Abstract

Background: Spinocerebellar ataxia (SCA) is a late onset neurodegenerative disorder in which coordination and balance are affected. Although many international guidelines have been established regarding presymptomatic testing, it is still a grey area in Indonesia. We report two large families with advanced stages of SCA who underwent presymptomatic genetic testing in children along counseling process.Case presentation: Thorough examination was performed, including pedigree construction, physical and neurological examination, gene mutation analyses for patients, and presymptomatic testing for family members, including children. SCA3/MJD1 gene mutation analysis was done in both cases, and a full penetrance CAG repeat expansion was found in both affected patients. Two different outcomes were observed in the offspring, who were both children. The risk and consequences of positive results had been explained in a counseling session to family members, who decided to keep the information until the child would have reached legally adult age.Conclusions: In developing countries such as Indonesia, problems arose due to ethical issues, knowledge of genetic diseases, and inaccessible molecular diagnostics. Culture, religion and tribe diversities may create additional challenges. These cases emphasize the need for careful consideration of presymptomatic testing in children, especially in complicated situations where psychological and ethical issues should be addressed.

Abstract

Background: Low zinc levels affects the relationship between the glutamatergic and serotonergic systems in major depressive disorders that cause stress and inflammation. Decreased zinc in the hippocampus can activates the HPA axis associated with an increase in cortisol. Several studies documented the relationship between zinc and clinical depression, however further research including biological measurements is needed to support these studies.Objective: To observe the correlation between zinc intake with serotonin and cortisol serum in patient with depressionMethods: This was an observational study with cross sectional design. Subjects were patients with depression who came to Dr. Kariadi Hospital, Tugurejo Hospital, Diponegoro National Hospital and Permata Medika Hospital met the inclusion and exclusion criteria. The food frequency questionnaire (FFQ) was used to assess daily zinc intake. The levels of serum serotonin and cortisol were measured using ELISA technique.Results: Of the 53 subjects, there was significant correlation between zinc intake with serotonin serum level (p=0,038), however there was no correlation between zinc intake with cortisol serum level (p=0,845)Conclusion: The higher zinc intake the higher serotonin serum level, however there was no correlation between zinc intake with cortisol serum level in patients with depression.

Abstract

Background: The COVID-19 pandemic by SARS-CoV-2 has caused many losses. One way to prevent the spread of this virus is to get vaccinated. However, the latest SARS-CoV-2 variants, including variant B.1.617.2 (Delta) are doubtful to be inhibited by existing vaccines because of mutations. Therefore, we need a new vaccine candidate that is effective against this SARS-CoV-2 variant. Through an immunoinformatics approach with various software and analysis websites, vaccine candidates can be predicted in a short time.Objective: Identity, analyze, obtain, and confirm the selected B-cell epitope sequence that can be used as a vaccine candidate for the SARS-CoV-2 B.1.617.2 (Delta) variant.Methods: This research was conducted by isolating the amino acid peptide sequence in the SARS-CoV-2 B.1.617.2 (Delta) variant protein spike from the Protein Data Bank which is suspected to be an immunogenic epitope and can be used as a vaccine candidate. A Series of tests were carried out such as antigenicity, toxicity, allergenicity, and BLAST® protein to ensure that this vaccine candidate is safe for later application into the human body. The next stage is a conservation analysis to see its potential by comparing it with the SARS-CoV-2 Delta (B.1.617.2) variant spike protein sequence in Indonesia. The study ended by mapping amino acid peptides to the SARS-CoV-2 Delta (B.1.617.2) variant spike protein using the Biovia Discovery Studio Visualizer v21.1.0.20298 2020 software to ensure that the selected sequences were epitope.Results: From the five amino acid peptides that have been isolated, the FTNVYADSFVIRGDEVRQIAPGQTGKIADYNYKLPDDFT epitope sequence has good results than the others. It is probable an antigen, non-toxic, non-allergen, and non-homolog to the human body protein.Conclusion: Based on this in silico study, it was found that the FTNVYADSFVIRGDEVRQIAPGQTGKIADYNYKLPDDFT epitope sequence was the best to be used as a vaccine candidate of SARS-CoV-2 B.1.617.2 (Delta) variant.Keywords: SARS-CoV-2 B.1.617.2 (Delta) variant, B-cell epitope, vaccine, in silico, immunoinformatics.

Abstract

Malaria is one of the deadliest infectious diseases in the world. Artemisinin-Based-Combination-Therapy (ACT) an antimalaria recommended by WHO, is starting to experience resistance in Asia. Meanwhile the natural-product-immunoprotective and antimalaria-effect may benefice for malaria. Objective of this study was to determine the effect of combination of ethanolic-extract from Songga-wood-stem (EESWS) and ACT on IL-10, a protective-cytokine against malaria-immunopathology. This experimental-study used post-test-only-randomized-controlled-group-design. The thirty-Swiss-webster-mice were grouped into 5 groups. The one-group of healthy-mice (K1), and the four-groups infected with Plasmodium berghei ANKA (PbA) which were untreated-K2-group, ACT-treated-K3-group, EESWS-treated-P1-group, and EESWS-ACT-combination-treated-P2-group. IL-10-level of stimulated-splenocytes-culture was examined by ELISA-method. Data-analysis-used was One-Way-Anova-Welch-test and Post-hoc Games-Howell. P1 and P2-groups had higher IL-10-levels than K1 (p=0.038). Groups of P1 and P2 showed lower IL-10-levels than K3 (p=0.001). IL-10-level of P2-group was not different than P1 (p=0.135). The conclusion is the EEWS or EESWS-ACT-combination-therapy restrains the increase-splenic-IL-10-production above normal value in the healing phase of malaria infection.

Abstract

Background: Increasing number of the older adults population results in increasing sarcopenia, a geriatric problem that may lead to poor quality of life, susceptibility to disease, malnutrition, and even death. Muscle mass is an important sarcopenia parameter that can be measured by Bioelectrical Impedance Analysis (BIA). Detection of decreased muscle mass can be done by measuring calf circumference, it is expected to provide an early diagnose of sarcopenia so that early intervention can be given and improve the quality of life of the older adults.Objective: To analyze the diagnostic accuracy of calf circumference for decreased muscle mass in older adults to provide simple way in diagnosing sarcopenia.Methods: This study involved 126 older adults, consisted of 57 men and 69 women aged 60-80 years in the community who met the inclusion criteria. Criteria of sarcopenia were defined based on the Asian Working Group for Sarcopenia (AWGS) Consensus, consisted of three components; muscle mass, handgrip strength, and walking speed. This study analyze the diagnostic accuracy of calf circumference for decreased muscle mass measured by single- frequency BIA and calf circumference was measured using a measuring tape. The analysis was carried out according to the receiver operating characteristic (ROC) curve to determine the cut-off point along with the sensitivity (Se) and specificity (Sp) values, positive and negative predictive values (PPV and NPV) of calf circumference as an indicator for low muscle mass.Results: Optimal cut-off point of calf circumference to indicate low muscle mass is 32.9 cm in women (Se 80.8%, Sp 79.1%, PPV 75.9%, NPV 87.5%) and 33.5 cm in men (Se 78.6%, Sp 74.4%, PPV 50%, NPV 91.4%). PPV in men is lower than women. This is due to a lower prevalence of decreased muscle mass in men than women. There were 49 participants with the calf circumference below cut-off point and 40 (31.7%) of the 126 participants had sarcopenia.Conclusion: Calf circumference has a diagnostic accuracy to find decreased muscle mass in  sarcopenia.

Abstract

ABSTRACT Background:Intra ocular pressure (IOP) elevationis one of topical steroids’ side effects. Corticosteroid will initiate matrix metalloproteinase cascade that leads to extra cellular matrix (ECM)of trabecular meshwork (TM) turnover(remodeling) and its acumulationin TM. Objective: To analyze the reversibility of ECM TM in Wistar rat after discontinuation oftopical prednisolone1% at different periode(14 days and 28 days). Methods:This was an experimental study with post test only controlgroup design. Total samples of 28 rats were divided into 4 groups: Treatment groups 1 was treated with  topical prednisolone for 28 days, and was terminated 14 days after discontinuation of the drug.Treatment goups2 was treated topical prednisolone for 28 days and was terminated28 days after discontinuation of the drug. Control 1 was treated withtopical prednisolone for 28 days, and Control 2 was treated with topical saline for 28 days and terminated without periode of discontinuation. Histopathological grading score was used to evaluate ECM TM deposition. Mann-Whitney test and Kruskal-Wallis test were used to analyze the data. Result: Deposition of ECM in TM was not statistically different intreamentgroup 1 and treatment group 2 (p>0.05). Deposition of ECM in TM were statistically differentbetween treatment group 2 and control group 1 (p<0.05). Comparative test showed p<0.001,which means that there was a change in the  thickness of ECM after discontinuation of instillation. Conclusion: ECM TM was thinner in the experimental animals with a longer duration of topical prednisolondiscontinuation, which demonstrate that maintenance remodeling of ECM was happen.

Abstract

Background: The SARS-CoV-2 rapid diagnostic test antibody (RDT-Ab) was most often used as an early detection tool for COVID-19 at the beginning of pandemic. Whereas the antibody response was formed in the second week after the onset of symptoms.Objective: To evaluate the diagnostic value of the SARS-CoV-2 RDT-Ab, including sensitivity (Se), specificity (Sp), positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (PLR), and negative likelihood ratio (NLR), in patients at Diponegoro National Hospital, Semarang, Indonesia.Methods: Data subjects have been selected retrospectively using purposive sampling based on inclusion criteria (patients who had shortness of breath, pneumonia, suspected, possible, or confirmed COVID-19, and data on the results of the SARS-CoV-2 RDT-Ab IgM and/or IgG (Leccurate® SARS-CoV-2 Antibody Rapid Test Kit) with a valid RT-PCR as gold standard) and exclusion criteria (patients who only had one of either SARS-CoV-2 RDT-Ab or RT-PCR). Researchers analyzed the diagnostic value of SARS-CoV-2 RDT-Ab with RT-PCR which gave the possibility of true-positive, false-positive, true-negative, and false-negative results arranged in a 2x2 table. According to WHO, the diagnostic value is said to be good at least having a sensitivity value of 80% and specificity of 97%.Results: The diagnostic value of SARS-CoV-2 RDT-Ab with RT-PCR, which was evaluated from 1142 patients retrospectively, included IgM (Se 65.25%, Sp 89.51%, PPV 46.70%, NPV 94.81%, PLR 6.22, NLR 0.39), IgG (Se 58.16%, Sp 93.01%, PPV 53.95%, NPV 94.04%, PLR 8.32, NLR 0.45), IgM and IgG (Se 53.90%, Sp 94.21%, PPV 56.72%, NPV 93.55%, PLR 9.30, NLR 0.49), IgM and/or IgG (Se 69.50%, Sp 88.31%, PPV 45.58%, NPV 95.36%, PLR 5.95, NLR 0.35).Conclusion: SARS-CoV-2 RDT-Ab (Leccurate® SARS-CoV-2 Antibody Rapid Test Kit) is not ideal to be used as a rapid diagnostic test for COVID-19.Keywords: COVID-19, Rapid diagnostic test, RT-PCR, SARS-CoV-2 antibody

Abstract

Overweight/ obesity is associated with cardiovascular diseases, which both contribute to the severity of coronavirus disease 2019 (COVID-19). Nutritional interventions focusing on dietary fibre and prebiotics interventions have been implemented. Fibre has been suggested to modulate gut-derived metabolites short-chain fatty acid (SCFA). We conducted a systematic review on fibre (including prebiotics) interventions to depict its effect on SCFA from faecal and blood using standard methodologies. PubMed, Cochrane, Embase, CINAHL, and Scopus databases were systematically searched to yield peer-reviewed articles published until 31 December 2021. We included 17 articles describing fibre (including prebiotics) intervention in adult individuals with overweight/obesity. These interventions were broadly described into 3 groups: (i) fibre type food items (n = 8); (ii) fibre supplementations (i.e. prebiotics) (n = 8); (iii) prebiotic supplementation combined with CRD (n = 1). Fibre type food items intervention mostly affected the changes of acetate in faecal, whilst propionate mostly changed in the blood. Interestingly, intervention with fibre supplementation affects more the increase of faecal and blood acetate. Furthermore, fibre intervention might have an impact on the gut microbiota. Nevertheless, more well-controlled human studies are needed, with a more personalized approach.

Abstract

Background: Spleen plays a role in human immune system as well as in recycling and filtering blood. The aging spleen is associated with fibrosis and impaired immune system, increasing individual vulnerability to getting infections. Regular physical activity is essential to maintain and enhance body fitness, endurance, and immune system.Objective: This study aimed to investigate the effects of mild and moderate-intensity treadmill exercise on the aging spleen by examining fibrosis and the number of macrophages in d-Galactose-induced aging rat models.Methods: Twenty-four male Wistar rats were randomly divided into four groups: G1 (control group), G2 (d‐Galactose, no exercise), G3 (d‐Galactose, low-intensity exercise), and G4 (d‐Galactose, moderate-intensity exercise). d-Galactose was administered intraperitoneally on day 0 and treadmill exercise was given for four weeks following the modified Brown et al. (2007) protocol. Spleens were histologically processed and stained for picrosirius red and against CD68+ antibody. Percentage of fibrosis fraction area and macrophage cell count were obtained using ImageJ, and the data were analyzed statistically using SPSS software.Results: The aging spleen did not show any differences in weight, length, and width among groups (P > 0.05). The administration of d-Galactose in rats causes fibrosis and an increased number of macrophages. Low and moderate-intensity treadmill exercise could not lower the percentages of fibrosis fraction area. However, the moderate-intensity treadmill exercise was effective in lowering macrophage number.Conclusion: Moderate-intensity treadmill exercise was effective in lowering macrophage cell count in the aging rat spleen induced by d-Galactose.

Abstract

Background: High level of malondialdehyde (MDA) and low level of superoxide dismutase (SOD) are markers of oxidative stress and indicate enhance metabolic syndrome. Tempeh gembus is a food that has antioxidant properties which can decreased MDA concentration and increased SOD enzyme activity.Objective: The aim of this research was to determine the effect of Tempeh gembus on plasma MDA and serum SOD enzyme levels in metabolic syndrome rats.Methods: A post-test only experimental design was used in which 25 Sprague Dawley rats were divided into 5 equal groups of 2 control groups (K- and K+) and 3 treatment groups (P1, P2, P3). Metabolic syndrome was induced in the K+ control and the 3 treatment groups. Tempeh gembus were given for 4 weeks with the doses of 2.5g (P1), 5g (P2), and 7.5g (P3). Plasma MDA levels were measured by the Thiobarbituric Acid Reactive Substance (TBARS) method and serum SOD enzyme levels were measured by the Enzyme Linked Immunosorbent Assay (ELISA) method. The statistical analysis used One Way Anova test.Results: Tempeh gembus significantly decreased plasma MDA levels of metabolic syndrome rats (P=0.000) but had no significant effect on serum SOD enzyme levels in each treatment group (P=0.449).Conclusion: The dose of 7.5g Tempeh gembus was the most effective dose in reducing the plasma MDA level. Tempeh gembus in the 3 treatment doses significantly decreased plasma MDA levels but had no significant effect on serum SOD enzyme levels.
