Abstract

Background: Maternal mortality rate is one of the indicators to measure the level of a woman's health. The maternal mortality rate in Indonesia is still high, it's about 190/100.000 live birth. The objective of the present study is to evaluate the cause of maternal death in Bantul at 2016 which related to the health services including human resources.Subjects and Method: This was a mixed method with case study design on maternal mortality in Bantul District. The qualitative data collected through Focus Group Discussion (FGD) and the quantitative data from the questionnaire were filled by a senior midwife throughout the hospital in Bantul. The number of hospitals in Bantul District was 11 consisting of 1 government hospital and 10 private hospitals.Results: The number of obstetricians who could be available 24 hours in hospitals was still limited. The skills of midwives were still considered to be limited in handling a number of obstetric emergency conditions due to the lack of regular training. The primary health care services which had been provided to support and handle obstetric emergencies, but the infrastructure and human resources were still considered inadequate. Thus, there was also a condition where the utilization of integrated ANC facilities in the primary health center (Puskesmas) was still low, soan early detection of complications of pregnancy was less optimal.Conclusion: There are three main topics causing the maternal mortality at Bantul, and those are 1)Maternal mortality related to hospital facilities, 2) Maternal mortality related to human resources at the hospital, 3) Maternal mortality related to health center facility.Keywords: maternal, obstetrician stand-by, hospitalCorrespondence: Arlina Dewi. Abstract

Background: Clinical pathway (CP) is a collaborative guidance for patients treatments that focused on diagnosis, clinical problems, and stages of care. CP implementation becomes a guideline on hospital quality and safety improvement for the patient services. Method: The research used a descriptive-qualitative mixed methods. Primary data collected through medical record and secondary data is done by interview and observation by using CP format. Results: From medical records total, 23 cases were found that met the inclusion criteria. The acute appendicitis case in adults is more than children in number (3,6:1). Acute appendicitis CP compliance is 86%. There are three problems in CP compliance of acute appendicitis i.e doctor's visit adherence (physician in charge of patient 87%, 0% anesthesia), 65% adherence therapy compliance, and 52% inpatient admission. The root of the problem is the lack of socialization, monitoring, and evaluation of established standards. Some activity variations may occur during each process from time to time that will produce a variety of the outcomes as well. How to reduce process variation is to standardize. The process of standardization includes the preparation, socialization, monitoring, control, evaluation and revision of thew standards. CP profit is that it could reduce variation, is a professional requirement, and the basis for quality measurement. Implementation of good standards will ensure the safety of patients and the healthcare providers. Conclusion: Almost all employees have conducted medical record (MR) in accordance with clinical pathway criteria as well, but the CP sheet is not included in the MR

Abstract

UU no.20 tahun 2013 tentang pendidikan kedokteran mengarahkan bahwa peran fakultas kedokteran selain sebagai produsen juga berperan dalam meningkatan kualitas pelayanan kesehatan untuk meningkatkan derajat kesehatan masyarakat. Academic Health Center (AHC) adalah suatu organisasi yang dibuat dengan tujuan terjadinya sinergi antara perguruan tinggi, RS. pendidikan, dan regulator. Dosen sebagai salah satu komponen penting dalam suatu fakultas kedokteran akan menjadi komponen penggerak jika ingin dibentuk AHC di fakultas kedokteran Tujuan penelitian ini adalah untuk mengetahui kesiapan dosen dalam mewujudkan Academic Health Center di Fakultas Kedokteran Universitas Muhammadiyah Makassar. Metode penelitian ini menggunakan desain penelitian deskriptif kuantitatif cross sectional dengan menggunakan kuesioner yang disebarkan kepada seluruh dosen fakultas kedokteran universitas muhammadiyah makassar. Data akan dianalisis untuk mengetahui pengetahuan dan sikap para dosen terhadap AHC. Hasil yang diperoleh bahwa hampir ½ dari responden belum pernah mendengar tentang AHC. Hanya sebagian kecil dosen yang memiliki pengetahuan yang cukup tentang AHC. Tetapi sebagian besar dosen bersedia untuk mencari informasi lebih banyak tetang AHC dan bersedia berkontribusi dalam pembentukan AHC. Sikap para dosen setuju dengan peran AHC yang akan dapat bemanfaat bagi sistem kesehatan di Indonesia. AHC dinilai mampu mengatasi masalah-masalah akibat dari perubahan lingkungan di dunia kesehatan. Persepsi para dosen tentang AHC yang positif perlu diimbangi dengan pengetahuan yang mumpuni dari para dosen.

Abstract

Emotional intelligence (EQ) contributed 80% in determining one ’s success. EQ was developed in 3 containers, namely: family, school and society, and experts agree that keluargalah have great influence on the development anak.Untuk, it is necessary to find whether the relationship between healthy functioning in the family (Apgar family) with adolescent emotional intelligence in family. This study aims to determine whether or not there any healthy relationship between family junction with emotional intelligence in adolescents in the family. Observational study conducted by the research design of cross-sectional survey. Samples were selected by simple random sampling from the population at SMU Negeri 1 Bantul Poor. Research instruments for assessing emotional intelligence using the instrument Baron Emotional Quotient Inventory Version andfamily function assessment using the Family Apgar instrument of Gabriel Smilkstein, which has been translated. Data analysis to determine relations between the two variables above using Pearson test. Statistical analysis showed that the Apgar Family related/correlated (r = 0.460) was significant (p 0.05) with adolescent emotional intelligence in the family. Concluded that there is a significant relationship between Apgar Families with teenagers Emotional Intelligence.Kecerdasan emosi (EQ) menyumbang 80% dalam menentukan keberhasilan seseorang. Kecerdasan emosi dikembangkan dalam 3 wadah, yaitu : keluarga, sekolah dan masyarakat, dan para ahli sepakat bahwa keluargalah yang sangat berpengaruh terhadap perkembangan anak.Untuk itu, perlu dicari hubungan antara sehat tidaknya fungsi di dalam keluarga (APGAR keluarga) dengan kecerdasan emosi remaja di dalam keluarga tersebut. Penelitian ini bertujuan untuk mengetahui adakah hubungan antara sehat tidaknya fungsi keluarga dengan kecerdasan emosi pada remaja di keluarga tersebut. Penelitian dilakukan secara observasional dengan rancangan penelitian cross-sectional. Sampel penelitian dipilih secara simple random sampling dari populasi di SMU Negeri 1 Kasihan Bantul. Instrumen penelitian untuk menilai kecerdasan emosi menggunakan BarOn Emotional Quotient Inventory Version dan penilaian fungsi keluarga menggunakan instrumen Family APGAR dari Gabriel Smilkstein, yang telah dialihbahasakan. Analisis data untuk mengetahui hubungan antara kedua variabel di atas menggunakan uji Pearson. Hasil analisis statistik menunjukkan bahwa APGAR Keluarga mempunyai hubungan/ korelasi (r=0,460) yang bermakna (p 0,05) dengan kecerdasan emosi remaja di keluarga tersebut. Disimpulkan bahwa terdapat hubungan bermakna antara APGAR Keluarga dengan Kecerdasan Emosi remaja.

Abstract

Affective domain (attitude, feeling, etc) is an important part of education in medical science besides cognitive and psychomotor domains. Some ofthe responsibilities of Indonesian doctors written in the “Kurikulum Inti Pendidikan Dokter Indonesia III (KIPDIIII) ” can be categorized into affective domain.Difficulties in evaluating affective domain include: 1) Disparity on the credibility of students ’ answers on the affective evaluation 2) Failure on the evaluation objectivity; 3) There is no agreement yet on how to measure and how far the affective domain will be evaluated in medical education. Therefore, medical faculties need to discuss together the methods and tools for measuring affective domain. This includes considering the objectives and function of the measurement and defining study goals on affective domain (using taxonomy of affective domain).Ranah afektif atau pengukuran terhadap sikap, perasaan, dll merupakan bagian penting dalam pendidikan di bidang kedokteran, selain ranah kognitif dan psikomotor. Di dalam Kurikulum Inti Pendidikan Dokter Indonesia (KIPDI) DI, dicantumkan tentang sejumlah tanggungj awab dokter di Indonesia yang dapat digolongkan dalam ranah afektif.Kesulitan yang muncul dalam penilaian ranah afektif yaitu: 1) Kesenj angan dalam kredibilitas jawaban-jawaban mahasiswa terhadap penilaian afektif; 2) Kegagalan dalam objektifitas penilaian; dan 3) Dalam dunia pendidikan, belum adanya kesepakatan cara mengukurnya dan seberapa jauh ranah afektif ini akan dinilai. Untuk itu perlu institusi secara bersama-sama membicarakan cara dan instrumen pengukuran yang digunakan dengan mempertimbangkan antara lain tujuan dan fungsi pengukuran serta menetapkan target belaj ar dalam ranah afektif (misal menggunakan taksonomi ranah afektif).

Abstract

Infant Mortality Rate (IMR) in Indonesia has been decrease but it is still high compared with the other ASEAN’s countries, the third higher rank in ASEAN, after Laos and Burma. Indirect causes of IMR include health services, relative difficult to be solved so it is still influence health status disparity of rural and urban area. High IMR regions should be more concerned, such as Pejawaran District which has a high IMR in 5years recent are 46,8/1.000 birth life on 2007, that is higher than national’s IMR are 26,9/1.000 birth life. The objective of this study is to evaluate health services (antenatal andperinatal care) in a high IMR region, Pejawaran District. This study use observational method with survey approach. The population is Pejawaran District ’s community. The samples are housewives who live at this district minimum 5 years and given birth minimum 1 life baby born. Ninety seven samples are selected by Lameshow formula of sample size and were selected by unproportional simple random from 3 villages that are selected by purposive sampling. The result are analyzed by descriptive analysis Generally, maternal-neonatal health care in Pejawaran district are good. There is needed an effort to make health cost and health care location more affordable and accessible for all Pejawaran ’s communities.Angka Kematian Bayi (AKB) di Indonesia telah menurun, tetapi masih tinggi dibandingkan anggota ASEAN lain yaitu peringkat ketiga setelah Laos dan Burma. Penyebab tidak langsung AKB termasuk pelayanan kesehatan relatif sulit ditanggulangi sehingga masih menimbulkan disparitas antarpedesaan-perkotaan. Daerah dengan AKB tinggi perlu lebih diperhatikan, seperti Kecamatan Pejawaran yang selalu tinggi dalam 5 tahun terakhir yaitu 46,8/1.000 kelahiran hidup pada tahun 2007, terlihat lebih tinggi dari AKB nasional yaitu 26,9/1.000 kelahiran hidup. Penelitian ini bertujuan untuk mengevaluasi pelayanan kesehatan (perawatan antenatal dan perinatal) di wilayah dengan AKB tinggi, yaitu Kecamatan Pej awaran. Penelitian ini menggunakan metode observasional pendekatan survei. Populasi penelitian adalah masyarakat Kecamatan Pej awaran, dengan sampel ibu rumah tangga yang pernah melahirkan minimal satu bayi lahir hidup dan tinggal di kecamatan ini minimal selama 5 tahun. Sampel sebesar 97 ditentukan berdasarkan rumus besar sampel Lameshow dan diambil secara unproporsional simple random dari tiga desa yang ditentukan secara purposive sampling. Data diolah dengan analisis deskriptif. Secara umum didapatkan hasil, pelayanan kesehatan maternal dan neonatal di Kecamatan Pejawaran cukup baik, biaya kesehatan dan lokasi tempat pelayanan kesehatan perlu diupayakan supaya lebih terj angkau dan tercapai bagi seluruh masyarakat Kecamatan Pej awaran.

Abstract

Patients with renal disease require special care.This study aims to develop a hemodialysis unit in the Army Hospital that require the act of washing the blood as a form to improve health services for patients with kidney failure. In this study, the majority of patients covered by the National Health insurance. Research methods namely method a case study with quantitative descriptive analysis.The results of this study show that the development of the hemodialysis unit will give a positive impact for the patient and for the hospital. The review will be the addition of hemodialysis machines, the addition of a new building, the addition of shift nurse can be seen that all these options will be profitable in the foreseeable future. This of course can make the best judgment for the Hospital in determining a decision for the sake of improving health care for the patient.

Abstract

Background: Diabetes Mellitus (DM) is the most chronic disease throughout the world which is a chronic metabolic disease due to pancreas that cannot produce enough insulin and the body cannot use effective insulin, as evidenced by the number of diabetes mellitus prevalence that can continue at any time increased. Diabetes mellitus is a metabolic disease which is very high risk of death. Patients with diabetes mellitus participate with thrombosis in blood vessels, one of which is diabetic foot ulcer. Thrombosis in patients with diabetic foot ulcers will increase thrombosis in the lower limbs that are part of venous thromboembolism, or Deep Vein Thrombosis (DVT). One effort to repair the wound can be done with practice. Exercise is one of the first management strategies proposed for new patients diagnosed with diabetes, because along with diet and policy modifications, exercise and exercise are important components of all diabetes intervention programs, which by doing exercises can help very much.Research Objectives: This literature review discusses the causes of deep venous thrombosis (DVT), causes of ulcer wounds and looks for the influence of the Angkle range of motion (ROM) on the risk of Deep Vein Thromosis (DVT) in patients with diabetes mellitus with ulcer wounds.Method: The literature database search method was conducted at Google Scholar and Pubmed. The literature used is literature that meets the inclusion criteria and published the last five years, is approved then selected and reviewed.Results: Final results There are 4 literatures that can be reviewed. Based on the literature conclusions, it can be found several exercises in patients with diabetes mellitus so as to increase diabetes, and can reduce the need for Deep Vein Thromosis (DVT). To find out the risk of Deep Vein thrombosis (DVT) can be determined by the D-Dimer test. If the D-Dimer test shows positive results, then the patient must use DVT

Abstract

Heart failure is a disease that can caused death. About 5.1 million people in the United States had heart failure. In 2009, one in nine deaths caused by heart failure. In Indonesia, the prevalence of heart failure in 2013 was 0.13%, the estimated number of heart failure patients in Yogyakarta was 6,943 (0.25%). Based on data from 2007 indicated CHF Riskesdas the third cause of death in Indonesia after stroke and hypertension. The purpose of this study to determine the characteristics of heart failure patients in the Poly Heart Gamping Sleman PKU Muhammadiyah Hospital in September-November 2016. The study was conducted using a descriptive design. The sample was 32 respondents. Characteristics of patients with heart failure in Poly Heart PKU Muhammadiyah Hospital Gamping Sleman highest respondent's age range 61-65 years was 59.38%, the highest gender was female  53.12%, the last education was elementary 34.38%, the worst job many are civil servants was 31.25%, stage 2 was  93.75%. The conclusion of this study is that most of the respondents are at the age of 61-65 years old, the sex of the respondents is bigger than the male, the most recent primary school education, the work of most civil servants, and almost all respondents had heart failure stage 2. The results of this study are expected to be a reference to know the description characteristics of patients with heart failure so that prevention or intervention can be done to reduce the recurrence of patients with heart failure.

Abstract

Background: Discomfort often occurs when catheter placement and also the removal of catheter. In health sector, nursing practice focus on patient quality and safety. Feeling safe is a form of feeling that if not controlled of anxiety. Using urinary catheter will affect to quality of life of patients such as physical, psychological, socio cultural, environmental and spiritual aspects.Objective: The aims of this study to describe how the fulfillment of safety and comfort needs in urinary catheter patients.Method: This study use literature review. The database were PubMed, Google Scholar and ProQuest. The search was restricted to English language articles and limited to last 5 years (2014-2019). Articles should be related to research. This study use main keywords experience AND “urinary catheter*”. Only fourteen journals met the eligbility criteria and were review by two researchers.Result: The Results of article that urinary catheter impact on several aspect of patients such as social aspect patient have limited social interaction, and difficult to adaptation. In psychological aspect, patient feel embarrassed, dissatisfaction, frustation, anxiety, and low self esteem. In physical aspect, patient feel blocking their movement, increase the risk of infection, painful, discomfort, and iritation. In other aspect, the patient has economic problem.Conclusion: The findings of articles show positive and negative impact for patient’s life when urinary catheter inserted. The review provides insight that can be useful for nurses in concerning patient’s need about safety and comfort especially patients with urinary catheters.
