Abstract

Background: Implementation and good practice of Occupational Health and Safety (OHS) based on state regulation should be sufficient for a company to promote safety culture. However, “Zero Accident” as its measurable goal has not been consistently achieved, thus we can deduce that OHS practice has not gained its optimum level. We need to know why this gap happens; which safety culture elements are considered as dominant obstacle and support factors at the same time. The research purpose is to measure safety culture maturity level as well as factors that influence safety culture in a company, as case study PT X. Method: As research designs, analytical descriptive and cross-sectional design using quantitative and qualitative approaches were mixed together. The research samples consisted of 96 individuals and 5 key informants. Data was gathered using NOSACQ-50 questionnaire, SCMM questionnaire, and in-depth interviews. Its independent variables were OHS priority management, commitment and competency, OHS empowering management, OHS equality management, worker commitment on OHS, priority and risk acceptance, communication in OHS practice, learning and trust between co-workers in OHS competency, confidence level on OHS system effectivity, work duration, education level, and job position, whilst the only dependent variable was safety culture maturity level. Result: The research result showed that there were correlations between all independent variables of NOSACQ-50 components and safety culture maturity level. It was also concluded that OHS priority management and commitment and competency were both considered as the most influential factors to safety culture (OR=6,29). Following up the research result, it is recommended to the company to officially state Safety Culture as OHS department vision, to improve training management especially for increasing training participation from each business unit, to educate employees to have better mindset on punishment and incident reporting activities, and to improve communication effectiveness within employees related to OHS practice. Keywords: Safety Culture, Safety Climate, Safety Culture Maturity Level, NOSACQ-50, SCMM.

Abstract

Background: Malaria is still becoming a public health problem in Indonesia. Menoreh Hills shows as one of the areas with endemic malaria in Java Island which has not been able to achieve the elimination target by 2015. Menoreh Hills is a cross-border administrative area, Central Java Province and Yogyakarta Special Region Province which has its own authority in financial management and budget allocation for malaria control. Furthermore, spatial analysis is very beneficial in controlling malaria, presenting geographic distribution of the disease, testing whether malaria is randomly distributed, evaluating the statistical significance of the disease cluster and showing as an early detection of outbreaks. Method: It applied an observational study with cross sectional spatial analysis design to observe the spreading, grouping pattern and the correlation between house distance to mosquito breeding and population density. The samples collected were 138 malaria cases and 138 controls. It required measurement of the coordinates to the house with GPS, identified breeding spots for mosquitoes around the house and collected the date data of malaria diagnosed in District Health Services. The location of the study covered 3 sub-districts as Malaria endemic areas namely Kaligesing, Bagelen and Kokap Sub-districs. Analysis of the data was conducted through ArcGIS, SaTScan dan Geoda softwares. Results: The malaria spreading in September-December 2015 was dominated in Kaligesing Sub-district of Purworejo Regency. Malaria cases were more common in areas with low density of population. The buffering analysis discovered that malaria patients were living near to the mosquito breeding (river, springs, and puddle). It was identified that there was 1 primary cluster and 2 secondary clusters which covered 3 districts at research location. Conclusion: The results of spatial analysis present that there is an incident of local malaria transmission in Menoreh hills. It requires an integrated malaria control program in Menoreh hills.

Abstract

Background: The Gajah Wonderland Kampong is a recreation area offering various games that is located in Lembang, Bandung. The favorite game to be played in this park is the so-called Buggy Game. The results of in-depth observations and close examination indicate that this particular Buggy Game is not ergonomic and that path information is inadequate so that accidents often occur. In order to solve these problems, the Participatory Ergonomics (PE) method has been employed. Method: The implementation of PE involves key stakeholders, namely the management in charge, workers, designers, and ergonomics experts. The stakeholders have discussed these matters to reach an agreement in the shape of a design that suits the user. Results: This study resulted in a chair design concept that can be adjusted according to needs, a gas pedal limiting design, a track plan design and manufacturing directions, a speed sensor design and an emergency button, in addition to a head frame design, and a design for anti-heat material in the engine area. Conclusion: PE turns out to be the right method for repeated repairs or improvements made to the Buggy Game because it involves the main user so as to produce a design that is able to improve both the comfort and safety of the users.

Abstract

Background: Clinical pathway (CP) is a collaborative guidance for patients treatments that focused on diagnosis, clinical problems, and stages of care. CP implementation becomes a guideline on hospital quality and safety improvement for the patient services. Method: The research used a descriptive-qualitative mixed methods. Primary data collected through medical record and secondary data is done by interview and observation by using CP format. Results: From medical records total, 23 cases were found that met the inclusion criteria. The acute appendicitis case in adults is more than children in number (3,6:1). Acute appendicitis CP compliance is 86%. There are three problems in CP compliance of acute appendicitis i.e doctor's visit adherence (physician in charge of patient 87%, 0% anesthesia), 65% adherence therapy compliance, and 52% inpatient admission. The root of the problem is the lack of socialization, monitoring, and evaluation of established standards. Some activity variations may occur during each process from time to time that will produce a variety of the outcomes as well. How to reduce process variation is to standardize. The process of standardization includes the preparation, socialization, monitoring, control, evaluation and revision of thew standards. CP profit is that it could reduce variation, is a professional requirement, and the basis for quality measurement. Implementation of good standards will ensure the safety of patients and the healthcare providers. Conclusion: Almost all employees have conducted medical record (MR) in accordance with clinical pathway criteria as well, but the CP sheet is not included in the MR
