Abstract

ABSTRACTBackground: The failure of artesunate-amodiaquine therapy in chloroquine resistant patients have been reported in several areas in Indonesia. One of the unexpected resistance mechanisms is associated with pfmdr1 gene polymorphisms. This study determine the frequency of pfmdr-1 polymorphisms and examine the relationship of pfmdr1 gene polymorphism with therapeutic responses to artesunate-amodiaquine therapy in uncomplicated falciparum malaria patients in Puskesmas Hanura, Pesawaran Regency, Lampung Methods: This study was conducted in accordance with the WHO protocol for anti-malarial drug efficacy test in uncomplicated falciparum malaria. Combination therapy used artesunate-amodiaquine and primaquine. Therapeutical response and Parasite Clearance Time was observed during 28 days of observation. Diagnosis polymorphism by PCR-RFLP technique used the patient's blood (EDTA and filter paper) were taken a tH0.Results: There were 63(85.1%) patients of Adequate Clinical and Parasitological Response, Early Treatment Failure were 5 (6.8%) people, Late Clinical And Parasitological Failure 2 (2.7 %) people, and Late Parasitological Failure 4 (5.4%) people. Seven (13.5%) patients were found with N86Y pfmdr1 gene polymorphism with mixed type (mutant type and wild-type) in which 1 (9.1%) people had failed therapy and 6 (14.6%) people were cured. There was no association between pfmdr1 gene polymorphism with artesunate-amodiaquine failed therapy (RR: 0.64, 95% CI0.97-4.27, p: 1.000). Parasite Clearance Time on pfmdr1 polymorphism group was 2.14 ± 1.21 days and 1.79 ± 1.06 days in group without pfmdr1 polymorphisms. There was no significant difference between both groups (p =0.434).Conclusions:The frequency of pfmdr1 N86Y polymorphism in Puskesmas Hanura, Pesawaran Regency, Lampung was 13,5%. There was no significant difference between the rate of treatment failure patients with pfmdr1 N86Y polimorphism compared with patients without pfmdr1 N86Y polymorphism. There was no significant difference between Parasite Clearance Time patients with pfmdr1 N86Y polymorphism compared with patients without pfmdr1 N86Y polymorphism.Keywords: falciparum malaria, artesunat-amodiakuin, polymorphism, pfmdr1

Abstract

Rumah Sakit Imanuel Bandar Lampung merupakan rumah sakit yang telah menggunakan sistem peresepan elektronik. Penelitian tentang manfaat dan kelemahan peresepan elektronik sudah banyak dilakukan, namun persepsi pasien tentang peresepan elektronik masih jarang dilakukan. Persepsi pasien perlu diketahui untuk mengetahui kualitas pemberian pelayanan kesehatan yang menggunakan sistem peresepan elektronik. Tujuan pada penelitian ini adalah untuk mengetahui hubungan persepsi pasien terhadap peresepan elektronik dengan kepuasan pasien di Instalasi Farmasi Rumah Sakit Imanuel Bandar Lampung. Metode penelitian ini adalah cross-sectional dengan rancangan studi deskriptif, melibatkan 190 responden yang dilaksanakan pada bulan Agustus sampai Desember 2018, di Instalasi Farmasi Rumah Sakit Imanuel Bandar Lampung. Data penelitian didapatkan melalui pengisian kuesioner yang telah diuji validitas dan reliabilitas. Analisis data yang digunakan adalah uji Fisher’s Exact Test. Hasil analisis data pada penelitian ini mendapatkan hasil p-value 0,031. Kesimpulan yang didapatkan bahwa terdapat hubungan antara persepsi pasien terhadap peresepan elektronik dengan kepuasan pasien di Instalasi Farmasi Rumah Sakit Imanuel Bandar Lampung. Abstract

Ada sekitar 64 juta remaja berusia antara 10 hingga 24 tahun, atau 28,64% dari total populasi Indonesia. Pendidikan kesehatan reproduksi bagi remaja merupakan upaya penting untuk menghindari pergaulan bebas, penyebaran penyakit menular seksual dan kehamilan yang tidak diinginkan. Tujuan dari pengabdian masyarakat ini adalah untuk memberikan penyuluhan tentang kesehatan reproduksi kepada remaja guna mewujudkan remaja yang sehat dan berprestasi. Metode pelaksanaan promosi kesehatan yang digunakan adalah Participatory Action Research (PAR) dengan pelatihan tentang masalah kesehatan reproduksi dan pencegahan penyakit menular seksual yang diikuti oleh siswa-siswi Madrasah Aliyah (MA) Dinniyah Putri, Kabupaten Pesawaran, Lampung. Hasil pengabdian ini menunjukkan terdapat perbedaan yang signifikan antara nilai rerata pretest (63,96) dan nilai posttest (92,2) tentang kesehatan reproduksi pada remaja di MA Dinniyah Putri, Pesawaran, Lampung. Kegiatan pengabdian ini terbukti dapat meningkatkan pengetahuan dan pemahaman remaja tentang kesehatan reproduksi sehingga dapat menjadi salah satu upaya dalam mencegah terjadinya penyakit.

Abstract

Background: Gastric ulcers are stomach lesions characterized by depleted mucosa with the defect extending into the submucosa or muscularis propria. Gastric ulcers could be caused by the use of Indomethacin as an analgesic drug. Turmeric rhizome is a herbal plant that contains curcumin which has been suspected to play a role in gastric ulcer prevention. Objective: The aim of this study was to perceive the effect of turmeric rhizome juice with concentrations of 100%, 75%, and 50% on indomethacin-induced gastric epithelium image in mice. Methods: This experimental study with a Post Test Only Control design was conducted by using 25 samples of male ddY mice for 3 days intervention. Samples were divided into 5 groups, two control groups, and three intervention groups. The organ samples had been observed by assessing the gastric epithelium injury of mice on the antrum under a microscope with an objective lens 100x for eight fields of view. Results: The Kruskal-Wallis test had been used for analysis and showed a p-value of 0,025 which means it had a significant difference. Turmeric juice with concentrations of 100%, 75%, and 50% showed different effects on mice gastric epithelium image induced by Indomethacin (18 mg/kg). Conclusion: There were different effects of turmeric juice on gastric epithelium image induced by Indomethacin in mice.

Abstract

Working in a hot environment can disrupt the balance of body temperature so that the body tries hardly to control the heat which can cause a lack of fluids and is called dehydration. Dehydration can affect cognitive performance, causing decreased productivity and increased risk of work accidents, and in the long term can trigger some non-communicable diseases such as hypertension, urinary tract disease, obesity and others. Pak Rama Education (Active Workers Diligently Drink Water) is an educational and empowerment activity for the worker community, especially those who work in hot environments, in an effort to prevent dehydration and non-communicable diseases. The aim of this activity was to increase workers' knowledge about healthy hydration and increase workers' ability to assess hydration status independently. This activity was carried out using the education method about healthy hydration and a demonstration of how to assess hydration status using a urine color chart. Pak Rama's activity was held on 17th November 2022 at PTPN VII Unit Way Berulu South Lampung. The participants were 50 workers consisting of management officers, factory and plantation workers. From the evaluation results, it was found that the average pre-test score of participants was 66.7 and there was an increase of participants' knowledge about healthy hydration which was seen from the increase in the average score on the post-test, namely 86.6. Based on analysis using the Wilcoxon test, it was found that there was a statistically significant difference between average pretest and posttest scores. The results of the assessment of participants' ability to assess hydration status using a urine color chart were also found increase. Pak Rama's education has proven effective in increasing workers' knowledge about healthy hydration and their ability to assess hydration status independently. Abstract

Asma merupakan penyakit heterogen yang disebabkan oleh inflamasi kronis saluran pernafasan. Pada tahun 2019, prevalensi asma di Indonesia berjumlah 4,5%. Provinsi Lampung tercatat memiliki prevalensi asma sejumlah 1,6% di tahun 2018. Kasus seorang wanita, usia 42 tahun, mengeluhkan sesak nafas 30 menit Sebelum Masuk Rumah Sakit (SMRS). Sesak nafas terakhir muncul 2 bulan yang lalu dan sesak nafas saat malam hari muncul 2 bulan yang lalu. Sesak nafas muncul ketika dipicu oleh debu, asap, dan cuaca dingin. Sesak nafas memberat ketika malam dan pagi hari. Keluhan penyerta berupa batuk tidak berdahak dan riwayat alergi terhadap debu, asap, dan cuaca dingin. Pemeriksaan fisik didapatkan pasien bernafas cepat, wheezing, dan obesitas grade II. Pada pemeriksaan penunjang, pasien mengalami anemia, eusinophilia, dan indikasi penyakit paru kronis pada gambaran rontgen thorax. Diagnosis pasien berupa asma eksaserbasi sedang persisten berat tidak terkontrol dengan obesitas dan anemia. Pasien diberikan tatalaksana farmakologi berupa nebulisasi Short Acting Beta Agonist (SABA) dan nebulisasi kortikosteroid. Setelah 3 hari rawat inap, pasien mengalami perbaikan klinis dan diperbolehkan pulang

Abstract

Keberadaan UKBM TOGA sangat bermanfaat sebagai upaya pengobatan sendiri atau swamedikasi. Desa Karanganyar, Kecamatan Jati Agung, Kabupaten Lampung Selatan memiliki potensi untuk budi daya tanaman TOGA dengan memanfaatkan pekarangan warga namun animo masyarakat masih rendah dan masyarakat masih sedikit yang memahami cara budi daya, manfaat TOGA sebagai swamedikasi. Pengabdian ini bertujuan untuk meningkatkan partisipasi masyarakat untuk menggunakan TOGA sebagai swamedikasi melalui Program CINTA (Curahan Ibu pada Tanaman Obat Keluarga). Manfaat kegiatan ini untuk meningkatkan pengetahuan dan keterampilan ibu-ibu PKK tentang budi daya, asuhan mandiri (asman) TOGA, dan status kesehatan masyarakat di Desa Karanganyar. Sasarannya adalah 30 orang ibu-ibu PKK, solusi yang ditawarkan adalah Program CINTA dengan metode Participatory Impact Monitoring (PIM) dengan kegiatan edukasi kesehatan dan pembentukan kader CINTA. Hasil yang didapatkan, sebelum dilakukan edukasi, pengetahuan ibu-ibu PKK yang rendah sebanyak 22 orang (73%), tinggi sebanyak 8 orang (27%) dan setelah dilakukan edukasi pengetahuan ibu –ibu PKK yang rendah hanya 1 orang (7%) dan tinggi sebanyak 29 orang (97%). Dari hasil analisis McNemar didapatkan perbedaan bermakna antara pengetahuan sebelum dan sesudah edukasi kesehatan (p=0,001). Kegiatan ini perlu terus dilaksanakan secara berkelanjutan dan komitmen bersama dari pihak mitra dalam program CINTA dan Desa Karanganyar sebagai desa sehat dan mandiri. The existence of UKBM TOGA is very useful as a self-medication or self-medication effort. Karanganyar Village, Jati Agung District, South Lampung Regency has the potential to cultivate TOGA plants by utilizing residents' yards. However, public interest is still low and few people understand how to cultivate and the benefits of TOGA as self-medication. This service aims to increase community participation in using TOGA as self-medication through the CINTA Program (Mother's Outpouring of Family Medicinal Plants). The benefit of this activity is to increase the knowledge and skills of PKK women regarding cultivation, TOGA self-care (asman) and community health status in Karanganyar Village. The target audience is 30 PKK mothers, the solution offered is the CINTA Program using the Participatory Impact Monitoring (PIM) method with health education activities and the formation of CINTA cadres. The results obtained, before the education was carried out, the knowledge of PKK mothers was low as many as 22 people (73%), high as many as 8 people (27%) and after the education was carried out the knowledge of PKK mothers was low only 1 person (7%) and high as many as 29 people (97%). From the results of McNemar's analysis, a significant difference was found between knowledge before and after health education (p=0.001). This activity needs to continue to be carried out in a sustainable manner and with a joint commitment from partners in the CINTA program and Karanganyar Village as a healthy and independent village.

Abstract

Obesity continues to be a major health problem worldwide and has become an epidemic that has worsened over the past 50 years. Obesity has a prevalence that continues to increase every year due to unhealthy lifestyles and energy intake that is greater than energy expenditure. Obesity is closely related to the habit of consuming high-fat foods, which can increase the risk of increased blood pressure. Several studies have found that a 15% increase in body weight can cause an 18% increase in blood pressure. The results of the 2018 Indonesian Basic Health Research showed that the prevalence of obesity in the Indonesian population aged 18 years and over reached 21.8%. The purpose of this study was to determine the difference in obesity status on blood pressure at the Panjang Health Center, Bandar Lampung City. This study used an observational analytical method with a cross-sectional approach. The population in this study were obese patients registered at the Panjang Health Center, Bandar Lampung City in the period August 2023 to August 2024. The total number of research subjects who met the inclusion and exclusion criteria was 113 research subjects obtained from medical records of obese patients at the Panjang Health Center, Bandar Lampung City in the period August 2023-August 2024. Data are presented in a distribution table and analyzed using the Mann-Whitney test. The results showed a P value of 0.001 which indicated that there was a difference in the effect of obesity status on blood pressure status at the Panjang Health Center, Bandar Lampung City.

Abstract

Insomnia is a condition characterized by difficulty initiating sleep despite having adequate time and opportunity. Globally, the majority of insomnia sufferers are women, with approximately 10% of the world’s population estimated to experience severe insomnia. The management of insomnia typically begins with the principles of Consultation, Information, and Education (CIE). In cases of severe insomnia, further intervention often involves pharmacological therapy through medication. The primary class of drugs prescribed for insomnia patients generally belongs to the benzodiazepine group, which acts as a central nervous system depressant, muscle relaxant, and sedative. However, pharmacological treatment should always be complemented with non-pharmacological therapy. Notably, non-pharmacological interventions serve as the first-line treatment for patients with mild to moderate insomnia. This review article aims to evaluate the effectiveness of various non-pharmacological therapies for managing insomnia, which is recognized as the most prevalent sleep disorder worldwide. The literature review was conducted by searching databases such as PubMed, Google Scholar, and ScienceDirect using the keywords “Insomnia,” “Therapy,” and “Non-Pharmacological.” Based on the search results, seven relevant articles were selected for review. The findings from these studies indicate that effective non-pharmacological interventions for insomnia management include Cognitive Behavioral Therapy for Insomnia (CBT-I), hypnotherapy, audiovisual stimulation, aerobic and endurance exercises, acupuncture, and Pre-Sleep Dim Light Therapy (LT).
