Abstract

Background: National data indicate an increase in chronic disease prevalence. Therapeutic outcomes need to be assessed based on the aspects of the patient’s psychology, including treatment satisfaction. In Indonesia, no native or cross-culturally adapted treatment satisfaction tool is available and validated for chronic disease patients.Objectives: The study is intended to conduct cross-cultural translation and adaptation of the Treatment Satisfaction with Medicines Questionnaire (SATMED-Q) into Indonesian and to evaluate the questionnaire’s reliability and validity in the Indonesian context.Methods: SATMED-Q translation and adaptation were conducted following a systematic procedure: forward translation, synthesis, backward translation, expert committee review, and pre-testing. The samples were hypertension and type 2 diabetes mellitus patients at the primary health care center. The internal consistency and known-group validity were analyzed statistically.Results: A total of 45 participants were involved in the research to assess the reliability and validity of the translated questionnaire. Some grammatical changes were made on the forward translation, the back translation, and the expert committee review stage. The major changes were the definition of the Likert-type scale on each domain. The reliability test showed good consistency (Cronbach’s alpha=0.842). Certain factors (gender and working status) showed known group validity, but not the number of antihypertensive or antidiabetic medications. Compared to individuals who took two medications, people who took one medicine indicated less satisfaction with their treatment.Conclusion: The translation and cross-cultural adaptation of the SATMED-Q to Indonesian was satisfactory. The Indonesian version of SATMED-Q was a reliable and valid instrument to assess treatment satisfaction.

Abstract

Stroke in Yogyakarta has the second highest prevalence rate in Indonesia, which requires long-term care and rehabilitation at a high cost. Stroke patients treated in classes 1, 2 and 3 are known to have experienced an increase in INA-CBG rates based on Minister of Health Regulation Number 3 of 2023. However, some hospitals often experience discrepancies between actual costs and INA-CBG's rates. Therefore, this study is the first to assess the cost of inpatient stroke treatment using the INA-CBG's tariff based on the Minister of Health Regulation Number 3 of 2023. This study aims to determine the average actual cost, the appropriateness of the average actual cost, and the difference in actual costs at a Regional General Hospital according to the INA-CBG's tariff in 2023. The research method used an observational design with a cross-sectional approach and retrospective data collection on stroke patients hospitalized from January to September 2023. Data were analyzed using the descriptive analysis method and one sample t-test if the data were normally distributed and the Wilcoxon test if the data were not normally distributed. Based on the study's results, it is known that the average actual cost does not exceed the INA-CBG tariff, so the hospital is profitable.

Abstract

The elderly population has the highest risks of bleeding due to oral anticoagulant use to prevent secondary stroke since causes higher levels of anticoagulant status fluctuation. However, the detailed information on safety issues regarding bleeding effect of oral anticoagulant from elderly population perspective with atrial fibrillation is still lacking. This narrative review was conducted to summarize the study findings from the present literature. This review was conducted using PubMed and Scopus database. Data were retrieved from 2017 until 2023. Articles were considered for inclusion if they discussed the safety of oral anticoagulant for secondary stroke prevention in Atrial Fibrillation (AF) patients. A total of 9 articles were extracted. The design of the studies, participant characteristics, and the risk of bleeding events were all assessed. The Hazard Ratio (HR) ranged from 2.82 to 0.58 across 5 trials. There were three occurrences of significant hemorrhage with delayed Non-Vitamin K Antagonist Oral Anticoagulants (NOACs) and seven cases with early NOACs. When comparing acute reperfusion to no reperfusion, the Odd Ratio (OR) of 1 study was 0.74. One study found that the incidence of significant bleeding was 2.1% in the case of standard warfarin use and 0% in the case of early apixaban use. Research comparing warfarin and rivaroxaban in patients with mild AF found a Relative Risk (RR) of 0.91. NOACs/Direct Oral Anticoagulants (DOACs) are relatively safe to use, while International Normalized Ratio (INR)-guided Vitamin K antagonists (VKAs) should be use with cautions in the elderly with AF for secondary stroke prevention concerning bleeding events, regardless of independence status (patient’s level of functional independence), presence of acute reperfusion treatment, Embolic Stroke of Undetermined Source (ESUS), Patent Foramen Ovale (PFO), and decreased renal function.

Abstract

The 2018 Riskesdas results showed that the prevalence of delivery by sectio caesarea in DIY was 23.1%. The high cost of sectio caesarea hinders access to public health, so the government implemented the INA-CBG's tariff. This study is the first to evaluate the INA-CBG's tariff based on the Permenkes RI Number 3 of 2023 for inpatient sectio caesarea delivery. This study aims to determine the difference between the cost of sectio caesarea delivery and the INA-CBG's tariff based on Permenkes RI Number 3 of 2023. Through this research, hospitals are expected to evaluate their services so that the costs of cesarean sections are under the INA-CBG's tariff. In addition, this research can serve as an evaluation of the INA-CBG's tariff for the government. This study used observational research methods and a cross-sectional approach. Data were obtained retrospectively by tracing medical records and treatment cost data of sectio caesarea patients from January to September 2023. Data were analyzed by descriptive analysis, one-sample t-test and one-sample Wilcoxon methods. The results of this study showed that the average actual cost was greater than the INA-CBG's rate, with significant differences in patients with class I code O-6-10-I, class II codes O-6-10-I and O-6-10-II, and class III code O-6-10-II, so that PKU Muhammadiyah Yogyakarta Hospital suffered losses.

Abstract

Permasalahan utama pasien DM adalah kepatuhan dalam minum obat mengingat DM merupakan penyakit kronis yang membutuhkan terapi jangka panjang. Ketidakpatuhan dalam manajemen terapi DM dapat meningkatkan biaya pengobatan dan perawatan. Penelitian bertujuan untuk mengukur efektivitas model edukasi pada PMO (Pengawas Minum Obat) dan pasien DM tipe 2 dalam meningkatkan kepatuhan minum obat pada pasien DM tipe 2 di RSI (Rumah Sakit Islam) Pondok Kopi. Metode penelitian yang dipakai adalah penelitian eksperimental dengan rancangan quasi experimental – pre-test/post-test study design yang dilakukan secara prospektif. Intervensi yang diberikan adalah edukasi berupa ceramah dan pembagian booklet yang diberikan sebanyak 3 kali selama 2 bulan periode penelitian. Uji statistik yang digunakan adalah paired-sample t test dengan CI 95% untuk melihat perbedaan kadar HbA1c sebelum dan setelah pemberian intervensi. Perbandingan kepatuhan pasien berdasarkan skor kuesioner MMAS-8 dilakukan dengan uji Wilcoxon. Nilai HbA1C pasien sebelum intervensi rata-rata 7,2 dan setelah edukasi nilai HbA1c rata-rata sebesar 5,5 (P<0,05). Skor MMAS-8 pada awalnya rata-rata 4,4 menjadi rata-rata 2,4 setelah diberikan intervensi berupa edukasi (P<0,05). Kesimpulan penelitian ini adalah model edukasi pada PMO dan pasien DM tipe 2 di Rumah Sakit Islam Jakarta Pondok Kopi efektif untuk meningkatkan kepatuhan pasien dengan menurukan nilai HbA1c dan skor MMAS-8.

Abstract

Indonesian children still have limited access to books, and reading activities have not become a culture in Indonesia. This problem worsens with the Covid-19 pandemic because many schools were closed or changing the method to learning from home. In addition to reading, health literacy is also crucial during this pandemic. This service activity aims to increase interest in reading through training and establishing the children's reading park. The method used was training and diffusion of science and technology. The training was conducted online through video conferencing applications and live streaming via popular video sites. Evaluation of activities was carried out using a pre-test and post-test. More than 80 participants joined the training activities. From the questionnaire results, there was an increase in the average score of the participants after receiving the training. The children's reading park has been formed and is ready to be opened to the public. The procurement of books was carried out through grants from donors, and more than 300 reading books were collected.

Abstract

The practice of self-medication is relatively high among Indonesian people. Inappropriate self-medication practices can have a negative impact. One of the diseases that is often treated with self-medication is a skin disease. Community service activities were carried out on February 19, 2023, at the Al Barokah Musholla, Cileungsi. The activity was attended by 27 participants consisting of mothers. The method used in this activity was Community Based Interactive Approach (CBIA). The activity begins with a presentation about the management of skin disease. Furthermore, participants were divided into several groups, and participants were asked to discuss the medicines that had been provided. Participants put the results of the discussion on a worksheet accompanied by a tutor. Participants filled out pre-test and post-test questionnaires to see changes after being given education. The results of statistical analysis showed that there was an increase in participants' knowledge after being given education with a p-value of 0.011. It can be concluded that self-medication counseling activities on skin diseases with the CBIA method can increase participants' knowledge

Abstract

This community service activity was carried out in the society-based library blok E, Pasir Angin village, Cileungsi, Jawa Barat. This activity aimed to increase the knowledge and skills of society-based library administrators in managing their society-based library.  Furthermore, a coloring competition for kids and counseling on healthy and safe snacks was also held which aimed to increase public knowledge and at the same time encourage public interest in visiting this society-based library. The training activities were carried out using the discussion method and practice on how to classify books, make catalog cards, book call numbers, and the membership lists of society-based libraries. Counseling activity was carried out using lecture and discussion methods. Evaluation of the training activity was carried out using the interview method, while the counseling activity was evaluated using a questionnaire instrument. The result be achieved from this activity is that the administrators can manage the society-based library well so that they are able to foster public interest in visiting and at the same time stimulate public interest in reading.

Abstract

The leaf of Mangifera indica L. contains flavonoids and mangiferin which showed positive effects on wound healing of diabetic ulcer. The used of suitable surfactant and co-solvent is required to ensure the high solubility of the active pharmaceutical ingredient (API), thereby optimizing the efficacy. This study aims to determine the ideal surfactant and cosolvent for a thermosensitive hydrogel formula of Mangifera indica leaf ethanol extract. The leaf was fextracted by maceration using 70% ethanol then determined for moisture content.  The extract was prepared for solubility tests of flavonoid and mangiferin on surfactants (Kolliphor® EL, Tween® 20, Tween® 80, Miranol® C2M) and co-solvents (Glycerin, PEG 400, and transcutol). Methods for total flavonoids content were Spectrophotometry UV-Vis and mangiferin with RP-HPLC method. The moisture content in the extract is 8.590 ± 0.754%. The surfactant demonstrating the highest capability in flavonoid dissolution is Tween 20 (1863.750 ± 0.838 µgQE/g extract), followed by Tween 80, Kolliphor EL, and Miranol C2M. In the context of co-solvents, PEG 400 (1309.583 ± 0.292 µgQE/g extract) show superior flavonoid dissolution capability, with glycerin and Transcutol following in descending order. Tween 20 and Tween 80 exhibite comparable efficacy in mangiferin dissolution, followed by Miranol C2M and Kolliphor EL. Among co-solvents, Transcutol demonstrate the highest aptitude for mangiferin dissolution, succeeded by PEG 400 and glycerin. This study indicates Tween 20 is a preferable surfactant and PEG 400 was considered as co-solvent to be further used in a thermosensitive gel formula for diabetic ulcer.Â
