Abstract

C-reactive protein is an acute-phase protein produced by the liver in response to inflammation. Dyslipidemia can trigger the inflammatory reaction in the blood vessels, causing atherosclerosis. The ratio of total cholesterol/High Density Lipoprotein (HDL) is considered as one of the sensitive predictor lipid ratio for the cardiovascular disease risk factor. Objectives: To determined the correlation of C reactive protein between cholesterol levels with total cholesterol/HDL ratio in obese people. Methods: This study was an analytical study with a cross-sectional design of 59 people who met the inclusion and exclusion criteria at the RSUP Dr. M. Djamil Padang from January to September 2019. Research subjects consisted of first degree obesity (25.0 ≤IMT <30.0 kg/m2) and second degree (≥30.0 kg/m2). C reactive protein levels were measured as high-sensitivity C-reactive protein. Total cholesterol and HDL levels were measured using automated clinical chemicals with the enzymatic colorimetric method. Data were analyzed by Pearson, significant if p <0.05. Results: The study subjects were 59 people (17 men and 22 women) with an average age was 35 (8.04) years. CRP levels in stage II obesity were slightly higher than grade I, with a mean difference between groups of 0.26 mg/L (95% CI: -1.80-1.2; p=0.74). The average ratio of total cholesterol/HDL levels in the obese group I and II was 5.13 and 4.80 (p=0.93). The correlation showed a very weak positive (r=0.12) and was statistically not significant (p=0.35). The results of the subanalysis showed relatively similar patterns between the first-degree obesity population (r=0.10; p=0.58) and second-degree (r=0.16; p=0.41). Conclusion: There is no significant correlation between protein C-reactive and total cholesterol/HDL ratio.Keywords: Obesity, C-Reactive Protein, total cholesterol/HDL ratio

Abstract

Microscopic examination of urine sediment using shih-yung system requires 10ml urine volume. Pediatric patients and kidney disease patients with oliguria difficult to get 10mL urine samples, so it needs to be studied to determine the accuracy of results with smaller sample volumes. Objectives: To analyze agreement of urine sediment examination using Shih-yung system in 10 mL and 5 mL urine volume. Methods: This study was a cross-sectional analysis of 37 urine specimens that met inclusion and exclusion criteria at the central laboratory of Dr. M. Djamil Hospital from July to September 2020. Examination of erythrocyte sediment, leukocyte sediment, and non-hyaline cast using Shih-yung system was done in 10 mL and 5 mL urine volume. Data were analyzed using kappa reliability/agreement test and significant if p<0.05. Results: Agreement test of erythrocyte sediment examination using Shih-yung system in 10ml and 5mL urine volume were moderate significantly (κ=0.600; p<0.05), agreement of leukocyte sediment examination were moderate significantly (κ=0.571; p<0.05), and agreement of nonhialin cast examination were substantial significantly (κ=0.767; p<0.05). Conclusion: Agreement test of erythrocyte and leukocyte sediment examination in 10ml and 5mL urine volume is moderate, so 5ml urine volume was not recommended. Manual examination with microscope and difference in urine sample volume causing difference sediment concentration and amount of sediment per microscope field of view.Keywords: Shih-yung,  urine sediment,  urine volume

Abstract

The COVID-19 pandemic has caused substantial morbidity and mortality worldwide. Older patients, male gender and those with preexisting comorbidities such as chronic kidney disease are reported to be more likely infected with SARS CoV-2 and are at higher risk of severe illness or death. It has been reported a 24 years old male was admitted to the hospital with shortness of breath, coughing, fever and paleness. The history of contact with confirmed COVID-19 cases was unclear. The patient works as a security officer. A history of hypertension is present. Laboratory results showed hemoglobin 7 g/dl, ureum 261 mg/dl, and creatinine 22,9 mg/dl. On the second day of admission, the patient experience increased shortness of breath, decreased consciousness and epistaxis. From the result of the nasopharyngeal swab, the patient tested positive for COVID-19 and was given Oseltamivir 75mg. The patient is prepared for hemodialysis, which was performed in the isolation room. After hemodialysis, the patient's condition improved with decreased shortness of breath and increased of consciousness. The patient comes out from the isolation room and discharges home in good condition. Antiviral therapy in CKD patients with Covid-19 infection requires dose adjustment. Immediate hemodialysis is required in patients with CKD and coexisting COVID-19 infection to improve the patient's condition. Prompt management for patients with CKD and COVID-19 will reduce the risk of mortality.Keywords:  COVID-19, chronic kidney disease, hemodialysis

Abstract

Platelet-rich fibrin (PRF) has a lot of benefits in the process of healing. The new generation of PRF, namely advanced PRF plus (A-PRF+), has a different fibrin structure and more growth factors compared to Advanced PRF (A-PRF), the generation before A-PRF+. Objectives:To compared the effects of A-PRF and A-PRF+ on the IIB degree burn. Methods: This experimental study used 20 male Sprague Dawley rats. All rats were given IIB degree burn and divided into four groups. Group 1 was treated with saline, Group 2 was treated with silver sulfadiazine 1%, Group 3 was treated with A-PRF, and Group 4 was treated with A-PRF+. Wound size reduction will be calculated every two days and measured with ImageJ and the wound would be made into a histological slide on day 14 to calculate angiogenesis. Results: Changes in the wound surface area for the group that was given A-PRF dan A-PRF+ are 2,333cm and 4,827cm. The wound surface area’s data were analyzed using Kruskal-Wallis and the results are statistically not significant (p>0,05). The number of angiogenesis for the group that was give A-PRF and A-PRF+ as followed are 5,88 and 4,867. The angiogenesis data were analyzed using one-way ANOVA and the results are statistically not significant (p>0,05). Conclusion: There is no difference between the effect of A-PRF and A-PRF+ towards changes in wound surface area and angiogenesis in IIB degrees burn wounds.Keywords:  A-PRF, A-PRF+, burn wound, angiogenesis, wound surface area

Abstract

Salah satu bentuk predominan dari lesi oksidatif yang diinduksi oleh radikal bebas adalah 8-hidroksi-deoksiguanosin (8-OHdG). Paparan sinar matahari pada sel, terutama sel kulit dapat menyebabkan reaksi fotooksidasi yang terjadi akibat pelepasan reactive oxygen species (ROS). Lama paparan sinar matahari merupakan salah satu faktor penentu berapa banyak sel kulit yang mengalami stres oksidatif. Tujuan: Menganalisis hubungan lama paparan sinar matahari dengan kadar 8-OHdG urin pada remaja perempuan. Metode:  Ini merupakan penelitian observasional analitik dengan desain cross-sectional dilakukan pada populasi mahasiswi Pendidikan Dokter Universitas Andalas dengan teknik systematic random sampling (n = 110). Lama paparan sinar matahari diperoleh melalui kuesioner dan kadar 8-OHdG diukur dengan metode ELISA. Analisis data dilakukan dengan uji Kruskal-Wallis. Hasil: Rerata usia subjek pada penelitian ini adalah 20,6 ± 1,23 tahun, rerata lama paparan sinar matahari didapatkan sebesar 49,01 ± 36,96 menit. Rerata kadar 8-OHdG pada subjek adalah 40,75 ± 39,62 ng/ml. Analisis bivariat menunjukkan bahwa lama paparan sinar matahari tidak berhubungan dengan kadar 8-OHdG urin (p = 0,396). Simpulan: Tidak terdapat hubungan antara lama paparan sinar matahari dengan kadar 8-OHdG urin pada remaja perempuan.Kata kunci: 8-OHdG urin, lama paparan sinar matahari, stres oksidatif

Abstract

Mycetoma is a chronic granulomatous infectious disease of the skin, subcutis, fascia and bones caused by bacteria (actinomycetoma) or fungi (eumycetoma) in adults aged 20-40 years old, male to female ratio is 3,7:1 respectively. The clinical finding is a painless, solid and cold subcutaneous node. It has been reported a 43-year-old woman with a painless suppurative lump in her chest, stomach, armpits and legs in the past year. The lump were hypertrophic scars, atrophic scars, nodules and erythematous papules with pustules and sinuses, which excretes exudate greenish-yellow. The patient had a history of working in an oil palm farm. Histopathological examination of hematoxylin-eosin showed suppurative granulomatous inflammation consisting of epitheloid histiocytes, solid lymphocytes and neutrophils, and datia cells. PAS (Periodic acid-Schiff) staining showed positive results. The diagnosis of deep mycosis eumycetoma was confirmed based on physical examination and histopathological examination. Eumycetoma is a rare deep fungal disease. Supporting investigations such as direct microscopy, histopathology with biopsy, and culture are needed to establish the diagnosis. The therapies are antifungal drugs and surgery. Death is possible if the infection occurs in the chest or head cavity.Keywords: deep mycosis, eumycetoma, granules, mycetoma, sinuses

Abstract

Depression is a mental disorder that is often found in hemodialysis patients. Depression had bad effects on patients. The study that determines the relationship between depression and all-cause mortality risk in hemodialysis patients is debatable. Objectives: To determine the role of depression as a predictor factor of all-cause mortality within one year in hemodialysis patients. Methods: This cohort study was conducted from February to August 2020 at the hemodialysis unit of Roemani Muhammadiyah Hospital, Semarang. The inclusion criteria were undergoing hemodialysis ≥3 months, hemodialysis frequency twice a week, being willing to participate in this study, being able to communicate well, and there was no history of previous mental disorders. Meanwhile, those whose data were incomplete, transferred to another hemodialysis unit, HBSAg +, and had other stressors unrelated to hemodialysis or Chronic Kidney Disease (CKD) were excluded from this study. Depression was assessed using the Beck Depression Inventory-II questionnaire. The collected data was then analyzed using Kaplan Meier and Cox regression. Results: From 82 respondents, 20 (24,4%) respondents categorized into depression group and 62 (75,6%) respondents into non depression group. Cox Regression analysis showed that depression plays a role as a predictor factor of all-cause mortality within one year in hemodialysis patients (p value=0,007, and Hazard ratio=3,587). Conclusion: Depression plays a role as a predictor factor of all causes of mortality within one year in hemodialysis patients.Keywords: depression, hemodialysis, mortality, end-stage renal disease

Abstract

Adiponectin and HDL cholesterol have a protective cardiovascular effect. Adiponectin levels decrease in obese people. This causes ATP-binding cassette transporter A1 expression and apolipoprotein A-I synthesis in hepatocytes to decrease, resulting in impaired HDL cholesterol synthesis. Objectives: To determined the correlation between adiponectin levels with HDL cholesterol levels in obese. Methods: This was the analytic study with a cross-sectional design of 59 obese people who met inclusion and exclusion criteria at RSUP dr. M. Djamil Padang, from January to August 2019. Research subjects were grouped into two groups based on WHO Asia Pacific body mass index (BMI) classification. First-degree obesity group (25.0 £ BMI <30.0 kg/m2) and second-degree obesity group (BMI ≥ 30.0kg/ m2). Adiponectin levels were examined by the enzyme-linked immunosorbent assay (ELISA) method and HDL cholesterol levels by the enzymatic colorimetric method. Data analyzed by Pearson correlation test, significant if p <0.05. Results: Research subjects 59 people (17 men, 42 women), average age 35 (8.04) years. Adiponectin levels in the first-degree obesity group were higher than second-degree obesity group, with a mean difference between groups 1.03 μg/mL (CI: 95%: 0.14 - 1.92, p = 0.02). Results showed adiponectin levels positively correlated with HDL cholesterol levels of moderate strength (r = 0.45) and statistically significant (p = <0.001). The results of the population subanalysis show relatively similar patterns. First-degree obesity population (r = 0.55; p = 0.002), second-degree obesity population (r = 0.41; p = 0.026). Conclusions: Adiponectin levels correlate with HDL cholesterol levels.Keywords: adiponectin, HDL cholesterol, obesity

Abstract

ABSTRAKLatar Belakang: Malnutrisi pada balita masih merupakan masalah kesehatan di Indonesia dan erat kaitannya dengan perawatan rumah sakit. Laporan kasus ini ditulis untuk memberi gambaran mengenai kasus gizi buruk sehingga semakin ditingkatkannya perhatian terhadap gizi anak oleh para tenaga kesehatan di lapangan.Kasus: Anak perempuan berusia 12 bulan datang dengan keluhan sudah 1 bulan berat badan tidak bertambah dan nafsu makan berkurang. Keluhan disertai batuk dan pilek  berulang serta demam. Ayah dan ibu merupakan keluarga dengan perekonomian menengah ke bawah sehingga anak hanya diberi makan seadanya. Pada pemeriksaan fisik, anak sadar, tampak sangat kurus dan sakit sedang, didapatkan berat badan menurut usia, panjang badan menurut usia, berat badan menurut panjang badan di bawah -3 SD grafik pertumbuhan WHO. Pemeriksaan toraks menunjukkan kecurigaan terhadap infeksi pada paru kanan. Hasil laboratorium menunjukkan anemia defisiensi besi dengan peningkatan CRP dan foto toraks memperlihatkan adanya infiltrat pada paru kanan. Anak dirawat di rumah sakit dengan pemberian terapi cairan, antibiotik, dan intervensi diet. Dalam 2 minggu keadaan anak menujukkan perbaikan yang bermakna dan dapat dipulangkan dengan memberi edukasi terkait gizi anak, perilaku hidup bersih dan sehat, serta pentingnya imunisasi.Simpulan: Gizi buruk pada balita harus menjadi perhatian karena 1000 hari pertama kehidupan merupakan periode perkembangan otak yang paling pesat. Pemahaman yang baik oleh tenaga medis di lapangan mengenai gizi anak sangat diperlukan dalam rangka memberi edukasi kepada masyarakat untuk mengurangi angka kejadian gizi buruk pada balita Indonesia.Kata kunci: Gizi buruk, balita, edukasi ABSTRACTIntroduction: Malnutrition in toddlers is still a problem in Indonesia and is related to hospitalization. This case is written to give an overview about severe malnutrition in children so that we, as healthcare workers will be more aware to children’s nutrition.Case: Twelve – month – old baby girl came with chief complaint of difficulty in gaining weight. This complaint was accompanied by reduced appetite and fever with repeated cough and cold. Her parents were from middle – to low – income family so this baby wasn’t fed well. On examination, she looked extremely thin. She was alert with weight – for – age, height – for – age, and weight – for – height <3 Z – score WHO growth chart. Thoracal examinations showed abnormalities on right chest. Laboratory findings showed anemia hypochromic microcytic and increased CRP. Chest radiograph showed infiltrates on right lung. She was hospitalized and treated with fluid therapy, antibiotic, and diet intervention. After 2 weeks, her conditions improved and she was discharged. Her parents were educated about feeding practices in children, hygiene, and the importance of immunization.Conclusion: Malnutrition in toddlers has to be a concern because the first 1000 days are the most dramatic phase of neurobehavioral development. Understanding children’s nutrition is essential to all healthcare workers in order to decrease the incidence of malnutrition in Indonesia.Keywords: Malnutrition, children, education

Abstract

The diagnosis of typhoid fever can be made by hematological examination and the Widal method. Hematologic change that often occurs in typhoid fever is thrombocytopenia. Platelet function can be observed by examining the platelet index, namely the Mean Platelet Volume (MPV), and Platelet Distribution Width (PDW). The titer on the serological examination of the Widal test is a significant diagnostic for infections caused by the bacterium Salmonella typhi. Objectives: To determine the relationship between the Widal titer and the number and index of platelets in patients with typhoid fever at the West Lombok Health Center. Methods: The research methodology used is an analytic observational method with a cross-sectional approach. This study was conducted from February 2021 to April 2021. The population in this study was all patients who underwent laboratory examinations diagnosed as positive for typhoid fever in the Gunungsari Health Center, West Lombok Regency, with a sample of 24 respondents. Data analysis using Spearman correlation. Results: Statistical test results obtained P≥0.05, namely the titer value against platelets p=0.429, the titer against the MPV platelet index p=1,000, PDW p=0.291. Conclusion: there is no relationship between widal titer to the number of platelets and the platelet index in patients with typhoid fever.Keywords:  typhoid fever, platelets, platelet  index, widal titer
