Abstract

Kata kunci: AIDS, pengawas minum obat (PMO), rifampisin, tuberculosis, AbstractPapua shows the highest prevalency rate of HIV-AIDS in Indonesia and almost 60% people who live with HIV develop tuberculosis (TB), hence TB is the most common opportunistic infection in HIV patients. This study was aimed to know and to describe the profile of TB-HIV co-infection patients among TB patients according to clinical symptoms and the side effects of FDC (Fixed Dose Combination) drugs. Methods used was descriptive study taken from medical record of 124  TB-HIV co-infection patients in Dok II Jayapura hospital, Papua during January  2011 – September 2012. Results showed that 35 of 124 patients indicated side effect of antituberculosis risk default when they did not undergo drug monitoring therapy. The patients should be controlled by his/her family to help them through all the side effects of antituberculose drugs to complete the treatment. Keywords: AIDS, drug monitoring therapy, rifampicin, tuberculosis

Abstract

This study aimed to translate and cross-cultural adapt the version of the Beliefs about Medicines Questionnaire (BMQ) into Indonesia language, and explored its psychometric properties, and establish preliminary norms. We followed the guideline for forward-backward translation and 201 HIV out-patients were recruited. They all completed BMQ-general and BMQ-specific scales that were previously culturally adapted and translated into the Indonesian language. All participants were patients visiting a referral hospital in Papua Province, Indonesia. One-third of participants were indigenous Papuans, and the majority were female. The overall Cronbach's alpha of BMQ was acceptable (0.80) however each domain of BMQ was lower than the original study. The Cronbach alpha value for each section of BMQ-Indonesian version was as follows: BMQ Specific-Necessity 0.56; BMQ Specific-Concerns 0.50; BMQ General-Overuse 0.65; and BMQ General-Harm 0.53. The reasons for the distinction between this study to original may be complex and HIV associates with the problem in neurological and cognitive symptoms at a later stage. Since the Cronbach alpha in this study was within the range of the original study the BMQ-Indonesian version is applicable to be used in Indonesia, especially among HIV patients but caution is still needed in this questionnaire.

Abstract

ABSTRACT Papua as the highest HIV-AIDS prevalence region in Indonesia will be against Tuberculosis (TB) as opportunistic infection of the majority of patients with HIV-AIDS. The success of the treatment is determined by how the patients handle the side effects of FDC (Fixed Drug Combination) successfully during taking the drug for 6-8 months. This study was aimed to find out some influenced factors leading to fail treatment for HIV-AIDS patients after getting FDC (Fixed Dose Combination) drug, from Januari 2011 to September 2012. The retrospective descriptive method was taken to collect data from the medical records, TB- 01 form, and the microbiological testing for TB patients from January 2011 to September 2012. Descriptive study from medical record of 124 TB-HIV coinfection patients in RS Dok II Jayapura, Papua in January 2011 – September 2012 resulted that 35 of 124 patients represented as default patients and they did not have someone as treatment observer because they wanted to keep their HIV status. Patient who has someone from the core family as observer treatment can complete TB treatment and getting help in through all the side effect of FDC drug. Abstract

Abstract

Malaria remains a leading cause of death in Papua and combination of Dihydroartemisin -Piperaquine (DHP) and Primaquine (PQ) is recommended treatment to replace chloroquine out since chloroquine resistance present. Moreover, awareness of rational prescription and adherence may be the first priority in malaria treatment in preventing death and resistance in Papua. This study aimed to assess the rationality of antimalaria prescription in the public and private sectors of Papua Province. This cross-sectional study was conducted. Overall, 571 prescriptions were included in this study and more than half of prescription was rational (65%). This study was predominantly female (65%). More than half was diagnosed by P. falciparum (54%), using referred using the public hospital for treatment (54%) and self-identified as Papuan (50%). According to type of malaria, incorrect of prescription DHP and PQ were 20% and 18%, respectively. The multiple logistic regression model showed that female was significantly associated to receive irrational of prescriptions than males (OR = 1.549; 95% CI =1.004-2.389). Participants who had P. falciparum increased the likelihood of irrational of prescriptions (OR= 1.641; 95% CI =1.144-2.355). There is a need to have a continuum strategy to improve access and utilization of malaria case management in both public and private health facilities. Inequality gender based on prescribers needs to be investigated. Competence improvement among healthcare providers needs to be a priority to prevent antimalarial resistance.
