Abstract

Increasing breast milk supply for breastfeeding mothers aged 0-6 months is a crucial step in supporting optimal infant growth and development. Many mothers experience challenges in producing sufficient breast milk, necessitating interventions that can support successful breastfeeding. Oxytocin massage and reflexology are methods to support breast milk production, each with its own distinct mechanisms. The purpose of this study was to determine the effect of reflexology and oxytocin massage on breast milk supply in breastfeeding mothers. This research method used a pre-experimental design with a one-group pretest-posttest design. Statistical testing used the Wilcoxon test to demonstrate significant changes before and after the intervention. The p-value was used to determine differences. A sample of 40 breastfeeding mothers was divided into two intervention groups. sampling using purposive sampling technique. The results of this study, using the Wilcoxon test, showed a significant change in breast milk sufficiency before the intervention of 4.80 and after the oxytocin massage intervention of 7.56, a difference from the pre-post results of 2.85 with a p-value of 0.000 (p-value <0.005). Meanwhile, the difference between reflexology massage before and after the reflexology massage intervention was 5.55 and 7.85, a difference from the pre-post results of 2.85 with a p-value of 0.000 (p-value <0.005). This indicates that breast milk sufficiency with oxytocin massage and reflexology massage can both increase breast milk sufficiency, but there is no significant difference between the two.

Abstract

Acute Respiratory Tract Infections (ARI), particularly pneumonia, are a leading cause of death in children under five years of age globally and in Indonesia. Vitamin D, in addition to its important role in bone health, has immunomodulatory functions by increasing the production of antimicrobial peptides such as cathelicidin and defensin, and suppressing excessive inflammatory responses. This literature review aims to explore the potential role of vitamin D supplementation in preventing ARI in children, focusing on innate and adaptive immune mechanisms, factors influencing its effectiveness, and current clinical evidence. Literature searches were conducted using PubMed, Scopus, the Cochrane Library, and Google Scholar with the following keywords “acute respiratory tract infection”, “children”, “pediatric”, “pneumonia”, “vitamin D”. The search was limited to publications from 2015–2025. A total of 356 articles were identified, of which 72 were screened based on title and abstract. After full-text eligibility assessment, 6 articles met the inclusion criteria and were included in this review. Articles were critically appraised by examining study design, sample size, intervention (dose and duration of vitamin D), and outcomes measured. Data were synthesized narratively to compare findings across studies. This review shows that vitamin D supplementation, especially in children with low baseline levels (<25 ng/mL), can reduce the incidence and duration of ARI, with a more pronounced protective effect in high-risk groups. Effectiveness is influenced by baseline vitamin D status, dose, duration of administration, age, and sunlight exposure. Clinically, these findings support the recommendation of monitoring vitamin D levels and providing supplementation as a significant preventive strategy to reduce the burden of acute respiratory infections (ARI) in children.

Abstract

The period of child development is important for the development of cognitive, motor, language, socio-emotional skills, religion, art and morals. The success of the educational process during this period is the basis for the next educational process. Child growth and development monitoring can be carried out through the toddler  mothers class program to improve knowledge, attitudes and behaviors of mothers in realizing optimal growth and development of toddlers. In this case, it is important for mothers to have a book on Maternal and Child Health. This study aims to find out the difference in knowledge and attitude of mothers towards the class of mothers under five to stimulate children's growth and development. This type of research is experimental with one group pre test post test design research. The samples in this study were taken in 37 respondent (total sampling) which was carried out to all members of the population as research respondents. The data analysis technique used the Wilcoxon  statistical test.The results of the study conducted by taking pretest and postest  data found that there was a difference in mother's knowledge as evidenced by  a p value = <0.001 and there was a difference in maternal attitudes as evidenced by a p value = <0.001 in the toddler mother's class to stimulate children's growth and development. Advice for mothers to be more routine and active in participating in classes for toddlers in order to achieve optimal growth and development of children.

Abstract

Stroke is one of the main diseases that causes long-term disability and increases the need for palliative care. Nurses play a central role in providing palliative nursing care, especially in improving the quality of life of patients experiencing physical, psychological, social, and spiritual disorders. Exploring the experiences of nurses providing palliative care to stroke patients in the Garuda Ward of S.K. Lerik Regional General Hospital in Kupang City. This study used a qualitative method with a phenomenological approach. Data were obtained through in-depth interviews, using purposive sampling technique with 10 nurses as participants. The participant was selected based on inclusion criteria as follow: nurses who have experiences in palliative nursing care for stroke patients. We identified five main themes as follow: (1) Nurses have a good understanding of palliative care, (2) Patience and empathy as the main keys for nurses, (3) Professional responsibility and initiative of nurses, (4) Collaboration and initiatives in providing palliative care, (5 The need for palliative care competency development. Nevertheless, nurses demonstrated strong commitment and high empathy in providing holistic support for stroke patients.

Abstract

Hand hygiene represents a fundamental infection prevention strategy, particularly critical among adolescent populations who serve as potential vectors for respiratory and gastrointestinal disease transmission. This study aimed to evaluate the efficacy of animated video-based educational interventions on hand hygiene knowledge and attitudinal outcomes among junior high school students. A quasi-experimental pre-test/post-test design without control group was implemented among 109 seventh-grade students at one public Secondary High School (SHS) in Bantul, Indonesia. Convenience sampling was employed across five class sections. Participants received a structured educational intervention utilizing animated video content. Assessment was conducted via validated knowledge and attitude questionnaires administered before and after intervention. Statistical analysis employed the Wilcoxon signed-rank test to evaluate intervention effects. Participants (58.7% female, 41.3% male) demonstrated statistically significant improvement in hand hygiene knowledge scores following the intervention (pre-test mean=9.55±0.687; post-test mean=9.76±0.507; p=0.001). However, attitudinal measures did not show significant change (pre-test mean=30.68±4.86; post-test mean=31.35±4.76; p=0.160). Animated video-based educational interventions demonstrate significant efficacy in enhancing hand hygiene knowledge among secondary school students, though attitudinal changes appear more resistant to short-term intervention.

Abstract

Hemorrhagic stroke with comorbid hypertension is a complex clinical condition with a high economic burden. In the National Health Insurance (JKN) financing system, INA-CBG package rates often do not reflect the actual costs incurred by hospitals. One potential factor contributing to this cost gap is Drug-Related Problems (DRPs), which include incorrect dosing, drug interactions, and inappropriate therapy.Objective to assess the impact of DRPs on the cost gap between INA-CBG tariffs and actual costs of hemorrhagic stroke care with hypertension, as well as to evaluate the effectiveness of the PCNE method in identifying DRPs. Following PRISMA guidelines, a literature search was conducted on PubMed, ScienceDirect, ProQuest, and Google Scholar (2020–2025) using keywords related to hemorrhagic stroke, hypertension, cost-effectiveness, INA-CBGs, and PCNE. Primary studies analyzing DRPs and/or comparing actual costs with INA-CBGs tariffs in hemorrhagic stroke patients were included. The data were synthesized narratively, focusing on types of DRPs, application of PCNE, and cost gaps. Of 195 articles, 11 were included in the synthesis, with 3 highly relevant. DRPs (eg, inappropriate drug selection) were detected in over 60% of cases using PCNE, leading to increased costs. The gap between actual costs and INA-CBGs tariffs reached up to IDR 8 million. DRPs are significant determinants of cost inefficiency in the care of hemorrhagic stroke patients with hypertension. Systematic implementation of the PCNE method integrated with pharmacoeconomic evaluation is recommended to support the sustainability of JKN. This study suggests tariff adjustments for INA-CBGs and an expanded role for pharmacists.

Abstract

Nurse well-being is a critical foundation for ensuring the quality of healthcare services. However, the high prevalence of physical fatigue, occupational stress, and biopsychosocial-spiritual (BPSS) imbalances among nurses indicates an urgent need for strategic interventions rooted in nursing management. This residency project aimed to develop and implement the Healthy Nurse Program as an innovative initiative to holistically enhance nurse well-being at X Hospital. The program adopted Kurt Lewin’s planned change theory, encompassing the stages of unfreezing, changing, and refreezing. Implementation began with problem identification through interviews, observations, and BPSS-based questionnaires, followed by the formulation of a Plan of Action, socialization, and interventions such as workplace stretching, ergonomic education, MBCA monitoring, and spiritual reflection sessions.The study involved 32 newly hired nurses (≤1 year experience) selected through purposive sampling, with data analyzed using a fishbone diagram. Initial evaluation revealed a high prevalence of muscle fatigue, work-related stress, and over-fat conditions among nurses, along with strong support for program implementation. The Healthy Nurse initiative strengthened core nursing management functions and fostered a culture of sustainable workplace wellness. These findings provide a critical foundation for integrating occupational health promotion into hospital systems based on evidence-based practice.

Abstract

Diabetes Mellitus is a group of metabolic diseases characterized by hyperglycemia resulting from abnormalities in insulin secretion, insulin action, or both. Diabetes Mellitus remains a significant health problem and a priority target for global follow-up due to its dangerous impacts and increasing prevalence annually. Complementary therapy, a combination of hydrotherapy, drinking water, and acupressure, is a non-pharmacological therapy that can be administered to control blood sugar levels in diabetes mellitus patients. This combination of therapies has been proven to be more effective in controlling blood glucose levels in diabetes mellitus patients because it can impact the excretion of excess sugar in the body while simultaneously affecting the hypothalamus by activating one of the carbohydrate metabolism enzymes (glucose-6-phosphate). In the pancreas, it increases insulin synthesis, increases receptors on target cells, and accelerates glucose utilization in cells. The purpose of this study was to analyze the effect of the combination of drinking water hydrotherapy and acupressure therapy on blood glucose levels during Type II DM patients in Ilir Village, Gunungsitoli District Health Center Working Area in 2024. This type of research is quasi-experimental with a nonequivalent control group design approach. The study population was all Type 2 DM patients in Ilir Village, Gunungsitoli District Health Center Working Area with a sample size of 32 respondents. The sampling technique was purposive sampling. The sample was selected according to the researcher's wishes with inclusion criteria: age 30-70 years, type 2 diabetes mellitus diagnosed by a doctor, stable drug therapy ≥ 4 weeks (no change in dose/type of insulin), random blood sugar 140-300 mg/dl, good hydration status, adequate kidney function and no fluid restrictions, controlled blood pressure; exclusion criteria: type 1 DM, gestational DM, changes in antihyperglycemic therapy < the last 4 weeks, suffering from kidney failure, heart failure, experiencing acute infections, uncontrolled blood pressure, skin disorders/wounds.   Researchers used repeated ANOVA tests if the data were normally distributed or Friedman Tests if the data were not normally distributed. For comparison of the intervention and control groups, researchers used t-tests if they were normally distributed or Mann Whitney if they were not normally distributed. From the results of the repeated ANOVA test, it was known that the Greenhouse-Geisser significance value was 0.001 (p = <0.05), which means there was a significant difference between blood sugar levels before and after the intervention in the intervention group. From the results of the independent t-test, it is known that the two-sided significant value is 0.001 (p < 0.05), which means there is a significant difference between random blood sugar levels in the intervention group and the control group after intervention 3. The conclusion of this study is that the combination of drinking water hydrotherapy and acupressure has an effect on the KGDS of diabetes mellitus patients.

Abstract

Pregnant women are often filled with anxiety, especially those who are pregnant for the first time, especially before labor. Mothers about to give birth experience excessive emotions, leading to high levels of anxiety, a state where they constantly think about potential negative outcomes. The lack of social support can exacerbate the emotional and physical burden on mothers. Many new mothers feel unprepared regarding infant care, breastfeeding, and infant sleep patterns. This study aimed to examine the relationship between husband’s support and maternal anxiety in facing childbirth. This can lead to feelings of insecurity and stress in their new role as mothers. This study used a quantitative cross-sectional design. This study analyzed the relationship between husband's support and anxiety levels. The population of pregnant women in their third trimester in the Bancak Community Health Center (Puskesmas) working area was 75 respondents in the last three months of January 2025. A sample of 63 respondents was obtained. The researcher used a questionnaire as an instrument. This study utilized univariate and bivariate data analysis. The Rank Spearman statistical test yielded a p-value of 0.000 < α (0.05). Therefore, it can be concluded that there is a relationship between husband's support and the level of anxiety facing childbirth in the Bancak Community Health Center area. There is a relationship between husband's support and the level of anxiety facing childbirth.

Abstract

The suboptimal competency development program for healthcare workers in regional general hospitals affects the fulfillment of professional competency requirements and integration as a superior, international-class referral hospital chosen by the community with excellent and comprehensive services. This study aims to explore the implementation of the healthcare worker competency development program through input, process, and output, as well as identifying problems encountered and solutions to overcome them at the NTB Provincial Hospital. This study uses a Qualitative Descriptive approach using the Donabedian framework (input, process, and output) with in-depth interviews, observation, and document analysis methods. The informants numbered 14 people, determined by purposive sampling, namely selecting several health workers who have experience, strategic positions, or perspectives that are considered capable of providing in-depth insights regarding training needs. Data analysis used the interactive model of Miles and Huberman involving data collection, data reduction, data presentation, and drawing conclusions. This study found that the implementation of healthcare worker competency development has been running normatively, but there are problems with the lack of number and type of competency development and its uneven distribution. The large number of employees, types of health workers and the many varied service units demand a very important need in the development of health worker competencies, problems with the Training Needs Assessment (TNA) have not been carried out properly, implementing personnel or training organizers who have not been trained, lack of infrastructure for certain specific training and limited budgets, and there is no binding policy to implement SPM 20JP. The development of health worker competencies at the NTB Provincial Hospital has not reached an optimal level due to limited budget allocations, the large number of personnel accompanied by the diversity of types of services that require must be a priority agenda because of the importance of equitable and planned competency development to ensure the quality, effectiveness, and sustainability of health services.
