Abstract

Coronary Heart Disease (CHD) is a significant public health problem with high morbidity and mortality. Although CHD cases continue to increase in Indonesia, including in East Kalimantan, there have not been many studies that have identified specific risk factors at Kudungga Sangatta Hospital, East Kutai Regency. This study aims to analyze the risk factors for CHD incidence at Kudungga Sangatta Hospital. This study uses an analytical observational design with a case-control approach. The sample consisted of 60 cases of CHD and 60 non-CHD controls taken from outpatients at the cardiac polyclinic of Kudungga Sangatta Hospital using purposive sampling techniques. Data was collected through questionnaires and medical records. Independent variables include hypertension, family history, and hypercholesterolemia, while dependent variables are CHD incidence. The data collection techniques included a preparation stage (obtaining research permits and conducting a preliminary study), followed by an implementation stage (collecting respondent data, sorting samples according to criteria into two groups, filling out questionnaires, and collecting research instruments). Data analysis was done quantitatively using a case control design with the chi-square test to see the relationship between variables. This study found a significant association with the incidence of dengue in the following variables: hypertension (p < 0.000) OR 10,394, family history (p < 0.000) OR 7.538, and hypercholesterolemia (p < 0.000) OR 10.846. It can be concluded that hypertension is the most dominant risk factor affecting the incidence of coronary heart disease at Kudungga Sangatta Hospital (Exp (β) = 11,311).

Abstract

Obesity is a complex disease that has a significant impact on public health and its prevalence is increasing worldwide. Obesity causes lipid accumulation in adipose tissue. The purpose of this study was to determine the effect of a high-fat diet on lipid profiles in male white rats. This study used a true experimental study on male white rats with a post-test control-only group design. The group division in this study was divided into two groups, consisting of a negative control group (K1) of 5 rats and a HFD group (K2) of 20 rats. This study was conducted from October 2024 to April 2025 at the Biomedical and Biochemistry Laboratory, Faculty of Medicine, Andalas University. The population in this study were male white rats of the Rattus norvegicus strain wistar obtained from the Biomedical Laboratory, Faculty of Medicine, Andalas University. The sampling technique in this study used purposive sampling. The number of samples used in this study was 25 rats with a minimum reserve of 10%. The results of the study with the average values of LDL cholesterol, HDL cholesterol, Total cholesterol, and TG levels in the K1 group were 45.72, 62.28, 67.66, and 107.60 mg/dL, respectively, while in the K2 group they were 61.62, 43.14, 83.17, and 167.20 mg/dL, respectively. Based on the results of the independent T test, the significance of the two groups was 0.001, 0.001, 0.001, and 0.005, respectively, which means there was a significant or real difference in the average levels of LDL, HDL, total cholesterol, and triglycerides in the K1 and K2 groups.

Abstract

Superficial burns often cause acute pain that interferes with patient comfort. One of the non-pharmacological methods used to reduce pain is cold compresses, but scientific evidence regarding their effectiveness is still mixed. To evaluate the effectiveness of cold compresses in reducing pain in patients with superficial burns. This systematic review was conducted by searching articles from the Scopus, PubMed, and ScienceDirect databases using the keywords: "cold compress", "burn injury", "pain management", "superficial burns", "RCT". The inclusion criteria were RCT articles published between 2015–2024, in English, and discussing the effects of cold compresses on pain in superficial burns. Selection was carried out through the PRISMA process. Of the 1,025 articles found, 14 articles met the inclusion criteria. Each article underwent title and abstract screening, followed by full-text assessment for eligibility. Methodological quality was then appraised using the JBI Critical Appraisal Tool, and only studies with moderate to high quality were included in the synthesis to ensure robust and reliable findings. The majority of studies showed that cold compresses significantly reduced the intensity of acute pain in the early phase of superficial burns (p < 0.05). This effect was reported to be effective especially in the first 24 hours after injury, with minimal side effects. Cold compress is an effective, safe, and easy-to-do nonpharmacological intervention to reduce pain in superficial burn patients. This intervention deserves to be part of standard care in emergency departments and primary health care.

Abstract

Guttate psoriasis is the second most common variant of psoriasis found in children and young adults. It is known that several factors such as infections, alcohol consumption, drugs, trauma, acute discontinuation of potent topical or systemic corticosteroids, body mass index, and endocrine disorders are known to be associated with the occurrence and development of guttate tu. This study aims to report a case of varicella with bronchopneumonia in an adult patient. Data were collected through anamnesis, physical examination, laboratory and microbiological investigations, and follow-up assessments, then analyzed qualitatively and presented descriptively as a case report. A 27-year-old man came to our polyclinic with multiple erythematous papules and multiple plaques, milliar-lenticular-nummular in size, discrete, accompanied by the white scale on the scalp, thoracic, vertebral, axillary, right, and left brachial and lower extremities. Two throat swab examinations showed different bacterias and were later found to be induced by periodontitis. This patient was then treated for chronic plaque psoriasis with methotrexate and showed good progress.

Abstract

Exclusive breastfeeding is the most optimal source of nutrition for infants during the first six months of life, yet its coverage in Indonesia remains suboptimal, particularly among primiparous mothers. This study aimed to analyze the effectiveness of exclusive breastfeeding education through a combination of audio-visual media and demonstration compared with leaflet-based education. This study employed a quasi-experimental design with a pretest–posttest control group, involving 48 primiparous mothers as research samples determined using the purposive sampling method based on the Slovin formula with a 10% margin of error, recruited from two midwifery practices in West Bogor District. Respondents were equally divided into intervention (audio-visual + demonstration, n=24) and control groups (leaflet, n=24). Data on knowledge, attitudes, and breastfeeding skills were collected using validated questionnaires and observation checklists, then analyzed with Wilcoxon and Mann–Whitney tests at a 5% significance level. The findings revealed that leaflet education significantly improved knowledge, particularly on basic breastfeeding concepts (p<0.05). However, audio-visual and demonstration methods were more effective in transforming attitudes and enhancing psychomotor skills, especially correct positioning and latching techniques (p<0.001). These results highlight that leaflet is useful for reinforcing cognitive understanding, while audio-visual and demonstration approaches provide more interactive and practical learning that supports behavioral change.

Abstract

Erythroderma, or Exfoliative Dermatitis (ED), is a serious dermatological emergency marked by erythema and scaling over more than 90% of the body surface area. Although etiologies are diverse, drug reactions remain a major cause. Beyond conventional pharmaceuticals such as antibiotics, antivirals, anti-inflammatory agents, cardiac drugs, and chemotherapeutics, herbal remedies have also been linked to hypersensitivity reactions. Sambiloto (Andrographis paniculata), a traditional antidiabetic widely used in Southeast Asia, has been reported to trigger severe cutaneous adverse reactions, including erythroderma. We present the case of a 53-year-old woman with diabetes mellitus who developed generalized erythematous macules, scaling, severe pruritus, fever, chills, and fatigue four days after consuming a Sambiloto decoction. She denied the use of other medications. Clinical findings supported by histopathological analysis confirmed erythroderma. Management involved discontinuation of the suspected herbal, systemic corticosteroids, antihistamines, moisturizers, antibiotics, and glucose control, leading to marked improvement within eight days. This case highlights that drug-induced erythroderma often has an acute onset and rapid course. Despite their reputation for safety, herbal medicines may provoke severe hypersensitivity reactions with potentially life-threatening outcomes. Comprehensive drug history-taking, including traditional remedies, is therefore essential in evaluating patients with exfoliative dermatitis. The objective of this report and journal is to raise awareness of herbal-induced erythroderma and to contribute to the growing body of evidence regarding the risks of traditional therapies. Early recognition and prompt withdrawal of the causative agent remain critical for preventing recurrence and improving patient outcomes.

Abstract

The Family Planning (KB) Safari Program is a government initiative aimed at increasing the use of long-term contraceptives to control population growth and improve family welfare. However, challenges remain, such as social stigma, lack of family support—especially from husbands—and varied public perceptions regarding contraceptive methods. This study aims to evaluate the implementation and effectiveness of the KB Safari Program in Depok City in 2025, as well as to identify supporting factors and obstacles in the program’s execution. This study employs an evaluative approach with a qualitative descriptive method aimed at exploring facts and assessing the success of the Safari Family Planning Program in the Depok City area. Informants were selected using purposive sampling and consisted of three main categories: one key informant, the Head of DP3AP2KB Office; five primary informants including field officers and implementing midwives; and seven additional informants comprising cadres and participants/acceptors of the Safari Family Planning Program. Data collection was carried out through in-depth interviews, participatory observation, and document studies, with thematic analysis using NVivo software. Research ethics were prioritized, including informed consent, data confidentiality, and the participants' right to withdraw during the interview process. The KB Safari Program was found to operate effectively, positively impacting increased community participation in using contraceptives, particularly long-term methods such as IUDs and implants. Success factors included cross-sector collaboration, efficient scheduling, easily accessible service locations, and comprehensive, responsive education tailored to community needs. Major challenges involved negative perceptions, insufficient spousal support, and the need for enhanced human resource capacity. The KB Safari Program in Depok City is effective in reducing birth rates and raising awareness of family planning. However, strengthening is needed through improved training for implementers and optimized socialization efforts targeting husbands and families.
