Abstract

Bronkhopneumonia is still disease with a wide spectrum of clinical presentation often with unpredictable clinical progression andoutcome. It is often difficult to predict the subset of patients who will progress from non-severe to severe disease. A 6 years old boyadmitted to the hospital for his acute fever for three days accompanied with shortness of breath. He also complained cough with pleghm, headache, nausea, and vomitting. There was rash on his limbs and his gums bleed several hours ago. He looks ill, compos mentis, temperature was 38.9°C, pulse was regular 112 x/min, respiration rate was 24 x/min, there was cyanosis, nostril breath (+), and found ronchi on auscultation. Blood workup showed thrombocytosis and increasing LED with no leukocytosis. He was diagnosed with bronkhopneumonia and then he received intravenous fluid, oxygen, i.v line broadspectrum antibiotic, andacetaminophen. Bronkhopneumonia has many cause of disease so it should receive more attention to symptoms and signs to get information of the severity of the disease. Abstract

Low birth weight (LBW) infants are born with a birth weight less than 2500 grams regardless of gestation. The prevalence of LBW is about 10% of all pregnancies. Some 3-5% of LBW occurred in healthy mothers, and more than 25% of events occurred in mothers with high risk pregnancies. A newborn boy, 1 day old, delivered from operating room (OR) Abdul Moeloek Hospital with preterm age and low birth weight. Baby born per abdominan with indication antepartum hemorrhage because placenta covering birth canal. Baby weighing 1800 g, body length 45 cm, with Apgar score 5/6/7. Physical examination obtained unwell state, compos mentis awareness, temperature 36.9 ° C, heart rate 138 x/min, respiration rate 64 x/min, nutritional status weight/age = <-3 SD (impression: poor nutrition), PB/age= -3 SD — <-2 SD (impression: short), BB/PB: <-3 SD (impression: very thin), Ballard score of 22 (32 weeks gestation), suprasternal and subcostal retraction, moaning (+), vesicular breathing (+/+). Hb: 16.8 g/dl; leukocytes: 24,400/ul, platelets: 255,000/ul. Patients was diagnosed LBW with Respiratory Distress Syndrome (RDS). Management of LBW infants with RDS in this case is installation of Continous Positive Airway Pressure (CPAP), trophic feeding, phototherapy, giving aminophylline and antibiotics. Abstract

Smoking is the most common disorder in a population with severe mental illness. Smoking frequency of individuals with schizophrenia is 2-4 times higher than the general population. Smoking also affects the metabolism and levels of psychiatric drugs in the blood. Mechanism of nicotine in influencing neurotransmitters in the central nervous system is through bonding with cholinergic receptors which in turn triggers the release of dopamine. Decreased plasma levels of antipsychotic drugs cause patients require higher doses of medication. Abstract

Marasmic is a malnutrition condition occurs when the diet is lack of calories and protein, or can also due to malabsorbtion, metabolic abnormalities or congenita l malformations. In malnutrition patients can occur growth disorder. Mortality rate quite large, about 55%. Death is often caused by an infectious disease due to a disorder of the body's defense mechanism, this also often cause severe life-threatening condition. A 1 year old child, came with chief complaints of cough since he was 5 months with no improvement, accompanied with decreased of app etite so there is no increase in body weight. Patients also experienced subfebrile fever with night sweating. Patient had contact with pulmonary tuberculosis patient. Physical examination found pulse 140x/min, respiration 32x/menit, temperature 38,1o C (per axiler), body weight 5,4 Kg and height 65 Cm. Conjunctival pallor, elderly face, sunken eyes, baggy pants, wasting, turgor returns slowly and no edema. Blood tests found Hb 10.6 g/dL. Radiological examination overview of active pulmonary tuberculos is. Patient was diagnosed with marasmic with failure to thrive and stunting accompanied pulmonary tuberculosis and nutritional anemia. Patient was threated based on management procedure of marasmic and management of pulmonary tuberculosis based on national guidelines tuberculosis for children. Malnutririon can lead to failure to thrive, anemia and immune system distur bance that cause the patient more likely to get any infection. Abstract

Leiner’s disease is a disease affecting infants (4-20 weeks) which is a complication from seborrhoeic dermatitis, universal erythema and squama (erythroderma), usually found with anemia, diarrhea and vomit. It is followed by secondary bacterial infection. The etiology of this disease has not been known yet and also so rare. A Baby, 5 months old, had chief complain of red spots and rough shells all over the body during a week, accompanied by fever, vomit, and diarrhea. From physical findings, the general condition seemed to be in a serious illness, consciousness was compos mentis, regular pulse 96 x/minute, respiration rate 24 x/minute, temperature 37,3 OC. Nutrient status was normal (Z - Score WHO 2006 Curve). From dermatologic findings, the lesion was general, diffuse erythematous covered by rough scaly layer of squama, yellowish white, oily, wax drop phenomenon test (-). Laboratory assessment found leukocytes 16.700/ul, hemoglobin 11,1 gr/dl, hematocrit 34,4% and platelets 717.000/ul. The patient was diagnosed with Leiner’s disease and given systemic therapy dexamethasone, chlorpheniraminemaleat, paracetamol, lacto-bio, and zinc, and topical therapy gentamicyn ointment and emollients lanolin 10%. The etiology of Leiner, although has not been known yet, commonly caused by spreading seborrhoeic dermatitis. The diagnosis determined based on anamnesis, physical examination, and laboratory examination. Abstract

Acute Post-Streptococcus Glomerulonephritis (APSGN) is characterized by the appearance of hematuria, proteinuria, red blood cell in the urine, swelling (edema), and hypertension with or without oligouria. It can follow beta haemolytic streptococcus bacterial infection group A in the upper respiratory tract or on the skin. Boy, 7 years old, complained of swelling in the face, legs, and genital since 5 days before entering the hospital. As long as swelling, the patient looks pale, weakness, fatigue, lethargy, and loss of appetite. A week before admission, the patient's  urine was reddish. Nine days before admission, the patient had pharyngitis. In physical examination found blood pressure was 120/100 mmHg, pale skin, eyes anemic conjunctiva, swelling of palpebra, swelling of genital, swelling of lower extremity pretibia the right and left (pitting edema). Laboratory results, Hb 9.3 mg/dl, positive Anti Streptolysin Titer O (ASTO) examination, on urinalysis the urine color was red, protein 150 mg/dl and blood cryptic 250/ul. Patient was diagnosed with acute post-streptococcus glomerulonephritis with hypertension grade I. Managements were bed rest, low salt diet, supportive therapy, IVFD D5% 15 drops/min (micro), antibiotic ceftriaxone 1gr/12h, and diuretic furosemide 25mg/8h. Abstract

Schizophrenia is an terminology of major psychiatric disorders that characterized by changes in perception, thought, affect, and behaviour. The disorder was on 1% of the adult people and generally at the onset of adolescence or age of consent. Paranoid type schizophrenia is the most stable type and often occurred. Mrs. K, 43 years old, present with chief complain of unreasonable anger since one week ago. The patient locked herself in her room and avoided the people. The patient has not drank the medicines since a month ago. The patient has mental disorders history since 1995, and went to outpatients clinic routinely but did not consume the medicines routinely. The medicines were risperidone, stelazine, and trihexylpenidyl, by consuming those patients felt improvement. General and neurologist examination was normal. Blood pressure was 130/80 mmHg, pulse was 82 /minute, respiratory rate was 20x/minute, and temperature was 36,70 C. The psychiatric status self care was good, compos mentis, calm, the talking was spontaneous, cooperative, mood eutimia, the affect was extensive, appropriate, and cognitif was good. There were auditory and visual halusination, no illusion, no depersonalisation or derealialisation. There were delusion of control, delusions of greatness, delusions of pursue, delusions of mystical magic. The judgement capability was interrupted and insight grade 1. The management of risperidone was 2x2 mg/day and also given supportive psychotherapy. The paranoid type schezophrenia is mental disorder that often occured at the end of adolescence or age of consent. Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a progressive airway obstruction, irreversible and occurs concurrent chronic bronchitis, emphysema or both. The prevalence of COPD in the ratio between men and women is 3 to 1. Predisposing factors of reversible COPD should be sought and treated, which include smoking, hiperesponsif respiratory tract, respiratory tract infection, occupational exposure, pollution, genetic. Mr. M, 47 years old, present with shortness of breath, coughing and weight loss. Patients has history of consumption of anti-tuberculosis drugs, smoking for a quite long time, and also his mother has history of shortness of breath disease. Besides that, the physical examination found bronchial breath sound and wet ronchi additional sound by auscultation. Patients was diagnosed with COPD exacerbation and treated with oxygen therapy, bronchodilators, and antibiotics. COPD is a progressive, reversible and associated with an abnormal inflammatory response of the lungs to noxious particles or gases. Abstract

Background: Tumor is a neoplastic condition, means the local tissue is overgrowth and incoordinated through the stimulus that triggers the growth has ceased. Histopathological examination is the gold standard method to determinate and diagnose tumors. This examination distinguish the degree of malignancy of a tumor, whether leading to a  benign  or  malignant changes. Currently, breast tumor is one of the neoplasm most often found, especially for malignant tumor, and cause high mortality in the world. The aim of study is to find out the histopathological type of the breast tumor.Methods: The method of this descriptive study is cross sectional, which is conducted in the Anatomical Pathology Installastion In Doctor Haji Abdul Moeloek General  Hospital,  Bandar  Lampung.  Data collecting procedure was carried out by analyses to each 75 data of medical records of breast tumor histopathology type during 2010-2012, using total sampling method. Histopathological type obtained  then grouped by the World Health Organization 2003 classifiation.Results: The results of this study indicate that the type of the most breast tumor histopathology on the Anatomical Pathology Installation in Doctor Haji Abdul Moeloek General Hospital, Bandar Lampung, during 2010-2012 is fibromadenoma mammae as the  most  benign  mammary  tumors  of 30  people (40%) and malignant tumor which is 45 people (60%).Conclusion: Fibroadenoma mammae is the most  benign mammary  tumor  and  invasive  ductal carcinoma mammae is the most malignant mammary tumor in Anatomical Pathology Installation  at Doctor Haji Abdul Moeloek General Hospital, Bandar Lampung,  during  2010-2012.  [J  Agromed  Unila 2014; 1(1):1-7] Keywords: breast tumor, descriptive, histopathology

Abstract

Background: Dengue hemorrhagic fever (DHF) is an endemic disease in Southeast Asia, including Indonesia. The use of insecticide induce negative impact, so it required natural ingridients like soursop leaf (Annona muricata). The research was conducted to perceive the effectiveness of soursop leaf extract as larvacide, LC50 and LT50 values.Method: The research design is an experimental, using completely randomized design. The amount of sample used was 480 larvas, which divided into 6 groups of soursop leaf extract concentration, 0% (negative control); 0,25%; 0,5%; 0,75%; 1%; and abate 1% (positive control). Each container contained 20 larvas and was given 200 ml soursop extract solution, repeated for 4 times. Kruskal-wallis test (p<0.05), Post-Hoctest Mann-Whitney (p<0.05) and Probit analized is used to analize LC50 and LT50 value.Result: The research showed that the amount of dead larvae was 28,75% at a concentration of 0,25%; 58,75% at a concentration of 0,5%; 95% at a concentration of 0,75%; and 96,25% at a concentration of 1%. The effective concentration were 0,75% and 1% because the larvacide potency is almost equal as abate 1%. LC50value was 0,989% at at 20th minute; 0,897% at 40th minute; 0,777% at 60th minute; 0,349% at 120th minute; 0,220% at 240th minute; 0,207% at 480th minute; 0,165% at 1.440th minute; 0,110% at 2.880th minute and 0,087% at 4.320th minute. LT50 value was 1528,747 minutes at concentration of 0,5%; 148,688 minutes at a concentration of 0,75% and 106,650 minutes at a concentration of 1%.Conclusion: Soursop leaf (Annona muricata) extract has larvacide effect against instar III Aedes aegypti larvae.
