Abstract

Background: Digital transformation has been crucial to the evolution of healthcare organizations, with most hospitals in Asia having launched smart hospital initiatives to enhance service quality and operational efficiency. However, a significant gap remains, as there is still no established model demonstrating that smart hospital implementation contributes to operational efficiency and patient satisfaction, as described above. Objective: This study aims to examine the implementation of affordable smart hospitals, operational efficiency, and patient satisfaction related to physiotherapy as a healthcare service highly dependent on direct contact and intensive physical therapy sessions. Methods: We performed a systematic literature review (SLR) in accordance with the PRISMA protocol, including articles published between 2020 and 2026. A total of 25 articles were included in the synthesis through narrative analysis, and themes were identified across the literature. Results: Smart hospitals, in combination with hospital information systems, electronic medical records, and telemedicine, have been found to improve operational efficiency. This improvement leads to faster service processes, higher levels of accuracy in data entry by medical professionals, better resource optimization, and ultimately greater patient satisfaction due to reduced waiting times and improved quality of service interactions. Conclusion: Computing technology is a promising mechanism for maintaining process standards across all forms of operational performance, particularly in the domain of patient-centered care services primarily represented in physiotherapy settings. This study supports the use of such technologies to enhance patient comfort and operational robustness.

Abstract

Background: Pesantren is an Islamic educational institution that has existed for a long time; however, in the era of globalization, it must be able to transform in order to survive. Objective: To analyze the practice of learning organization in Pesantren X Islamic Boarding School, Cirebon, and the factors that support the development of organizational learning and its relationship to institutional performance. Method: The study adopted a qualitative case study methodology complemented by quantitative operationalization through the Learning Organization Index (LOI) based on Peter Senge’s five disciplines. Data collection was conducted through 24 semi-structured interviews, 38 observations (187.5 hours), and analyses of 47 institutional documents. Results: The LOI results indicated a score of 3.76 (75.2%), identifying the Pesantren as a developed learning organization. The dimension with the highest score was shared vision (81.7%), whereas the lowest was mental models (55.8%). The Aggregate Performance Index (API) value was 61.6%, with mental models identified as the primary area for improvement. The assessment was highly reliable, with an ICC value of 0.87. Conclusion: The findings from Pesantren X Islamic Boarding School demonstrate a high potential for becoming a learning organization, particularly in the aspects of shared vision and team learning. However, the institution continues to face significant challenges related to transformative mindsets concerning digitalization, gender, and sustainability. The theoretical contributions of this research are situated within the context of Islamic education, while the practical implications are directed toward reforming the management of Indonesian Islamic boarding schools.

Abstract

Background: Diaphragmatic hernia following laparoscopic Roux-en-Y gastric bypass (LRYGB) is a rare but life-threatening complication. Diagnosis is challenging due to non-specific symptoms, and delayed recognition may result in intestinal incarceration, strangulation, ischemia, and respiratory compromise. Computed tomography (CT) is the primary imaging modality for timely diagnosis and surgical planning. Objective: To report a case of incarcerated diaphragmatic hernia following LRYGB, highlighting the CT imaging findings, postoperative radiological follow-up, and the role of multidisciplinary management. Methods: Case report following CARE guidelines. Clinical, laboratory, and radiological data were obtained retrospectively from the medical record of a patient treated at Bali Hospital in April–May 2024. Imaging evaluation included non-contrast abdominal CT, serial chest radiographs, and postoperative thoracoabdominal CT. Results: A 59-year-old woman with a history of LRYGB (2019) developed acute epigastric pain and vomiting during hospitalization. Non-contrast CT revealed extensive herniation of the stomach, jejunum, ileum, mesenteric fat, and part of the spleen through the esophageal hiatus into the right hemithorax, resulting in significant pulmonary compression, leftward mediastinal shift, and obstructive ileus with suspected intestinal ischemia. Emergency laparotomy confirmed these findings, revealing dense adhesions. Postoperatively, CT demonstrated hemopneumothorax and bilateral pleural effusion, which were managed with chest drainage and video-assisted thoracoscopic surgery (VATS). Serial imaging confirmed successful hernia repair with no recurrence. Conclusion: Multidetector CT with multiplanar reconstruction is essential for rapid diagnosis, operative planning, and postoperative monitoring of incarcerated post-LRYGB diaphragmatic hernia. A multidisciplinary radiological–surgical approach is critical for optimal outcomes in this rare emergency.

Abstract

Background: Operation Ketupat is an annual national agenda in Indonesia that mobilizes large numbers of travelers and requires intensive cross-institutional coordination. This study investigates the social dynamics, cultural practices, and institutional mechanisms shaping the implementation of Operation Ketupat 2025 through the perspective of police anthropology. Objective: This study aims to explore how work culture, interagency coordination, and traveler behavior influence the effectiveness of the operation, highlighting the socio-institutional processes that support large-scale traffic management during the 2025 Eid exodus. Methods: A qualitative ethnographic approach was employed, including in-depth interviews and participant observation conducted across six provinces on the island of Java (Banten, DKI Jakarta, West Java, Central Java, the Special Region of Yogyakarta, and East Java) during the 2025 Eid exodus and return period. Results: The findings indicate that operational success is shaped by three interrelated dimensions: (1) cross-sectoral collaboration functions as a coordination ritual that builds shared language and institutional trust; (2) field officers exercise discretionary practices and bricolage, improvising solutions amid resource constraints; and (3) travelers act as rational agents by actively navigating traffic regulations, reconstructing safety norms, and demonstrating adaptive capacity. These dimensions reveal that operational effectiveness relies on social relationships and collective adaptation rather than on technology alone. Conclusion: Operation Ketupat 2025 demonstrates that social relationships embedded in institutional coordination are central to operational success. Cross-sectoral collaboration, officer discretion, and traveler agency form an interconnected socio-institutional system. Policy recommendations include strengthening soft-skills training for field officers, formalizing interagency communication protocols, and implementing structured traveler feedback mechanisms to enhance future large-scale traffic operations.

Abstract

Background: Jakarta's rapid urban expansion has intensified housing demand, leading the government to deploy the Housing and Settlement Information System (SIRUKIM) as a digital tool to enhance transparency and administrative efficiency in subsidized rental flat management. Despite this initiative, notable shortcomings persist in areas such as service responsiveness, openness of information, and the willingness of users to engage with the platform. Objective: This study examines the key determinants of SIRUKIM's operational success by employing an augmented version of the DeLone and McLean Information Systems Success Model, enriched with trust and perceived security as supplementary explanatory variables. Methods: A quantitative research approach was applied, collecting primary data from 396 active SIRUKIM users during April to May 2024. The data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) via SmartPLS. Results: Findings reveal that service quality significantly shapes both system use and user satisfaction, whereas system quality and information quality show no meaningful impact on usage behavior. User satisfaction emerged as the strongest predictor of both system use and net benefits. Trust was found to be a critical antecedent of all three quality constructs, while perceived security did not significantly influence usage. Conclusion: The results affirm that user satisfaction and institutional trust are central to the success of government-run information systems. These insights offer practical guidance for Jakarta policymakers and the SIRUKIM development team, particularly in terms of improving service quality and cultivating trust-enhancing mechanisms to drive broader system adoption.

Abstract

Background: This study explores the impact of mindful learning in English as a foreign language (EFL) among high school students, with a focus on affective engagement. Mindful learning techniques have gained attention for enhancing students’ focus, anxiety management, and overall academic performance; however, their effects in EFL settings have been less studied. Objective: To examine the influence of mindful learning on students’ writing skills and their engagement during the writing process in EFL contexts. Methods: The study employed a convergent parallel mixed-methods design integrated with a quasi-experimental post-test control group design. Two intact classes from a high school in Singaraja, Indonesia, were selected, with one group receiving mindful learning instruction and the other receiving conventional EFL teaching. Data were collected through post-test writing scores, classroom observations, teacher interviews, and student journals. Results: Post-test results, analyzed using an independent-samples t-test, revealed that the experimental group outperformed the control group in writing scores. Qualitative data indicated that students in the experimental group demonstrated better concentration, lower anxiety, and a more positive attitude toward their learning. Conclusion: Mindful learning significantly enhances writing competency and affective engagement among high school EFL students. This study contributes by integrating analysis of writing skills and affective engagement in the application of mindful learning within a high school EFL context.

Abstract

Background: This paper examines how corporate income tax, leverage, exchange rates, company size, profitability, and bonus mechanisms affect transfer pricing decisions. Inflation is included as a moderating variable. This study analyzes the interaction of these factors in the transfer pricing behavior of Indonesian multinational companies. Objective: The aim of this study was to examine the relationships among various financial and macroeconomic determinants affecting transfer pricing, with inflation acting as a moderating factor. Methods: Panel data regression and Moderated Regression Analysis (MRA)-based quantitative frameworks were used. The sample includes 19 multinational companies listed on the Indonesia Stock Exchange between 2017 and 2024, with a total of 152 observations. After specification testing, the random-effects model was selected as the best estimator for this study. Results: This study reveals that corporate income tax schemes and bonus mechanisms have a significant negative effect on transfer pricing. On the other hand, leverage, exchange rates, company size, and profitability do not partially affect transfer pricing. Transfer pricing behavior is largely explained by the independent variables. This research is characterized by the use of inflation as a moderating variable. Furthermore, the moderation analysis shows that inflation is a quasi-moderating factor that weakens the effect of corporate income tax. In contrast, inflation does not weaken the effects of leverage, exchange rates, company size, profitability, or the effectiveness of bonus mechanisms. Conclusion: These findings underscore the importance of fiscal pressures and managerial incentives in determining transfer pricing behavior, even across different macroeconomic environments.

Abstract

Background: Waste management in Indonesia is a complex and ongoing issue affecting environmental, public health, and economic development, as well as the overall quality of life. Objective: This paper examines the issue from a socio-political and philosophical perspective, recognizing that waste is more than a technical issue it is also an ethical issue and one of social responsibility. Methods: This research uses normative qualitative research, combining philosophical analysis with systematic policy evaluation. Results: The waste crisis is interpreted as reflecting three interrelated failures: (1) individual moral failure widespread disregard for collective environmental responsibility; (2) socioeconomic injustice the disproportionate burden of poor waste management falling on marginalized communities; and (3) governance failure characterized by weak institutional coordination, poor policy implementation, and inadequate enforcement. This study contributes a normative ethical framework that bridges political philosophy and public policy analysis in the Indonesian context. Conclusion: These efforts are essential for developing fair, sustainable, and human rights-based waste management systems in the public interest. The novelty of this study lies in its application of a normative political ethics framework drawing on Suseno (2016) to systematically evaluate waste governance as a moral and institutional accountability issue, thereby bridging philosophical analysis with public policy discourse in the Indonesian context.

Abstract

Background: Cystic renal cell carcinoma (CRCC) is a rare renal malignancy. Complete radiologic response of metastatic CRCC with liver metastasis following pazopanib therapy is rarely reported. This case highlights a successful treatment outcome. Objective: To describe the clinical course, diagnostic workup, treatment strategy, and complete radiologic response in a patient with cystic renal cell carcinoma (CRCC) presenting with liver metastasis and treated with pazopanib. Methods: This case report describes a single patient diagnosed with cystic renal cell carcinoma with hepatic metastasis. Clinical, laboratory, imaging, including computed tomography (CT) and magnetic resonance imaging (MRI), and histopathologic data were collected and analyzed. The CARE guidelines for case report writing were followed. Results: A 50-year-old woman presented with a 3-month history of weakness, decreased appetite, weight loss, and persistent anemia (Hb 8 g/dL) that was unresponsive to blood transfusion. Imaging studies revealed a large left cystic renal mass measuring 13.6 × 11.8 × 11 cm, with suspected liver metastasis in segment VIII. Following cytoreductive nephrectomy, histopathologic examination confirmed papillary renal cell carcinoma, Fuhrman grade 2, leading to a diagnosis of cystic renal cell carcinoma with hepatic metastasis. The patient subsequently received pazopanib 400 mg twice daily. After 10 months of treatment, complete radiologic disappearance of the liver metastasis was achieved. Conclusion: This case demonstrates that pazopanib therapy, in combination with cytoreductive nephrectomy, can achieve a complete radiologic response of hepatic metastasis in CRCC. Pazopanib represents a viable and well-tolerated targeted therapy option for metastatic CRCC, warranting further clinical investigation.

Abstract

Background: Acne vulgaris is common in adolescents and young adults and can decrease quality of life. Self-medication with creams containing topical corticosteroids is still prevalent and has the potential to trigger inflammatory acne. Objective: This study assessed the relationship between exposure to steroid-containing creams and the appearance of inflammatory acne. Methods: A cross-sectional observational study was conducted among adolescents and young adults aged 15–41 years with acne complaints, recruited via an online questionnaire using consecutive sampling (n = 110). Topical steroid exposure (yes/no) and inflammatory acne occurrence (yes/no, assessed using reference lesion images) over the past 3 months were the primary independent and outcome variables, respectively. The association was evaluated using the Pearson chi-square test, with prevalence ratio (PR) and 95% confidence interval (CI). Results: Of 110 subjects (mean age, 22.95 years; 54.5% female), 28.2% reported topical steroid exposure and 69.1% had inflammatory acne. The proportion of inflammatory acne was significantly higher in the exposed group (90.3%) than in the unexposed group (55.7%), with a PR of 1.62 (95% CI, 1.28–2.06; p = 0.001). The dominant symptoms in the inflammatory acne subgroup were a burning/heat sensation (56.6%) and pain (50.0%). Conclusion: Topical steroid exposure was significantly associated with inflammatory acne in this population. These findings underscore the need to avoid unsupervised use of steroid-containing anti-acne creams. The cross-sectional design limits causal inference; therefore, prospective studies with clinical confirmation and control of confounding factors are warranted.
