Abstract

Diabetes Mellitus is one of the degenerative diseases with high prevalence and is increasing every year in Lampung province by 22,345 cases or 1.37%. Obesity is one of the predisposing factors for diabetes mellitus. The prevalence of obesity in the population aged 18 years and over increased from 14.8% to 21.8%. In Lampung province, it was 17.3%. Diabetes mellitus and Obesity must be managed comprehensively which is expected to prevent complications and improve the quality of life of patients. Objective: Implementation of Evidence Based Medicine-based services in patients by identifying risk factors, clinical problems, and patient management based on a patient problem-solving framework with a patient-centered, family-focused, and community-oriented approach. Method: This study is a case report. Primary data was obtained through anamnesis, physical examination and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Results: After the intervention, there was an increase in patient and family knowledge regarding the importance of health care provider check-ups, regular medication consumption, appropriate diet and physical activity patterns for patients with Diabetes Mellitus and Obesity. Conclusion: Holistic management has been carried out with a family doctor approach for Mrs. A, 54 years old with type II Diabetes Mellitus and grade II Obesity which was adjusted based on the initial holistic diagnosis. The intervention has increased patient and family knowledge regarding the importance of health care provider check-ups, regular medication consumption, diet and physical activity patterns as indicated by improvements in the final holistic diagnosis.

Abstract

Thyroid carcinoma is the most common cancer found in the endocrine system that arises from thyroid parenchymal cells.  Its prevalence has increased over the past few decades with a mortality rate of 0.5% per 100,000 cases. The incidence of thyroid carcinoma in 2020 ranks 7th in the world as the 9th most frequent incidence occurs from all types of cancer. Objective: This study aims to determine the characteristics of thyroid carcinoma patients at Dr. H. Abdul Moeloek Regional General Hospital for the period of January 2023 – August 2024. Method: This study used a descriptive observational design and Total Sampling technique with a data source in the form of secondary data on medical records from 82 thyroid carcinoma patients at the Dr. H. Abdul Moeloek Regional General Hospital for the period January 2023 to August 2024.  Results: In this study, thyroid carcinoma patients were mostly female, with an age group in the range of 41-60 years. The most common type of thyroid carcinoma is papillary carcinoma and the least common type is anaplastic carcinoma. Conclusion: In this study, the characteristics of thyroid carcinoma patients were obtained based on gender, age group, and type of histopathological diagnosis.

Abstract

Dengue fever (DF) is an acute viral infectious disease caused by the dengue virus which in general can be characterized by symptoms of fever for 2-7 days, accompanied by other symptoms in the form of bleeding, such as nosebleeds and red spots on the body (ptekiae), bleeding gums, decreased platelets, the form of hemoconcentration in the form of plasma leakage with signs such as increased hematocrit, ascites, pleural effusion. The purpose of this case report is to Implementing family doctor services by identifying risk factors, clinical problems, and patient management based on Evidence-Based Medicine that is family-approach, patient-centred and community-oriented. The study conducted is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical records at the puskesmas. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. Patient is female with complaints of fever since 4 days ago. Fever followed by weakness, hedache, and nausea. In this case, the diagnosis and treatment have been carried out according to the latest theory and journals. After the intervention, there was an improvement in patient symptoms and an increase of patient knowledge.

Abstract

Excessive consumption of monosodium glutamate can form free radicals and is nephrotoxic which causes damage to organs, one of which is the kidneys, especially in the proximal tubule. Chlorogenic acid compounds contained in coffee beans have potential as antioxidants and nephroprotective agents. Coffee consumption at a certain dose is assumed to prevent and or reduce damage to the proximal kidney tubules of rats caused by free radicals. This research is an experimental study with a completely randomized design (CRD) with a Posttest Only Control Group Design approach. This research was conducted for 14 days. The samples used were 25 rats which were divided into 5 groups, namely K- (aquades 3 ml/day), K+ (MSG 4 g/kgBW/day), P1, P2, and P3 (MSG 4 g/kgBW/day). and Lampung robusta coffee bean extract 1 ml/200gBW/day with a concentration of 0.006 g/ml; 0.012 g/ml; 0.024 g/ml respectively) with each group consisting of 5 rats. Kidney histopathological assessment was carried out by observing kidney preparations in 5 visual fields each in 400x magnification. The mean total score of proximal tubular damage in groups K-, K+, P1, P2, and P3 respectively was 2.48; 3.40; 2.80; 2.64; 3.04. One Way ANOVA test obtained p value = 0.001 (p <0.05). Post Hoc LSD test on proximal tubular damage scores showed a significant difference (p<0.05) between K- and K+ (p=0.000), K- and P3 (p=0.008), K+ and P1 (p=0.005) , K+ with P2 (p=0.001), and P2 with P3 (p=0.047). There is an effect of administration of Lampung Robusta (Coffea canephora) coffee bean extract on the histopathological appearance of the kidney proximal tubules of male rats (Rattus norvegicus) strain Sprague-Dawley induced by monosodium glutamate.

Abstract

Hypertension is a condition when blood pressure in the blood vessels increases chronically. Data from the World Health Organization (WHO) in 2015 showed that the incidence of hypertension in the world reached 1.13 billion and in 2025 the number of hypertension sufferers will continue to increase to reach 1.5 billion individuals. Type 2 DM is a metabolic disease characterized by hyperglycemia due to insufficiency of insulin secretion by pancreatic beta cells or impaired insulin function. According to the International Diabetes Federation (IDF), it is estimated that one in 11 adults or 90 million people (aged 20-79 years) in Southeast Asia will suffer from DM in 2021. Hypercholesterolemia is a condition characterized by high levels of cholesterol in the blood. According to (Riskesdas) 2018, it shows 21.2% of the Indonesian population aged ≥ 15 years. Hypertension, type 2 diabetes mellitus (T2DM) and hypercholesterolemia are chronic disease disorders that are interrelated with each other due to overlapping risk factors such as endothelial dysfunction, vascular inflammation, arterial remodeling, atherosclerosis, dyslipidemia and obesity. There are many cardiovascular complications in diabetes and hypertension, especially related to microvascular and macrovascular diseases such as stroke, heart failure, kidney disease, retinopathy, neuropathy and amputation. To reduce morbidity and mortality rates, Evidence Based Medicine-based service management is carried out in patients by identifying risk factors, clinical problems, and patient management based on a patient problem-solving framework with a patient-centered, family-focused and community-oriented approach. Abstract

Dyslipidemia is a lipid metabolism disorder characterized by an increase or decrease in the levels of lipid fractions in plasma. Dyspepsia is a feeling of discomfort in the upper stomach. In Maret, in 2018 there were 28.8% of the Indonesian population aged ≥ 15 years with abnormal total cholesterol levels. In 2020, the prevalence of dyspepsia was 28 million people, 11.3% of the Indonesian population. The aim of this research is to determine risk factors, clinical problems, and manage patients by applying the principles of family medicine according to evidence-based medicine through a patient centered and family approach. The method of this study is a case report. Primary data through history taking, physical examination and home visits, to complete family data, psychosocial and environmental data. The assessment is carried out based on a holistic diagnosis of the beginning, process and end of the study quantitatively and qualitatively. Result: 69 years old patient with complaints of neck stiffness and headache since two weeks ago. The patient has a history of high cholesterol since the past year and does not regularly take cholesterol medication. The patient likes to eat oily foods and coconut milk. The patient has insufficient nutritional intake. The patient’s total cholesterol level was 271 mg/dL. The patient complained of upper stomach pain, a feeling of bloating in his stomach that had been getting worse for the past 2 days. Complaints were caused by sour, spicy and late eating. The patient experienced this complaint since 30 years ago. The patient was diagnosed with dyslipidemia and dyspepsia. In this case, diagnosis and treatment are carried out in accordance with theories and journals. After the intervention, there was improvement in clinical symptoms, increased knowledge, and changes in the patient’s lifestyle.

Abstract

Diabetes mellitus, a metabolic disorder characterized by elevated blood glucose levels due to insulin deficiency or insulin resistance, is a pressing global health issue, with more than 537 million people affected worldwide, according to WHO data from 2023, and an annual mortality rate of 1.5 million. In Indonesia, the prevalence of type 2 diabetes reached 19,572,564 cases in 2021, with Lampung reporting 38,923 cases. One key area of concern is the Kebon Jahe Community Health Center, which has recorded a high incidence of diabetes. A lack of awareness about diabetes contributes to a high rate of complications such as neuropathy, which can lead to damage to sensory, autonomic, and motor nerves in the peripheral nervous system if not well controlled. The management aimed to implement a holistic and comprehensive family physician approach tailored to the patient’s problems, utilizing evidence-based medicine that is patient-centered, family-oriented, and community-oriented. Primary data were obtained through auto-anamnesis, physical examination, and home visits. Secondary data were gathered from the patient’s medical records. Evaluations were conducted based on a holistic diagnosis from the initial process to the final study, analyzed qualitatively and quantitatively. Mr. H, a 66-year-old man, visited the Kebon Jahe Community Health Center with complaints of tingling and numbness in his hands for the past two months. The patient also reported frequent urination, excessive thirst, and increased hunger. He experienced weight loss of 3 kg over the past three months. A physical examination revealed a deficit in protopathic sensory function. The patient had a habit of consuming sugar from tea or coffee, adding 1-2 tablespoons in 5 servings daily. The patient’s knowledge about his condition was limited. Management was carried out according to related theories and journal guidelines. During the evaluation, the patient was able to adhere to both pharmacological and non-pharmacological therapy recommendations.

Abstract

Horner's syndrome is a group of symptoms consisting of a slightly drooping upper eyelid (ptosis) and a smaller pupil (miosis) on the affected side (ipsilateral), less commonly accompanied by a lack of sweat production (anhidrosis) over the ipsilateral eyebrow or face. Horner’s syndrome can be congenital, acquired, or inherited disorder, but the cause is sometimes unknown. Based on the anatomical location of the underlying pathological process, Horner's syndrome is classified into central, preganglionic, and postganglionic. Although in most cases clinical examination may predict the etiology, in other cases pharmacological testing can help in localizing the lesion. Pharmacological testing agents used in the diagnosis of Horner's syndrome include apraclonidine, cocaine, hydroxyamphetamine, or phenylephrine. Imaging approaches such as targeted Magnetic Resonance Imaging (MRI) or Computed Tomography Angiography (CTA) are recommended, given the financial burden of imaging the entire oculosympathetic pathway. This article reviews the clinical signs and symptoms as well as the pharmacological and imaging modalities that can help in the diagnosis and localization of Horner's syndrome and the cause of the condition.

Abstract

Skin is the separator organ between the environmental parts of the body outside the body. Skin continuously is exposed to environmental factors in the form of physical, chemical, and biological. Therefore, if there is disease. This skin disease is caused by work (occupational dermatitis) namely a skin inflammation that results in a person’s activities. Contact dermatitis due to work is (DKAK) is skin inflammation that occurs due to the interaction between workers and the substance in the work environment. Objective: to find out the risk factors related to the occurrence of contact dermatitis due to work for car washing workers in Bandar Lampung. Method: The type of research used is analytic with cross sectional design. The sample in this study car washing workers in 5 sub-district area Rajabasa, Enggal, Labuhan Ratu, Way Halim, and Sukarame. Results: The distribution of frequency on the working shift in car washing workers who work more than 2 hours by 30 (75%) of workers, at a more contact order on 5x in a day 35 (87,5%) for workers, in the characteristics of chemicals that contain surfactant as much as 37 (92,5%) of workers, and get 22 (55%) of workers experience contact dermatitis due to work. Conclusion: There is a significant relationship between working shift with dkak with the results of the chi-square test 0,001, there is a significant relationship in the speed condition with dkak with the results of the chi-square 0,008. And there is a significance relationship between the chemical charateristics with the results of the chi-square test 0,046.

Abstract

Low Back Pain (LBP) is a musculoskeletal disorder caused by poor body activity. Data for the number of LBP sufferers in Indonesia is estimated at 11.9%. Work-related lower back pain is often reported in workers who perform certain physical actions. Applying the principles of family medicine services and carrying out holistic management based on a framework for solving patient problems with a patient-centered, family focused and community oriented approach based on Evidence Based Medicine. This study is a case report study. Primary data was obtained through auto-anamnesis, physical examination and home visits, filling in the family folder, completing family data, psychosocial and environmental data. The assessment is carried out based on a holistic diagnosis of the beginning, process and end of the study quantitatively and qualitatively. Patient Mr. A, male, 40 years old, works as a fisherman and has a main complaint of lower back pain since approximately one year ago but the pain has worsened in the past one week. The pain improves when resting and gets worse when the patient is active. The patient was diagnosed as Low Back Pain. Pharmacological and non-pharmacological interventions were carried out in the form of education regarding ergonomic body positions and exercise to reduce complaints of lower back pain. The diagnosis and management of Low Back Pain in this patient has been carried out in a holistic, patient-centered, family approach and based on several theories and the latest research.
