Abstract

Combination of valproate and fluoxetine are the most used combination in bipolar disorder therapy in Bhayangkara Hospital, Kediri. However, this combination has been controversial because of its risk of triggering mania or hipomania episode in 12 months. The aim of this study was to analyze mood change symptoms with YMRS and MADRS scale after treatment of valproate and fluoxetine combination in continuation phase treatment of bipolar disorder. This cross-sectional, observational study was conducted in psyciatry clinic in Bhayangkara Hospital Kediri between August 2016-October 2016 on 15 patients in YMRS. MADRS questionnaire was filled by physician before treatment and after 8 weeks treatments. Within 8 weeks, mean YMRS score changed from 1.4 to 1.26 (p>0.05). Whereas, mean MADRS score changed from 31.8 to 10.93 (p<0.05) after 8 weeks. There was no significant different in YMRS score between pre- and post-treatment, but there was significant different in MADRS score after treatment.

Abstract

Introduction: Olfactory Reference Syndrome (ORS) is a rare, under-recognized, and understudied psychiatric condition characterized by a persistent belief about the perceived odor, ideas of reference, and engagement in repetitive cleansing behaviors. The condition causes significant psychological distress, leading to social withdrawal, impaired occupational functioning, and, in severe cases, complete isolation. Greater clinical awareness and accurate diagnosis are essential to ensure appropriate psychiatric intervention and improve patient outcomes. Methods: The method of writing a case report is based on direct observation of the patient’s condition at the Bhayangkara Hospital, Kediri, in 2024 during the treatment period. Case: This report discusses a 21-year-old patient who has experienced a persistent belief that he smells and is being avoided by others for the past 9 months. The patient said that people around him liked to cover their noses when they were near him. The patient often took a shower 5 times a day because he continued to feel smelly. The patient was prescribed lorazepam 0.5 mg two times a day as an antidepressant and anti-insomnia agent. Conclusion: In patients with Olfactory Reference Syndrome (ORS), symptoms that appear include preoccupation with unpleasant body odor, delusional beliefs about the impact of the odor on other people’s judgments, and compulsive behavior such as frequent bathing or using deodorant to overcome these concerns. In this case, the patients are included in the symptoms of Olfactory Reference Syndrome (ORS). The administration of a combination of antipsychotic and antidepressant therapy in this patient showed significant improvement in symptoms.

Academic stress is associated with mental disorders such as depression, anxiety, physical, and academic challenges. Medical students are particularly susceptible to high levels ofstress due to demanding academic workloads, intense competition, and prolonged studyperiods. Assessments like Objective Structured Clinical Examination (OSCE), whichevaluate students' clinical skills, also contribute to stress. One of the key factors insuccessfully facing this test is ego strength and emotional intelligence. This study aims toinvestigate the relationship between ego strength, emotional intelligence, and clinicalskills performance. A quantitative research design employing an analytical observationalapproach and a cross-sectional methodology was utilized. The instrument used tomeasure academic stress is the MMPI-1, while emotional intelligence is the TEIQue-SF.A total sampling method was employed for participant selection. The research findingsshow that the majority of respondents have low levels of ego strength (60%), a mediumlevel of emotional intelligence (77%), and OSCE scores was predominantly A grades. There was a weak association between emotional intelligence and OSCE performance (p= 0.784). Similarly, relationship between ego strength and OSCE performance (p =0.314). It is important that the identified relationships are correlative rather than causaland their impact is not statistically significant.
