Abstract

Background: Systemic complications that occur in children with thalassemia like hepatomegaly, hemosiderosis, bone deformities can affect nutrition disorder and growth retardation. The longer someone has suffered thalassemia, the more likely for the systemic disorder occurred. The aims of this study to find out the correlation between duration of illness with nutritional status of children with thalassemia in Dr. Moewardi Hospital. Methods: This is an observational analytic study using cross sectional design which was conducted from April to May 2013. Thirty children with thalassemia in the Dr. Moewardi Hospital were fulfill the restriction criteria. The duration of illness was acquired from medical record, while the nutritional status was obtained through measurements of weight, height, and the mid-upper arm circumference. The correlation between the duration of illness, gender, blood transfusion, iron chelating agents, splenomegaly, and 1 day food recall with nutritional status was analyzed by using Spearman test followed by the multivariate logistic regression analysis. Results: Spearman test analysis showed no significant correlation between duration of illness with nutritional status in children with thalassemia according to the weight/age and height/age indicator (p = 0,403 and p = 0,408). From the result of analysis on correlation between gender, blood transfusion, iron chelating agents, splenomegaly, and 1 day food recall with nutritional status according to the weight/age and height/age indicator (p > 0,05). Conclusions: There is no significant correlation between the duration of illness with the nutritional status of children with thalassemia according to the weight/age and height/age indicator in Dr. Moewardi Hospital. There is no correlation between gender, blood transfusion, iron chelating agents, splenomegaly, and 1 day food recall with nutritional status. Abstract

Background: Thalassemia major patients need a regular blood transfusion that may cause an iron overload and vital organ dysfunction. This was an observational analytic study to analyze the relationship between ferritin level and transaminase enzymes level in thalassemia patients at Dr.Moewardi General Hospital. A positive relation may remind doctor to pay attention of liver function in thalassemia major patients. Methods: The research was an observational analytic study with cross sectional approach during April-June 2013 in Dr. Moewardi General Hospital. The samples was taken using exhaustive sampling and all were children with beta thalasemia major. These patients had a regular blood transfusion, high ferritin level (>1000 ng/l) and iron chelating agents usage. Ferritin level as independent variable was measured with ELFA method. Transaminase enzymes as dependent variables were measured with IFCC method. These variables were analyzed with Pearson correlation, diagnostic odds ratio and linier regression. Results: Thirty thalassemia patients’ test results of ferritin, ALT and AST level were collected. Eighteen patients had a very high ferritin level (≥3000 ng/l) and the other twelve patients’ ferritin level were between 1000-3000 ng/l. Eighteen patients had a high ALT level and seventeen patients had a high AST level. Pearson analysis showed a significant result between ferritin and ALT level (p=0.043) also between ferritin and AST level (p=0.028). Pearson correlation strengths were moderate (0.373 and 0.401). Odds ratio between ferritin level with ALT and AST level were 3.333 and 3.111. Conclusions: There was a positively moderate correlation between ferritin level and both ALT and AST level in thalassemia patients in Dr. Moewardi General Hospital. Odds ratio analysis showed that ferritin level above 3000 ng/l is a risk factor of ALT and AST level elevation. Keywords: Thalassemia, iron overload, ferritin, transaminase

Abstract

Background: Acute lymphoblastic leukemia is one of the most common malignancies in children with the highest incidence aged 3-7 years. Management and treatments such as chemotherapy and radiotherapy have musculoskeletal and neuromuscular side effects which can significantly reduce the physical activity of leukemia patients and survivors. This study aims to determine the profile of the physical activity level of children with acute lymphoblastic leukemia as assessed by the Global Physical Activity Questionnaire (GPAQ) in Surakarta, Indonesia. Subjects and Method: This was a study with a cross-sectional design. Children who survived acute lymphoblastic leukemia who had completed chemotherapy at Dr. Moewardi Hospital and met the inclusion criteria were included in this study. A sample of 26 patients was selected through purposive sampling. Demographic data were obtained through interviews and the profile of the patient's physical activity level was assessed using the GPAQ questionnaire. Data analysis was performed using a Pearson Correlation test with SPSS 22.0. Results: The total sample in this study was 26 children with acute lymphoblastic leukemia. As many as 11.5% of children with acute lymphoblastic leukemia experienced decreased physical activity. There was a significant relationship between length of stay (p=0.001), age (p=0.004), gender (p=0.031), and the degree of risk of acute lymphoblastic leukemia (p=0.004) with the category of physical activity based on the GPAQ score. Conclusion: As many as 11.5% of children with acute lymphoblastic leukemia experienced decreased physical activity. The length of time being a survivor, the degree of risk of acute lymphoblastic leukemia, the age of the survivor, and gender are risk factors for decreased physical activity in children.

Abstract

This study aims to analyze the attitude of Central Aceh society towards religious differences. Central Aceh is a region with significant religious diversity, with the majority of the population adhering to Islam. However, religious differences can also be found within the Central Aceh society, such as Christianity, Hinduism, and Buddhism. This study employs a quantitative approach by collecting data through a survey conducted among 300 respondents representing the Central Aceh community. The survey instrument used is a reliable measurement scale of tolerance attitudes. The obtained data were analyzed using descriptive and inferential statistical methods. The analysis results show that, overall, the Central Aceh society exhibits a relatively high level of tolerance towards religious differences. The majority of respondents express respect for the right of every individual to choose and practice their own religion. However, there are several interesting findings regarding the factors influencing tolerance attitudes. Factors such as education, interreligious interaction experiences, and understanding of human rights significantly influence the tolerance attitudes of the Central Aceh society. This research contributes significantly to the understanding of tolerance attitudes in the Central Aceh community and provides insights for stakeholders, including the government and educational institutions, in developing programs that promote more inclusive and respectful tolerance attitudes among communities with different religious backgrounds. The policy implications derived from this research are expected to enhance harmony and inter-religious coexistence in Central Aceh, as well as foster sustainable social development processes.

Abstract

Background The advent of chemotherapy with multimodal treatment has improved the survival rate of osteosarcoma. However, the survival rate of patients with osteosarcoma in low- and middle-income countries (LMICs) remains lower than in high-income countries (HICs). Objective To assess prognostic factors associated with survival rate of pediatric patients with osteosarcoma in Moewardi Hospital, Surakarta. Methods This retrospective study was performed in pediatric osteosarcoma patients at Moewardi Hospital, Surakarta from 2018 to 2022. We described patients’ clinical presentations and assessed prognostic factors. Survival rate was calculated using Kaplan–Meier survival analysis. Prognostic factors were analyzed using univariate and multivariate Cox regression methods. Results Of the 82 patients included in the study, 40 (48.8%) were male and 42 (51.2%) were female. Most subjects (75.6%) were diagnosed with osteosarcoma at the age of >10 years, 45 patients (54.9%) had primary tumors in the femur, and 16 patients (19.5%) had limb salvage surgery. The mean event-free survival duration was 13.6 (95%CI 9.7 to 17.5) months. The mean duration of overall survival was 22.7 (95%CI 17.3 to 28.1) months, with a 44.1% survival probability. Type of surgery was a significant prognostic factor (P=0.018), with limb salvage having better survival probability (93.3%) than amputation (19.8%). Conclusion The overall survival rate of osteosarcoma patients at our center is still lower than in other regional centers. Limb salvage surgery was a significant prognostic factor for survival, while gender, location of primary tumor, histological subtype, and metastasis at the time of diagnosis were not.
