Abstract

HIGHLIGHTS 1. Myomectomy during cesarean section is a recommended and safe procedure. 2. This procedure benefits the patient because it avoids a second operation and anesthesia complications and is cost-saving.   ABSTRACT Uterine myoma is a tumor in the uterus that is generally benign. There are many types of uterine myomas. These tumors can grow on the outer wall of the uterus, on the uterine muscle, or it can also be on the inner wall of the uterus. The most frequent kind of uterine tumor is myoma. Uterine myoma mostly occurs in women over thirty years of age. Its prevalence ranges from 5.4% to 77%, with uterine fibroids accounting for up to 5% of pregnancies. The myomectomy procedure is an option for women who still want children but are concerned about the possibility of subsequent surgical intervention. The myomectomy cesarean section is indicated if there were complications related to the myoma in a previous pregnancy. It also avoids the possibility of repeat laparotomy for fibroid removal in the future. Myomectomy surgery should be planned based on fibroids' location, size, and quantity, using suitable imaging. Myomectomy cesarean section can be an option compared to cesarean section without myomectomy, especially if it is performed by experienced surgeons with proper hemostatic techniques and performed in tertiary-level health facilities. This article discusses the details of the benefits and safety of myomectomy during cesarean section so that it might be considered before performing this procedure.

Abstract

HIGHLIGHTS PE is still one of the contributors to morbidity and mortality which is quite high for pregnant women in Indonesia. The use of NLR and MEOWS modalities as predictors of preeclampsia incidence shows significant potential and can be applied especially in facilities with limited resources.   ABSTRACT Objective: To describe modalities of detecting the severity of preeclampsia (PE) between neutrophil-to-lymphocyte ratio (NLR) levels and Modified Obstetric Early Warning Systems (MEOWS). Materials and Methods: We used a systematic search in PubMed, ScienceDirect, and Google Scholar for fully-accessed English articles, published between 2015 – 2023 that evaluated modalities between NLR levels and MEOWS for detecting the severity of preeclampsia, which resulted in 208 works of literature. Among the studies, we found 14 literature that met the PICO inclusion criteria and included in this study. Results: PE is still one of the high causes of morbidities and mortality in maternal patients. It is important to predict the possible risk of PE events. NLR is one of the markers that become a prognostic modality to determine the severity of PE. MEOWS is one of the tools to assess the level of care of patients who carry out the treatment. NLR and MEOWS can be modalities for predicting PE severity. Conclusion: NLR and MEOWS can be used even with limited resources. The combined use of NLR and MEOWS can be an alternative and potential modality in detecting PE severity.

Abstract

HIGHLIGHTS In malaria endemic area, pregnant women are highly prone to suffer from malaria infection. Vivax malaria in pregnancy can contribute to anemia and preterm delivery.   ABSTRACT Objective: This study aimed to report a preterm delivery and anemia as part of P. vivax malaria infection complications in a pregnant woman in Timor Tengah Selatan regency, East Nusa Tenggara, Indonesia. Case report: A 42-year-old pregnant woman, gravida 6 para 5,36-week of gestational age pregnant woman came with complaints of water breaking since one day before admission. She had fever with chills for three days, especially at night along with muscle, headache, joint soreness, dizziness, and palpitations. Rapid diagnostic test for malaria showed positive result. Peripheral blood smear examination revealed microcytic hypochromic due to iron deficiency or chronic infection and presence of trophozoites-ring form of P. vivax with 4,235 parasitemia. A baby boy was born with weight of 2,470 grams (percentile 28%), fetal head 31 cm (percentile 13%), birth length 43 cm (percentile 4%), and Apgar Score (AS) 8 and 9 at 1 and 5 minutes, respectively. The treatment was provided according to anti-malarial guideline in Indonesia using dihydroartemisin 120 mg and piperaquine phosphate 960 mg fixed dose as DHP for 3 days and primaquine 15 mg for 14 days. Conclusion: Anemia as part of vivax malaria complication in pregnancy contributes to preterm delivery.

Abstract

HIGHLIGHTS 1. Mitotic index and grade are prognostic factors for endometrial cancer, but both are independent.2. Stage and mitotic index associated with cell proliferation affect the prognosis of endometrial cancer.   ABSTRACT Objective: This study aimed to analyze the correlation between the mitotic index and the stage and grade of endometrial cancer. Materials and Methods: We collected pathology reports of endometrial cancer from the Pathology Laboratory at Dr. Soetomo General Hospital in Surabaya, Indonesia, covering cases diagnosed between 2018 and 2020. A total of 106 cases of endometrial cancer were included in this study. For each case, detailed records of the cancer stage, grade, and mitotic index were recorded. The mitotic index, an indicator of cell proliferation, was quantified, and its correlation with cancer stage and grade was assessed. To determine the strength and direction of these relationships, we performed a Spearman rank correlation statistical analysis for non-parametric data. Results: Our findings indicated a significant positive correlation between the mitotic index and the stage of endometrial cancer. An increase in the mitotic index, reflecting a higher proliferation rate of cancer cells, was associated with a more advanced cancer stage, suggesting that the mitotic index could potentially serve as a prognostic marker for assessing tumor progression in endometrial cancer. However, our analysis revealed no significant correlation between the mitotic index and the histological grade of endometrial cancer, implying that the grade, which typically reflects the differentiation status and morphological characteristics of the tumor cells, is independent of the proliferation rate as measured by the mitotic index. Conclusion: The mitotic index is positively correlated with the stage of endometrial cancer but does not show a correlation with the histological grade. These findings highlight the potential use of the mitotic index in staging endometrial cancer.

Abstract

HIGHLIGHTS There is a significant higher level of stress during COVID-19 pandemic. The highest menstrual abnormalities during pandemic cases were found in premenstrual syndrome and dysmenorrhea, and the lowest was found in menstrual cycle disorders (polygomenorrhea, oligomenorrhea, amenorrhoea).   ABSTRACT Objective: COVID-19 pandemic had caused a considerable increase in stress level due to reduced human mobility, interaction, and activity. High stress levels are often associated with an adverse health outcome. Stress influences female reproduction systems, where it is an important indicator of female health. This systematic review aimed to analyze the relationship between stress levels due to the COVID-19 pandemic and the incidence of abnormal menstruation. Materials and Methods: This systematic review was registered in PROSPERO international database (CRD42023433296). Literature search was conducted across four databases, Pubmed, Web of Science, Science Direct, and Google Scholar, with boolean AND and OR operators as keyword search assistants. Literature was searched and limited to journals published during the pandemic (2019-2023), written in English, an open access article, and meet the keywords. Literature not a research study, duplicated and limited access were not included. Inclusion to the journal was carried out by screening using PRISMA flowchart and EPHPP Assessment Tool for Quantitative Studies for the quality assessment. Results: Nine out of 1150 journals were eligible for review. Seven studies revealed that there was a significant relationship between stress and anxiety due to the COVID-19 pandemic and the incidence of menstrual abnormalities. However, one study revealed that these changes were not significant. Conclusion: There is a significant relationship between stress during the COVID- 19 pandemic and the incidence of menstrual abnormalities.

HIGHLIGHTS Neutrophil Lymphocyte Ratio (NLR) and Platelet Lymphocyte Ratio (PLR) have potential as independent prognostic markers for ovarian cancer. This research was to evaluate NLR and PLR levels in ovarian cysts with epithelial ovarian cancer patients at RSUP H. Adam Malik Medan, Indonesia.   ABSTRACT Objective: To assess Neutrophil Lymphocyte Ratio (NLR) and Platelet Lymphocyte Ratio (PLR) as independent prognostic markers in epithelial ovarian cancer patients at RSUP H. Adam Malik Medan, Indonesia, considering the disease's common occurrence and typically late-stage diagnosis with a poor prognosis due to inflammatory processes implicated in malignancy mechanisms. Materials and Methods: A comparative case-control study was conducted at the Department of Obstetrics and Gynecology, RSUP H. Adam Malik Medan, from December 2019 to February 2020. The study included 40 patients each diagnosed with ovarian cysts and epithelial ovarian cancer. Bivariate analysis was conducted using independent T-test for normally distributed data and the Mann-Whitney test for non-normally distributed data. Significance was established at p <0.05. Results: The median NLR in the ovarian cancer group was 2.45 (1.06 to 38.44) and in the ovarian cysts group was 2.34 (1.44 to 3.78). Median PLR levels in ovarian cancer were 12813.94 (3178.08 to 19040.0) and in ovarian cysts were 11138.15 (5026.18 to 22839.51). Statistical analysis revealed no significant difference in NLR and PLR levels between ovarian cancer and ovarian cysts (p >0.05). Conclusion: NLR and PLR levels demonstrated no significant difference between ovarian cancer and ovarian cyst patients at RSUP H. Adam Malik Medan, Indonesia. However, NLR and PLR can still serve as valuable markers for identifying malignant processes in patients suspected of malignancy.

Abstract

HIGHLIGHTS Complementary treatments have been shown to greatly reduce the intensity of dysmenorrhea pain. The use of medicinal plants, herbal concoctions, or compresses might reduce dysmenorrhea discomfort more effectively than taking medications like mefenamic acid, ibuprofen, piroxicam, etc   ABSTRACT Objective: The study's objective was to obtain latest data on alternative therapy for dysmenorrhea in adolescent females of reproductive age. Materials and Methods: A scoping review was conducted using the PRISMA ScR protocol. A search was conducted on PubMed, Science Direct, and Wiley, yielding 848 studies. Ten literature studies were identified that satisfied the criteria for population, exposure, and outcome. The studies also incorporated inclusion criteria, focusing on women of reproductive age who had dysmenorrhea. Results: Out of 848 articles, 32 were considered potentially relevant and met the inclusion criteria. The articles indicated that teenagers commonly used warm compresses and herbal drinks as supplementary therapies to alleviate dys-menorrhea due to their perceived effectiveness and comfort. Other complement-ary therapies for the condition included massage, relaxation, herbal products, self-care practices, acupuncture, and therapeutic methods. Conclusion: Herbal and warm water compress are the most commonly applied alternative therapies for treating dysmenorrhea.

Abstract

HIGHLIGHTS The size of curcumin was modified to nano scale in order to enhance its bioavailability and facilitate its absorption in the body. As a herbal medicine, nano-curcumin has the ability to reduce proteinuria and serve as a preventive measure against preeclampsia.   ABSTRACT Objective: Since preeclampsia is one of the most serious hypertensive disorders in pregnancy, as it occurs in 5-7% of all pregnancies, and causes around 70,000 maternal deaths and 500,000 fetal deaths worldwide each year, this study aimed to determine the effect of nano-curcumin on proteinuria in pregnant white rats (Rattus norvegicus) with preeclampsia. Materials and Methods: In this study, 24 white rats (Rattus norvegicus) were randomly selected and divided into six groups. Inclusion criteria included healthy rats aged 8 weeks or older, with normal blood pressure and weight, while exclusion criteria included sick, deceased, or prematurely birthing rats, and those with high blood pressure. Treatment, administered over six days from gestational days 13-18, involved L-NAME and nano-curcumin injections. Groups included K- (no treatment) and K+, P1, P2, P3, and P4 (treated with L-NAME and varying nano-curcumin doses). Blood pressure and proteinuria were evaluated on gestation days 12, 15, and 19 to confirm the preeclampsia model and assess nano-curcumin's effect on proteinuria. Urine collected over 24 hours in metabolic cages preceded the rats' termination. Data analysis utilized IBM SPSS version 23, including the Shapiro-Wilk test, parametric independent sample t-tests, One-Way ANOVA tests, and LSD post-hoc tests to identify group differences.   Results: The results of this study showed that nano-curcumin had the effect of reducing proteinuria in white rats with preeclampsia. The significant results of the One-Way Anova test was p=0.001 (p <0.05) and the LSD post-hoc test revealed that the effective dose was 25 mg/ml. It was found that higher nano-curcumin dose had a higher average of proteinuria. Conclusion: Nano-curcumin can affect proteinuria in preeclampsia. The most effective dose is 25 mg/ml.

HIGHLIGHTS 1. In pregnant women, vitamin D insufficiency may plays a role in the development of post-delivery illness.2. Vitamin D supplementation is significant in remodeling of post-delivery levator ani muscle.   ABSTRACT Objectives: Vitamin D is considered a crucial vitamin for the restoration of levator ani muscle strength. Therefore, this study aimed to evaluate the association between vitamin D and levator ani muscle remodeling in the post-delivery period.Materials and Methods: The literature search was conducted across three electronic databases, namely PubMed, Google Scholar, and Springerlink. Our investigation yielded a total of 2613 studies, out of which 8 studies were found to meet the inclusion criteria and were subsequently included in our study. Among these, 4 studies specifically examined the impact of vitamin D micronutrient status on the levator ani/pelvic muscles during the post-delivery period.Results: The mean maximum contraction strength of the levator ani muscles following the administration of vitamin D supplements was 26.77 ± 7.15 cmH2O. The analysis conducted utilizing a paired t-test yielded a p-value of less than 0.05, indicating statistical significance. Additionally, a coefficient correlation of 0.831 was observed, with a p-value also less than 0.05. The findings of this study indicate a noteworthy correlation between levels of vitamin D and the magnitude of levator ani muscle contractions, as evidenced by a statistically significant p-value of less than 0.05.Conclusion: The administration of vitamin D supplements has been found to play a significant role in the remodeling of the levator ani muscle during the post-delivery period. This is evidenced by the observed increase in strength of the levator ani muscles following vitamin D supplementation.

The odds of having a prolonged menstrual cycle were increased by a factor of 1.109 with an increase of one unit of body fat percentage, while the odds of having a prolonged menstrual bleeding period was decreased by a factor 0.887 with an increase of one unit of body fat percentage.   ABSTRACT Objectives: Body fat percentage measures overall amount of fat as a proportion of total body weight. Basal metabolic index (BMI) is an unreliable predictor of body fat percentage as excess fat, lean, muscle, or bone density are indifferentiable. However, the relation between body fat percentage and BMI on menstrual characteristics are still unclear. The aim was to compare the correlations between body fat percentage and BMI towards menstrual characteristics.Materials and Methods: A cross-sectional sample of 211 young adults was taken by means of cluster random sampling. Cross tabulations were performed between variables and Pearson's chi square value were observed. Multiple logistic regressions were performed to observe the odds ratio and 95% confidence interval.Results: Body fat percentage was found to be associated with menstrual cycle (p=0.000) and menstrual bleeding period (p=0.000) but not daily pads usage, intermenstrual bleeding, and menstrual pains. Age was found not to correlate with any of the collected menstrual characteristic data. BMI was also found to be associated with menstrual cycle (p=0.008) and menstrual bleeding period (p=0.003). Further analysis showed that a one unit increase of body fat percentage was linearly correlated with increased of menstrual cycle by a factor of 1.109 days (p<0.01) and a decreased of menstrual bleeding period by a factor of 0.887 days (p<0.01).Conclusion: Although both body fat percentage and BMI showed associations with menstrual cycle and menstrual bleeding period, only body fat percentage was linearly correlated with menstrual cycle and menstrual bleeding period.
