Abstract

Hypertension is a global health problem and is recognized as a major contributor to the burden of cardiovascular disease. The prevalence of hypertension continues to increase due to lifestyle changes such as smoking, obesity and psychosocial stress. It is important to educate people with hypertension to self-manage themselves at home. This community service activity aimed to provide health information about hypertension and self-management in the form of exercise and massage. This activity was attended by 30 Prolanis (Chronic Disease Management Program) participants at Johar Baru Health Center in Jakarta. There were 5 sessions in this activity: Session 1&2 for registration and measurement of physiological parameters, followed by cardiovascular exercises. Session 3 for health education about hypertension in general, then followed by next session on isometric exercises with a tennis ball and final session on of self-massage exercise. The evaluation done on this activity showed that the participants demonstrated isometric exercises and back massage techniques that can be carried out independently. Hypertensive patients are expected to be actively involved in Prolanis activities and conduct self-management intervention to maintain their blood pressure and prevent the complications.

Abstract

Diabetic ulcers are open wounds on the skin surface accompanied by necrotic tissue. Patients with diabetic ulcers tend to experience a significant increase in emotional distress, where individuals will feel negative emotions in daily life. This study aims to determine the effectiveness of mindfulness-based intervention: Open Heart prayer on emotional distress and blood sugar levels in diabetic ulcer patients at X Bekasi Hospital. This study used a quasi-experimental method with a two-group pre-test and post-test approach, involving 98 samples divided into a control group (49) and an intervention group (49). Open Heart prayer therapy was given 4 times a week for 4 weeks, with the research instrument being the DASS 42 questionnaire. The results showed a significant relationship between mindfulness-based intervention: Open Heart prayer in reducing emotional distress in the intervention group before and after the intervention (p-value <0.001). Diabetic ulcer patients who underwent Open Heart prayer experienced a 1-time decrease in emotional distress compared to patients who did not undergo the intervention. In addition, the decrease in emotional distress had an impact on reducing blood sugar levels (p-value <0.001). Patients who experienced a decrease in emotional distress showed a decrease in fasting blood sugar levels by 12 times (Exp (B) = 11.795) compared to patients whose emotional distress increased.  Mindfulness-Based Intervention Therapy: Open Heart prayer proved to be effective in helping reduce emotional distress and fasting blood sugar levels in diabetic ulcer patients. Multivariate results showed that the risk factors of gender and degree of injury had a significant effect (p-value <0.05) in reducing emotional distress, while age and degree of DM also had a significant effect (p-value <0.05) in reducing emotional distress.

Abstract

Diabetes mellitus is a metabolic disorder requiring patients to manage and stabilize their condition to prevent complications. Education using e-booklets can facilitate information access and blood sugar control. This study aimed to eval_uate the effectiveness of e-booklet-based education on self-care behavior and blood sugar levels in diabetes mellitus patients. Conducted from June to August 2023 at X Hospital's internist clinic in Bandung, the research employed a quasi-experimental pre-test and post-test design with a control group. The study involved 126 respondents, with 63 in the intervention group and 63 in the control group, selected through purposive sampling. The intervention lasted 4 weeks, covering 8 materials with ongoing monitoring and active participation. Self-care behavior and random blood sugar levels were measured at the beginning and end of the study using the Summary Diabetes Self-Care Activity (SDSCA) questionnaire and blood glucose tests. The Wilcoxon test revealed significant differences in self-care behavior and blood sugar levels before and after the intervention (p < 0.05). The Mann-Whitney test showed significant differences between the intervention and control groups (p < 0.05). Multivariate analysis indicated that age, gender, education, disease duration, and e-booklet-based education accounted for 22.2% of the variance in self-care behavior and 30.6% in blood sugar levels. E-booklet-based education effectively improves self-care behavior and blood sugar control. The study recommends ongoing development and supervision to establish healthy habits and prevent complications.

Abstract

Type 2 diabetes mellitus (DM) is a prevalent metabolic disorder often accompanied by complications such as diabetic peripheral neuropathy (DPN), which impairs sensation and quality of life. Non-pharmacological interventions such as patient education and Buerger Allen Exercise (BAE) have been recommended for managing symptoms and glycemic control, but evidence on their combined effectiveness in clinical settings in Indonesia remains limited. This study evaluated the effectiveness of structured diabetes education combined with Buerger Allen Exercise (BAE) in reducing peripheral neuropathy symptoms and improving fasting blood glucose (FBG) levels among type 2 diabetes mellitus patients. A quasi-experimental one-group pretest-posttest design was employed with 120 purposively sampled patients at Hospital X in Bandar Lampung. The intervention consisted of comprehensive DM education followed by supervised BAE training, performed twice daily for 15 minutes over 14 consecutive days. Neuropathy symptoms were assessed using the Michigan Neuropathy Screening Instrument (MNSI), and FBG was measured with a digital glucometer. Results showed significant reductions in both outcome measures post-intervention (). The mean MNSI score decreased from 5.600 to 2.996, and mean FBG declined from 146.48 mg/dL to 112.59 mg/dL. The proportion of patients with normal FBG increased from 38.3% to 79.2%. Multivariate analysis (MANCOVA) confirmed the intervention's significant effect after controlling for age, gender, and diabetes duration, contributing 44.2% to neuropathy reduction and 18.8% to FBG improvement. The findings demonstrate that integrated education and BAE is an effective, non-pharmacological strategy for managing diabetic complications and glycemic control, suitable for implementation in clinical and self-care settings.

Abstract

Peripheral Arterial Disease (PAD) merupakan penyempitan pembuluh darah arteri perifer yang disebabkan karena aterosklerosis akibat komplikasi dari Diabetes Mellitus (DM). Akibatnya terjadi penurunan aliran darah ke ekstremitas yang bila  tidak ditangani akan menyebabkan klaudisio intermiten, luka gangrene hingga berisiko amputasi. Penelitian ini bertujuan untuk mengetahui Efektivitas Buerger Allen exercise (BAE) dibandingkan dengan Rendam Kaki Air Hangat Terhadap Nilai Ankle Brachial Index (ABI) dan Gula Darah Sewaktu(GDS) pada pasien DM tipe 2. Jenis penelitian ini adalah Quassy Eksperimen prepost test and between two groups. Total sampel 54 penderita diabetes melitus di Rumah Sakit Sumber Waras yang diperoleh secara random dengan blok permutasi. Sampel dibagi menjadi 2 kelompok perlakuan yaitu BAEdan Rendam kaki air hangat. Perlakuan diberikan selama 5 hari berturut-turut dengan pelaksanaan 2 kali sehari yaitu pagi dan sore selama 15-20 menit. Alat yang digunakan untuk mengukur nilai ABIadalah Vascular Dopler. Berdasarkan uji paired T test di temukan ada perbedaan yang bermakna nilai ABI sebelum dan sesudah intervensi pada kelompok BAE (p=0.00), dan pada kelompok Rendam kaki air hangat (p=0.00); namun tidak ada perbedaan bermakna pada nilai GDS sebelum dan sesudah intervensi pada kedua kelompok perlakuan (p>0.05).Uji statistic Ancova di temukan ada pengaruh BAE terhadap nilai ABI dan Gula Darah Sewaktu yang dikontrol oleh variabel confounding dengan Corrected Model 0,000; namun tidak ada pengaruh rendaman kaki air hangat terhadap nilai ABIdan GDS yang dikontrol oleh variabel confounding dengan Corrected Model 0,065. BAE merupakan intervensi yang efektif dalam meningkatkan nilai ABI dibandingkan rendam kaki air hangat, namun Rendam kaki air hangat  lebih efektif dalam menurunkan nilai GDS.

Abstract

Diabetes Mellitus (DM) merupakan penyakit yang membutuhkan pengontrolan status metabolik untuk meminimalkan terjadinya komplikasi multi organ. Diperlukan edukasi yang terarah untuk meningkatkan perawatan diri dan indeks glikemik yang terkontrol. Tujuan penelitian ini untuk mengetahui efektivitas edukasi Self-care terhadap perilaku manajemen diri dan Gula Darah Sewaktu (GDS) pada pasien DM. Penelitian ini dilakukan bulan Juni-Juli 2020 diwilayah kerja Puskesmas Kelurahan Sukapura Jakarta. Desain penelitian adalah quasy eksperimental dengan pre-test dan post-test one group. Sampel dalam penelitian sebanyak 25 responden dipilih secara purposive sampling. Variabel GDS dan perilaku manajemen diri dinilai pada minggu 1 dan 4 dengan menggunakan kuesioner Diabetes Self-Management Instrument (DSMI). Berdasarkan uji Paired T-test ditemukan ada perbedaan bermakna pada perilaku manajemen diri sebelum dan sesudah diberikan edukasi Self-care (p 0,000 < 0,05 ), namun, tidak ditemukan adanya perbedaan bermakna pada GDS sebelum dan sesudah diberikan edukasi Self-care (p 0,956 > 0,05). Berdasarkan uji multivariate ditemukan bahwa lamanya menderita DM dan kadar GDS sebelum intervensi berpengaruh secara parsial terhadap variabel GDS setelah intervensi (p<0.05), sedangkan variabel usia dan lamanya menderita DM berpengaruh secara parsial terhadap perilaku manajemen diri (p< 0.05). Edukasi Self-care merupakan program efektif untuk memperbaiki perilaku perawatan diri dan GDS sebagai kontrol glikemik. Penelitian ini merekomendasikan perlunya menerapkan edukasi Self-care secara berkelanjutan untuk mencegah terjadinya komplikasi pada pasien DM.

Abstract

Latar belakang: Tingginya kejadian obesitas menjadi salah satu pemicu terjadinya prediabetes, dan banyak masyarakat yang tidak menyadari bahwa mereka menderita prediabetes. Penelitian bertujuan untuk mengetahui hubungan antara self-awareness pola makan terhadap kejadian Prediabetes di wilayah kerja Puskesmas Johar Baru. Tujuan: Penelitian yang bertujuan untuk menganalisa hubungan Self-awareness pola makan terhadap kejadian Prediabetes pada responden yang berada di wilayah kerja Puskesmas Johar Baru. Metode: Penelitian menggunakan Kuantitatif-non eksperimental dengan pendekatan cross sectional. Jumlah responden sebanyak 64orang yang dipilih menggunakan teknik purposive sampling. Penelitian ini menggunakan kuesioner Self Awareness Pola Makan dan Finnish Diabetes Risk Score (FINDRISC) untuk mengukur prediebetes. Analisa data menggunakan uji statistic Kendall Tau-C. Hasil: Analisis univariat menunjukkan mayoritas responden memiliki self-awareness rendah (62.5%) dan responden memiliki risiko prediabetes sedikit meningkat (34.4%) diikuti dengan risiko prediabetes tinggi (26.6%). Analisis bivarat menunjukkan ada hubungan yang bermakna antara self-awareness pola makan terhadap kejadian prediabetes (p <0.05). Kesimpulan: Hasil penelitian ini menunjukkan terdapat bahwa self-awareness pola makan berhubungan dengan kejadian prediabetes di wilayah kerja Puskesmas Johar Baru, Jakarta.

Abstract

Latar belakang: Salah satu komplikasi yang paling sering terjadi pada penderita Diabetes Mellitus (DM) adalah adanya ulkus diabetik. Masih banyak pasien DM yang tidak menjalankan perawatan kaki dikarenakan rendahnya pengetahuan pasien. Kurangnya informasi yang didapat oleh pasien menyebabkan pasien kurang memahami tentang tindakan yang perlu dilakukan untuk perawatan kaki. Tujuan: Penelitian ini bertujuan untuk mengetahui efektivitas pendidikan kesehatan terhadap pengetahuan perawatan kaki pada pasien DM di RS X, Bekasi. Metode: Penelitian ini menggunakan metode penelitian quasi eksperimen dengan bentuk posttest with control group. Terdapat 60 responden pasien DM yang dipilih menggunakan tehnik purposive sampling dan dibagi menjadi kelompok intervensi dan control masing-masing sebanyak 30 responden. Kelompok intervensi diberikan Edukasi perawatan kaki DM, dan tingkat pengetahuan diukur menggunakan kuesioner Knowledge of Foot Care. Hasil: Mayoritas responden pada kelompok intervensi memiliki pengetahuan yang baik (96,7%), sedangkan kelompok control memiliki pengetahuan sedang (36,7%). Menggunakan uji statistik Mann Whitney U Test didapatkan adanya perbedaan signifikan pada pengetahuan perawatan kaki antara kelompok intervensi dan kelompok control dengan p value=0,000. Kesimpulan: Pendidikan kesehatan perawatan kaki DM harus diberikan sejak dini sebagai upaya pencegahan terjadinya ulkus diabetikum berulang, terutama pada pemeriksaan kaki secara rutin, kebersihan kaki dan pemilihan alas kaki.

Abstract

Children of all ages can experience illness.  One of the many invasive procedures that hospitalized children will undergo is IV insertion. IV insertion is painful because it can cause pain, so management is needed to minimize pain. One of the non-pharmacological pain management is to use distraction techniques that can reduce the pain scale during infusion.  This study aims to analyze the effect of distraction techniques watching animated cartoons on the pain scale during infusion of children pre school at X Jakarta Hospital. The research design used was Quasi Experiment Non Equivalent Control Group Design involving 70 respondents who were divided into an intervention group of 35 respondents and a control group of 35 respondents.  The research instrument used a pain measurement sheet wong baker faces scale. The results of the Man Whitney test analysis showed that there was an effect of distraction techniques watching animated cartoons on the pain scale in children pre school during infusion at X Jakarta Hospital (p value = 0.000 <0.05). It is expected that nurses can start applying infusion installation with distraction techniques, especially audiovisual distraction techniques to minimize pain in pediatric patients.
