Abstract

Coronavirus disease-2019 (COVID-19) is an infectious acute respiratory disease caused by SARS-CoV-2. The protein that plays a role in the entry of SARS-CoV-2 into human cells is the surface protein, or the Spike, which is thought to be the effective vaccine target to prevent SARS-CoV-2 infection. Until December 2020, Indonesia has reported 106 SARS-CoV-2 genome sequences identified from COVID-19 positive patients. The purpose of this study was to analyze the phylogenetic relationship of the Spike protein of the Indonesian isolates of SARS-CoV-2 Indonesian, as well as the virus mutations and their effects on changes in the amino acid. The 106 Indonesian SARS-CoV-2 genomes were downloaded from GISAID and  the Spike nucleotide and amino acid sequences were analyzed by multiple sequence alignment (MSA) and mutation analysis using the ClustalW method. Phylogenetic trees were created using the Neighbor-Joining method in MEGA-X software. The results showed that 30 of the 106 Indonesian isolate SARS-CoV-2 Spike were 100% identical to the Wuhan-Hu-1, while the remaining 76 had experienced mutations at 1-4 sites. There were 43-point mutations in the Spike gene, 27 of which led to amino acid changes and four had not been reported in other countries. The global mutation D614G was found in 60 Indonesian isolates , of which West Java was the province with the most reports. The phylogenetic of Spike showed that the Indonesian samples have been divided into several branches that are far from Wuhan-Hu-1. This study indicates the possibility of differences in the protein structure of Indonesian isolate SARS-CoV-2 Spike that need to be further studied to manufacture a vaccine against the Indonesian strain of SARS-CoV-2.

Abstract

Cardiotoxicity is a condition where damages caused by toxic chemical exposure are observed in heart and blood vessels. Doxorubicin is the most common chemotherapy agents for various types of cancer therapy. However, doxorubicin is converted into doxorubicinol known to trigger cardiac disfunctions and release of several cardiac biomarkers, such as CK-MB and LDH. Turmeric is known to be an alternative medical treatment that has the effect of neutralizing oxidative stress. This study aimed to evaluate CK-MB and LDH levels in doxorubicin-induced rats (20 mg/kgBW) that received turmeric ethanolic extract from curcuma. This was an experimental study conducted in February 2020 in the Faculty of Pharmacy Universitas Sumatera Utara. The rats were divided into six group and each group consisted of wistar albino male rats. The groups were normal (CMC-Na), positive control (Vitamin E 1%+Dox 20 mg/kgBW), negative control (Dox 20 mg/kgBW), treatment I (EEC 100 mg/kgBW+Dox 20 mg/kgBW), treatment II (EEC 300 mg/kgBW+Dox 20 mg/kgBW), and treatment III (EEC 500 mg/kgBW+Dox 15 mg/kgBW). Doxorubicin was given 5 mg/kgBW once a week for four weeks. Results showed that the groups that received 100 mg/kg BW, 300 mg/kgBW, and 500 mg/kgBW of turmeric ethanol extract demonstrated a reducing effect on the biomarkers of cardiac damage, i.e. CK-MB and LDH. Statistically, serum CK-MB and LDH levels at dose 500 mg/kgBW showed no significant differences (p>0.05) with the normal and positive treatment group. In conclusion, turmeric has a cardioprotective effect. Pengaruh Kardioprotektif Ekstrak Etanol Curcuma Longa pada Tikus yang Diinduksi DoxorubicinKardiotoksisitas adalah kondisi terjadinya jantung dan pembuluh darah akibat paparan kimia. Doxorubicin diubah menjadi doxorubicinol yang diketahui menyebabkan gangguan fungsi jantung dan pelepasan biomarker jantung sseperti CK-MB dan LDH. Kunyit dikenal sebagai pengobatan alternatif yang memiliki efek menetralkan stres oksidatif. Penelitian ini dilakukan pada Februari 2020 di Fakultas Farmasi Universitas Sumatera Utara. Tujuan penelitian ini adalah mengevaluasi kadar CK-MB dan LDH pada tikus yang diberi ekstrak etanol kunyit yang diinduksi doksorubisin (20 mg/kgbb). Penelitian dibagi menjadi enam kelompok dan tiap-tiap kelompok terdiri atas 4 ekor tikus wistar albino jantan: normal (CMC-Na), kontrol positif (Vitamin E 1%+Dox 20 mg/kgBB), kontrol negatif (Dox 20 mg/kgBB), group I (EEC 100 mg/kgBB+Dox 20 mg/kgBB), group II (EEC 300 mg/kgBB+Dox 20 mg/kgBB), dan group III (EEC 500 mg/kgBB+Dox 15 mg/kgBB). Doxorubicin 5 mg/kgBB diberikan 4 kali. Hasil penelitian menunjukkan bahwa kelompok yang diberi ekstrak etanol kunyit dengan dosis 100 mg/kgBB, 300 mg/kgbb, 500 mg/kgBB memiliki efek penurunan biomarker kerusakan jantung yaitu CK-MB dan LDH. Berdasar atas hasil uji statistik kadar CK-MB dan LDH serum pada dosis 500 mg/kgBB tidak terdapat perbedaan yang bermakna (p>0,05) dengan kelompok normal dan positif, dapat disimpulkan bahwa kunyit memiliki efek kardioprotektif.

Abstract

Annona muricata L. is widely known throughout Indonesia as having great potentials as an antidiabetic agent. This study aimed to evaluate the antihyperglycemic effectiveness of ethanol extract 96% from soursop leaves and to compare its effects to metformin and insulin aspart injection as antidiabetic agents. This was a cross-sectional experimental study with random sampling approach which was performed at Phytochemistry and Biopharmacy Laboratory, Faculty of Pharmacy, Hasannudin University, Indonesia, from February 17 to March 3, 2020. Twenty-four Mus musculus subjects induced by streptozotocin were divided into six groups, which received 96% ethanol extract from Annona muricata in various doses. The extract was produced using the maceration process. The fasting blood sugar level was measured using the point of care testing (POCT) method, and data collected were then analyzed using a univariate approach. Results showed that the average fasting blood glucose level (FBG) was decreasing with increasing doses (5 g/kgBW=127 mg/dL, 10 g/kgBW =114 mg/dL, and 15 g/kg BW=97.75 mg/dl with a p-value of 0.000 based on repeated ANOVA. When compared to the positive control 1 (metformin), the decrease in FBG level in this control group (70.67 mg/dL) was better than in the group that received ethanol extract from Annona muricata L. In subjects in positive control 2 (insulin aspart), the average FBG level remained high, which was 473 mg/dL. Hence, 96% ethanol extract of Annona muricata L. can effectively lower fasting blood glucose in Mus musculus. Uji Efektivitas Antihiperglikemik Ekstrak Ethanol 96% Daun Sirsak pada Mus musculus yang diinduksi dengan Streptozotocin Tanaman sirsak menyebar di pelosok Indonesia serta banyak dimanfaatkan masyarakat sebagai obat herbal untuk berbagai macam penyakit, diantaranya  sebagai antidiabetes. Tujuan penelitian ini adalah mengetahui efektivitas antihiperglikemik ekstrak ethanol 96% daun sirsak pada mencit yang diinduksi dengan streptozotocin dan membandingkan efeknya  dengan metformin dan insulin aspart. Penelitian ini adalah penelitian eksperimental dengan desain cross-sectional dan teknik sampling secara acak yang dilakukan di Laboratorium Fitokimia dan Biofarmasi Fakultas Farmasi Universitas Hasanuddin pada 17 Februari–3 Maret 2020. Sebanyak 24 ekor mencit yang diinduksi streptozotocin dibagi menjadi enam kelompok yang menerima ekstrak etanol 96% dari daun sirsak dalam berbagai dosis. Pembuatan ekstrak menggunakan proses maserasi dan untuk mendapatkan gula darah puasa (GDP) menggunakan metode point of care testing (POCT). Hasil penelitian menunjukkan bahwa rata-rata kadar GDP menurun dengan peningkatan dosis pada dosis 5 g/kgBB=127 mg/dL, 10 g/kgBB=114 mg/dL, 15 g/kgBB=97,75 mg/dL dengan nilai p=0,000 berdasar atas uji ANOVA. Jika dibanding dengan kontrol positif 1 (metformin) penurunan nilai GDP jauh lebih baik pada kelompok kontrol 70,67 mg/dL lebih baik dibandingkan pada kelompok yang mendapat ekstrak etanol dari daun sirsak. Pada subjek kontrol positif 2 (insulin aspart) nilai kadar GDP tetap tinggi, yaitu 473 mg/dL. Simpulan, ekstrak ethanol 96% daun sirsak efektif menurunkan kadar glukosa darah puasa pada mencit.

Abstract

There are currently still limitations in the diagnosis of tuberculosis (TB). Sputum collection as specimen for diagnosis is not only difficult but also has low sensitivity.In blood, IP-10/CXCL-10 chemokine plays a role in inducing the movements of chemotactic inflammatory cells towards the sites of inflammation. A high level of IP-10 is found in active pulmonary TB patients and significantly decline after the patients have completed the TB treatments. The aim of this study was to analyze the decline of the IP-10 level before and after 2 months of TB treatment. This study was conducted from March­ toJuni 2020. This was a comparative observational cohort study on active pulmonary TB patients who were >18 years old at the DOTS Clinic of Dr. Hasan Sadikin General Hospital Bandung. Thirty patients who met the inclusion criteria were followed up until 2 months of TB treatment. Serum of  these patients were collected and examined for the IP-10 level before and after 2 months of TB treatment. It was demonstrated that the median IP-10 level in new active pulmonary TB patients was 384.1 pg/mL (136.70–779.80) and dropped to 251.85 pg/mL (91.10–698.30) (p<0l001) two months of TB treatment. Thus, the IP-10 level in the active pulmonary TB patients is significantly declined (p<0.001) after 2 months of TB treatment and that serum IP-10 level could be considered as a non-sputum-based marker to monitor TB treatment.

Abstract

Kuhnle et al. has reported that Sunkist peels contain 36 µg/100 g wet weight of phytoesterogen. This study was designed to explore Sunkist peels' potential to recover the infertility due to ovulation failure. This study was performed in June 2020 at the Pharmacology Laboratory, Universitas Prima Indonesia, on 25 rats. These rats were divided into five groups: Control, Standard, and Sunkist Peels Extract-I, II, and III that received 1 ml of 0.5% Sodium Carboxymethyl cellulose suspension, ethinyl estradiol 50 g/kgBW, 300 mg/kgBW, 450 mg/k BW, and 600 mg/kgBW of ethanol extract from Sunkist peel, respectively. Every 8-12 hours, the rat’s vagina were swabbed and smear into on object glass to be stained by 10% Giemsa and viewed at 40x and 100x magnifications. The leucocytes, especially neutrophils, epithelial cells, cornified and epithelial cells as well as the density of cells in vaginal swab were evaluated to determine the estrus cycle of the rats. The length of estrus phases was express as median (IQR) and analyzed using the Kruskall-Wallis test. The result of this study showed that the standard and highest dosage extract showed a significant extending of estrus and metestrus phases compared to the control group and significantly shortened diestrus phases. Hence, the highest dosage of Sunkist extract can prevent infertility by increasing the period of estrus cycle (ovulation) at the same level as the standard group.

Abstract

Testicular torsion is an emergency urological condition that is caused by the torsion of the spermatic cord structures, causing disruption of circulation of the affected testicle. This study aimed to describe the characteristics of patients with testicular torsion treated at Dr. Hasan Sadikin General Hospital Bandung from January 2016 to January 2020. This was a retrospective descriptive study on 34 medical records of patients diagnosed and treated for testicular torsion. Nine patients (29.4%) were <20 years old and 25 patients were >21 years old. The onset was mostly between 6 to 24 hours (38.2%), followed by between 2–7 days (23.5%), less than 6 hours (20.6%), between 1–2 weeks (8.8%), and between 2–4 weeks (8.8%). Left testicular torsion were more frequent than the right torsion (61.8% vs. 38.2%). The etiology of the torsion was mostly idiopathic with no identifiable precedent (88%). Orchidectomy was more frequently performed compared to orchiopexy (78.4% vs. 21.6%). All but one patient (97.1%) presented with testicular pain as the main symptom. Patients presented mostly with a high risk TWIST score (64.7%); however, more presented with low risk compared to the intermediate risk TWIST score (26.5% vs. 8.8%). Orchidectomy is the most frequently performed operation on pre-pubertal and adult patients, possibly due to relatively delayed presentation (>24 hours) to the hospital to receive treatment. Patients were mostly younger; predominantly with high TWIST score and affected left testicle.

Abstract

Increased prevalence of DM is accompanied by increased in its various complications include diabetic nephropathy, which can lead to end stage renal disease (ESRD). Urinary albumin-creatinine ratio (uACR) is the gold standard for diabetic nephropathy; however, it has several limitations, including the inability to early diagnose due to the absence of increased level in uACR. Uromodulin is produced by thick ascending limb (TAL) at Henle’s loop and early distal tubule of nephron. Uromodulin will decrease when tubular atrophy occurs in early stage of renal impairment with normoalbuminuria. The aim of this study was to determine the validity of serum uromodulin in type 2 diabetes mellitus (T2DM) in identifying diabetic nephropathy by comparing it with uACR as gold standard. This study was a cross-sectional analytical observational study at Dr. Hasan Sadikin (RSHS) Bandung from June to August 2020. Subjects consisted of 62 patiens with T2DM. Results showed that the serum uromodulin level had a sensitivity of 93.3%, a specificity of 88.2%, a positive predictive value of 95.5%, a negative predictive value of 83.3%, and an accuracy of 91,9% and an AUC value of 0.975. The cut-off point of serum uromodulin in this study was 47.195 ng/mL. In conclusion, serum uromodulin, when compared to the uACR as the gold standard, has good sensitivity and specificity for identifying diabetic nephropathy.

Abstract

Prostate cancer is a malignant disease of the urogenital system, which is the second most common cancer in men and one of the leading causes of death in male population. In Bandung City, Indonesia, no data are available regarding the profile of prostate cancer patients. This study aimed to determine the profile of prostate cancer patients treated at Dr. Hasan Sadikin General Hospital Bandung, Indonesia, which is the tertiary hospital in the most populated province in Indonesia, during the period of 2015-2019. This was a a retrospective descriptive study on secondary data from medical records. Results showed that most prostate carcinoma patients were was 67.95 ± 8.946 years old with most of them (n=92, 81.4%) had a history of hypertension and 62 patients (54.9%) did not have any history of smoking. Most patients had 20-40 cc prostate volume with hard consistency and without nodules; a PSA level of > 20 ng/ml; and a gleason score of 8-10 or poorly differentitated. Age affected increased incidence of prostate carcinoma with the highes prostate volume (20-40cc). The  average PSA was 504.66 ± 10.26 and the median gleason score was 8 (6-10). The highest prostate volume is 20-40cc with hard consistency without nodule and the highest PSA value is > 20 ng/ml with a Gleason score of 8-10, showing a poorly differentiated condition.

Abstract

Type 2 Diabetes Mellitus (Type 2 DM) is a metabolic disorder group with mechanisms that include hyperglycemia, insulin resistance and hyperinsulinemia. Type 2 DM has a close association with IGF-1, where the active form of the IGF-1 becomes elevated by reason of the fact that hyperinsulinemia inhibits the production of IGF-binding proteins (IGFBP) 1/IGFBP 2. The active form of IGF-1 has the ability to promote cell proliferation and inhibit apoptosis, hence becomes one of the risk factors for cancer cell growth. This was an analytical study conducted in August at the Harapan Bunda Clinic, Medan, Indonesia to determine the difference between the IGF-1 level and blood glucose level in type 2 DM patients in different age groups. Twenty subjects with Type 2 DM participated in this study and were divided based on their age into 35-50 years old or Group 1 and 51-65 year old group or Group 2. The IGF-1 levels in both groups were compared and analyzed using the T-test dependent method. Results showed that the IGF-1 and blood glucose levels were higher in Group 1 (35-50 years old) when compared to Group 2 and the difference was significant. The change in the IGF-1 level in diabetic patients cannot be determined only by the blood sugar level.

Abstract

Immunization, which is usually done by giving vaccinations, is the process of forming immunity in individuals against infectious diseases. The target in the Strategic Plan of the Ministry of Healht of the Republic of Indonesia in 2015-2019 aimed for 93 % of coverage. However, according to the report of the Indonesian Basic Health Survey (IBHR), there is actually a decrease in basic immunization coverage for the period of 2012–2015 when compared to the previous period. This study aimed to determine the correlation between parental knowledge, attitudes, and behavior towards complete course of basic immunization among parents of an elementary school in North Jakarta, Indonesia, in 2018 and 2019. This was a cross-sectional analytical descriptive study involving 110 respondents. Data were obtained through a questionnaire that was distributed to parents that included questions on knowledge, attitude, and behavior towards complete basic immunization. Out of 110 respondents, 70% stated that their child received the full basic immunization course while the remaining 30% stated that their child only received some of the vaccines required in the basic immunization course. There was no correlation, based on the bivariate analytical test, between parental behavior (p = 0.868), parental attitude (p = 0.647), and full course of basic immunization. In contrast, parental knowledge influences whether the child will receive the full course of basic immunization or not (p=0.000).
