Abstract

Background: Triple elimination is the elimination of transmission of HIV, syphilis, and hepatitis B from mother to child. The Prevention of Mother to Child Transmission (PMTCT) program is a comprehensive activity. The basic condition of the case is carried out because of the importance of health promotion and early detection in strategies for managing HIV, Syphilis, and Hepatitis B infections. This study aimed to analyzecontextual effect of community health service on the use of triple elimination service in pregnant women.Subjects and Method: This was a cross sectional study conducted at 25 community health centers in Sragen Regency, Central Java, from June to July 2019. A sample of 200 pregnant women was selected by stratified random sampling. The dependent variable was the use of triple elimination. The independent variables were age, perceived susceptibility, perceived seriousness, cues to action, attitude, motivation, occupation, income, and distance to health service. The data were collected by a set of questionnaire and analyzed by multilevel multiple logistic regression. Results: The use of triple elimination increased with age ?35 years (b=1.29; 95% CI=0.37 to 2.21; p=0.006), working outside the home (b=1.20; 95%CI=0.17 to 2.23; p=0.012), income by ?Rp 1,600,000 (b=1.52; 95% CI=0.54 to 2.50; p=0.002), perceived susceptibility (b=1.46; 95% CI=0.53 to 2.38; p=0.002), perceived seriousness (b=1.53; 95% CI=0.55 to 2.50; p=0.002), cues to action (b=1.30; 95% CI=0.30 to 2.30; p=0.010), attitude (b=1.10; 95% CI=0.02 to 2.18; p=0.046), and strong motivation (b=1.68; 95% CI=0.66 to 2.71; p=0.001). The use of triple elimination decreased by a distance ?5km (b=-1.36; 95% CI=-2.27 to -0.46; p= 0.003). Community health center had strong contextual effect on the use of triple elimination with ICC= 22.6%.Conclusion: The use of triple elimination increases with age ?35 years, working outside the home, income by ?Rp 1,600,000, perceived susceptibility, perceived seriousness, cues to action, attitude, and strong motivation. The use of triple elimination decreases by a distance ?5km. Community health center has strong contextual effect on the use of triple elimination. Keywords: triple elimination, Health Belief Model, Theory of Planned BehaviorCorrespondence: Prissy Verasita. Abstract

Abstract

Osteoporosis is a metabolic bone illness which involves bone density loss and micro destruction, enabling bone to develop into fragile, porous, and readily shattered. There are both controllable and non-modifiable risk variables for osteoporosis. Smoking, drinking behavior, and numerous other modifiable variables are examples. The goal of this research is to identify the possibility of osteoporosis among senior smokers. Research purposes to determine the influence of smoking behavior and the incidence of osteoporosis The following PICO criteria are used in this systematic review and meta-analysis investigation: Population: The elderly are the majority of the population. Intervention: smoking. Comparison: not smoking. Outcome: Osteoporosis. The articles used in this research were obtained from three databases: Google Scholar, Pubmed, and Science Direct. Keywords to search for articles are "Obesity" AND "Preeclampsia" AND Multivariate AND Pregnancy. Over 2013 and 2023, articles were utilized. To choose articles, the PRISMA flow diagram has been used. The papers were evaluated using the Review Manager 5.3 instrument. Seven cohort studies in the country of the United States (America), Europeans (Bosnia and Herzegovina), China, Iran, and Korea are among the countries in Asia that have been included in the systematic review and meta-analysis.. In accordance to the cohort study's forest plot results, elderly smokers have a 1.30 times greater risk of osteoporosis than elderly nonsmokers (aOR= 1.30; 95% CI= 0.81 to 2.08), and the results of this study are statistically significant (p <0.001).

Abstract

Blood vessel disorders can occur due to the narrowing and hardening of the artery walls, which is called atherosclerotic plaque. Stroke is a serious health problem because of the high mortality and morbidity rates and its impact can cause chronic disability and occurs not only in elderly people but also in young people. This study aims to estimate and analyze the effect of diabetes mellitus on the incidence of stroke. This research is a systematic review and meta-analysis research using the PRISMA diagram. Article searches were carried out based on the PICO Model eligibility criteria. P= DM patient; I= Diabetes mellitus; C= Normal blood sugar; O= Stroke. The articles used come from Google Scholar. With keywords including "Diabetes mellitus" AND "Normal blood sugar" AND "Stroke" AND "adjusted Odds Ratio". Articles were analyzed using the PRISMA diagram and the Review Manager 5.3 application. 5 articles (2016-2022) with a randomized controlled trial study design will be used as a source for meta-analysis of the effect of diabetes mellitus on the incidence of stroke. Shows that people with diabetes mellitus increase the likelihood of stroke. Patients suffering from diabetes mellitus increase the likelihood of stroke by 2.41 times compared to those with normal blood sugar. (aOR= 2.41; 95% CI= 1.71 to 3.39; p=0.0001), and statistically significant. The forest plot also shows high heterogeneity of effect estimates between studies (I2 = 73%). The funnel plot shows that there is publication bias which tends to exaggerate the true effect (overestimate). Meta-analysis of 5 studies cross-sectional concluded that people with diabetes mellitus increase the likelihood of stroke.

Abstract

Preeclampsia is the onset of high blood pressure accompanied by proteinuria and edema in pregnancy after 20 weeks of gestation or after the postpartum period. Obesity has been widely studied as a risk factor for preeclampsia. This study aims to estimate and analyze the impact of obesity on the incidence of preeclampsia in pregnant women. This research is a systematic review and meta-analysis research using the PRISMA diagram and using 4 articles from 2015 to 2020. Article searches were carried out based on the PICO Model eligibility criteria. P= Pregnant women; I=Obesity; C= Not obese; O= Preeclampsia. The articles used come from 1 database, namely: Google Scholar. With keywords including "Obesity" AND "Preeclampsia" AND "Pregnant Women" AND "Cross Sectional".Articles were analyzed using the PRISMA diagram and the Review Manager 5.3 application. 4 articles with a cross-sectional study design will be used as a source for meta-analysis influence of obesity on the incidence of preeclampsia in pregnant women. Shows that obesity increases the possibility of preeclampsia in pregnant women. Obese pregnant women have an increased risk of preeclampsia by 2.04 times compared to non-obese pregnant women (aOR= 2.04; 95% CI= 1.90 to 2.19; p=0.001), and the results are statistically significant. Meta-analysis of 4 cross-sectional studies concluded that obesity in pregnant women increases the occurrence of preeclampsia.
