantages.

Abstract

Influenza virus infection is common in populations of all ages. Vaccination is the most effective measure to prevent infection. However, influenza viruses are prone to antigenic drift, causing the virus to mutate within a few months. Predicting influenza virus evolution plays a crucial role in ensuring the protective effect of vaccines, allowing for the selection of the appropriate vaccine type. Stacking models, convolutional neural network (CNN) models, Gaussian processes vector autoregressive models, and susceptible-exposed-infectious-removed (SEIR) models can predict influenza virus antigenic variants. Various modern models and approaches, such as influenza antigenic variants (IAV)-CNN models, sequence-based antigenic distance approach (SBA), and ensemble of nonlinear regression models, have been used to improve the efficacy and relevance of vaccination strategies.

Abstract

Introduction: Asthma patients are at higher risk of experiencing respiratory morbidity due to influenza, vaccination is recommended for this group. This study aims to compare the efficacy and side effects between live-attenuated influenza vaccines (LAIV) and inactivated influenza vaccines (IIV) in asthmatic children. Methods: This research use PRISMA guidelines, with studies sourced from PubMed, Cochrane, and Embase. ROB2 and ROBINS were employed to assess the risk of bias. Quantitative analysis was performed using Review Manager v5.4. Results: A total of 4,753 samples were collected, consisting of 2,390 intervention patients and 2,363 control patients. The meta-analysis results showed that influenza infection was more likely to occur in patients who received the inactivated influenza vaccine [pooled OR = 0.58; 95% CI (0.43, 0.78)}. Conclusion: The recommended vaccine type is the live-attenuated influenza vaccine, as no significant differences in side effects were observed between the two vaccines.

Abstract

Anak dengan gangguan perkembangan neurologis (GPN) termasuk dalam kelompok risiko tinggi untuk mengalami komplikasi berat akibat influenza. Hal ini disebabkan oleh keterbatasan mobilitas, penurunan kekuatan otot pernapasan, serta gangguan tonus otot yang dapat memengaruhi fungsi sekresi paru. Meskipun kelompok berisiko ini relatif kecil, anak dengan GPN merupakan penyebab sejumlah besar rawat inap dan kematian akibat influenza. Vaksinasi tahunan adalah pencegahan yang efektif dan direkomendasikan untuk semua anak usia 6 bulan hingga 18 tahun, termasuk anak dengan GPN. Selain itu, anggota keluarga atau pengasuh di lingkungan sekitar juga harus menerima vaksinasi influenza setiap tahun. Namun demikian, penelitian khusus mengenai vaksin influenza pada populasi kelompok ini masih terbatas, dan tingkat efektivitas vaksin dapat bervariasi setiap tahunnya. Artikel ini akan membahas vaksinasi influenza, baik secara umum maupun khusus pada kelompok anak dengan GPN.
