Abstract

Background: Stunting is still a serious public health problem, especially in developing countries like Indonesia. The incidence of stunting in children is multifactorial, one of which is thought to be caused by environmental factors such as lack of access to safe drinking water, sanitation facilities and hygiene practices (WASH). Purpose: To explore previous studies that are relevant in identifying water, sanitation, and hygiene (WASH) factors that play a role in the incidence of stunting in toddlers in Indonesia Methods: This research is a systematic review prepared based on the PRISMA flowchart method as a guide. Three electronic databases were used in the literature search, namely: Garuda Portal, Pubmed and Science Direct. Articles were selected based on publications in the last 3 years, namely from 2022-2024. Result: There were 8 articles from a total of 1,088 publications identified through three online databases that were deemed suitable for analysis. The results of the analysis show that Water (low community access to clean water and drinking water sources), Sanitation (means for disposing of feces and waste water), and Hygiene (the habit of washing hands with soap) have a significant influence on the incidence of stunting among toddlers in Indonesia. Implication: Policy makers can integrate WASH programs into programs that can support the reduction of stunting rates, such as nutrition programs, namely emphasizing the importance of clean water in food preparation, as well as educating about the relationship between poor sanitation and the risk of infection that can interfere with nutrient absorption.

Abstract

Stunting remains a pervasive public health concern, particularly in developing countries such as Indonesia. The etiology of stunting in toddlers is multifactorial, with one potential causative factor being maternal. The objective of this study is to examine extant studies that are pertinent in identifying maternal factors for the incidence of stunting in toddlers in Indonesia. The present study is a systematic review, meticulously organized according to the PRISMA flowchart method. A comprehensive literature search was conducted using three major electronic databases: Garuda Portal, Pubmed, and Science Direct. The selection of articles was based on the most recent three years of publications, commencing from 2022 to 2025. A total of 2,042 publications were identified through three online databases, and 12 articles were deemed eligible for analysis. The results of the analysis demonstrated that maternal characteristics (i.e., age, education, income, height, nutritional status, parity, and birth spacing) and maternal health conditions during pregnancy (i.e., weight gain, infectious diseases, history of anemia, and chronic energy deficiency) were significantly associated with the incidence of stunting in children under five in Indonesia. Additionally, maternal parenting related to nutrition fulfillment (i.e., exclusive breastfeeding and feeding practices) was associated with the incidence of stunting in children under five in Indonesia.

Snack habits and peer influence were found to have no significant effect on the incidence of diarrhea (ρ-value = 0.063 and ρ-value = 0.083). However, factors such as hand washing with soap (p-value = 0.023), toilet use (p-value = 0.000), and nail cleanliness (p-value = 0.002) had significant effects on the incidence of diarrhea. The use of toilets received an OR value of 35, indicating that respondents with toilet hygiene behaviors have a 35 times higher risk of experiencing diarrhea. Conclusion: Toilet use is the strongest predictor (35 times higher risk when proper toilet hygiene is not practiced).
