Abstract

Preoperative fasting is a standard applied in surgical services to prevent aspiration, but in hospital practice, fasting often occurs that exceeds the recommendations of the American Society of Anesthesiologists (ASA) which recommends a minimum fast of 6-8 hours, the negative impact of extended fasting will affect the hemodynamic and metabolic status of patients. This study aims to analyze the impact of variations in fasting duration with hemodynamic status and blood sugar levels in patients. This study used a quantitative design with a cross-sectional approach with a sample of 252 elective surgical patients collected by purposive sampling. Data collected included demographic characteristics, ASA physical status I-II, fasting duration, and preoperative clinical parameters. with inclusion criteria: respondents aged 18-55 years, ASA physical status I-II, undergoing elective surgery while exclusion criteria were patients with metabolic disorders such as diabetes mellitus or unstable cardiovascular disorders and cytotoxic patients. Data were collected using observation sheets related to demographics, physical status, type of surgery, last meal, and post-operative complaints, as well as a Non-Invasive Blood Pressure Monitor (NIBP) to measure hemodynamics, a pulse oximeter for oxygen saturation, and a glucometer for glucose levels. The results of the one way anova test showed that variations in fasting duration did not affect hemodynamics (p > 0.05), but did affect blood sugar levels where the value (p < 0.05). It was found that these findings confirm that pre-operative fasting beyond the recommendation can affect the patient's metabolic status, so that fasting duration management according to ASA standards is important to maintain the patient's metabolic and physiological stability.

Abstract

Stunting remains a major chronic nutritional problem among children under five in developing countries, including Indonesia. Low maternal knowledge and limited ability to monitor child nutrition are key contributing factors. Nutritional education interventions play an essential role in improving nutrition literacy, self-monitoring capacity, and stunting prevention practices among mothers of young children. This literature review aims to analyze the impact of nutritional education interventions on enhancing maternal knowledge and self-monitoring skills, as well as their effectiveness in preventing stunting among children under five, based on quasi-experimental and randomized controlled trial (RCT) studies. The review process followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Articles were retrieved from PubMed, ScienceDirect, Google Scholar, and Garuda (Garba Rujukan Digital) databases using the keywords: nutrition education, maternal knowledge, self-monitoring, stunting prevention, and children under five. The publication range included studies from 2020 to 2025. Out of 312 initially identified articles, duplicates were removed, and eligibility was assessed based on inclusion and exclusion criteria, resulting in 12 studies selected for in-depth analysis. Methodological quality was appraised using the Joanna Briggs Institute (JBI) Critical Appraisal Tools. Inclusion criteria comprised: (1) quasi-experimental or RCT study design; (2) population of mothers with children under five; (3) interventions involving nutrition education, health promotion, or maternal empowerment programs related to child nutrition; (4) open-access full-text availability; and (5) publications in English or Indonesian. Exclusion criteria included review articles, case reports, descriptive studies without intervention, populations other than mothers, studies not assessing outcomes related to knowledge, behavior, or child nutritional status, and articles lacking peer review or full-text access. Analysis of the 12 selected studies revealed that nutritional education interventions consistently improved maternal knowledge, feeding behavior, and self-monitoring practices while reducing the prevalence of stunting. Interventions combining education with maternal empowerment and community-based mentoring demonstrated greater effectiveness compared to education-only approaches. Overall, nutritional education interventions are effective in improving maternal knowledge and self-monitoring skills, contributing to stunting prevention among children under five. Continuous, community-based education integrated into primary health services is recommended to strengthen maternal roles and enhance the effectiveness of stunting prevention programs.

Abstract

Hypertension among elderly may result in a number of complications if left untreated. Objective to determine the effect of giving MU hypertension herbal drink and 30 minutes of walking to reduce blood pressure. Quasi-experimental with two groups pre and post test. The study sample was 60 elderly (intervention= 30; control= 30), which was selected using purposive sampling technique. The intervention group was given MU hypertension tea and walked for 30 minutes, while the control group was only given MU hypertension tea, for 7 days each. Data was collected in April-May 2025, then analysed by Wilcoxon and Mann Whitney test (≤0.05). The intervention group experience decrease in average systolic and diastolic blood pressure of 43.87 and 30.36 mmHg. The control group experience decrease in mean systolic and diastolic blood pressure of 40.8 and 26.16 mmHg. Differences in systolic blood pressure before and after treatment in both groups (p = 0.000; p = 0.000) is found. Differences in post-test systolic and diastolic blood pressure between two groups (p = 0.000; p = 0.000) is found. MU hypertension tea and 30 minutes of walking have been proven to be effective to lowering blood pressure in the elderly.

Abstract

Metastatic Bone Disease (MBD) is one of the most aggressive forms of tumor spread, reshaping the tumor microenvironment into a hypoxic and acidic state that enhances lactate dehydrogenase (LDH) activity. Primary lung and breast tumors possess distinct predictive biomarkers, EGFR and HER2, which drive the Warburg effect and further elevate LDH levels. MBD often leads to skeletal-related events that impair quality of life and require prompt, comprehensive management. This study aimed to examine the the association between LDH levels and EGFR/HER2 expression, and evaluate LDH’s potential as an early, efficient indicator for clinical decision-making in MBD patients. Method: A cross-sectional study was conducted through medical record reviews at Dr. Moewardi General Hospital, Surakarta. Samples included patients with primary lung and breast tumors diagnosed with MBD from 2023–2024, selected using total sampling. Serum LDH, EGFR, and HER2 data were then retrieved from the electronic medical record. Associations between LDH and EGFR/HER2 were analyzed using binary logistic regression. LDH showed a positive but statistically non-significant association with EGFR mutation status in lung cancer patients with MBD (OR=0.999, 95% CI=0.994–1.004, p=0.713) and with HER2 status in breast cancer patients with MBD (OR=1.750, 95% CI=0.084–36.287, p=0.718). LDH was positively associated with EGFR and HER2 expression in MBD patients, though the associations were not statistically significant.

Abstract

Patient safety is a fundamental aspect of nursing practice that must be instilled from the time of education. However, many nursing students do not have adequate competence in applying the principles of patient safety during clinical practice. A Simulation Training-based Clinical Base Learning Model (CBL-ST) was developed to answer these challenges. This study uses a mixed methods approach with a sequential explanatory design. The first stage was carried out qualitative exploration through in-depth interviews with 10 clinical supervisors to prepare the CBL-ST intervention module. The second stage used a quasi experiment design. Sample with purposive sampling with the criteria of a student with a minimum of semester 5, have participated in clinical practice and are willing to be respondents as  80 nursing students, divided into intervention and control groups. Patient safety competency was measured using the Patient Safety Attitude (PSA) instrument, analyzed by the t-test. Results showed a significant improvement in scores in the intervention group (pretest 68.5; posttest 85.3; p < 0.05) compared to the control group (pretest 67.9; posttest 70.1; p > 0.05). The five key themes of the qualitative outcomes include: patient-centered service, patient safety goals, teamwork, effective communication, and safety culture. The CBL-ST model is proven to not only improve cognitive and technical aspects, but also shape professional values and student safety culture. The CBL-ST model is effective in significantly improving the safety competence of nursing student patients. The application of case-based simulation learning can be a systematic clinical education strategy that has a long-term impact on the quality of health services.

Abstract

Diabetes mellitus is a chronic metabolic disease characterized by elevated blood glucose levels due to beta cell dysfunction in the pancreas, which prevents the effective production and/or utilization of insulin. Various factors contribute to the increase in the prevalence of diabetes in 2021, with 12.1% of cases occurring in urban areas compared to 8.3% in rural areas. Antidiabetic drugs such as metformin are widely used and have gastrointestinal side effects. Using natural alternatives can provide safe effects. Basil leaves (Ocimum basilicum) and red ginger (Zingiber officinale) are active ingredients that can be used as herbal medicines that are easily found anywhere and have long been known in various traditional medicines for diseases. Usually, people consume them as vegetables and herbal medicine. Basil leaves and red ginger contain natural compounds that are beneficial to health and can boost the immune system, including as antidiabetic agents. The purpose of this review is to present comprehensive information on basil leaves and red ginger that have antidiabetic activity, conducted using a scoping review starting with the PRISMA guidelines. The information for this article was obtained using Google Scholar, Science Direct, Semantic Scholar, and PubMed from 2015 to 2025 resulting in 1229 articles, The results of this review were obtained from 8 articles that were used and met the inclusion criteria. Basil leaves contain flavonoids, phenols, and tannins, which play a role in lowering blood glucose levels by increasing insulin sensitivity and antioxidant activity. Red ginger is rich in gingerol and shogaol, which can lower blood glucose levels and reduce oxidative stress in the body. Both plants have significant potential as natural antidiabetics in lowering blood glucose levels through various mechanisms.

Abstract

Nausea and vomiting that occurs in early pregnancy is a common occurrence, it is due to an increase in the hormones estrogen, progesterone and the release of the hormone HCG (humanchorionic gonadothropin). Nausea and vomiting occur between 4 – 8 weeks and will continue for 14 -16 weeks. If this is not handled properly, it will harm the mother and fetus, because of a decrease in appetite resulting in a lack of electrolyte balance such as sodium, calcium, and potassium. The purpose of this study is to determine the effectiveness of jederasa powder drink in reducing nausea and vomiting in pregnant women at 12-16 weeks in the working area of UPTD Tanjung Baru Health Center, Ogan Komering Ulu Regency. This research employed a quantitative approach in the form of a pre-experimental study with a one-group pretest-posttest design. One group underwent pre- and post-treatment testing. The study focused on pregnant women experiencing emesis gravidarum symptoms. Only one group received treatment, and the results were then observed. Sampling was conducted using a purposive sampling technique based on previously identified population characteristics. Thirty-three respondents experienced nausea and vomiting. The study was conducted for 14 days, from September 27 to October 10, 2024. Data analysis used a paired sample t-test. The administration of jederasa powder drink showed the results of the frequency of nausea was known, the value of the "Paired Samples Test" showed a value of "Mean Paired Differences" of 3,303. This value shows the difference between the average Pre Test results and the average Post Test results, which is 9.45 –6.15 = 3,303. It is known that the value of Sig. (2-Tailed) is 0.000 <0.05. The results showed that there was an average difference between the results of the pretest and the posttest where there was an effect of giving jederasa powder drink in reducing the frequency of nausea in pregnant women at 12-16 weeks. Meanwhile, the results of the "Paired Samples Test" showed a "Mean Paired Differences" value of 3.818. This value shows the difference between the average pre-test results and the average post-test results, which is 7.03-3.21 = 3.818. It is known that the value of Sig. (2-Tailed) is 0.000 < 0.05. So it can be concluded that there is an average difference between the results of the pre test and the post test. Which means that there is an effect of giving a powdered drink. Jederasa powdered drinks are effective in reducing the frequency of nausea in pregnant women at the Tanjung Baru Health Center UPTD by 76%, reducing vomiting by 64% and jederasa powdered drinks can be accepted by more than 70% of pregnant women.

Abstract

Hypertension is a common non-communicable condition with inadequate drug adherence.  Noncompliance with antihypertensive treatment can lead to stroke, heart failure, and kidney problems.  Health education through the HEART (Hypertension Education and Resource Toolkit) animated movie being tested to improve medication adherence in hypertension patients at Sebelas Maret University Hospital (UNS) Solo.  The study was quasi-experimental with a single group pretest–posttest.  Purposive sampling yielded 94 individuals. The Wilcoxon Signed Rank test was used to analyze the results before and after the intervention  using the MMAS-8 questionnaire with Cronbach's Alpha validity test. The Morisky Medication Adherence Scale 8 (MMAS-8)  was 0.83, indicating high internal consistency. Reliability was tested using the test-retest reliability method with a Cronbach's Alpha coefficient of 0.898.  Education significantly increased adherence (p = 0.001), with 96.8% of patients non-adherent before the intervention and 91.5% moderately and 3.2% totally adherent afterward.  These findings demonstrate that audiovisual educational media improves patient comprehension and adherence.  In conclusion, UNS Solo Hospital hypertension patients' medication adherence has improved using the HEART animated movie.

Abstract

Online registration systems have emerged as digital innovations to improve the efficiency of outpatient services, which often experience long queues and extended waiting times that negatively affect patient satisfaction. By enabling remote appointment scheduling, these systems aim to streamline administrative processes and enhance healthcare delivery. This scoping review followed the PRISMA 2020 guideline to identify studies published between 2015 and 2025 examining the implementation of online or electronic registration systems in hospital outpatient settings. Searches were conducted across PubMed, Google Scholar, and EbscoHost, including English and Indonesian articles. A total of 2,148 abstracts were screened, and data were extracted on study design, system type, outcomes, and key findings related to waiting time and patient satisfaction. Ultimately, six studies were included. Each study was independently reviewed by two reviewers, and in cases of disagreement, a third reviewer was consulted to reach a consensus. Six studies from Indonesia and India met inclusion criteria. Most used quantitative designs, while one employed a qualitative approach. All studies reported significant reductions in waiting time (up to 75%) and improvements in patient satisfaction, driven by ease of access, reduced congestion, and system responsiveness. Integration with hospital information systems enhanced adoption, although barriers such as low digital literacy and technical limitations persisted. Online outpatient registration systems effectively reduce waiting times and improve satisfaction, supporting their integration into hospital digital transformation strategies.

Abstract

Hypertension is one of the major health problems in Indonesia that contributes significantly to increased morbidity and mortality due to cardiovascular disease. Physical activity has been proven effective in controlling blood pressure, but the level of compliance of hypertensive recipients with physical activity is still low. One of the psychological factors that plays a role is self-efficacy, which is an individual's belief in their ability to perform a behavior. This study aims to determine the connection between self-efficacy and the ability to perform physical activity in hypertensive patients. This study used a correlational design on 108 hypertensive patients selected through purposive sampling. The instruments used were the Exercise Self-Efficacy Scale (ESES) and the International Physical Activity-Short Form (IPAQ-SF) Indonesian version, which have good validity and reliability (Cronbach’s Alpha ESES = 0.78–0.92; ICC = 0.91; Cronbach’s Alpha IPAQ-SF = 0.884; S-CVI = 0.94). The data were analyzed using the Chi-Square test. The outcomes showed a significant connection among self-efficacy and the ability to perform physical activity (p < 0.05). These outcomes indicate that participants with high self-efficacy tend to perform moderate to high levels of physical activity contrasted to participants with low self-efficacy.
