Abstract

Obesity continues to be a major health problem worldwide and has become an epidemic that has worsened over the past 50 years. Obesity has a prevalence that continues to increase every year due to unhealthy lifestyles and energy intake that is greater than energy expenditure. Obesity is closely related to the habit of consuming high-fat foods, which can increase the risk of increased blood pressure. Several studies have found that a 15% increase in body weight can cause an 18% increase in blood pressure. The results of the 2018 Indonesian Basic Health Research showed that the prevalence of obesity in the Indonesian population aged 18 years and over reached 21.8%. The purpose of this study was to determine the difference in obesity status on blood pressure at the Panjang Health Center, Bandar Lampung City. This study used an observational analytical method with a cross-sectional approach. The population in this study were obese patients registered at the Panjang Health Center, Bandar Lampung City in the period August 2023 to August 2024. The total number of research subjects who met the inclusion and exclusion criteria was 113 research subjects obtained from medical records of obese patients at the Panjang Health Center, Bandar Lampung City in the period August 2023-August 2024. Data are presented in a distribution table and analyzed using the Mann-Whitney test. The results showed a P value of 0.001 which indicated that there was a difference in the effect of obesity status on blood pressure status at the Panjang Health Center, Bandar Lampung City.

Abstract

Dermatophytosis is a superficial infection caused by dermatophyte fungi. This infection is also often called tinea. Classification is based on the affected body part, namely tinea capitis (head), tinea faciei (face), tinea barbae (beard), tinea corporis (body), tinea manuum (hands), tinea cruris (groin), tinea pedis (feet), tinea unguinum (nails). The prevalence of dermatophytosis in Asia is 35.6%, while in Indonesia it is 52%. The risk factors are related to the agent, host, and environment. These factors include immunocompromised, high humidity, high temperature, increased urbanization, variations in fungal virulence. The purpose is application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient problem-solving framework using a patient-centered and family approach. This study is a case report. The data obtained are primary data obtained through autoanamnesis, alloanamnesis, physical examination, and home visits and secondary data obtained from the patient's medical record. The results of this study are, patient Mrs. I 62 years old came with complaints of itching and redness in the foot area since two week before coming to the puskesmas. Complaints of itching worsen if the patient feet are damp. Physical examination revealed a numular sized eritematous lesion, the edges of the lesion had fine scales. Clinically the patient was diagnosed with Tinea pedis (ICD-10 B35.3). Furthermore, management, intervention and evaluation are carried out. In conclusion, holistic management can increase knowledge and change attitudes and behavior of patients, especially in patients wuth dermatophytosis. The role and support of the family is needed in the care and treatment of patients.

Abstract

Schizophrenia is characterized by fundamental and characteristic deviations from thought and perception, as well as by abnormal affect. Based on Riskesdas in 2019, the prevalence of schizophrenia in Indonesia was 6.7% per 1000 households. Proper handling holistically using a family medicine approach is needed to improve the patient's quality of life. Applying the principles of a holistic and comprehensive family doctor approach in detecting internal and external risk factors and solving EBM problems. This study is a descriptive study of case reports. Primary data were obtained through autoanamnesis and alloanamnesis, mental examination, and home visits. Secondary data were obtained from the patient's medical record. Patient Mr. P, 52 years old, has been diagnosed with schizophrenia paranoid, has difficulty sleeping at night accompanied by frequent talking to himself, hearing voices, and seeing shadows that others cannot see. Furthermore, comprehensive management is carried out, namely carrying out interventions using poster media and inviting patients to socialize. In the evaluation, the results were obtained in the form of a better understanding of the disease and changes in behavior that have an impact on the success of therapy. Holistic management with family medicine approach is needed for patients with paranoid schizophrenia disease to identify problems in various aspects to support the success of therapy and improve the patient's quality of life.

Abstract

Gastroesophageal Reflux Disease (GERD) is a medical condition where stomach acid rises into the esophagus, with primary symptoms such as heartburn, regurgitation, chest pain, chronic cough, and hoarseness. GERD occurs due to the weakness of the valve between the stomach and esophagus. According to Riskesdas 2018, the prevalence of GERD in Indonesia is 40.8%, making it a significant health issue. Family doctor services that are oriented towards evidence-based medicine focus on identifying risk factors, clinical issues, and patient management through an approach that actively involves the patient and their family. This study used primary data through anamnesis, physical examination, and home visits to assess the environmental conditions that may impact the patient’s health. The assessment was done holistically, covering diagnosis, process, and research results both qualitatively and quantitatively. Indicators evaluated include diet, stress levels, adherence to therapy, physical activity, and environmental factors such as home cleanliness and access to healthcare. Before the intervention, the patient and family had limited knowledge about GERD, with unhealthy eating habits, such as irregular meals, overeating in one sitting, minimal intake of fruits and vegetables, and lack of exercise. After the intervention, the knowledge score increased from 4 in the pre-test to 9 in the post-test, indicating improved understanding of GERD and preventive measures. The holistic approach proved effective in improving the patient’s understanding, attitude, and behavior, as evidenced by improvements in the diagnostic evaluation at the end of the study.

Abstract

Family doctors provide evidence-based medicine services with a patient-centered and family-focused approach to identify clinical problems, risk factors, and patient management. This article is compiled using a case report method according to research needs. The data used are primary data obtained through anamnesis, both auto and alloanamnesis, physical examination, home visits to obtain data related to family, psychosocial and environment. A holistic diagnosis of various factors is carried out from beginning to end to solve the problem holistically. A 60-year-old woman was diagnosed with Grade I Hypertension and Scabies. The patient has internal risk aspects, namely obesity, poor hygiene, unbalanced diet, low knowledge of the disease, and stress. The external risk in this case is the lack of family knowledge regarding the patient's disease. Then, comprehensive management is carried out for the patient and family. Through this approach, it is expected to be able to improve the patient's knowledge, perspective, and actions. The role of the family to change the patient's lifestyle to be healthier as support for the patient, such as reminding to take medication and inviting exercise greatly affects the results of holistic management. The complexity of this problem requires time and cooperation between family doctors, patients, and families.

Abstract

Tonsillitis can strike any age but is more common in children. Inflammation of the tonsils can be caused by bacteria or viruses.  Inflammation of the tonsils will result in enlargement which causes difficulty swallowing or feels like there is a lump in the throat. Enlarged tonsils can also cause other symptoms such as hoarseness (muffled voice) or even breathing problems in severe cases. If there is no complete healing of the tonsils, recurrent infections can occur. Applying the principles of family medicine services and carrying out holistic management based on a framework for solving patient problems with a patient-centered, family-focused, and community-oriented approach based on Evidence-Based Medicine. This study is a case report using primary data obtained through history taking, physical examination, and home visits to complement family data, and psychosocial and environmental data. Secondary data was obtained from patient medical records. Patient An. N aged six years old with complaints of swallowing pain since four days ago. Patients have a habit of consuming iced drinks and light snacks which may be unhygienic and can be a risk factor for infection. Holistic management with a family doctor approach has been carried out for An. N aged six years with Acute Tonsillitis  which was adjusted based on the initial holistic diagnosis. Before the intervention, the patient and family are given a pretest to measure the patient's level of knowledge regarding the disease they are suffering from. During the intervention, the family is explained about the disease she is suffering from using media in the form of posters, and the family is given education regarding the importance of treatment and how to prevent the disease. At the last visit a posttest was given to the family with the results being a significant increase in knowledge about the disease they were suffering from and awareness of the importance of changing patient habits.

Abstract

Varicella or chicken pox is an acute and highly contagious disease caused by infection with the Varicella Zoster virus (VZV). Apply the principles of family medicine services and carry out holistic management based on a framework for solving patient problems with a patient-centered, family focused and community-oriented approach based on Evidence Based Medicine. This study analysis is a case report. Primary data is obtained through history taking, physical examination and home visits to complete family data, psychosocial and environmental data. Secondary data was obtained from patient medical records at the Community Health Center. The assessment is carried out based on a holistic diagnosis of the beginning, process and end of the study quantitatively and qualitatively. Male patient Mr. C 27 years-old came with complaints that he had tingles on his face, neck, body, back, arms, and leg for two days ago accompanied by itching. Elastic in the form of vesicles the size of billions, round shape, well-defined, with an erythematosus base, distributed regionally. Clinically and on physical examination the patient was diagnosed with Varicella (ICD 10 B01.9). In this case, diagnosis and treatment have been carried out according to theory and related journals. After the intervention was carried out, there was a reduction in clinical symptoms and an increase in patient and family knowledge. Holistic management can improve the knowledge, attitudes and behavior of patients and families in maintaining health.

Abstract

Urinary tract infection (UTI) is an infection caused by the growth of microorganisms in the urinary tract that causes inflammation and bacteriuria. It is estimated that there are around 150 million cases of UTIs occurring each year worldwide, which can burden health care expenditures. Dyspepsia is a group of symptoms that include uncomfortable sensations in the abdomen, such as pain, bloating, nausea, and vomiting. It is estimated that almost 30% of cases encountered in general practice and 60% in gastroenterology practice are dyspepsia. Implementing comprehensive and holistic family doctor services by identifying risk factors, clinical problems, and patient management based on evidence-based medicine with a patient-centered, family approach and community-oriented strategy. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits. Secondary data were obtained from the patient's medical records. Patient Mrs. H, 33 years old, came to the polyclinic with pain when urinating for 2 days, and nausea that had been felt for 7 days. The patient was diagnosed with UTI and Dyspepsia. Interventions were carried out in the form of providing health promotion using health posters regarding UTI and dyspepsia. The results of the intervention in patients were increased knowledge about the disease and compliance in implementing hygiene patterns, regular eating patterns, physical activity and reduced complaints experienced by patients. Holistic management can improve the knowledge, attitudes and behavior of patients and families in maintaining health.

Abstract

Anemia is a body condition characterized by a lower number of red blood cells or hemoglobin concentration than normal limits. According to WHO, 64 million or 23.9% of elderly suffer from anemia worldwide. One of the causes of anemia in the elderly is chronic kidney disease (CKD). According to RISKESDAS, the reported prevalence of CKD at ages ≥15 years is 0.38% in Indonesia with the highest incidence at ages 65-74 years. Appropriate holistic treatment using a family medicine approach is needed to improve the patient's quality of life. Implement comprehensive and holistic family doctor services by identifying risk factors, clinical problems, and patient management based on evidence-based medicine with strategies that are patient centered, family approach and community oriented. This study is a case report. Primary data was obtained through history taking, physical examination and home visits. Secondary data was obtained from patient medical records. Patient Mrs. T, 72 years old, has been diagnosed with anemia, complains of weakness accompanied by fatigue, dizzy eyes and unsteadiness when walking. An intervention was carried out in the form of providing health promotion using health posters regarding anemia in chronic kidney disease. The results of the intervention in patients obtained increased knowledge about the disease and compliance in maintaining health for successful therapy. Holistic management can improve the knowledge, attitudes and behavior of patients and families in maintaining health. Key words: Family Doctor, Anemia, Chronic Kidney Disease.

Abstract

Chronic obstructive pulmonary disease (COPD) is defined as a lung disease with clinical symptoms as shortness of breath, coughing, phlegm production due to airway abnormalities that cause persistent airflow obstruction. This airflow obstruction in COPD is progressive and often associated with chronic-inflammation caused by exposure to noxious substances, such as cigarette smoke. Tuberculosis (TB) is a contagious-infectious disease caused by Mycobacterium-tuberculosis and most often manifests in the lungs (pulmonary TB), although-M.Tb can also infect other organs (extrapulmonary TB). History of pulmonary TB could cause permanent-damage to lung-anatomy and associated with loss of lung-function, such as bronchiectasis, fibrosis, or post-TB emphysema, which can increase the risk of COPD. Research by Fan H et al in 2021 stated that there was a risk of COPD in patients with a previous history of pulmonary TB. Providing family doctor services rooted in evidence-based medicine involves the identification of risk factors and clinical issues, along with the management of patients, all within a patient centered and family oriented problem solving framework. This study involves a case-report-analysis. Primary data were gathered through history taking, physical examinations, and home visits. The evaluation is conducted based on a comprehensive diagnosis-spanning the initial, process-oriented, and final phases of the study, using both quantitative and qualitative methods. Interventions for the patient-consisted of educational measures for patients and their families using posters and direct discussions. Following these interventions, there was an observed improvement in both knowledge and behavioral-changes both patient and family-behaviors, as evidenced by improvements in the final holistic-diagnostics.
