Abstract

Background: Medication adherence is essential to achieving controlled blood sugar in diabetic patients. Insulin generally provides better glycemic control but is considered painful and requires special techniques. Insulin administration in patients with neurological complications requires particular consideration because these complications can cause physical and cognitive barriers. Objective: This study analyses the effect of insulin administration on medication adherence in diabetic patients with neurological complications and the influence of various confounding variables (baseline characteristics, medical and medication history). Methods: This observational study was conducted with a cross-sectional design at a government hospital in East Jakarta from September 2021 to January 2022. The sample was type 2 diabetes mellitus patients with neurological complications who received antidiabetics for at least six months. The neurological complications include central nervous disorders (such as stroke) and peripheral nervous disorders (such as neuropathy). The independent variable was insulin administration, while the dependent variable was adherence, measured using subjective methods [Adherence to Refills and Medications Scale (ARMS)] and objective methods (Medication Refill Adherence (MRA) and HbA1c]. Results: Of 175 respondents, based on the three methods (MRA, ARMS, HbA1c), 13 respondents (7.4%) were adherent, namely one respondent (1.8%) in the insulin group and 12 respondents (10.1%) in the non-insulin group. Insulin administration affects adherence to antidiabetics by 0.123 times (95% CI: 0.015 - 1.024), or patients who use insulin have 87.7% lower adherence controlled by antidiabetic changes and the total number of medicines used. Conclusion: Insulin administration significantly affects medication adherence in diabetes mellitus patients with neurological complications.

Abstract

Background: Diarrhea can be treated with probiotic bacteria such as Bifidobacterium bifidum, which decreases the intestinal environment's pH to become acidic so that pathogenic bacteria cannot thrive. Objective: To make fermented coffee that can increase the number of probiotic bacteria Bifidobacterium bifidum and has antidiarrheal activity against pathogenic bacteria Escherichia coli. Methods: Robusta coffee (20.25% and 19.75%) was fermented using Saccharomyces cerevisiae, and then the probiotic bacteria Bifidobacterium bifidum was added. Unfermented coffee was compared with the same concentration (20.25% and 19.75%) to obtain four formulas. Organoleptic panelists tested all formulas to determine the best formula for fermented and non-fermented coffee. The number of Bifidobacterium bifidum and antibacterial activity was calculated on the optimum formulation using the Total Plate Count and Disc Diffusion Method. Result: The optimum formula obtained at fermented and unfermented coffee concentration was 20,25%. The number of probiotic bacteria Bifidobacterium bifidum growing in fermented and non-fermented coffee was 7.3 x 108 ± 32.4 and 3.1 x 108 ± 30.7 (p < 0.05). The diameter of the inhibition zone of the best fermented and non-fermented coffee was 11.5 ± 0.5 mm and 8.5 ± 0.5 mm, respectively (p < 0.05). Conclusion: Fermented coffee can increase the growth of the probiotic bacteria Bifidobacterium bifidum and has strong antibacterial activity against Escherichia coli bacteria.

Abstract

Background: Noni leaf (Morinda citrifolia L.) extract is a natural product that can be used as a sunscreen. The extract contains flavonoids which function act as an antioxidant. In this work, sunscreen cream formulated with noni leaf extract was prepared using a combination of tween 80 and lecithin. Objective: The purpose of this study is to evaluate how the combination of tween 80 and lecithin affects the physical qualities of the cream, such as organoleptic, homogeneity, emulsion type, spreadability, adhesion, pH, and stability over 28 days of storage at room temperature, and to find the best formula. Methods: This study used 10% of noni leaf extract in the cream formulation. The Simplex Lattice Design (SLD) method was used to determine the effect of different concentrations of the two emulsifiers on the cream's spreadability, adhesion, and pH. Furthermore, the SLD was used to find the best formula. Results: The results showed that different concentrations of the emulsifier, which are the tween 80 and lecithin combination, affected the physical properties and storage stability of cream preparations. The interaction of tween 80 and lecithin is having a significant impact on the cream's adhesion and spreadability; however, the effect of the interaction on the pH value was not significant. Conclusion: The formula containing 2.5 % tween 80 and 2.5 percent lecithin was found to be the most effective in fulfilling the cream physical properties while remaining stable during storage.

Abstract

Background: Cardiovascular disease is one of the chronic diseases with an increasing prevalence from year to year. The results of Indonesia's basic health research (RISKESDAS 2018) showed that it was experienced by 2.9% of the population. Ensuring the continuity of drug availability is very important in pharmacy services for this patient group. The history of drug use is considered in the planning dan procurement of drug products. Objective: This study aims to analyze the drug use pattern and cost of drugs for cardiology outpatients in the National Health Insurance scheme (JKN). Methods: A retrospective observational study was conducted using the prescription of cardiology outpatients JKN for March-May 2021. The ABC method carried out the analysis of drug use patterns and cost. Results: From 2,986 prescriptions for cardiovascular disease, there were 94 types of drugs from 37 therapeutic classes. In the ABC analysis of drug use for class A, 68.82% contained seven types of drugs, class B, 20.55% contained nine types of drugs, and class C, 10.63% with 78 types of drugs. The results of ABC analysis for investment value (cost) of drugs class A 66.96% contained two types of drugs, class B 19.81% with four types of drugs, and class C 14.23% with 88 types of drugs. Conclusion: ABC analysis of the drug use pattern and cost/investment showed different patterns which had value to consider in procurement planning to maintain service continuity.

Abstract

Background: In addition to minerals, honey contains carbohydrates (glucose and fructose), protein, amino acids, water, enzymes, ash, vitamins, and other substances. Compounds of honey can affect the chemical properties of honey. Knowing the physicochemical properties of honey is very important because physicochemical properties affect the quality of honey. One of the biological activities of honey is an antioxidant. Antioxidants can interfere with oxidative processes, prevent disease, and play an important role in the body's defence system. Objective: to determine and compare physicochemical properties (color, viscosity, ash content, water content, reducing sugar (glucose), total phenolic compound, HMF) and antioxidant activities of monofloral honey samples from Indonesia. Methods: The color of honey are categorized using the Pfund scale. Viscosity measurement is carried out using a Brookfield viscometer. The water content is carried out using a refractometer. Phenolic content and antioxidant activities analysis were carried out by UV-VIS spectrophotometer. Results: The results show that rambutan honey from Malang has the highest physicochemical properties and antioxidant activity, which had an amber color, water content of 21.7% b/b, acidity 20.7 mL NaOH/Kg, viscosity of 33.08 poise, ash content of 0.17% b/b, reducing sugar 69.38%b/b, total phenolics content 533.7 mg GAE/Kg sample and IC50 0.111 µg/mL. Conclusion: The quality of honey varies from region to region. The best honey (according to SNI) is rambutan honey from Malang.

Abstract

Background: Hypertension is currently a non-contagious disease that primarily affects the elderly population in Indonesia. Medication adherence is critical in managing hypertension and reducing the risk of morbidity and mortality. Previous research has found that loneliness and support received by older adults with hypertension influence medication adherence. Objective: This study aimed to examine the impact of feeling lonely and receiving social support on medication adherence in the elderly with hypertension at the Community Health Center in Surabaya. Methods: The study design of this research was a descriptive cross-sectional study from December 2021 to March 2022. A total of 235 eligible subjects fulfilled the inclusion criteria. The instruments used in the data collection were the patient's information form, UCLA-Loneliness Scale, MOS-Social Support Survey, and the ARMS (Adherence to Refill and Medication Scale). Results: The results revealed that the correlation between loneliness and social support was significantly associated with medication adherence in the elderly with hypertension (p < 0.05). In addition,  other factors, such as occupation status, living status, comorbidity, the number of drugs taken, and antihypertensive drug therapy, showed a significant correlation with medication adherence (p < 0,05). The most influential factor on medication adherence was loneliness (35.5%), followed by social support (24.4%), the number of drugs taken (7.1%), antihypertensive drug therapy (monotherapy or combination therapy (2.5%), occupation status (2.4%), comorbidity (1.6%), and living status (0.2%). Conclusion: This study confirms that feeling lonely and receiving social support affect medication adherence in the elderly with hypertension at the Community Health Center in Surabaya.

Abstract

Background: Hepatitis C is caused by hepatitis C virus (HCV) infection. HCV infection is one of the biggest causes of chronic liver disease. About 60-80% of patients with acute hepatitis C will develop chronic hepatitis C. Objective: This study aimed to analyze the potential of mango peel compounds as HCV NS5B inhibitors. Methods: The methods in this study are ligand preparation, physicochemical and pharmacokinetic predictions, protein structure preparation, molecular docking, data analysis, and visualization. Results: The results showed that the test ligands had binding free energies close to the reference ligands, namely Mangiferin -7.862 kcal/mol and respectively D-(+)-Maltose -6.453 kcal/mol, Dibutyl – phthalate -6.326 kcal/mol, bis-β-D-fructofuranose 1,2':2,3'-dianhydride -6.249 kcal/mol, 16-Heptadecyne-1,2,4-triol -5.476 kcal/mol, 3,4,5-trihydroxycyclohex-1-ene-1-carboxylic acid -5,360 kcal/mol, Trigonelline -4.905 kcal/mol, Hexitol -4.552 kcal/mol, α-Glucoheptitol -4.403 kcal/mol. All the test ligands bind the NS5B active site with hydrogen bonds. Furthermore, the ligand-receptor complex has a dissociation constant value and hydrogen bond length. Conclusion: The results showed that Mangiferin was the most potential ligand in inhibiting NS5B HCV of all the test ligands used.

Abstract

Background: The side effects of synthetic anti-inflammatory drugs have become a known problem in medicine as well as in the general public. These side effects problems drive patients to seek alternatives to common anti-inflammatory drugs, especially natural alternatives. Phytochemicals such as flavonoids, alkaloids, and many different others have been identified as potential anti-inflammatory agents. Red dragon fruit (Hylocereus polyrhizus) is one source of such phytochemicals. Whilst the consumption of the flesh of red dragon fruit is very common, utilization of the peel is very rare and often becomes waste. Objective: This study aimed to explore the potential of red dragon fruit peel as an anti-inflammatory agent. Methods: This experimental study with a control group post-test-only design. This study involved in-vitro (protein denaturation inhibition using bovine serum albumin) and in-vivo (carrageenan-induced inflammation on an air-pouch model on 25 white rats) experiment. Results: This study showed that the red dragon fruit peel extract contains flavonoid, alkaloid, saponin, and tannin compounds. Administration of red dragon fruit peel extract suspension was found to be effective at inhibiting protein denaturation and reducing white blood count in the inflammation exudates, and the effectiveness is increasing along with the dose. Conclusion: It can be concluded that the red dragon fruit peel extract was effective as an anti-inflammatory agent, especially at a higher dose.

Abstract

Background: Although a SARS-CoV-2 vaccine is readily available, new cases of COVID-19 are still occurring. New drug discovery is needed to treat COVID-19. Protein E is one of the potential targets. Two synthetic compounds of bicycloproline derivatives have the potential to be developed. Objective: This study aimed to estimate the interaction of bicycloproline compounds to protein E in-silico. Methods: There were two bicycloproline-derived compounds, MI-09 and MI-30, used in docking. Remdesivir was used as a reference ligand. The crystal structure of the E protein was created using homology modeling, while the test compound was drawn using the Marvin Sketch. MOE 2022.02 and BDS 2021 were used for docking and visualization processes. Results: The pentamer of the SARS-CoV-2 E protein obtained a clash score (1.06); poor rotatomer (0.00%); favored rotamers (98.11%); Ramachandran favored (96.43%); Ramachandran outlier (1.78%); Rama Z-score (-1.08); and mol probity (1.04). Research shows promising inhibition potential of the MI-09 and MI-30. The MI-30 has the best binding energy of -10.3326 kcal/mol. Conclusion: The docking results show that MI-30 has potency as an inhibitor of protein E and can be developed in treating COVID-19. Further research is needed to confirm the result by in vitro and in vivo studies.

Abstract

Background: Green tea possesses abundant polyphenols that exert antioxidant activity. However, green tea's hydrophilicity and instability limit its penetration into the skin layers. Recently, a non-ionic surfactant-based elastic nanovesicular system called spanlastic can enhance the delivery of hydrophilic and unstable substances. Spanlastic composed of vesicle builder and edge activator, which influence the characteristics of the vesicle.  Objective: The study aimed to evaluate the influence of the ratio of the components on the characterization of green tea extract-loaded spanlastic using three different weight ratio of vesicle builder and edge activator that is 7:3, 8:2, and 9:1. Methods: Spanlastic is prepared by ethanol injection methods using Span 60 as vesicle builder (VB) and Tween® 60 as edge activator (EA). The characterization includes visually observed organoleptic, particle size (PS) and polydispersity index (PDI) using dynamic light scattering, entrapment efficiency (EE) and drug loading (DL) using total phenolic content assay. The most optimum ratio will be tested its zeta potential value using Zetasizer and viscosity using Brookfield Cone and Plate. Results: Selected spanlastic formula composed of Span 60 and Tween® 60 at a weight ratio of 8:2 has given characteristics as follows: entrapment efficiency 60.85±1.70%; drug loading 11.07±0.65%; the particle size is 419.70±7.42 nm; and  PDI value 0.26±0.05. The prepared spanlastic has a greenish liquid form, with a zeta potential value of 28.53±2.78 mV and viscosity of 14.65±0.32 cP. Conclusion: The optimum weight ratio of vesicle builder and edge activator for green tea extract spanlastic is Span 60:Tween® 60 8:2.
