Abstract

Aerva lanata (Linn.) contains secondary metabolites with antifertility potential, such as alkaloids, tannins, saponins, steroids, and flavonoids. The increasing need for accessible, cost-effective, and reversible contraceptive options further underscores the importance of investigating plant-based antifertility agents such as Aerva lanata. This study evaluated the effects of ethanol extract of A. lanata on implantation sites and endometrial thickness in female mice (Mus musculus L., Swiss Webster). Using a posttest-only control group design, five treatment groups (200, 400, 800, and 1000 mg/kgBW) and a control group received daily extract administration for 10 days. On day 12 of pregnancy, mice were sacrificed to count implantation sites and measure endometrial thickness. Statistical analysis by ANOVA followed by Duncan’s Multiple Range Test revealed that the 800 mg/kg and 1000 mg/kg groups showed complete inhibition of implantation (100% anti-implantation activity; p< 0.05). Additionally, the endometrial thickness was significantly reduced from an average of 150 ± 10 µm in control mice to 85 ± 8 µm and 70 ± 7 µm in the 800 and 1000 mg/kgBW groups, respectively (p < 0.05). These dose-dependent reductions in implantation and endometrial thickness suggest that the ethanol extract of A. lanata is a promising antifertility agent in female mice.

Abstract

Nifedipine and amlodipine are both used in clinical practice as antihypertensive options for the management of preeclampsia. Although nifedipine is more commonly recommended, amlodipine is also used in practice, and evidence suggest that both offer comparable outcomes. Existing studies on antihypertensive use in preeclamptic patients have primarily focused on clinical outcomes, with limited evaluation from an economic perspective. Therefore, this study applied a cost minimization approach to identify which antihypertensive is more affordable. A cost-minimization analysis was conducted to compare the total cost between nifedipine and amlodipine. A cost analysis was performed using direct medical costs from a healthcare perspective. Data were collected retrospectively from January 2021 to June 2025 using a purposive sampling among uncomplicated preeclampsia patients. In addition to analyzing patient characteristics and average total cost per patient, a one-way sensitivity analysis was also performed. The results showed that the total cost significantly was lower in the nifedipine group compared to the amlodipine group (IDR 1,467,225 vs. IDR 1,938,051). However, the daily cost of amlodipine therapy use was significantly lower than nifedipine. Medication costs accounted for only 3–4% of the total treatment expenses among patients with preeclampsia. Sensitivity analysis confirmed that medical supply and nursing care cost were the most dominant factors influencing the total treatment cost. These findings indicate that nifedipine use in uncomplicated preeclampsia may contribute to overall cost efficiency in patient management.

Abstract

Papaya (Carica papaya L.) var. California leaves are widely used as traditional medicine. Several scientific studies have demonstrated that papaya leaves possess biological and pharmacological properties, with potential to be developed into a therapeutic agent. Papaya contains major secondary metabolites from the alkaloid group, including carpaine, pseudocarpaine, emetine, and dehydrocarpaine. As a major compound, we need to know its physicochemical properties, target and receptor binding properties, and potential biological functions. This research aims to determine ADME characteristics, predict target proteins, and assess drug similarity. We identified an alkaloid compound in the ethanol extract of Carica papaya L. var. California is using LC-HRMS. We utilized the Swiss ADME Webserver to analyze ADME properties, predict protein targets, and perform drug similarity analysis. We used various references to study the biological activity and effect of alkaloids on various receptor binding. LC-HRMS showed that the Carica papaya L. var. California leaves contain ethanol extracts that contain some alkaloids, including carpaine (58.3%), pseudocarpaine (2.49%), emetine (0.12%), dehydrocarpaine II (0.07%), and dehydrocarpaine I. ADME analysis showed good oral absorption, widespread distribution, no CYP-mediated interactions, and low clearance. Target prediction studies showed that papaya leaf alkaloids have three main targets: protein kinases, G protein-coupled receptors (GPCRs), and proteases. Drug similarity studies showed that papaya leaf alkaloids share structural similarities with several drugs that play roles, including anti-inflammatories, antivirals, immunosuppressants, anticancer agents, vasodilators, modulators of the nervous and cardiovascular systems, and muscle relaxants.

Abstract

Ulin seed oil has been empirically used by the people of Kalimantan as a natural hair darkener and hair growth promoter. However, standardized extraction methods and sample characteristics for ulin seed oil (USO) have not yet been clearly established. This study aims to determine the optimal. characteristics of ulin seeds for oil extraction and to identify USO adulteration using FTIR spectrophotometry combined with principal component analysis (PCA). Oil extraction was performed using the Soxhlet method with n-hexane as the solvent, employing variations in seed characteristics. Adulterant oils used to detect the authenticity of USO were candlenut oil (CNO) and virgin coconut oil (VCO). The results showed that the most optimal ulin seeds for oil extraction were fresh, mature, and resinous seeds. Adulteration of USO could be detected at wavenumbers 3008, 1745, and 1118 cm⁻¹. PCA analysis of the FTIR spectral data showed that the wavenumber 1745 cm⁻¹ is a variable that can be used to detect adulteration of USO.

Abstract

Avocado seeds, often considered agricultural waste, contain bioactive compounds, such as flavonoids and alkaloids, with promising antidiabetic properties. This study aimed to develop fast disintegrating tablet (FDT) formulations utilizing avocado seed extract with varying concentrations of the superdisintegrant croscarmellose sodium (2%, 4%, 6%, and 8%). The tablets were evaluated for their physical characteristics, including hardness, friability, disintegration time, and uniformity of weight, to ensure compliance with pharmacopeial standards. All formulations demonstrated acceptable physical properties, indicating that the incorporation of avocado seed extract did not compromise tablet quality. In vivo testing was conducted in diabetic mice to assess the antidiabetic activity of each formulation. The results revealed that all FDTs were effective in reducing blood glucose levels, with the formulation containing 8% croscarmellose sodium showing the most significant hypoglycemic effect. This enhanced activity is attributed to the rapid disintegration and improved bioavailability of the active compounds, which allows a faster onset of action. The findings suggest that avocado seed extract can be successfully formulated into fast-disintegrating tablets, providing a novel and efficient delivery system for antidiabetic therapy. Moreover, the utilization of avocado seed waste highlights the potential of natural resources in pharmaceutical innovation, offering both therapeutic benefits and sustainable value.

Abstract

Medication Therapy Management (MTM) represents a patient-centered pharmaceutical care strategy that necessitates robust interprofessional collaboration, especially between pharmacists and physicians. Within Indonesia's National Health Insurance framework administered by BPJS Kesehatan, healthcare delivery operates through a tiered referral system prioritizing primary care via First-Level Health Facilities (FKTP). The growing emphasis on strengthening primary care engagement and alleviating hospital overcrowding underscores the critical need for optimized health professional collaboration at this level. This study investigated the impact of MTM-based collaborative practice between pharmacists and physicians on clinical outcomes in type 2 diabetes mellitus patients at primary healthcare settings. A quantitative quasi-experimental approach employing a non-randomized pre–post intervention–control design was carried out across FKTP facilities in Central, South, and West Jakarta. Study participants consisted of pharmacist–physician pairs executing MTM-based collaboration alongside type 2 diabetes mellitus patients registered under the Chronic Disease Management Program (PRB). Eight collaborative pairs were established, encompassing 80 patients divided equally into intervention and control groups of 40 each. Measured clinical indicators included Fasting Blood Glucose (FBG), 2-hour Postprandial Glucose (2h-PPG), blood pressure, and total cholesterol, assessed at baseline and following a three-month intervention period. Statistical analysis utilized paired and independent tests based on data distribution, with significance established at p < 0.05. Findings revealed statistically significant improvements across multiple clinical parameters relative to baseline values (p < 0.05), encompassing glycemic indices, blood pressure levels, and lipid profiles. Within the scope of this study's limitations, MTM-driven pharmacist–physician collaboration demonstrated potential in enhancing clinical outcomes for type 2 diabetes mellitus patients in primary care settings. Subsequent research incorporating larger multicenter populations and extended intervention durations is encouraged to assess the long-term sustainability of this collaborative model.

Abstract

Hypoglycemia caused by medications is still a major safety issue for people with type 2 diabetes who are in an ambulatory therapy. To support pharmacists lead interventions, structured risk stratification is needed. The Medication-Related Hypoglycemia Risk Score Assessment Tool (HYPOGLYRISK) has been subjected to psychometric validation and digital development. However, its reproducibility among practitioners is crucial prior to broader implementation. This study assessed the inter-rater reliability and risk classification concordance of the digital HYPOGLYRISK instrument among practicing pharmacists. A methodological reliability and agreement study was performed with 46 licensed pharmacists who independently evaluated three standardized simulated patient cases categorized as low, moderate, and high risk using a digital platform. We used the intraclass correlation coefficient to look at the inter-rater reliability of total scores and the descriptive method to look at categorical agreement. The reliability test showed almost perfect agreement, with an intraclass correlation coefficient of 0.995 (95% confidence interval: 0.988-0.999). There was no difference in how the raters classified the risk categories for any case. There was perfect agreement for the clinical risk factor part, but there was a small difference in numbers for the behavioral medication safety part in the moderate-risk case, which involved 8.7% of raters. This did not change the final risk categorization. There was no misclassification between extreme risk categories. These results show that the digital HYPOGLYRISK tool gives very consistent scores and stable risk stratification across pharmacists. This finding supports its use as a standardized tool for pharmacist-led medication safety services in ambulatory diabetes care.
