Abstract

A B S T R A C TResearch is an activity carried out based on scientific principles and methodssystematically to obtain information, data, and information from related subjects, withunderstanding the theory and proving assumptions and / or hypotheses. The resultsobtained are conclusions that can be applied or become additional knowledge for theadvancement of science. However, research activities must still respect the rights anddignity of research subjects. Health research includes biomedical, epidemiological,social, and behavioral research. Some health research can be done in vitro, usingmathematical models, or computer simulations.

Abstract

A B S T R A C TBackground: Traumatic brain injury is the main cause of death in the populationunder the age of 45 years, and the fourth leading cause of death in the entire ofpopulation. Based on the degree of traumatic brain injury, it is commonly categorizedbased on the Glasgow Coma Scale (GCS). Post-Concussion Syndrome (PCS) is theset of somatic, emotional / behavioral and cognitive symptoms that occur after atraumatic brain injury. The aim of this study was to find out the prevalence andcorrelation of the degree of traumatic brain injury based on the Glasgow Coma Scale(GCS) and the emersion of Post-Concussion Syndrome (PCS) acute onset in patientswith head injuries Method: This study was a cross-sectional analytic study ofpatients who experienced Post-Concussion Syndrome (PCS) after traumatic braininjury at DR. M. Djamil Hospital Padang in 2020 from June to November 2020. Datawere collected by filling in a questionnaire (The Rivermead Post ConcussionSymptoms Questionnaire) and medical record data of neurosurgical patients thatmet the inclusion and exclusion criteria. Result: : It indicated that 70 patients wereincluded in the inclusion criteria of this study. A total of 38 (54.3) respondents didnot undergo the acute onset of PCS, meanwhile respondents who experienced acuteonset of PCS were 32 (45.7) respondents. The results showed that 25 (67.6%)respondents with mild traumatic brain injury had PCS acute onset, while 4 (17.4%)respondents with moderate degree of traumatic brain injury had PCS acute onset,and 4 (17.4%) respondents had PCS acute onset PCS 3 (30%) respondentsexperienced severe traumatic brain injury with acute onset PCS and statistically thedifference in the proportion of data from each of these variables was significant witha p-value of 0.0001. The results of statistical tests showed that p value> 0.05 on thecorrelation between PCS and GCS, thus, it can be concluded that there was nocorrelation between the direction of the relationship between PCS and GCS.Conclusion There was no correlation between the degree of traumatic brain injurybased on GCS and the incidence of PCS acute onset, either it was unidirectional orvice versa in patients with head injuries at RSUP M. Djamil Padang.

Abstract

A B S T R A C TBackground: Head injuries occur every 15 seconds worldwide, with patientsdying every 12 minutes. The value of the Neutrophil Lymphocyte Ratio (NLR) is afactor that influences the assessment and prognostic value of patients withintracranial hemorrhage and without intracranial hemorrhage in head injury.The aim of this study was to determine the relationship between increased NLRbased on the severity of head injury patients with or without intracranialhemorrhage. Methods: This study is a retrospective cross sectional study of headinjury patients with hemorrhage and without intracranial hemorrhage who weretreated at Dr. M. Djamil Hospital Padang in 2020 from May to December 2020.Data was collected in the Medical Records Department of Dr. M. Djamil HospitalPadang who met the inclusion and exclusion criteria. Results : A total of 92patients were included in the inclusion criteria for this study. The results showedthat the mean NLR value in head injury patients was 11.72 with a variation of7.31. The mean NLR level based on the severity of mild head injury was 10.15with a variation of 6.38. The mean NLR level of moderate head injury severity was12.70 with a variation of 7.09. The mean NLR level of severe head injury, was14.69 with a variation of 9.30. The results showed that there was no difference inthe mean of NLR levels in mild head injury patients with hemorrhage and withoutintracranial hemorrhage. The results showed that there was a significantrelationship between NLR levels in moderate head injury patients withhemorrhage (14.20) and without intracranial hemorrhage (7.20) (p value = 0.029),whereas in severe head injury it could not be assessed because there was nosample without hemorrhage. Conclusion: There is an association betweenincreased NLR and intracranial bleeding in moderate head injury patients

Abstract

A B S T R A C TIntroduction Corona Virus Disease 19 (COVID-19) infection was first reported inWuhan, China, in December 2019. The author wants to know how the OncologySurgery Division elective surgery patient services before and during early pandemicera of COVID-19 at Dr. M Djamil Padang Hospital. Method: This research is acomparative study with a quantitative approach to elective surgery patients in theOncology Surgery Division before and during early pandemic era of COVID-19 at Dr.M Djamil Padang Hospital. The sampling technique in this study was total sampling.Data analysis will be carried out using statistical analysis in accordance with theexisting data scale and using SPSS software. Results: In this study, 137 sampleswere obtained consisting of 80 samples before the COVID-19 pandemic and 57samples during early pandemic era of COVID-19. Of the 57 patients who underwentelective surgery from the Surgical Oncology Division during early pandemic era ofCOVID-19, all patients were not suspicious / confirmed. The time for oncologyservice assessment was 14.26 days before COVID-19 pandemic and 8.93 days duringearly pandemic era of COVID-19. Conclusion: More patients who underwent electivesurgery in the surgical oncology division were female, both before and during earlypandemic era of COVID-19. The mean age of patients before COVID-19 pandemicwas 48.72 years and 49.53 years during early pandemic era of COVID-19. There isa disparity between the length of time the assessment of elective surgery patients inthe surgical oncology division before and during early pandemic era of COVID-19pandemic.

Abstract

A B S T R A C TIntroduction : Dr. M. Djamil Padang General Hospital (RSUP) still appliesconventional methods as thyroidectomy treatment. Satisfaction and comfort to thescar is one of the issues because conventional thyroidectomy surgery scars arelocated in the neck area that will affect the confidence of patients, especially women.Method: This research was a quantitative research using cross sectional studydesign. The research was conducted at The Oncology Surgery Polyclinic of RSUPDr.M. Djamil Padang with a sample of 50 respondents. Result: More than half ofthyroid tumor patients found were women (56%), most patients had totalthyroidectomy surgery (70%) with malignan tumor type (72%), and the most tumorsize in the T4 group (52%). The average age of patients during surgery was 54.26years. The average patient satisfaction score for post-thyroidectomy scars was 14.72with a total score of 16, the lowest score was 6 and the highest score was 22.Significant relationships were found in variable tumor size (p= 0.000), type of surgery(p= 0.005), tumor type (p=0.004) and gender (p = 0.028), while there was nostatistically significant relationship between age and patient satisfaction with post-thyroidectomy scars (p=0,176). Conclusion: There were significant relationshipsbetween gender, type of surgery, type of tumor, tumor size to patient satisfactionwith post-thyroidectomy scars, while age has no signifacnt relationship to patientsatisfaction with post-thyroidectomy scars. A small number of PSAS score showedthat the average patient was satisfied with post-thyroidectomy scar.

Abstract

A B S T R A C TIntroduction : Inguinal herniation is a protrusion of peritoneal component throughabdominal wall caused by the weakness of aponeurotica tissue of abdomen. Inguinalherniation is the most common herniation cases in which 291.145 patient registeredin Indonesia at 2012 and 87 patients in General Government Hospital of M. DjamilPadang at 2017-2018. Herniation treatment is consisted of herniorrhapylaparoscopy using the TEP or TAPP techniques and both of them are related to theincidence of chronical post-operative pain that was defined by IASP as anuncomfortable sensation lasting for more than 3 months. Method: This researchwas using retrospective study design. The research was conducted at RSUP Dr. M.Djamil Padang in August – October 2020 with 54 samples. Result: The study foundthere was 54 cases of inguinal herniation fit to the inclusion criteria with peak ageof 82 years old and average of 59.1 years old. Most patients were male (79.62 %).There were 2 patients having chronic pain (7.41%) after herniorrhapy using the TAPPtechnique and 1 more (3.71%) having chronic pain with TEP technique. Bivariateanalysis was done by Fisher Exact Test and the study concluded there was nostatistical significance in relationship of chronic post-operative pain incidence withthe particular technique chosen at herniorrhapy laparoscopy (p=1.000). Conclusion:There was no significant relationship between the incidence of postoperative chronicpain in patients undergoing laparascopic hernioraphy and TEP or TAPP procedure.

Abstract

A B S T R A C TIntroduction Diabetic foot is an erosion or injury to the epidermis or the distal soleof the foot and the base of the foot in people with diabetes mellitus (DM). Diabeticfoot is a major cause of limb amputation in non-traumatic cases. Per year, as manyas 2-3% of people with diabetes experience complications of diabetic foot. Theprevalence of DM sufferers with diabetic foot is 20-40% in developing countries. InIndonesia, the prevalence of DM sufferers with diabetic foot is around 15% with 32%mortality rate. One factor plays an important role in predisposing the occurrence ofdiabetic foot is the lipid profile. The aim of this study was to determine the correlationbetween lipid profile and Wagner Severity Level on diabetic foot patients at Dr. M.Djamil Central Public Hospital, Padang in the period of July 2020 to December 2020.Method: This research is an observational analytic study with cross sectionalanalytic design. The research sample of 47 cases was taken by non-probabilitysampling with the consecutive sampling method. This research is an observationalanalytic study with cross sectional analytic design. The research sample of 47 caseswas taken by non-probability sampling with the consecutive sampling method.Result: There were 47 cases that met the inclusion criteria where the 51-60 yearsage group (55.3%) was the largest group in cases of diabetic foot. Of these groups, itwas dominated by female patients, namely 31 patients (66.0%). Meanwhile, the BodyMass Index (BMI) was dominated by the group with BMI <25 kg/m2 with a total of26 patients (55.3%). Diabetic foot patient at Dr. M Djamil Central Public HospitalPadang who underwent amputation was 25 patients (53.2%) where most of them,namely 40 patients (85.1%) had suffered from Diabetes Mellitus for> 5-10 years. Thefindings obtained based on the Wagner Severity Level were that the Wagner grade 3-5 (heavy) group consisted of 24 people (51.1%) and Wagner 1-2 (mild) as many as 23people (48.9%). The results of the bivariate test between lipid profile levels withWagner Severity Level in diabetic foot patients showed a significant correlation asindicated by a p value <0.05. Conclusion: Wagner Severity Level which is gettinghigher in diabetic foot patients shows low HDL levels, high LDL levels and high levelsof triglycerides in blood plasma.

Abstract

Breast cancer is a hormone-dependent disease that relies on the mitogenic effect ofestrogen to increase tumorigenesis and tumor growth. Clinically significant levels ofestrogen-α receptor (ERα) expression are seen in 80% of human breast cancers,whereas progesterone receptor is expressed in 55% of human breast cancers. Thesedata are one of the bases for the development of endocrine therapy. Endocrinetherapy is therapy that targets the pathway and synthesis of estrogen, by blocking itvia receptors, reducing circulating levels of estrogen, or suppressing estrogensynthesis in the tissues of women diagnosed with breast cancer.

Abstract

Introduction : Anxiety is one of the most non-motorized symptoms in patients with Parkinson's which greatly affects the quality of life, but in clinical practice it is often neglected. Anxiety in patients with Parkinson's can accelerate motor deterioration / disability and also increase mortality. Methods: This cross-sectional study was conducted at the Neurology polyclinic Dr. M Djamil Padang from July to December 2020 in patients who had been diagnosed with Parkinson's Disease (PD) by excluding secondary Parkinson's and a history of stroke. Anxiety was measured using the Hamilton Anxiety Rating Scale (HARS). The research subjects were 60 people. Univariate analysis to present baseline characteristics and bivariate tests assessed factors associated with anxiety and the relationship between variables. A p value <0.05 was considered statistically significant. Results: Most of the subjects were male (55%) with a mean age of 58.05 ± 9.7 years and disease duration of 6.35 ± 5.29 years. By examining the Hamilton Anxiety Rating Scale (HARS), it was obtained 38.3% of Parkinson's sufferers with anxiety. There is a significant relationship between marital status, duration of illness and degree of disease with the incidence of anxiety (p <0.05) and there is no significant relationship between age and sex with the incidence of anxiety in patients with Parkinson's. Conclusion: There is a significant relationship between marital status, duration of illness and degree of disease with the incidence of anxiety in patients with Parkinson's and there is no relationship between age and sex with the incidence of anxiety in patients with Parkinson's.

Abstract

Intoduction: Deep neck infection is the accumulation of pus in the potential space and facial area in the deep neck. The incidence of deep neck space infections is 1/100.000 in adults and 2/100.000 in children. Risk factors such as age, oral hygiene, and comorbidities such as DM are predisposed factors in deep neck infection. Effective pus collection of specimens is essential, to obtain the organisms and determine the effective antibiotics. Purpose: The aim of this study was to describe the characteristics and bacterial patterns of abscess in Dr. Mohammad Hoesin General Hospital Palembang. Methods: This study was conducted retrospectively using medical records of patients who diagnosed deep neck infection in Dr. Mohammad Hoesin General Hospital Palembang during January to December 2020 who took pus collection and been checked in microbiology laboratory. Data recorded include gender, age, site, onset, risk factors, comorbidities, bacteria culture, antibiotic sensitivity, and complications. Results: Of the 36 patients, 23 (63.8%) were males, followed by 13 (36.1%) were female, and the age of 31-45 years old group were found in 13 subjects (36.1%). The onset was mostly found at 0-1 week as many as 19 subjects (52.7%). The highest complaints of dysphagia and odynophagia were found in 22 cases (26,2%), odontogenic factors were found in 35 cases (97,2%). Abscess location in multiple space were found in 17 cases (47,2%), while in single space there were 19 cases (52.7%), where the most location was in submandibular space in 7 cases. Diabetes was the most comorbid factors found in 6 subjects (50%). The most bacterial were Klebsiella pneumoniae in 4 cases (11.1%) and sensitive antibiotics including ciprofloxacin, meropenem, and tigecycline, in 6 cases (20%) respectively. And the most complication was mediastinitis in 6 cases (75%). Conclusion: Klebsiella pneumoniae is the most common found in deep neck infection with sensitive antibiotics including ciprofloxacin, meropenem, and tigecycline.
