Abstract

Manual femoral sheath removal following percutaneous coronary intervention (PCI) remains a major challenge in interventional cardiac nursing practice. Variations in manual pressure, prolonged compression duration, and nurse fatigue can lead to inconsistent hemostasis and potential peripheral complications. To address these issues, an innovative mechanical compression device Femovenart Mechanical Compression (Femopress) was developed as a locally engineered nursing tool designed to provide consistent, ergonomic, and safe compression after cardiac catheterization. This action research study was conducted at the Cathlab Unit of Dr. Loekmono Hadi General Hospital, Kudus, Central Java, from May to June 2025. The study involved four Cathlab nurses as collaborative practitioners and twenty post-PCI patients with femoral access. Data were collected through direct observation, peripheral safety assessment using six indicators (pulse, skin color, oxygen saturation, hematoma, paresthesia, and pain), and focus group discussions (FGDs). Triangulation was performed with three experts: an academic supervisor, an interventional cardiologist, and a senior Cathlab nurse. Quantitative data were analyzed descriptively, while qualitative data were processed using thematic content analysis. Prior to the innovation, manual compression required 15–20 minutes and often caused nurse fatigue and inconsistent hemostasis. Implementation of Femopress reduced compression time to 13–18 minutes, maintained optimal hemostasis, and improved peripheral safety. Based on the Abukays Peripheral Safety Scale, 90% of patients achieved “safe” scores (9–12), with no secondary bleeding or hematoma. Six FGD themes emerged: (1) manual practice limitations, (2) unavailability of special tools, (3) inadequacy of facilities, (4) need for dedicated compression devices, (5) positive evaluation of Femopress, and (6) expectations for future development. The Femopress innovation effectively improves procedural efficiency, reduces nurse workload, and enhances patient comfort and safety in femoral sheath removal after PCI. This device demonstrates strong potential as a standard nursing technology for Cathlab practice and a model of nursing-led innovation in cardiovascular care.

Abstract

Accurate hemodynamic assessment is critical for guiding fluid therapy and avoiding fluid overload in patients with heart failure (HF). Leg elevation maneuvers, including passive leg raising (PLR) and leg lifting (PLL), have emerged as functional diagnostic interventions. Still, their multifaceted roles across different HF phenotypes have not been fully synthesized. This literature review aimed to evaluate the effect of leg elevation on preload responsiveness and its broader clinical applications in adult patients with HF. A comprehensive search of online studies published between 2019 and 2025 was conducted across multiple databases (Sage Journals, Scopus, ScienceDirect, PubMed, Wiley Online Library, and ProQuest). Keywords included “heart failure”, “passive leg raising”, and “hemodynamics.” A total of 1551 articles were identified, and 5 articles were thoroughly reviewed. Five articles that met the inclusion criteria were systematically analyzed to synthesize evidence on the impact of PLR on key hemodynamic parameters. Leg elevation serves distinct, context-dependent roles. PLR/PLL is a provocative test in patients with suspected HF with preserved ejection fraction (HFpEF), confirming latent diastolic dysfunction with a significant PCWP increase. A blunted increase in the stroke volume index (SVi) during PLR/PLL signifies exhausted preload reserve and is an independent predictor of poor clinical outcomes in patients with HF with reduced ejection fraction (HFrEF). A significant decrease in PPV during the maneuver accurately predicts fluid responsiveness, which is particularly crucial for vulnerable patients, such as those with heart failure, as it remains accurate even with low-tidal-volume ventilation. Leg elevation is a versatile and reliable non-invasive maneuver with a multifaceted role in the management of HF. Beyond simply predicting fluid responsiveness, it functions as a crucial diagnostic tool for HFpEF, a prognostic tool for HFrEF, and an accurate predictor of fluid needs in critical care settings. Integrating PLR/PLL into clinical practice allows for precise, physiology-based decision-making, fluid management optimization, and improved patient outcomes.

Abstract

Effective nursing care documentation is a fundamental component of improving service quality, ensuring patient safety, and supporting professional accountability. This study aimed to evaluate the effectiveness of monitoring and evaluation in enhancing the quality of nursing documentation in an inpatient ward of a hospital in South Jakarta. This pilot study employed a management function framework, with activities conducted through planning, organizing, implementing, and controlling phases. Monitoring and evaluation interventions using digital tools were carried out by head nurses and primary nurses for 10 staff nurses. This study used a total sampling technique involving 10 nurses, with data analyzed through descriptive quantitative and qualitative methods to compare pre- and post-feedback documentation quality and identify factors influencing monitoring and evaluation effectiveness. The results showed an increase of 40.7% in the average documentation score after monitoring-evaluation feedback was provided. Significant improvements were found in the areas of nursing diagnoses and interventions. Structured monitoring and direct feedback were effective in improving documentation consistency and accuracy. Monitoring and evaluation should be institutionalized as a routine program supported by standard operating procedures, continuous training, and supervision to ensure the sustainability of high-quality nursing documentation.

Abstract

Primary nursing is a care delivery model emphasizing continuity and individualized nurse responsibility. Although implemented at X Hospital West Jakarta since 2017 through MPKP, challenges remain regarding role clarity, documentation, and assignment consistency. Objective to optimize the implementation of primary nursing through educational interventions and structured observation using the POSAC managerial approach. This study used a case study and pilot study approach conducted from May 27 to June 30, 2025. he sampling technique used was purposive sampling, involving 21 nurses as participants. Data were collected via interviews, observations, and questionnaires. Problems were prioritized using the USG method and root causes analyzed using the Fishbone diagram (5M+1E), followed by a Plan of Action based on POSAC functions. Interventions such as fixed team assignment, role education via Zoom, bedside handover, nurse identification labeling, and dashboard utilization improved consistency of care. The dashboard scored 75 on the System Usability Scale (SUS), indicating good usability. Optimizing nursing management functions especially planning and control proved effective in enhancing the implementation of primary nursing and improving continuity of care in the inpatient unit.

Abstract

Advances in early detection and treatment have improved breast cancer survival rates; However, survivors often face psychosocial challenges such as Fear of Cancer Recurrence (FCR) and unmet needs. FCR refers to anxiety or worry about cancer returning, while unmet needs represent the gap between the support required and that received—particularly in emotional and informational aspects. Understanding their interplay is vital for developing comprehensive, needs-based nursing interventions. This study aimed to systematically review the relationship between FCR and unmet needs among breast cancer survivors, focusing on prevalence, contributing factors, and their impact on quality of life and health-related behaviors such as screening adherence and healthcare utilization. A systematic search was conducted across PubMed, Scopus, and ProQuest using the keywords “fear of cancer recurrence,” “unmet needs,” and “breast cancer survivors.” Articles written in English with systematic reviews, cross-sectional, cohort, qualitative, or mixed-methods designs were included. From 1,414 identified studies, eight met inclusion criteria and were analyzed qualitatively following PRISMA guidelines. Moderate to high FCR levels were found in 50–70% of survivors, and 60–70% reported at least one unmet need, mainly emotional and informational. Higher FCR levels were strongly associated with increased unmet needs. Unmet needs mediated FCR's impact on health behaviors, leading to lower screening participation and higher unplanned healthcare use. Younger age, comorbidities, and low social support exacerbated both factors. FCR and unmet needs are interrelated psychosocial issues affecting survivors' emotional well-being and behavior. Oncology nursing should integrate structured assessments and tailored interventions to improve survivors' quality of life.

Abstract

Supervision is an active and continuous process aimed at ensuring that employees carry out their duties and responsibilities optimally in accordance with applicable standards, through monitoring, guidance, and performance evaluation activities, both directly and indirectly. At Jakarta Barat x Hospital, the implementation of tiered supervision in the General Outpatient Installation still faces several challenges, such as the suboptimal involvement of head nurses in the supervision process and the lack of standardized evaluation instruments. This study aims to optimize the implementation of tiered supervision by designing a daily work plan for head nurses and developing applicable and measurable supervision tools.  The method used is a case study approach through a pilot study. The study involved 16 nurse respondents, with data collected through questionnaires, interviews, and observations. The data were analyzed qualitatively and descriptively using a fishbone diagram for problem analysis, followed by priority setting, POA formulation with the POSAC approach, and implementation guided by Kotter’s eight-step change theory. The results show that systematic implementation of tiered supervision can enhance compliance with standard operating procedures, improve team communication, and ultimately raise the quality of patient care services. This optimization is expected to serve as a pilot model for strengthening the role of head nurses as frontline supervisors in nursing care units. The optimization of tiered supervision through structured daily work plans for head nurses and the development of measurable supervision tools significantly improves adherence to standard operating procedures, enhances team communication, and strengthens the overall quality of patient care. This approach not only clarifies the managerial role of head nurses as frontline supervisors but also provides a scalable model that can be replicated in other nursing units to ensure consistent, high-quality supervision and service delivery.

Abstract

Adolescent obesity is a significant global health problem that can persist into adulthood and lead to various degenerative diseases such as hypertension, coronary heart disease, and type 2 diabetes mellitus. The prevalence of obesity among adolescents in Indonesia is quite high, with 2018 Basic Health Research (Riskesdas) data showing that 31.0% of 15-year-olds suffer from central obesity. The high rates are often associated with multifactorial causes, including both genetic and external factors such as disproportionate dietary patterns and a lack of physical activity. Objective to analyze the relationship between various lifestyle factors such as diet, physical activity, sleep duration, and screen time with the incidence of obesity in adolescents using the literature review method. The review shows that unhealthy dietary patterns, such as the consumption of junk food and fast food, as well as excess energy intake, have a significant relationship with the incidence of obesity. Additionally, a lack of physical activity is proven to be a dominant risk factor. Concurrently, an increase in screen time is identified as having a strong relationship with obesity, as it reduces time for physical activity. Findings from one of the journals also show that peer influence has a significant correlation with junk food consumption behavior in adolescents, which can ultimately affect their nutritional status. The incidence of obesity in adolescents is a multifactorial problem that is highly influenced by a combination of poor dietary habits, a lack of physical activity, a sedentary lifestyle as measured by screen time, and social factors such as peer influence. Interventions to address obesity must be comprehensive, focusing not only on the individual but also on their social environment.

Abstract

Anxiety is a psychological problem often experienced by students, especially when facing final exams. Anxiety that is not handled properly can interfere with academic performance and mental health. One non-pharmacological approach that can be used to overcome anxiety is butterfly hug therapy. Butterfly hug therapy is a simple bilateral stimulation technique that can be done independently to help reduce anxiety. This study aims to determine the effectiveness of butterfly hug therapy in reducing student anxiety. this study used a quasi experiment with one grup pretest-posttest design. Sampling in this study used purposive sampling with the following criteria: willingness to be a respondent, being a sixth-semester final-year student who had not yet taken their final exams, and having mild to moderate anxiety score as screened using the Hamilton Anxiety Rating Scale (HARS). The sample obtained after screening consisted of 22 students experiencing mild to moderate anxiety. Anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS) before and after six intervention sessions. HARS has a Cronbach's Alpha reliability coefficient of 0.86, indicating an excellent level of internal consistency. In addition, content validity testing has been conducted by nursing and psychology experts, showing that each item is relevant for measuring the dimension of anxiety. The paired t-test analysis showed that butterfly hug therapy was proven to be significantly effective in reducing anxiety (p 0.000 (<0.05)). butterfly hug therapy can be used as a practical and effective non-pharmacological intervention in managing student anxiety and can be integrated into mental health promotion in higher education institutions.

Abstract

The demand for palliative care in Indonesia continues to rise as more patients live with chronic and life-limiting conditions. Yet, discharge planning—which should serve as a bridge between hospital care and home care remains limited, particularly when it is not integrated with Advance Care Planning (ACP). This study aimed to design an ACP-based discharge planning model that supports smoother transitions and strengthens continuity of care for palliative patients after hospital discharge.  A descriptive design with a developmental action research approach was carried out at the Hospital. Thirty nurses participated through surveys, complemented by in-depth interviews with the head nurse, nursing committee, and nursing division, as well as direct observation of clinical practice. This study used a purposive sampling technique, with data collected through observations, in-depth interviews, and structured surveys, and analyzed using thematic analysis for qualitative data and descriptive analysis for quantitative data to explore nurses’ knowledge, attitudes, and readiness in implementing ACP-based discharge planning. Data were analyzed descriptively and further explored using SWOT analysis and a fishbone diagram to map strengths, weaknesses, opportunities, threats, and root causes. Findings showed that most nurses understood the concept of ACP (80%) and expressed readiness to be involved, although the majority (73.3%) had never received formal training. Key barriers included limited empathic communication skills, the absence of standard operating procedures (SOPs), and the lack of standardized ACP forms. As an initial step, the study produced draft SOPs, technical guidelines, and ACP forms, which were considered useful but still require refinement and pilot testing. Overall, ACP-based discharge planning has strong potential to improve continuity of care and enhance the quality of life for palliative patients at home. Its success, however, will depend heavily on sustained managerial support, continuous training for nurses, and the active involvement of patients and their families in the care process.

Abstract

Hypertension is a major noncommunicable disease that requires effective nonpharmacological interventions, where its management depends not only on pharmacological therapy but also on regular physical exercise. The combination of hypertension exercises, brisk walking, and handgrip exercises has the potential to lower blood pressure through increased cardiorespiratory capacity and vascular vasodilation. This literature review aims to analyze the effectiveness of these interventions, both individually and in combination, and to develop evidence-based recommendations for the management of hypertension. This literature review was conducted to analyze the effectiveness of physical exercise interventions, namely hypertension exercises, brisk walking, and handgrip exercises, on blood pressure reduction and to develop evidence-based recommendations for hypertension management. The study was conducted systematically following the PRISMA guidelines, with articles searched in four electronic databases (PubMed, Scopus, ScienceDirect, Google Scholar) for the period 2020–2025 using keywords related to hypertension, hypertension exercises, brisk walking, handgrip exercises, and randomized controlled trials. Of the 520 articles found, 35 studies met the inclusion criteria (experimental, quasi-experimental, RCT) and were reviewed narratively by category based on exercise type, duration, intensity, and blood pressure outcome. The results of the study were reviewed using a narrative analysis method, which included a thematic synthesis of findings or main categorization based on study variables, referring to PICOS to ensure that the review was targeted at the research objectives. The results of the review show that hypertension exercise, brisk walking, and handgrip exercise interventions, either alone or in combination, are effective in lowering blood pressure with varying durations and intensities depending on the population. The main categories include: 1) Effectiveness of Combination vs. Single: Combined exercises (such as brisk walking and handgrip) are more effective at lowering blood pressure and improving fitness than single exercises. 2) Biological Mechanisms: Blood pressure reduction occurs through improved endothelial function, vasodilation, and sympathetic-parasympathetic nervous system regulation as a result of regular physical exercise. 3) Adherence & Structured Programs: Adherence increases with combination programs and education, resulting in more optimal BP reduction in patients who are consistent and educated. 4) Psychosocial Impact: Combination physical exercise improves quality of life, reduces depression and anxiety, and brings psychosocial benefits to hypertensive patients. Hypertension exercises, brisk walking, and handgrip exercises are effective nonpharmacological interventions for lowering blood pressure in patients with hypertension, with the combination providing more optimal synergistic benefits for long-term compliance.
