Abstract

Asthma is a chronic inflammatory disease that ranksfifth out of the top ten causes of morbidity1. Thedrug therapy commonly used for asthma iscorticosteroid, for example is prednisone2. Thedetermination of the drug in the pharmaceuticaldosage form is very important to ensure its safetyand efficacy. Assay method of prednisone in tabletaccording to Farmakope Indonesia is using HighPerformance Liquid Chromatography (HPLC)3, whichhas good accuracy and precision, but high cost andtook a longer analysis time. These aspects aremaking it less suitable for quality control on a regularbasis. UV-Vis spectrophotometry method could beused as the alternative assay method to determineprednisone in tablet, as prednisone haschromophore and auxochrome groups4. UV-Visspectrophotometry method has shorter analysistimes and lower operating costs while still providinga high precision5. Spectrophotometric method UVVishas been used in previous studies of prednisoneand proven to give good results, for example in thedissolution test6,7. Singh and Verma8 conductedresearch to determine prednisone in tablets basedon the color occurred after prednisone reacted withFeCl3 and K4Fe(CN)6, forming a blueish greensolution. The solution then measured byspectrophotometer giving the analytical wavelengthat 780 nm.The addition of coloring agents in pharmaceuticalpreparations intended to identify products inproduction and distribution, distinguishing the samedose of the drug, preventing product counterfeitingand stability indicators9. However, coloring agent ordye in the formulation could affect the analysisprocess, such as background interfering in theanalytical instrument measurement. In Indonesia, usually we can find prednisone tablet in green color.Dyes used in this study is a mix of light green andtartrazine resulting in a light green color. These dyesalso have a cluster of chromophore and auxochrometo provide absorbance in the UV-Vis spectrumregion. Observing the prednisone solution containing0.1% light green and tartrazine solution at themaximum wavelength of prednisone (238 nm) willgive absorbance of the solution 5% higher. To overcome this problem, the analysis of thespectrophotometry is applied by multi-wavelengthmethod. Dual- or three-wavelengthspectrophotometry methods are very useful toeliminate the absorbance influence of interferingcomponents.a.b.c.Figure 1. Chemical Structure of : a. Prednisone, b. Abstract

The main purpose of health service is toenhance degree of health and to prevent the diseasewith the main target were the society. There wereseveral public health assurances in Indonesia, suchas Jamkesmas, Jamkesda, SPM, Askes. People coulduses these assurance or with own payment.Jamkesmas was the social program from theIndonesian government for the poverty. Thisprogram organized nationally with the crosssubventionto gain the whole of health service inpoverty1.Poverty had same right with other people, including the right to get the health services. Manypeople felt that health service was so expensive. Thiscondition generates a barrier for the poverty.Government action to distribute health service forlow economic class was by applies JamkesmasProgram. This program was including public healthservice (Puskesmas) with their networks, like aadvance health service, emergency health service, and the other limited health services. Jamkesmasprogram was run in many health care service unitslike hospital, public health service, some privatehospital, etc.Nurse was the biggest number in of theseareas, and cares the patient in 24 hours. As a frontliner health provider, nurse performance was veryimportance in hospital success. Nurse performanceis means the work accomplishment done by nursewhen nurse care process, this include assessment, diagnose, intervention, implementation, andevaluation.Nurse performance can be evaluated by usingnursing practice standard. Nursing practice standardis norm or explanation about nurse work quality.Nurse work quality can be considered as good, right,or correct. This can be used as guideline for nursingpractice and evaluation of nurse performance3.The main purpose of Jamkesmas programwas to increase the access and quality of health carefor the poverty which could rise the people healthdegree optimally and efficiently4. In the other hand, some opinions consider that the service forJamkesmas patient was different.Ixora (2008) studied about the comparationof Social Safety Net (Jaring Pengaman Sosial) danNon Social Safety Net (Non Jaring Pengaman Sosial)perception in Seruni and Raflesia Ward, Dr.Soedomo Public Hospital, Treggalek showed thatmayority perception was negatif. This study taken in9 days with 40 respondents.Saputra (2010) studied about The Effect ofNurse Performance and Patien’s Satisfaction as theuser of Jamkesmas Card in RSUD Cengkareng, WestJakarta showed that nurse performance influencedpatient’s satisfaction. But, the questionair used inthis research was not based on nursing process, which includes; assesment, nursing diagnose, intervention, implementation and evaluation.Linder-Pelz (1982) in Krowinski and Stevens(1996), stated that client satisfaction was positiveevaluation in the scope of health services (caringprocess evaluated in single clinical visit, graduallymedical intervention, planning or health service andgeneral health service. Generally, patient satisfactionis showed by the suitable result with patient’s hopeduring or after service.Patient satisfaction was generated fromcomparation between hope and services duringhealth services in hospital. In this case, nurses hasgreat impact for the patient satisfaction. During 24hours nurses always dealing with patient. So, nurseperformance could be the patient attention. The patients of Jamkesmas program werepoverty. Some health problem were suffered fromthem such as tuberculosis, malaria, malnutrition, sexual transmitted disease, and the othercommunicable disease. Generally, these diseasecould be found in internal ward. From the preliminary studies in Dahlia IIWard, Ngudi Waluyo Wlingi Hospital on 05-06September 2012 by interviewed the head wardindicated that all of the Jamkesmas patient hadsame right and intervention with the other patient(non assurance or other assurance). But,administration process influenced the nurse timerespone to do nurse or medical intervention. Somerequirement should be prepared to claim theProceeding ICMHS 2016 ISBN 978-602-60569-3-1267medicines, medical tools, etc. More over, emergencyintervention could be delayed by this process.Patient perspective to health services indicatethe quality of nursing care. To understand the realcondition, researcher investigated the correlationbetween nursing performance & the level ofJamkesmas patient satisfaction in dahlia II ward,Ngudi Waluyo Wlingi Hospital. The aim of this research was to understandthe correlation between nursing performance & thelevel of Jamkesmas patient satisfaction in Dahlia IIward, Ngudi Waluyo Wlingi Hospital. Abstract

Caffeine was an alkaloid commonly used as activeingredient in cosmetics due to its high biologicalactivity for skin health as anticelulit, sunscreen, antioxidant, and photoaging inhibitor [6]. Caffeinehad potential antioxidant properties that couldprotect cells against the UV radiation inducecarcinogenesis with stimulatory effect on apoptosisin the UVB-treated epidermis cell in mice skin [8].High hidrophilic properties of caffeine (log P = -0.07)caused difficulties to penetrate through the stratumcorneum of human skin [9]. Nanoemulsion was adrug delivery system that had good stability andcould potentially increase penetration of the drugthrough the skin that had particle size approximately1 – 100 nm. Oil phase, aqueous phases, andsurfactant are basic component of nanoemulsion[11]. Surfactant was the important compound inpreparation of nanoemulsion. Type, amount ofsurfactant, and HLB value of surfactant can influencenanoemulsion stability [1]. Isopropyl miristate (IPM)was chemical penetration enhancer (CPE), wasadded in skin drug formulation to incrase thepenetration ability of the drug [3]. This study aimedto investigate the effect of hydrophile lipophilebalance (HLB) value of surfactant combination(tween 80 and span 80) and IPM concentration onthe stability and penetration ability of nanoemulsionusing factorial design method and obtain theoptimum formula of caffeine nanoemulsion usingDesign Expert 9.0 analysis.

Abstract

Healthcare facilities such as hospitals, public healthclinics, and private practices are places that peoplevisit to obtain thorough medical care. The state billno.44 of 2009 stipulates that hospitals areinstitutions that provide complete healthcare servicefor individuals. The service included is providinginpatient facilities, home care, and medicalemergency service. According to the bill, among thefunctions of a hospital is to establish and maintainthe quality standard of healthcare service within thehospital, which is then used as a blueprint or areference in day to day patient care. The general public are becoming evermoreconscientious on choosing which hospital to go towhen they become ill. Which is why for everyhospital that offers medical service, the challengethen arises on how to implement various strategiesin order to develop and not merely survive in thegrowing healthcare industry. Among the strategiesthat needs attention is sustaining the qualitystandard of the hospital care service. The quality standard of the hospital care service canbe measured using the SERVQUAL model (ServiceQuality model), which gives rate to the indicators ofvarious qualities of service (Tjiptono, 2011). Thereare 5 main aspects that are measured and arrangedbelow according to the relativesignificance/importance (Tjiptono dan Chandra,2011):A. Reliability, which signifies the institution's abilityto provide error-free and accurate medical careupon patient arrival, within the agreed upon timeconstraint.B. Responsiveness, signifies the healthcarepersonnel's availability and capability to provide fastresponse aid and cater to their needs and request. Italso involves the tactful knowledge of informing thepatient when the medical care will be given andperforming the promised care accordingly.C. Assurance, signifies how the healthcare personnelestablish trust with patients which in turn earns thepatient's trust towards the institution. This can beachieved by providing healthcare personnel whomaintain courteous conduct at all time yet possessthe skill and ability to answer any arising questionsor handle any given situation.Furthermore, it involves the institutions ability tofoster a safe and secure environment for patients.D. Empathy, signifies the ability of healthcarepersonnels to establish effective communicationwhile delivering proper diagnosis. Caring is anessential component which can be implemented bygiving personal attention to each patient.E. Tangibles, correlates with the physicalenvironment of the establishment, which can comein the form of an attractive office design, availabilityof a full range medical equipment, and furthercomplemented by the pleasant appearance of thehealthcare personnels.Delivering optimum service is essential forinstitutions who thrive on gaining customersatisfaction and building towards patient loyalty inthe long run.

Abstract

This study aims to identify the internal factors (strengths and weaknesses) and external factors (opportunities and threats) agribusiness coffee in Jember district and to formulate action plan (plan of action) on each value factors to increase the income of coffee farmers folk seed. This study will use three approaches to collecting data, namely participant observation, semi-structured interviews and focus group discussions. The analytical method used is the SWOT analysis. The research result obtained is the process of obtaining seed until the sale has not done a system of partnerships that benefit farmers but actually beneficial owners of capital. There are limitations to the ability of human resources, facilities and technology to farmers as well as the supply of capital is still limited, and only assisted by a cooperative that level of progress is still lacking due to limited funds cooperative management. The results of further research, programs to increase the production and quality of sustainable coffee plants. Needed gathering coffee development strategy through the revitalization of land, seed, infrastructure and facilities, improvement of human resources, financing farmers, institutional and industrial technology downstream farmers.

Abstract

Coffee plant is commodity exports that wasencouraging because it has relatively high economicvalue in the world market. Coffee plantations aremanaged by people, and this day its grow continuallyin several provinces in Indonesia so expansion isincreasing continuously. Coffee production inIndonesia ranks the fourth largest in the world afterColombia, Brazil and Vietnam (Zainuddin andMurtisari, 1995; Simanihuruk and Sirait, 2010). Inaddition, the coffee plant is one of the leadingcommodities developed in Jember. Chemical contentof coffee such as flavonoids, xanthine, antioxidants, alkaloids, polyphenols can serve as a bitter taste, antibacterial, platelet aggregation. Thebioavailability of polyphenols coffee has also beenstudied (Scalbert et al., 2000; Coralie et al., 2006;Naziq, 2012).Namboodiripad and Srividya (2009) prove theexistence of the inhibition zone coffee against S.mutans. These bacteria are structurally andantigenetically express a surface protein that arecalled antigen I / II, B, Sr and PAC that have amolecular weight of 185 kDa. These antigens by theresearchers were assigned that play a role in thepathogenesis of dental caries, and its are effective asa vaccine in the prevention of dental caries. I/IIantigens of S. mutans have properties of adhesive, that the bacteria are used attachment to hostcomponents during colonize and infection, so itsbecome the focus of a number of researchers. Thesesurface-antigen proteins have an effect in theattachment of S. mutans with acquired pellicles ontooth surfaces (Yuliati, 2005). The immune responsesof dental caries were associated with cytokine suchas IL-1 β, α, IL-1 and TNF-α that were expressedodontoblast layer. It is said that bacteria excretion aextracellular virulence-immunomodulatory protein(VIP, which has a mitogenic effect on lymphocytes, suppress the immune response of the host andinduces production of IL-10, which is animmunosuppressor cytokines. So, VIP is animportant virulence factor produced microorganismsand closely associated with the pathogenicity ofbacteria (Gomes et al., 2006).Thereby, it is said that coffee could be expectedto inhibit dental caries by means of modulate theimmune response. The paradigm change stated thatthe tissue replacement turns into tissueregeneration that was cause approach of medicalmateria that geared to a biologically tissueimprovement. Biocompatible materials indicate thatit can be accepted by the body (Harty and Ogston(1995), and the coffee was clearly meets theserequirements. The current utilization of the coffee plant in themodulates an immune response against S. mutansthat is cause a dental caries that have not beenstudied, so arises problem how theimmunomodulatory potential of robusta coffeebeans against dental caries ?The general objective of this research is to analyzethe immunomodulatory potential of robusta coffeebeans against dental caries, whereas the aim inparticular is analyze the influence of robusta coffeebeans against TNF-α levels in response to S. mutans.
