Abstract

This review brings into view the prognosis attributable to glioblastoma (GBM) and resistance to treatment, surgical interventions and chemotherapy seem ineffective at procuring a better prognosis for patients with the disease. Albeit there exist varying interventions for GBM, the median survival still comes to 12 to 15 months for afflicted patients, this has aroused the need for improvement in treatment success The principal goal is to create a better prognosis and have a decline in treatment resistance invariably leading to better survival rates via adequate treatment for GBM. A relationship exists between HOX genes (homeobox genes) and glioblastoma as is evident from literature. Treatment resistance has been observed in overexpression of HOX genes, the effectiveness of treatment could result from silencing these genes A series of studies have highlighted the role that HOX genes play in glioblastoma prognosis. Promotion of human glioblastoma initiation, aggressiveness, and resistance to Temozolomide has been associated with HOXA9 as shown by Pojo et al. The role of HOX gene expression in cancer stem cells should be studied as it could provide a means of designing CSC-targeted therapies, as CSCs play a part in initiation and progression of solid tumors.

Abstract

Background: Coronavirus disease 2019 (Covid-19) caused by SARS-Cov-2 had been spreading worldwide including Indonesia, but In Indonesia, it was difficult to diagnose confirmation Covid-19 by Polymerase Change Reaction (PCR). and in clinical practice to treat covid-19,  Indonesia Health Ministry use term probable covid-19 for patients under observation Covid-19, but study for neurological manifestation on  probable Covid-19 is very lacking.Objective: Knowing the neurological manifestation of patient probable Covid-19.Methods: Observational study from Covid-19 medical record and neuro-Covid-19 medical record of all probable Covid-19 hospitalized in the isolation ward of Saiful Anwar Hospital (RSSA) Malang since March-May, 2020 (3 months). Probable Covid-19 criteria were given by Covid-19 task force team based on Health Minister Indonesia criteria. This criterion was based on clinical manifestation (severe upper respiratory symptoms, or mild-severe pneumonia), radiologic examination (lung infiltrate and or chest CT scan), and laboratory examination. Neurological manifestation is based on clinical symptom appeared while patient hospitalized, supported with radiologic and laboratory data which was supervised by neurologist or neurologist resident. All data was inputted to neuro-Covid-19 e-registry on the smartphone application, sent periodically to the data center in Neurology Department RSSA Malang.Results: A hundred and fourteen probable Covid-19 was registered with mean age 34,5 year old and mostly at the young age of 20-50 years (38%). The neurological manifestation was observed in 4 patients (3,4% from all possible Covid-19 patients) with thrombotic stroke as the most common manifestation (50%), followed by myelum tumor (25%) and hypoxia encephalopathy (25%). Stroke patients in probable Covid-19 had old age (72 and 82 years) accompanied by several comorbid such as hypertension, DM and CVA history and Covid symptoms of cough, shortness of breath and fever. Hemiparesis was found improved with aspirin and neurotropic therapy. Patient with myelum tumor had a covid symptom of shortness of breath, cough, and fever with UMN tetraparesis. Previous MRI examination results suspected meningioma. The patient died while hospitalized. Patients with hypoxia encephalopathy admitted with decreased consciousness due to shortness of breath with acidosis respiratory symptoms (pH 7,22 ). This condition improved after oxygenation without focal neurologic deficit.Conclusion: Neurological manifestation in probable Covid-19 was 3,4% of all patients. The most common diagnosis was a thrombotic stroke, followed by myelum tumor, hypoxia encephalopathy, and mortality rate is 25%.

Abstract

Background: Dementia is a syndrome caused by decreased cognitive function due to aging, genetics, vascular disease and low physical and cognitive activities. Alzheimer's is the most common cause of dementia. Despite experiencing holistic changes in their lives, elderly with dementia Alzheimer's are entitled to well-being. Measurement of Subjective Well-being (SWB) is used to determine the well-being state of elderlies.Objective: The purpose of this study is to analyze the expression of Subjective well-being (SWB) and the healthy behavior of elderlies with dementia Alzheimer's.Methods: This study uses a qualitative phenomenological approach with a descriptive hermeneutics type. The data collection was done by in-depth interview techniques with the type of auto-anamnesis, observed, and family statements. The data analysis process was carried out in 3 stages (Verbatim, initial and focused coding, determination of categories were made in the form of themes). Data validity test used data triangulation techniques.Results: Participants in this study are 3 elderlies age 75 and 76 years who had mild dementia Alzheimer's. This condition affects participants ability in expressing their SWB and health behavior that are described in physical disorders, life satisfaction, satisfaction with important domains, positive influence, behavior change, treatment-seeking, and cognitive failure.Conclusion: Reported high subjective well-being is closely related to the low level of unpleasant emotions during their life journey. Healthy behavior is strongly influenced by knowledge, demographic factors, personal goals and values, social influence, and access to health services. Two participants managed to express SWB and healthy behaviors better than one participant.

Abstract

Background: Ischemic stroke is an emergency state condition of brain which needs immediate rescue so the sufferer will not get any injury or risk of death. Interval of departing period still becomes problem. Level of severity perception and process of decision making of family are factors correlated to interval of departing time of the patient to Emergency Department.Objective: The purpose of this research is to find out correlation of severity level perception and process of decision making of family to interval of departing time of patient suffering stroke to Emergency Department of Public Regional Hospital Blambangan Banyuwangi.Methods: The method of this research is quantitative with cross sectional approach by involving 113 respondents. The technique of sampling is purposive sampling. The instruments are interview and observation sheet. The data analysis is done by spearman test to find out the correlation between variables namely level of severity and process of decision making of family’s patient to interval of departing time.Results: Findings show that p value between perceptions of severity level to the interval is 0.000 and process of decision making of the family to the interval with p value = 0.000 < α 0.05 meaning that variable of severity level and process of decision making are correlated to interval of departing time.Conclusion: Nurses need to improve standard of stroke severity by determining half-body weakening symptom into severity of ischemic stroke then it is socialized to society and to let them know the importance of immediate transport to emergency department when ischemic stroke symptom is found

Abstract

Multiple sclerosis is a chronic autoimmune disease that attacks myelin in the central nervous system. About 2.5 million people worldwide have been diagnosed with multiple sclerosis. Its clinical presentation could vary according to the location of the lesion. Interferon beta is the most commonly used as immunomodulation therapy. However, its effectiveness for long-term use is still questionable. We report a case of 24-year-old woman with complaints of ataxia and limb weakness which were diagnosed as multiple sclerosis relapsing remitting (MSRR) and treated with interferon beta 1a for five years. During routine interferon beta 1a treatment three times a week, patient has still experienced four episodes of relapse in spite of good compliance. Hence, the rare presence of neutralizing antibodies was suspected. It commonly occurs after a year of interferon therapy, which is consistent with the patient’s treatment history. Further biomarker testing of drug-specific antibodies might be valuable to find out the possibility of interferon resistance

Abstract

Background: Stroke is affected by several factors, age, infection during treatment, obesity, and total cholesterol (TC) levels.  Objective: The study aimed to prove low TC serum levels as an unfortunate outcome predictor for intracerebral hemorrhage stroke. Methods: This study is a prospective cohort design. The research did in the Stroke Unit of Sanglah Hospital, Denpasar, from July 1st, 2018 to January 31th, 2019. The study involved patients with hemorrhage strokes with low TC serum levels and aged 40-95 years. Depending on variable divided into two groups with low TC serum levels (<180 mg/dL) and normal TC serum levels (>180mg/dL). Independent variable is The National Institutes of Health Stroke Scale (NIHSS) as an outcome. The processing of data analysis using Mac for SPSS version 23. Result: This study used 70 subjects. The age characteristics of the study subjects with a median value of 62 years in for low TC levels, and 57 years for normal TC levels, dominant in male (60% ), working (71.5%), not obese (58.6%), stroke-related infections (51.4%), and mean arterial blood pressure (MABP) £130mmHg (90%). Subjects with low TC had a risk of 64 times experiencing adverse outcomes (RR = 64: CI 95% = 14.06-291.32: p <0.001). Multivariate analysis showed a low TC serum level as an independent variable.Conclusion: Low TC serum levels as an independent predictor having a poor outcome in intracerebral hemorrhage stroke

Abstract

Background: The central nervous system neoplasms frequency is 1 per 1700 children aged from 0 up to 15 years old. The problem of the early diagnostics has not been solved yet. The recognition of the pathology, especially in childhood and adolescence is considerably difficult and affects the patient’s life prediction and quality.Objective: To reveal the value of ophthalmological signs in the diagnosis of astrocytoma of the brain.Methods: The young man with the only complaint of the binocular horizontal diplopia during the last year. One month prior to the visit, the patient lost his consciousness. Standard ophthalmic examination, optical coherence tomography (Cirrus HD-OCT), computer perimetry (Humphrey), MRI of the head on (Siemens symphony) with contrast were carried out.Results: Visual acuities were 20/20 for both eyes, an outward motions limitation of both eyes, the presence of paracentral and peripheral scotomas in computer perimetry, the reduction of the visual evoked potentials, pallor of the optic disk, thinning of the nerve fiber layer and ganglion cell on OCT. MRI with contrast revealed the existence of large tumor with a with clear, hilly contours compressing the brain stem. After confirming the presence of a large tumor size, signs of development of occlusal internal and external hydrocephalus, compression of brain stem with the development of life-threatening condition, the patient was referred for neurosurgical consultation. According to the urgent indications the surgical resection of the intracerebral tumor was performed. Biopsy result: pilocytic astrocytoma of the fourth ventricle region.Conclusion: Thus, in the absence of specific cerebral and focal neurological symptoms, the appearance of oculomotor disorders, defects in field of vision and fundus changes can be decisive factors in establishing an accurate diagnosis and timely referral of the patient to a neurosurgeon.

Abstract

Background: Carpal Tunnel Syndrome (CTS) is the most common mono-neuropathy caused by median nerve entrapment on the wrist. Major risk factor of the CTS is repetitive use of the hand, whether from occupational activities or hobbies. One of the most frequent occupation affected with CTS is the batik craftsmen.Objective: To investigate the relationship between working period of batik craftsmen with the incident of CTS at Kampung Batik Tulis Laweyan.Methods: Participants of this cross-sectional study were 30 batik craftsmen from Kampung Batik Tulis Laweyan who met the inclusion and exclusion criteria. They were classified into two groups based on working period (less or more than five years), clinically examined, and underwent the Nerve Conduction Velocity studies. Bland's classification was administered for grouping the nerve injury. Spearman correlation test was performed to investigate the correlation between working period and incident of CTS.Results: There was a significant medium positive correlation between working period and nerve injury based on Bland’s classification (r=0.559; p=0.013). While the chief complaints were also correlated with the working period, the association was not statistically significant (p=0.355).Conclusion: This research suggests an association between the incident of CTS and longer working period in batik craftsmen.

Abstract

Increasing aging population causes an increased prevalence of neurodegenerative diseases such as dementia that is associated with memory decline. Developing strategies for the prevention and therapy of age-related dementia is important to reduce the burden of treatment costs. Physical exercise is known to prevent cognitive decline and improve cognitive abilities. Physical exercise with moderate intensity for at least 150 minutes/week or 30 minutes/day for 5 days can reduce the incidence of degenerative diseases in the elderly. Thus, physical exercise appears as a simple, inexpensive, and affordable non-pharmacological therapy for most people. The processes of neurogenesis and neuronal survival involve the role of neurotrophic factors including BDNF, IGF-1 and VEGF, which are the three main neurotrophic factors that are known to increase after exercise. Many publications discuss about these neurotrophic factors, but their mechanism of signals and changes related to aging and exercise have not been completely studied. The purpose of this review is to discuss the mechanism of signals and changes of neurotrophic factors (focuses on BDNF, IGF-1, and VEGF) related to aging and exercise.

Abstract

Background:  The majority of caregivers are family members and within the symptoms that the caregiver may present, there are manifestations of stress, psychological state of overload, stress or discomfort caused by the continuous provision of care for the family member, feelings and negative perceptions regarding their caring function.Objective: The purpose of this research is to study the psychosocial vulnerability of family caregivers of patients with Mucopolysaccharidoses (MPS).Methods: This study is based on a mixed method with a cross sectional and a descriptive approach. The first stage was quantitative and the second stage was qualitative, both of them to explore the psychosocial vulnerability of a group of family caregivers of patients (n=111) with MPS belonging to the Colombian association of patients with lysosomal storage diseases ACOPEL by its acronym in Spanish. This study collects quantitative data using the patient admission request format from ACOPEL, employed to obtain relevant sociodemographic variables: Gender, Age, Marital status, Education, Occupation, living area, type of housing, live with, socioeconomic stratum, monthly income and time spent as a caregiver. And this study collects qualitative data using a structured interview held to psychologists and healthcare professionals belonging to ACOPEL.Results: The results show that most of them are women, relatives in a relationship or married; most of them belonging to a low socioeconomic stratum and a low professional education. The categories of analysis revealed were the attitude is towards illness, taking account diagnosis and health staff, support networks and psychoeducation. Secondly, the impact of care work on the physical and psychological health of female caregivers. Third, coping resources and finally the double mother role-caregiver and her emotional bond with the patient.Conclusion: In conclusion, the attitude and knowledge towards the disease depend on the diagnosis and the prognosis according to the type of MPS. It is important more research the psychosocial aspects and vulnerability of family caregivers in patients with MPS to develop evidence-based knowledge, psychoeducation, and, appropriate interventions.
