Abstract

Pregnancy is a physiological phase that needs to be prepared as well as possible. Sometimes when feeling complaints or symptoms of illness, pregnant women decide to take medication. However, most pregnant women do not know the safe use of drugs during pregnancy. This can certainly affect the health of the mother and the fetus in her womb. Therefore, it is necessary to provide information about self-medication so that pregnant women have sufficient knowledge of the right and safe drug consumption patterns. Based on these problems, this community empowerment program (PKM) is carried out to increase the capacity of village midwives. For this purpose, the study employed a quasi-experimental method. A total of 25 village midwives participated in the research. The program that will be carried out includes providing self-medication materials and communication methods, and training on how to make an RTL (follow-up action plan). After that, village midwives were taught to apply the snakes and ladders media as a means of delivering educational materials to pregnant women. The results of the activity show that there was an increase in the knowledge of village midwives after participating in the community empowerment program, leading to the conclusion that this snakes and ladders media can make it easier for pregnant women to understand the right self-medication during pregnancy so that pregnant women have additional knowledge and benefits in managing themselves regarding the right drug consumption.

Abstract

Background: The Surabaya City Government launched the Sayang Warga (Citizens' Love) application as a means of real-time data collection and reporting of PHBS indicators to support the Clean and Healthy Living Behavior (PHBS) Program. However, its implementation faced various technical and non-technical obstacles that impacted data quality. Objective: This study aims to identify the implementation challenges of the Sayang Warga application and analyze their root causes and alternative solutions based on the Diffusion of Innovation Theory perspective. Methods: This qualitative descriptive study used a health information system evaluation approach at the Surabaya City Health Office and involved seven informants, including Puskesmas heads, officers, and health office staff. Data were collected through interviews, observations, and document reviews and were analyzed thematically. Problem prioritization used the USG method with FGDs, followed by fishbone analysis and interpretation using the Diffusion of Innovation Theory. Results: The study identified obstacles in implementing the “Sayang Warga” application within the Surabaya City PHBS program, particularly related to complexity, compatibility, observability, and the innovation diffusion stages of knowledge and confirmation. The main issue was the absence of a feature to distinguish between inputted and non-inputted data, leading to potential duplication. Conclusion: A system update is recommended to include an automatic data cleansing feature and a warning notification for duplicate inputs or unsaved data. This innovation is expected to enhance data validity, efficiency, and overall quality of PHBS reporting, thereby supporting the optimization of the Surabaya City health information system.

Abstract

Background: The implementation of Indonesia’s National Health Insurance (JKN) has expanded healthcare access. As of December 31, 2024, JKN coverage had reached 98.45%, yet disparities persist across socioeconomic groups and geographic regions. These disparities contribute to unequal service quality, low utilization, and financial burdens among low-income households. Objective: This study identifies determinants of disparities in healthcare utilization in Indonesia using Andersen’s Behavioral Model and provides an evidence base for policymakers. Methods: A systematic review was conducted following PRISMA guidelines. Searches of CrossRef, Scopus, and PubMed (2015–2025) identified studies on healthcare utilization and disparities in Indonesia. Eligible articles were screened in Covidence and thematically synthesized using Andersen’s Behavioral Model to classify determinants into predisposing, enabling, and need factors. Results: Enabling factors, such as socioeconomic status, insurance ownership, and geographic access, were the strongest determinants, compared with predisposing factors (age, sex, marital status, and educational background) and need factors (self-rated health, chronic conditions, perceived healthcare needs). High health needs did not always lead to service use, particularly among low-income and remote populations. Financial protection alone is insufficient without improvements in health literacy, equitable workforce distribution, and stronger primary care. Conclusion: Although JKN has expanded access, disparities remain. An integrated approach is needed to strengthen primary care, ensure equitable health worker distribution, improve health literacy, and enhance geographic access. These actions can shift JKN’s focus from coverage expansion to equitable service access, supporting Indonesia’s progress toward Universal Health Coverage (UHC) and the Sustainable Development Goals (SDGs).

Abstract

Background: Strengthening family and community capacity for healthy eating remains a key challenge in health promotion, particularly in Indonesia, where child stunting persists. Objective: This study aimed to develop a salutogenic food-based taiyaki using moringa leaves and mung bean flour and to interpret its potential role in strengthening family and community capacity for healthy eating. Methods: A two-phase descriptive analytic design was applied. Phase 1 involved experimental food product development through five formulation trials using partial substitution with moringa leaves and mung bean flour, followed by expert sensory evaluation and nutritional content analysis using NutriSurvey software. Phase 2 consisted of a literature-based salutogenic health promotion analysis guided by salutogenesis theory, health promotion principles, and a social marketing framework to interpret potential contributions to capacity building. Results: The optimized taiyaki formulation with moringa leaves and mung bean flour demonstrated good sensory acceptability by expert panelists, alongside improved nutritional content, particularly in protein, iron, zinc, dietary fiber, and selected vitamins. These findings indicate that familiar snack foods can be nutritionally enhanced using locally available ingredients without compromising their acceptability. A Salutogenic perspective showed that the product’s familiar format and clear nutritional value enhance comprehensibility, the use of accessible ingredients supports manageability, and the culturally acceptable, child-friendly presentation strengthens meaningfulness. Conclusion: Food-based taiyaki modified with moringa leaves and mung bean flour shows potential as a salutogenic health promotion strategy to strengthen family and community capacity for healthy eating and provides a foundation for future community-based implementation.

Abstract

Background: In Indonesia, approximately 7.7% of individuals aged 10–17 years live with disabilities, representing a substantial portion of the adolescent population. These adolescents experience puberty similarly to their non-disabled peers but face unique social and cognitive challenges that may influence their sexual behaviour. This study aims to identify the determinants of sexual behaviour among adolescents with hearing and intellectual disabilities. Method: A quantitative, cross-sectional study was conducted at a Public Special Needs School in Semarang City. The study population consisted of 73 students with hearing impairments and 101 students with intellectual disabilities. Based on total sampling and predefined inclusion criteria, 34 adolescents with hearing impairments and 38 with intellectual disabilities were included as participants. Data were collected using validated, closed-ended questionnaires administered with the assistance of teachers. Statistical analyses were performed using Spearman’s rank correlation and the Mann–Whitney U test. Result: Attitudes (p = 0.004), peer influence (p = 0.001), and access to health information (p = 0.015) were significantly associated with sexual behaviour. However, no significant differences in sexual behaviour were found between adolescents with hearing impairments and those with intellectual disabilities. Among adolescents with hearing impairments, peer influence and access to information were the strongest predictors, whereas among those with intellectual disabilities, attitudes and peer roles emerged as the dominant determinants. Conclusion: Sexual behaviour among adolescents with disabilities is influenced by both interpersonal and cognitive factors. School-based programmes that enhance peer support, foster positive attitudes, and improve access to accessible reproductive health information are essential for promoting safer sexual behaviour within this population.

Abstract

Background: The widespread use of the internet among adolescents has transformed how they access health information. However, the abundance of unverified online content poses risks for individuals who lack the skills to assess its credibility. Digital health literacy is therefore essential for enabling adolescents to seek, evaluate, and use health information responsibly. Objectives: This study examined the relationship between digital health literacy and health information–seeking behavior among Indonesian adolescents, providing empirical evidence to inform digital health promotion strategies. Methods: A cross-sectional analytic study was conducted with 524 adolescents aged 12–19 years from five provinces in Indonesia. Data were collected using an online questionnaire comprising the eHealth Literacy Scale (eHEALS) and a Health Information–Seeking Behavior (HISB) instrument developed by the researchers. Descriptive statistics were used to profile respondents, and Spearman’s rank correlation was applied to assess the relationship between variables. Results: Overall, 38.2% of adolescents demonstrated high, 46.7% moderate, and 15.1% low digital health literacy. Adolescents with higher literacy levels exhibited more active and critical health information–seeking behavior, including more frequent use of credible sources, information verification, and appropriate application of information in decision-making. A significant positive correlation was found between digital health literacy and health information–seeking behavior (ρ = 0.307; p = 0.007), indicating that digital health literacy substantially influences adolescents’ engagement with online health information. Conclusion: Digital health literacy plays an important role in shaping adolescents’ health information–seeking behavior. Strengthening digital health literacy should be prioritized in health promotion initiatives targeting young populations in the digital era.

Abstract

Background: Dengue Hemorrhagic Fever (DHF) is still a significant burden to global public health. Successful prevention is a function of community capacity, including locus of control (internal belief in one’s ability) and collective potential to implement prevention. Objectives: The objective of this research was to measure K, A and P with a specific emphasis on examining the degree to which these mentored K, A and P align with community sense-making (meaning), constructed capacity (function), and perceived controllability for making useable space free from vectors. Methods: A cross-sectional design was used, with 102 respondents (mainly women, productive age group and primary education level). Descriptive statistics, Chi-Square test for bivariate analysis and Multivariate Logistic Regression were used to determine independent predictors. Results: A noticeable gap between the KAP was determined. Although the proportion of respondents with favorable attitudes is high (66.7%), that of preventive practices was low (54.9% poor score). There was marginal knowledge (45.1% poor) with a high proportion of critical misconceptions (80.4% incorrect response to key transmission point). On multivariate analysis, significantly associated with good knowledge were education (Junior High vs Primary School) (OR=4.1, p=0.015) and middle income for favorable attitudes (OR=2.9, p=0.041). Notably, neither demographic variable (age, gender, education nor income) showed association with good preventive practice. Conclusion: The poor practice indicates a lack in the ability of DHF preventive activities, i.e. the capacity and collective control despite of high motivation (attitude). That practice was not also predicted by demographics implies that barriers are structural and environmental. Hence, interventions will need to move away from knowledge-based approaches to community mobilization for control and resource movement towards dismantling structural drivers for closing the attitude-practice gap.

Abstract

Background: The high rate of stunting is closely linked to cultural influences within local communities. Therefore, health promotion intervention strategies need to be adapted to align with the cultural context of each community. Objective: This study aims to analyze culturally based interventions for preventing stunting in children aged 0–5 years. Methods: This literature review was conducted systematically using the PRISMA 2020 guidelines. Searches across five databases (ProQuest, ScienceDirect, Taylor & Francis, Scopus, and Google Scholar) yielded seven articles involving 352 participants who received culturally based stunting prevention interventions. Results: Seven articles were analyzed in depth, including three experimental studies without control groups. Five studies reported that culturally based health education interventions effectively improved knowledge, attitudes, and stunting prevention practices among caregivers of toddlers (p<0.05). The remaining two studies found that culturally grounded supplementary feeding interventions improved toddlers’ nutritional status (p<0.05). Conclusion: Culturally based interventions are effective in strengthening stunting prevention efforts for children aged 0–5 years. Integrating cultural approaches enhances community acceptance and can serve as a strategic option for governments to accelerate stunting reduction programs.

Abstract

Background: Type 2 Diabetes Mellitus (T2DM) is a metabolic disorder. In Surabaya, it has been a growing public health challenge, with rising prevalence and increasing strain on people and healthcare. Objective: The purpose was to identify a root cause of T2DM, and to provide possible solutions to tackle the problem. Methods: A mixed-method design was used. Qualitative data were obtained from three structured interviews capturing the perspectives of a patient, a physician, and a community health center worker. Problems contributing to DMT2 were brainstormed and then prioritized using the USG method. A “web of causation” approach was applied to identify the root cause of the problem. Results: Unhealthy lifestyle behaviors were prioritized based on the USG analysis. Knowledge insufficiency was identified as the root cause. Two evidence-based solutions are proposed: (1) mobile messaging and (2) “point-of-decision” education in workplaces, canteens, and other public spaces. Conclusion: WhatsApp/SMS education through Puskesmas registries and “point-of-decision” education in public spaces are feasible interventions to enhance diabetes knowledge, promote healthier eating, and reduce sugary product consumption.

Abstract

Background: Salutogenesis emphasizes factors that promote health and well-being, focusing on strengths rather than diseases. Central to this framework is Sense of Coherence (SOC), which reflects an individual’s capacity to perceive life as comprehensible, manageable, and meaningful. A higher SOC is associated with better stress management, resilience, and overall well-being. Advances in predictive analytics, including statistical modelling and machine learning, offer opportunities to integrate the SOC into models that forecast health outcomes, identify at-risk individuals, and guide interventions. Despite the growing interest, research on SOC in predictive models is fragmented and varies in methods, highlighting the need for a comprehensive review. Objectives: To review the literature on the use of SOC in predictive analyses and evaluate its effectiveness as an indicator of resilience and well-being. Discussion: Evidence shows that SOC is a reliable predictor of psychological resilience and well-being. Predictive models incorporating SOC, ranging from regression to machine learning, often outperform models that rely solely on traditional risk factors. The limitations of this study include variability in SOC measurement, limited longitudinal validation, and heterogeneity in the modelling approaches. Nevertheless, SOC provides important insights into resilience outcomes at the individual and population levels. Conclusion: Integrating SOC into predictive analyses is a promising strategy for assessing resilience and well-being in older adults. Standardizing SOC measurements and combining them with multidimensional data can enhance predictive accuracy and inform personalized health interventions.
