Abstract

Background: Stroke is the third leading cause of death in the world. Approximately 80% of strokes are ischemic strokes. One of the significant prognostic factors causing morbidity of stroke patients is the Body Mass Index (BMI). National Institutes of Health Stroke Scale (NIHSS) is a systematic examination to determine the severity stroke. The aim of this study is to determine the relationship of body mass index (BMI) with ischemic stroke severity in patients at Neurology Ward Dr. Moewardi Hospital. Methods: This was an observational study with cross sectional analytic. A total of 32 subjects were selected by purposive sampling, they were ischemic stroke patients in the Neurology Ward Dr. Moewardi Hospital. Data were collected by direct interview, direct measurement of weight and height also examination of severity stroke patient by using NIHSS score. Data analysis used multiple logistic regression models and were processed with Statistical Product and Service Solutions (SPSS) 17.00 for Windows. Results: Multiple logistic regression showed OR = 17,690; CI 95% 1,190 – 263,039; and p = 0,037. These results have been earned by controlled the confounding variables of age, blood pressure, blood sugar levels, cholesterol levels and smoking history. Conclusions: Ischemic stroke patients with high BMI score has risk of ischemic stroke severity 17.690 times higher than whose normal BMI score.There is a strong relationship between body mass index (BMI) with the severity of ischemic stroke patients at Neurology Ward of Dr. Moewardi Hospital, the relationship is statistically significant in this study. Abstract

Background: Systemic complications that occur in children with thalassemia like hepatomegaly, hemosiderosis, bone deformities can affect nutrition disorder and growth retardation. The longer someone has suffered thalassemia, the more likely for the systemic disorder occurred. The aims of this study to find out the correlation between duration of illness with nutritional status of children with thalassemia in Dr. Moewardi Hospital. Methods: This is an observational analytic study using cross sectional design which was conducted from April to May 2013. Thirty children with thalassemia in the Dr. Moewardi Hospital were fulfill the restriction criteria. The duration of illness was acquired from medical record, while the nutritional status was obtained through measurements of weight, height, and the mid-upper arm circumference. The correlation between the duration of illness, gender, blood transfusion, iron chelating agents, splenomegaly, and 1 day food recall with nutritional status was analyzed by using Spearman test followed by the multivariate logistic regression analysis. Results: Spearman test analysis showed no significant correlation between duration of illness with nutritional status in children with thalassemia according to the weight/age and height/age indicator (p = 0,403 and p = 0,408). From the result of analysis on correlation between gender, blood transfusion, iron chelating agents, splenomegaly, and 1 day food recall with nutritional status according to the weight/age and height/age indicator (p > 0,05). Conclusions: There is no significant correlation between the duration of illness with the nutritional status of children with thalassemia according to the weight/age and height/age indicator in Dr. Moewardi Hospital. There is no correlation between gender, blood transfusion, iron chelating agents, splenomegaly, and 1 day food recall with nutritional status. Abstract

Background: Socioeconomic factors in Indonesian citizen was different on every person. Especially on work, many variants of works would cause on variants of workload. On the other side, work varied effect on different income. Earnings would affect on the food consumed. Low collagen contained food intake could affect the low level of collagen. Risk factors for uterine prolapse was higher workload, higher parity and lower collagen level. Methods: This research was an analytic obervational with case control design. Sampling technique was fixed disease sampling for the case group and random sampling for the control group. The research was conducted by interviewing a sample directly in the  obstetric clinic  Dr. Moewardi hospital The sample was 36 person, divided into 18 samples of group cases and 18 control group samples. Dependent variable was the incidence of uterine prolapse and independent variables were socioeconomic factor. Data were analyzed by chi-square test then followed by multivariate analyse  logistic regression. Results: Distribution characteristics of uterine prolapse by age most results were in the age of 40-49 (16.67%). Based on the results, the highest number of children was 4 children (16.7%). The test results with regression analysis showed significance between socioeconomic factors to the incidence of uterine prolapse. Workload had p = 0.017 and OR = 0.186, p = 0.039 whereas income., and OR = 7.208, 95% confidence interval. Nagelke R square of 38.2%. Conclusions: Socio economic factor include workload and income significantly affected incidence of uterine prolapse. Keywords: Socio-economic factors, workload, income, uterine prolapse

Abstract

Background: Asthma is a chronic inflammatory airway disease characterized by episodic wheeze, coughe, and tightness in chest due to airway obstruction. Asthma attack is usually preceded by various kinds of triggers and different in each person. Asthma management is very complex and requires collaboration between patient with their families, especially parent. In order to properly manage asthma, patient and parent should have specific knowledge about asthma. This study aimed to determine the association between parent knowledge about asthma triggers and asthma attack frequency in children. Methods: This analytic study was observational using cross sectional approach. The population of this study was child patient with asthma at RSUD Dr. Moewardi Surakarta and Balai Besar Kesehatan Paru Masyarakat. A sample of 33 study subjects was selected by purposive sampling from outpatient and inpatient with asthma who visited Pediatric Clinics, RSUD Dr. Moewardi Surakarta and Balai Besar Kesehatan Paru Masyarakat and suitable with the inclusion criteria. The data was collected by interview using a set of questionnaire from Asthma General Knowledge Questionnaire (AGKQ) and International Study of Asthma and Allergies in Childhood (ISAAC) filled by the parent of patients. The data was analyzed using Chi Square test (χ2) on SPSS 17.0 for Windows program. Results: From the analysis of relationship of parent knowledge about asthma triggers and asthma attack frequency on children obtained χ2 count rate of 5.629,  p = 0.026, and C = 0,382. The C value indicates level of relationship of parent knowledge about asthma triggers and asthma attack frequency on children is 38,2%. Conclusions: There is a statistically significant relationship of parent knowledge about asthma triggers and asthma attack frequency on children. Keywords: parent knowledge, asthma attack frequency

Abstract

Background: Health promotion can improve knowledge, attitudes and behavior, particullary knowledge of Clean and Healthy Behavior. This study aims to prove the effect of health promotion on knowledge of Clean and Healthy Behavior. Methods: This research is a pre-experimental design with the plan of one-group pretest posttest design was held on 12 April 2013 at SD Negeri Ngemplak Surakarta. The study population was all students of SD Negeri Ngemplak Surakarta totaling 230 people. While the sampling carried out by simple random sampling in grade students IV and V. The sample consisted of 32 grade students IV and 38 grade students  V. Then the total number of samples is 70 students. The independent variable in this study is a health promotion of clean and healthy behavior  in the order of school a measurement tool slides and posters. As for the dependent variable is the knowledge of clean and healthy behavior are measured using a questionnaire that have been tested for validity and reliability. Analyzed using marginal homogeneity test through SPSS 16.0 for Windows program. Results : This research shows a increased knowledge after health promotion Clean and Healthy Behavior a number of 62 students or 88.57% of all students in attendance, knowledge of students decreased a number of 2 students or  2.8% and a fixed 6 students or 8.57 % and the marginal homogeneity test results showed p = 0.000. Conclusion:. There is the influence of health promotion of clean and healthy behavior  in the order of school to knowledge clean and healthy behavior students in class IV and V SD Negeri  Ngemplak Surakarta. Abstract

Background: Aging is a natural process of gradual disappearance of the tissues ability to repair itself and maintain it’s normal structure and function. Erectile dysfunction is the inability to maintain a sufficient erection to achieve satisfaction. In line with aging, the sexual function of men will decrease. The aim of the study was to determine the relationship between elderly and erectile dysfunction. Methods: This was an observational study using cross sectional approach. The study was conducted at Geriatric Policlinic RSUD Dr.Moewardi Surakarta. Samples were elderly men who met the inclusion criteria which was set by the writer. 30 males were chosen to participate in the study. Data were collected through self-administered questionnaires and an interview. Data analyzed by Lambda correlation test. Results: Most patients with erectile dysfunction samples were elderly category (60-74 years) as many as 22 people (73.33%). Lambda correlation test showed no correlation of 0.000 indicates that there is a very weak relationship between elderly and the incidence of erectile dysfunction. Conclusions: Based on this study, it can be concluded that there was a very weak relationship between elderly and the incidence of erectile dysfunction. Keywords: Elderly, erectile dysfunction.

Abstract

Background: Asthma and COPD are most pulmonary chronic disease. Recurrence dispnea attack in both can trigger the depression. Beside that, in COPD patient has mediator proinflamation are TNF-α and IL-6 that can make severe depression. Methods: This study is an observational analytic with cross sectional approach. The subjects are moderate to severe persistent asthma and moderate to severe Chronic Ostructive Pulmonary Disease (COPD) who came to pulmonary medicine clinic of Dr. Moewardi Hospital on April until June 2013. Subject were selected through purposive sampling. The study data obtained by doing interview qoestionnaire Beck Depression Inventory II (BDI-II). The data were analyze by t-test through SPSS Program 18 for Windows. Results: Data from 39 subjects consisting of 21 moderate to severe persistent asthma patients and 18 moderate to severe COPD patients. Median score of BDI-II moderate to severe COPD (11.89) is higher than moderate to severe persistent asthma (8.71). Data was also analyze by t-test by the significance p = 0.062. Conclusion: The study suggested no significant differences of depression between moderate to severe persistent asthma and moderate to severe COPD in Dr. Moewardi Hospital. Abstract

Background    : Stroke is a disease caused high morbidity and mortality rate in low-middle income countries. Low physical activity is one of the risk factors for ischemic stroke. The impact of low physical activity is high level of NADPH, atherosclerosis, and vascular elasticity disorder. Global Physical Activity Questionnaire version 2 (GPAQv2) is a questionnaire released by WHO which has been especially designed to measure the physical activity of people in developing countries. The aim of this study is to determine the correlation between man physical activities and ischemic stroke in Dr Moewardi Hospital Surakarta. Methods: This was an analytical observational study with case control approach. This study was conducted during May - July 2013 at the Neurology Department of Dr. Moewardi Hospital Surakarta. Subjects were acute ischemic stroke patients in neurology ward of Dr. Moewardi Hospital Surakarta which were taken by using fixed disease sampling. Data were collected by interviewed patients based on GPAQv2 questionnaire. Data analysis used multivariate with double logistic regression and have been processed by Statistical Product and Service Solution (SPSS) 16.00 for Windows. Results: This study showed statistically significant (p=0,036) between man physical activities and acute ischemic stroke in Neurology Department Dr. Moewardi Hospital Surakarta. Age and smoking as comfounding factors have been controlled. Male patients with low physical activity had 13.95 times (OR = 13.95; CI 95% 1.19 – 163.58) to develop ischemic stroke than ones with adequate physical activity. Conclusion:Men who use to have less physical activity more likely to develop ischemic stroke. Keywords : level of physical activity, acute ischemic stroke, GPAQv2

Abstract

Background: Premature rupture of membrane (PROM) occurs when amniotic membranes rupture without the signs of delivery. PROM occurs in 6 – 8% pregnancies and about 80% PROM occurs at term gestational age (>37 weeks). PROM is always terminated for preventing complication whether in mother and baby. Previews studies assumed that delivery process can affect the Apgar score of the newborn. This study aimed to know the relationship between delivery process with Apgar score in premature rupture of membrane case. Methods: This analytic study was observational using cohort retrospective approach. 151 study subjects were selected by fixed exposure sampling and simple random sampling from all PROM cases on January 2011 – December 2012 period at RSUD Dr. Moewardi and fulfill the intrinsic and extrinsic criteria. The data was collected from medical record. The data was analyzed using Chi square test and binary logistic regression test with Odd Ratio (OR) which shows the score of the relationship on SPSS 20 for windows. Result: This study showed that Sectio cesaria increases the risk of worse Apgar score than labor induction, although there is no significant relationship between delivery process with Apgar score in Chi square test (p = 0.198; OR = 1.861). So do the analysis multivariate with binary logistic regression test shows p = 0.404 and OR = 1.607. Conclusion: Sectio cesaria increases the risk of worse Apgar score than labor induction in PROM case at RSUD Dr. Moewardi. Keyword: premature rupture of membrane, delivery process, Apgar score.

Abstract

Background: Thalassemia major patients need a regular blood transfusion that may cause an iron overload and vital organ dysfunction. This was an observational analytic study to analyze the relationship between ferritin level and transaminase enzymes level in thalassemia patients at Dr.Moewardi General Hospital. A positive relation may remind doctor to pay attention of liver function in thalassemia major patients. Methods: The research was an observational analytic study with cross sectional approach during April-June 2013 in Dr. Moewardi General Hospital. The samples was taken using exhaustive sampling and all were children with beta thalasemia major. These patients had a regular blood transfusion, high ferritin level (>1000 ng/l) and iron chelating agents usage. Ferritin level as independent variable was measured with ELFA method. Transaminase enzymes as dependent variables were measured with IFCC method. These variables were analyzed with Pearson correlation, diagnostic odds ratio and linier regression. Results: Thirty thalassemia patients’ test results of ferritin, ALT and AST level were collected. Eighteen patients had a very high ferritin level (≥3000 ng/l) and the other twelve patients’ ferritin level were between 1000-3000 ng/l. Eighteen patients had a high ALT level and seventeen patients had a high AST level. Pearson analysis showed a significant result between ferritin and ALT level (p=0.043) also between ferritin and AST level (p=0.028). Pearson correlation strengths were moderate (0.373 and 0.401). Odds ratio between ferritin level with ALT and AST level were 3.333 and 3.111. Conclusions: There was a positively moderate correlation between ferritin level and both ALT and AST level in thalassemia patients in Dr. Moewardi General Hospital. Odds ratio analysis showed that ferritin level above 3000 ng/l is a risk factor of ALT and AST level elevation. Keywords: Thalassemia, iron overload, ferritin, transaminase
