Abstract

Breast cancer is known as carsinoma mammae that malignant tumor growing in breast tissue. Patient of breast cancer will being have in the change of body shape, psychology (such as depression and anxiety level), socializing, sexual and daily activities. That is will affected to quality of life of patients (QOL). This study aims to determine the factors that affect the quality of life of breast cancer patients in the city of Padang West Sumatra. This research uses analytics design with cross sectional approach. The number of samples are 34 patients with total sampling technique which adjusted to inclusion and exclusion criteria. Data processing use univariat and bivariat method. The result showed that responden characteristic of breast cancer in padang city has average ages around 52,58±10,323 years old with the oldest is 74 years old and the youngest is 33 years old. The average height of respondents 153,77±5,346cm, the level of education is SMA and University (38,5%), the respondents’ job of this research are mostly unemployment (65,4%),  the most stadium level is II and III which is 12 people each other (46,2%). Overall the quality of life of breast cancer sufferer in padang city from 2014-2017 was mostly excellent (15.4%), good (46.2%), moderately good (42.3%) and moderate (7.7% ). However, the symptoms was complained by the respondents are fatique (38,9%) and nausea (80,8%). It can be concluded that age, job, education level, bodymass index and stadium are not related to quality of life of breast cancer respondents in Padang city of West Sumatra. Kanker payudara disebut juga carcinoma mammae yaitu tumor ganas yang tumbuh pada jaringan payudara. Penderita kanker payudara akan mengalami perubahan fisik, psikologis (seperti tingkat depresi dan kecemasan), fungsi sosial, seksual serta aktifitas sehari-hari. Hal ini akan berpengaruh terhadap kualitas hidup atau quality of life (QOL) penderita. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berpengaruh terhadap kualitas hidup penderita kanker payudara di Kota Padang Sumatera Barat. Penelitian ini menggunakan desain analitik dengan pendekatan cross sectional. Sampel berjumlah 34 penderita, dengan teknik pengambilan sampel menggunakan total sampling yang disesuaikan dengan kriteria inklusi dan ekslusi. Pengolahan data menggunakan analisis univariat dan bivariat. Hasil penelitian menunjukkan bahwa karakteristik responden kanker payudara di kota Padang rerata umur responden adalah 52,50±10,332 tahun dengan usia tertua 74 tahun dan termuda 33 tahun. Tinggi badan responden penelitian rerata adalah 153,77±5,346cm. Tingkat pendidikan responden adalah SMA dan Perguruan Tinggi masing-masing 10 orang (38,5%). Pekerjaan responden penelitian ini adalah tidak bekerja 17 orang (65,4%), tingkat stadium terbanyak adalah stadium II dan III yaitu masing-masing 12 orang (46,2%). Kualitas hidup keseluruhan penderita kanker payudara di Kota Padang tahun 2014-2017 sebagian besar masuk dalam skala sangat baik (15,4%), baik (46,2%), agak baik (42,3%) dan sedang (7,7%). Namun gejala yang dikeluhkan responden adalah fatique sering pada 10 orang (38,9%), dan nause selalu pada 21 orang (80,8%). Dapat disimpulkan bahwa umur , pekerjaan, tingkat pendidikan, indeks masa tubuh dan stadium berpengaruh terhadap kualitas hidup responden kanker kanker payudara di Kota Padang Sumatera Barat.

Abstract

AbstrakPengembangan teknologi yang salah satunya teknologi informasi komunikasi (TIK) membantu manusia menyelesaikan pekerjaan sehari-hari baik secara individu maupun organisasi. Sistem informasi daerah binaan (SIDARBIN) Kota Dumai adalah merupakan media pencatatan dan pelaporan berbasis teknologi informasi, yang berisi form pendataan keluarga sesuai indikator keluarga sehat (PIS-PK) ditambah dengan indikator SPM serta data indikator lain sesuai kebutuhan daerah. Tujuan penelitian ini adalah untuk mengetahui pengaruh Human Organization Technology (HOT) terhadap Net Benefit SIDARBIN. Jenis penelitian ini adalah penelitian kuantitatif dengan pendekatan cross-sectional. Variabel penelitian adalah yaitu komponen human, organization, technology dan Net Benerfit SIDARBIN. Pengumpulan data dilakukan dengan menggunakan kuesioner dan wawancara. Analisis data dilakukan dengan analisis multivariat yaitu uji regresi logistic berganda. Hasil penelitian menunjukkan variabel komponen HOT mempengaruhi Net Benefit SIDARBIN dengan kekuatan pengaruh 68,1 % secara keseluruhan cukup kuat. Terdapat pengaruh signifikan komponen HOT (Human, Organization, Technology) terhadap Net Benefit aplikasi SIDARBIN di Puskesmas Jaya Mukti Kota Dumai Tahun 2019. Bagi pihak puskesmas agar dapat melakukan analisis data dan informasi yang dihasilkan dari pendataan keluarga menggunakan aplikasi SIDARBIN, untuk dapat gunakan sebagai bahan perencanaan kegiatan dan program dalam intervensi masalah kesehatan yang evidence base di Puskesmas. Kata kunci: Human, Organization, Technology, Net Benefit SIDARBIN

Abstract

To maintain the continuity of a business/service is important to retain existing customers. It is necessary to see what factors affect customer loyalty. So the purpose of this study is to find a model of the relationship between perceived value, brand image, satisfaction and customer loyalty. Data are collected from 140 customers who had been treated more than once at the Oncology Clinic in the Ropanasuri Hospital for Special Surgery in Padang to test the relationship model. The results show: 1) perceived value has a positive effect on brand image, 2) perceived value has a positive effect on customer satisfaction, 3) perceived value has no effect on customer loyalty, and 4) brand images have a positive effect on customer satisfaction, the five patient satisfaction has a positive effect on customer loyaltyhttps://doi.org/10.26905/jmdk.v8i2.5094

Abstract

Indonesia is now encountering moral hazard problems in the implementation of social health insurance. BPJS, as the administrator of Indonesia’s National Health Insurance, reported that there was an increase in deficit in the 4 years of the implementation of National Health Insurance from US$ 228 million in 2014 to US$ 470 million in 2016. Despite efforts conducted to overcome the problem, no evidence-based predictor that might be significantly associated with moral hazard in a rural province hospital in Indonesia. The purpose of this research is to identify the incidence of moral hazard in the implementation of National Health Insurance in Indonesia. Data consisting of 180 medical records obtained from three public hospitals in rural province of Indonesia were selected as samples in this study. These medical records were reviewed by Independent Senior Coder (ISC) who had more than 5 years experiences as a coder. The indicators of moral hazard in this study were upcoding, readmission, and possible unnecessary admission. Logistic regression was used to explore determinant of moral hazard from patient, coder, and physician side. The results show that rate of moral hazard cases for upcoding is 10%, readmission is 2.8%, and possible unnecessary admission is 18.9%. It can be seen from multivariate analysis that discharge status, severity level and LOS have a significant relationship with moral hazard. Illness severity level, Discharge against Medical Advice, and higher Length of Stay are risk factors for moral hazard incidence.

Abstract

Upcoding is one of important indicators of moral hazard and fraud in Social Health Insurance scheme. However, there seems to be little evidence about incidence of upcoding and how upcoding occurs in hospital, especially in rural province hospital. The objective of this study is to determine incidence and root cause of upcoding in the implementation of Social Health Insurance in Rural Province Hospital in Indonesia. The data used in this study were both qualitative and quantitative data (mixed method). Three hundred and sixty (360) inpatient medical records from six rural province hospitals were examined in this study. Diagnosis and procedure codes recorded in these medical records were re-coded by an independent senior coder (ISC). Codes from hospitals’ coders and codes from ISC were then re-grouped using INA-CBG casemix grouper to determine the casemix groups and the hospital tariffs. If the hospital tariff obtained by hospital coder is higher than that obtained by ISC, it is considered as upcoding. This qualitative study was conducted using Focus Group Discussion (FGD) and in-depth interviews in hospitals located in a rural province of Indonesia. In depth interview was held for two hospital directors and two officers from the Social Security Administrator (Indonesia: BadanPenyelenggaraJaminanSosial/BPJS), an agent that manages the Statutory Health Insurance (SHI). Six clinicians and six coders attended the FGD. We asked open-ended questions about their perceptions on upcoding in hospitals. The interviews were recorded and transcribed verbatim. The transcripts were then thematically analyzed. Upcoding cases were found in 11.9% (43/360) medical records. Upcoding cases were dominated by Deliveries Group 2.8% (10/360) and Female reproductive system Groups 1.7% (6/360). The potential loss of income due to upcoding was IDR 154.626.000 or 9% of hospital revenue. Appointment of non-medical doctors as internal verifiers, lack of clear coding guidelines, lack of training for doctors and coders, and poor coordination between hospital and BPJS to resolve coding disagreement were root causes of upcoding in hospital. Policies to prevent and manage upcoding should be urgently developed and implemented in the Statutory Health Insurance (SHI) program in Indonesia especially to prepare upcoding guidelines, enhance medical coding trainingregularly, increase number of coders and verification staff from medical background, and strengthen coordination for coding problem solving in hospital.

Abstract

Complaint handling is one of the many strategies for improving health services. It is recognized that patients’ satisfaction is an indicator of the quality of care. A survey on public services revealed that public satisfaction at Sijunjung Hospital was 43.78% in the year 2015 but decreased to 40.18% in 2016. This decrease in public satisfaction is due to patients’ complaints. From 2015 to 2016 there were 198,739 complaints while from 2016 to 2017 the number increased to 208,415. The purpose of this study is to determine the factors of patient’s dissatisfaction with health care services and complaint handling to improve health services quality at Sijunjung Public Hospital. This study uses the mixed method. The initial phase of this research is done by quantitative approach using a cross-sectional design in the form of data collection and analysis. The study involves a qualitative approach using observation method, in-depth interviews, Focus Group Discussions. The study reveals that only half of the inpatients, outpatients, and Emergency Unit patients get excellent health services at Sijunjung Public Hospital. The results of the study suggest that satisfaction of patients requires an individual analysis of the patient’s needs; implementation in the health care system of functions that meet these needs.

Abstract

Integrated Marketing Communication (IMC) is a promising communication concept, more than just integration, coordination and unification of communication instruments. IMC has great potential in influencing business performance, to provide a competitive advantage to companies, to increase brand equity, and to ensure the effectiveness and consistency of communication by facilitating the achievement of communication and marketing objectives. This research is a type of qualitative research using the Grounded Theory method conducted at the Naili DBS Hospital in April to December 2019. The results of this study was IMC program design of the Naili DBS Hospitalthrough eight parameters such as advertising where hospitals can carry out internet advertisements containing testimonials or endorsements as well as billboard installations, sales promotion can be carried out through consumer loyalty programs where hospitals form consumer communities in order to facilitate the implementation of sales promotions. Sales people in personal selling can sell to FKTP because 89% of Naili DBS Hospital patients are BPJS Health participants. For public relations / publicity it is better to hold a seminar and create a company magazine, for the parameters of hospital wom can create marketing from patients to deliver their experiences to others, this also relates directly to the implementation of direct marketing in which many customers have formed word of mounth no paid at Naili DBS Hospital. Interactive marketing must be re-enforced in order to be widely known by patients. The event and experience can be held by public exhibition in the form of seminars to introduce the Naili DBS Hospital to the public. Integrated Marketing Communication (IMC) merupakan konsep komunikasi yang menjanjikan, lebih dari sekadar integrasi, koordinasi, dan penyatuan instrumen komunikasi.IMC berpotensi besar dalam mempengaruhi kinerja bisnis, untuk memberikan keunggulan kompetitif pada perusahaan, untuk meningkatkan ekuitas merek, dan untuk memastikan efektivitas dan konsistensi komunikasi dengan memfasilitasi pencapaian komunikasi dan tujuan pemasaran.Penelitian ini merupakan jenis penelitian kualitatif dengan metode Grounded Theory yang lakukan di RS Naili DBS pada bulan April sampai dengan Desember 2019. Hasil dari penelitian ini berupa rancangan program IMC RS Naili DBS melalui delapan parameter yaitu advertising dimana rumah sakit dapat melaksanakan iklan internet yang berisikan testimony atau endorsement serta pemasangan billboard, sales promotion dapat dilaksanakan melalui program loyalitas konsumen dimana rumah sakit membentuk komunitas – komunitas konsumen agar memudahkan dalam pelaksanaan promosi penjualan. Sales people dalam personal selling dapat melakukan penjualan kepada FKTP dikarenakan 89 % pasien RS Naili DBS adalah peserta BPJS Kesehatan. Untuk public relation/ publicity sebaiknya dilaksanakan seminar dan membuat majalah perusahaan, untuk parameter wom rumah sakit dapat membentuk marketing – marketing baru dari para pasiennya agar menyampaikan pengalamannya kepada orang lain, hal ini juga berhubungan langsung dengan pelaksanaan direct marketing dimana sudah terbentuk banyak pelanggan yang melakukan word of mounth no paid di RS Naili DBS. Interactive marketing belum dikenal banyak oleh pasien sehingga harus di tinggatkan kembali pelaksanaannya.Pelaksanaan event and experience dapat dilakukan pameran publik berupa seminar – seminar untuk lebih mengenalkan lagi RS Naili DBS kepada masyarakat.

Abstract

AbstrakGizi lebih adalah keadaan tubuh seseorang yang mengalami berat badan berlebih karena kelebihan jumlah asupan energi yang disimpan dalam bentuk cadangan berupa lemak. Prevalensi gizi lebih pada anak di Indonesia mencapai 10,4. Salah satu faktor risiko terjadinya gizi lebih adalah kebiasaan mengonsumsi junk food. Tujuan penelitian ini adalah menentukan hubungan antara konsumsi junk food dengan status gizi lebih pada anak usia sekolah. Penelitian ini adalah penelitian analitik dengan rancangan cross sectional terhadap populasi penelitian yaitu siswa kelas 1 – 5 di SD Pertiwi 2 Padang sebanyak 250 siswa. Teknik pengambilan data dilakukan dengan menggunakan kuesioner semi kuantitatif FFQ.  Status gizi ditentukan dengan pengukuran berat badan dan tinggi badan yang hasilnya dikategorikan menggunakan standar antropometri penilaian status gizi anak menurut Depkes Indonesia. Pada hasil penelitian didapatkan bahwa siswa yang memiliki status gizi lebih hanya 23,6% dengan rata – rata IMT 16,9±3,69. Rata – rata frekuensi konsumsi junk food 4,6±2,9 kali per hari dan rata – rata asupan energi junk food 1046,5±918,4 kkal per hari. Kesimpulan penelitian ini adalah terdapat hubungan yang bermakna antara konsumsi frekuensi konsumsi junk food dengan kejadian gizi lebih (p = 0,013) dan tidak terdapat hubungan yang bermakna antara asupan energi junk food dengan kejadian gizi lebih (p = 0,120).Kata kunci: gizi lebih, junk food, FFQ AbstractOverweight is a condition of an individual who suffer from excessive weight due to an excess of energy intake stored in fat tissue. Prevalence of overweight in children in Indonesia reaches 10,4 %. One of the risk factors contributing to this condition is consuming junk food. The objective of this study was to identify the relationship of junk food consumption and overweight in students. This research is an analytic study conducted in a cross sectional design using 250  students of class 1-5 in SD Pertiwi 2 Padang as subjects. The data was obtained using semi-quantitative FFQ. Nutrition status is defined using the measurement of body weight and height. The results are categorized using anthropometric standard for nutrition status assessment developed by Depkes Indonesia. The result revealed that the students with overweight was 23,6 % with average BMI of 16,9±3,69. The average frequency of junk food consumption was 4,6±2,9 times per day and the average energy intake from junk food was 1046,5±918,4 kcal per day. The conclusion is that the frequency of junk food consumption has a significant relationship to overweight and there’s no significant relationship between energi intake from junk food and overweight.Keywords: overweight, junk food, FFQ

Abstract

Penanganan keluhan di RSUD Rasidin masih menjadi permasalahan, hasil observasi tahun 2018 masih terdapat beberapa keluhan pasien terhadap pelayanan di ruang rawat inap. Hal ini dapat dilihat dari survey Indeks Kepuasan Masyarakat terhadap penanganan keluhan masih dibawah standar yaitu 76,56 artinya kinerja dari unit pelayanan masih kurang baik. Tujuan penelitian ini adalah menganalisis tingkat kepuasan pasien dalam penanganan keluhan (Service Recovery) di RSUD Rasidin Kota Padang. Jenis penelitian ini merupakan penelitian cross sectional dengan instrument penelitian kuisioner terhadap 155 responden rawat inap di RSUD Rasidin Kota Padang. Variabel yang diukur yaitu Procedural Justice, Interactional Justice dan Distributive Justice. Analisis data menggunakan importance performance analysis dengan diagram kartesius sehingga didapatkan pemetaan alur permasalahan. Hasil penelitian menunjukkan 26% responden mendapatkan pelayanan kurang baik dan masih ada beberapa keluhan yang harus menjadi prioritas utama untuk ditingkatkan dari atribut Interactional Justice dan Distributive Justice diantaranya keahlian dokter dalam memberikan informasi mengenai masalah kesehatan penyakit yang diderita pasien, keahlian doker dalam menanyakan keluhan pasien, keahlian perawat dalam memberikan informasi mengenai masalah kesehatan penyakit yang diderita pasien, dan komunikasi antara pasien dengan dokter. Keempat atribut ini lebih menekankan kepada perilaku dan komunikasi yang kurang baik antara tenaga medis dengan pasien.
