Abstract

Integrated Primary Care (IPC) is one of the strategic initiatives of Indonesia’s Primary Healthcare Transformation Program aimed at strengthening primary healthcare services through integrated, comprehensive, and life-cycle-oriented care. The implementation of IPC is expected to improve service quality and ultimately enhance patient satisfaction. However, empirical evidence regarding the relationship between IPC implementation, service quality, and patient satisfaction remains limited, particularly within community health center settings. This study aimed to analyze the influence of Integrated Primary Care implementation on patient satisfaction and to examine the mediating role of service quality at Talango Community Health Center, Sumenep Regency. A quantitative study with a cross-sectional design was conducted among 125 healthcare service users at Talango Community Health Center. Respondents were selected using accidental sampling techniques. Data were collected through structured questionnaires measuring IPC implementation, service quality, and patient satisfaction. Data analysis was performed using Structural Equation Modeling–Partial Least Squares (SEM-PLS) with SmartPLS version 4. The findings revealed that IPC implementation had a positive and significant effect on service quality (β = 0.415; p < 0.001). Service quality demonstrated a positive but statistically insignificant effect on patient satisfaction (β = 0.168; p = 0.087). Furthermore, IPC implementation did not significantly influence patient satisfaction (β = −0.085; p = 0.432). Mediation analysis indicated that service quality failed to significantly mediate the relationship between IPC implementation and patient satisfaction (β = 0.070; p = 0.127). IPC implementation significantly improved perceived service quality but did not significantly increase patient satisfaction, either directly or indirectly through service quality mediation. These findings suggest that patient satisfaction is influenced by broader factors beyond service quality, including patient experience, healthcare accessibility, communication effectiveness, and facility comfort. Strengthening patient-centered healthcare approaches may be essential to maximize the benefits of IPC implementation.

Abstract

Stage 5 chronic kidney disease (CKD) is a progressive condition requiring long-term renal replacement therapy; haemodialysis imposes a complex physical, psychological, social, and existential burden on patients. Although the impact of haemodialysis on patients’ lives has been extensively studied, the integration of psychosocial adaptation and self-transformation toward finding meaning in life within a comprehensive theoretical framework remains largely unexplored. To map and synthesize scientific evidence regarding the dynamics of psychosocial adaptation and life meaning among stage 5 chronic kidney disease (CKD) patients undergoing haemodialysis, and to develop an integrative conceptual framework based on the Roy Adaptation Model (RAM) and Self-Transcendence Theory (STT). This study employed a scoping review design in accordance with the Joanna Briggs Institute (JBI) guidelines and was reported in accordance with PRISMA-ScR. A literature search was conducted via Scopus, PubMed, and Google Scholar using the keywords “chronic kidney disease”, “end-stage renal disease”, “haemodialysis”, “adaptation”, “coping”, “psychosocial”, “spirituality”, “self-transcendence”, and “qualitative” for the period January 2022–May 2026, which initially identified 330 articles, of which nine qualitative articles ultimately met the inclusion criteria for analysis using thematic synthesis. The results of the thematic synthesis identified four main themes, namely: (1) psychosocial burden as a stimulus for adaptation, (2) the dynamics of psychosocial adaptation and the reconstruction of life meaning, (3) spirituality, hope, and coping strategies in the search for life meaning, and (4) social support as a factor that strengthens adaptation and the search for life meaning. Haemodialysis patients exhibit complex and multidimensional psychosocial adaptation dynamics, with the RAM explaining adaptive response processes, while the STT describes self-transformation toward life meaning and resilience. These findings underscore the importance of a holistic nursing approach and support a paradigm shift from disease-centered care toward meaning-centered care.

Abstract

Hypertension is a medical condition in which blood pressure increases above 140/80 mmHg. If it is not controlled, it can have negative effects on health and may develop into a chronic disease. Hypertension control depends on tertiary prevention behavior. This study used the Information Motivation Behavioral Skills construct to analyze tertiary prevention behavior among patients with hypertension. This study used a cross-sectional design conducted in Boyolali Regency. The sampling technique used stratified random sampling. Five community health centers were selected, with a total sample of 200 respondents. Data were collected using a questionnaire that had been tested for validity and reliability. Multivariate data analysis was performed using path analysis. The results of the study showed that behavioral skills had a direct association with tertiary prevention behavior among patients with hypertension (b = 0.40; 95% CI = 0.28–0.51; p < 0.001). Motivation was also directly and significantly associated with tertiary prevention behavior (b = 0.26; 95% CI = 0.14–0.38; p < 0.001). Information had an indirect association with tertiary prevention behavior through behavioral skills (b = 0.17; CI 95% = 0.06–0.28; p = 0.003), while motivation also had an indirect association with tertiary prevention behavior through behavioral skills (b = 0.40; 95% CI = 0.29–0.51; p < 0.001). In addition, information had a direct association with tertiary prevention behavior (b = 0.17; 95% CI = 0.06–0.28; p = 0.006). The path model demonstrated a good fit to the data (CFI = 1.00; TLI = 1.00; RMSEA = 0.00; SRMR = 0.00). behavioral skills and motivation influence tertiary prevention behavior among patients with hypertension. Information has a direct effect on tertiary prevention behavior, but it has an indirect effect through behavioral skills. Behavioral skills also mediate the effect of motivation on tertiary prevention behavior.

Abstract

Puskesmas Bungus recorded high coverage for K1 (94.3%) and K4 (92.4%) visits; however, which dimensions of antenatal care (ANC) quality most influence pregnant women's satisfaction remain unclear. This study aimed to analyse pregnant women's satisfaction with ANC quality based on expectations versus actual experience across the SERVQUAL and SERPERF dimensions, and to identify the dimension most strongly influencing satisfaction at Puskesmas Bungus. A mixed-methods sequential explanatory design was employed. The quantitative sample comprised 113 pregnant women selected via accidental sampling, analysed using chi-square testing, multiple logistic regression, and Importance-Performance Analysis. Qualitative data were obtained from six informants through in-depth interviews and analysed using structured thematic analysis. Bivariate testing showed all five SERVQUAL dimensions were significantly associated with satisfaction (p<0.05). Gap analysis revealed tangibles as the only dimension with a negative gap (−0.282). Multivariate analysis identified tangibles as the most dominant predictor of satisfaction (OR=125.434, p=0.002). Qualitative findings confirmed constraints including prolonged waiting times (1–2 hours), limited physical facilities, inadequate privacy, and insufficient communication during the standard 10T examination. All SERVQUAL dimensions were significantly associated with satisfaction, with tangibles being the most dominant factor and the only unmet expectation. Strengthening physical facilities and queue efficiency should be prioritised.

Abstract

Low breast milk production in the first postpartum week may compromise infant nutrition and increase maternal psychological distress. Anxiety and stress can reduce prolactin levels, thereby affecting lactation. Objective: To determine the effect of combining Oketani massage and jasmine aromatherapy on prolactin hormone levels and anxiety in primiparous postpartum mothers. A quasi-experimental study with a pretest–posttest control group design was conducted at Klinik Mutiara Medika in 2023. Thirty-two primiparous postpartum mothers were selected through purposive sampling and assigned to intervention (n=16) and control (n=16) groups. Prolactin levels were measured using the ELFA method, while anxiety was assessed using the Postpartum Specific Anxiety Scale (PSAS), a validated and reliable instrument widely used to measure postpartum-specific anxiety. Data were analyzed using Paired Sample T-Test, Independent T-Test, and Mann–Whitney test. The intervention significantly increased prolactin levels and reduced anxiety (p<0.001). Post-treatment prolactin levels were higher in the intervention group (266.26 ng/mL) than in the control group (170.02 ng/mL). The intervention group showed an average prolactin increase of 111.55 ng/mL and an average anxiety reduction of 34.32 points. The combination of Oketani massage and jasmine aromatherapy is an effective non-pharmacological intervention for increasing prolactin levels and reducing anxiety among primiparous postpartum mothers.

Abstract

Chronic rhinosinusitis (CRS) is a common inflammatory disease of the sinonasal mucosa with significant morbidity. Laryngopharyngeal reflux (LPR) has been suggested as a contributing factor in CRS pathogenesis through direct mucosal irritation and neurogenic inflammation.  This study aims to determine the relationship between laryngopharyngeal reflux and chronic rhinosinusitis. This study was an analytic observational study with a cross-sectional design. The subjects were adult patients presenting with nasal symptoms at Haji Adam Malik General Hospital and Prof. Dr. Chairuddin P. Lubis Hospital, Universitas Sumatera Utara. Chronic rhinosinusitis (CRS) was diagnosed based on clinical symptoms and nasoendoscopic findings according to the European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) 2020 criteria. Laryngopharyngeal reflux (LPR) was assessed using the Reflux Symptom Index (RSI) and Reflux Finding Score (RFS). A total of 44 patients who met the inclusion and exclusion criteria were enrolled in this study using a non-probability consecutive sampling technique and provided written informed consent to participate. Data were analyzed using the Chi-square test, with statistical significance set at p < 0.05. A total of 44 subjects meeting the inclusion criteria were analyzed. The prevalence of CRS was 61.4% and LPR was 50%. Patients with LPR showed a higher proportion of CRS compared to those without LPR. However, statistical analysis demonstrated no significant association between LPR and CRS (p = 0.353). There is no statistically significant relationship between laryngopharyngeal reflux and chronic rhinosinusitis. However, LPR may still play a role as a contributing factor in CRS based on descriptive findings.

Abstract

Stunting remains a serious health issue, including in the city of Surakarta. This problem requires comprehensive efforts, one of which is through preconception interventions targeted at prospective brides and grooms as a preventive measure to tackle chronic malnutrition from an early stage. However, individual background characteristics influence health knowledge and literacy levels. This study aims to identify individual characteristics—including age, educational level and employment status—that are associated with the level of knowledge among prospective brides regarding the prevention of stunting. A quantitative study using a cross-sectional approach was conducted in April–May 2025 at the Office of Religious Affairs (KUA) in Banjarsari Sub-district, Surakarta City, with a sample size of 102 respondents selected using total sampling and who met the inclusion criteria. Data analysis was performed using univariate and bivariate analyses with the chi-square test at a 95% significance level. A total of 51% of respondents had a high level of knowledge, whilst 49% had a low level of knowledge. The majority of respondents did not yet understand clinical aspects such as the characteristics of haemophilia and external risk factors relating to the mother. The results of the bivariate analysis indicated a significant association between educational level and knowledge of stunting (p = 0.044). Educational attainment is a key factor underpinning the cognitive abilities and health literacy of prospective brides in relation to stunting. There is a need for pre-marital education programmes to optimise conceptual and analytical skills, with a particular focus on groups with low educational attainment, such as Sultanikah Capingan.

Abstract

Long-term treatment for pulmonary tuberculosis often leads to non-adherence due to low self-management and limited involvement in self-help group. health belief model (HBM)-based Tele-support intervention has the potential to improve patients' health behaviors by strengthening perceptions and self-efficacy. Objective to analyze the effect of HBM-based tele-support on improving self-management and self-help group among pulmonary TB patients. This quasi-experimental study used a pretest-posttest control group design involving 70 respondents (35 each in the intervention and control group). The independent variable was HBM-based tele-support, while the dependent variables were self-management and self-help group. Data were collected using validated Self-Management and Self-Help Group questionnaires. The Self-Management questionnaire demonstrated good construct validity based on Confirmatory Factor Analysis (factor loadings >0.50) and excellent internal consistency (Cronbach’s alpha = 0.91; subscale reliability = 0.80–0.85). The Self-Help Group questionnaire also showed good construct validity (factor loadings >0.60) with excellent internal consistency (Cronbach’s alpha = 0.88–0.92) and analyzed using the Wilcoxon and Mann-Whitney tests. The intervention group showed significant improvements in self-management (P= 0.003) and self-help group (P= 0.025) post-intervention, whereas the control group showed no significant changes (P> 0.05). Inter-group analysis revealed a significant difference in self-management (P= 0.003), but no significant difference in self-help group (P= 0.063). HBM-based Tele-support is effective in improving self-management in pulmonary TB patients, but has not yet shown optimal results for self-help group. This intervention is recommended for implementation in outpatient services as a medium for education and remote monitoring.

Abstract

Undernutrition among children under five remains a nutritional problem that may impair growth and development. Supplementary feeding based on local food resources is one strategy to improve nutrient intake and nutritional status. Jelawat fish (Leptobarbus hoevenii) is a local freshwater fish rich in protein, fat, and energy that has the potential to support nutritional improvement among undernourished children. To analyse the effect of jelawat fish supplementation on energy, fat, and protein intake, as well as body weight among undernourished children under five years of age. This study applied a quasi-experimental pretest–posttest control group design involving 48 undernourished children between 24 and 59 months of age. Participants were assigned to a control group, a fried jelawat fish group (79 g/day), and a jelawat fish soup group (121 g/day) for 14 days. Participants’ dietary intake was evaluated using two non-consecutive 24-hour food recall interviews, body weight was recorded using a calibrated digital scale with 0.1 kg precision. Data were analysed using Paired Sample t-test, One-Way ANOVA, Post Hoc test, Kruskal–Wallis test, and Mann–Whitney test. Significant increases in energy, fat, and protein intake, as well as body weight, were observed in both intervention groups (p<0.05). Mean changes in energy intake were 170.73±85.95 kcal and 128.14±108.82 kcal in the fried and soup groups, respectively. Mean changes in fat intake were 11.29±6.74 g and 7.34±7.82 g, while mean changes in protein intake were 4.45±5.43 g and 3.26±2.73 g. Mean body weight gains were 0.250±0.146 kg and 0.244±0.186 kg. Significant differences were found between the control group and both intervention groups (p<0.05), whereas no significant differences were observed between the fried jelawat fish and jelawat fish soup groups (p>0.05). Supplementation with jelawat fish (Leptobarbus hoevenii) increased energy, fat, and protein intake and body weight among undernourished children under five in Balangan Regency. Jelawat fish has the potential to be utilised as a local real-food supplementary feeding intervention to support nutritional improvement among undernourished children.

Abstract

Atopic dermatitis (AD) is a chronic inflammatory skin disease associated with skin barrier dysfunction and immune dysregulation. In moderate-to-severe cases, systemic therapy is often required. Dupilumab has been widely used as a biologic therapy targeting the IL-4 and IL-13 pathways, whereas Janus kinase inhibitors (JAK inhibitors, JAKi) exert broader effects through inhibition of the JAK–STAT signaling pathway. This systematic review aimed to evaluate the efficacy and safety profile of JAK inhibitors compared with dupilumab in patients with moderate-to-severe AD. A systematic literature search was conducted using PubMed, ScienceDirect, and the Cochrane Library in accordance with the PRISMA guidelines. English-language articles published between 2020 and 2026 were independently screened by two authors. Eligible studies included adult and/or adolescent patients with moderate-to-severe atopic dermatitis and evaluated oral Janus kinase inhibitors (JAK inhibitors) and/or compared them with dupilumab. The primary outcome was the achievement of EASI-75, while secondary outcomes included the safety profile of the therapies. Eight studies met the inclusion criteria. JAK inhibitors, particularly upadacitinib and abrocitinib, demonstrated rapid clinical improvement, especially in pruritus reduction and achievement of EASI-75 responses. However, JAK inhibitors were more frequently associated with infections, herpes zoster, eczema herpeticum, acne, nausea, and laboratory abnormalities. In contrast, dupilumab was more commonly associated with conjunctivitis, keratitis, and injection-site reactions. JAK inhibitors demonstrated a faster onset of symptom improvement in several studies, particularly with respect to pruritus relief and early EASI responses. Nevertheless, their use should be carefully balanced against the risks of infections, herpes zoster, acne, and laboratory parameter abnormalities. Dupilumab remains an important biologic treatment option with a relatively stable safety profile, although ocular adverse events, such as conjunctivitis and keratitis, should be anticipated.
