Abstract

Disasters can occur at any time and without knowing the time of occurrence. Some of the impacts of disasters that occur in life are damaged health facilities, lack of implementation of posyandu for the elderly and limited reach of health services, thereby increasing morbidity rates in the community, especially the elderly. One of the non-communicable diseases (NCDs) that often occurs in the elderly is hypertension. In the post-disaster period, elderly people who suffer from hypertension need to have their blood pressure monitored and controlled to prevent complications from hypertension. Hypertension is one of the main risk factors for global death and is estimated to cause 9.4% of world deaths annually (WHO, 2015). Cadre training in preventing post-disaster illnesses in the elderly is very important to minimize and reduce the risk of complications from hypertension in disaster victims. The aim of this community service activity is to control potential complications of hypertension in the elderly in the post-disaster period. The target of this community service activity is elderly posyandu cadres who live in Gheogoma Village, Ende District, Ende Regency. The method used in this activity is providing health education through counseling, mentoring and training of cadres, training in checking blood pressure. The location and time of the activity will be held on 12-13 June 2023 in Gheogoma Village, Ende District, Ende Regency. The results of the activity showed that 80 percent of the cadres had good knowledge and of the 12 participants, 11 cadres were able to measure blood pressure well, and 3 cadres were adequate at measuring blood pressure. Abstract

The role of cadres is very important in efforts to anticipate and handle hypertension complications such as cardiac arrest. One of the roles of cadres when the elderly experience cardiac arrest is that cadres are able to prevent and treat cardiac arrest by providing basic life support and are able to give advice to the elderly to avoid things that can be at risk of causing sudden death. The Community Service activity was held on May 1-2, 2024 in the Gheoghoma Village office hall and was attended by 11 village cadres and the Head of Gheo Ghoma Village. The method used is education and skills to do basic life support. At the beginning of the activity, a test of cadre knowledge about basic life support was carried out and then simulations and demonstrations were carried out and then closed with a post test. From the results of the activity, data was obtained that most of the cadres had understood and were able to provide basic life support, as evidenced by the results of the answers to the questions given where the respondents had less knowledge than 75% and in the post stage after being given material and demonstrations on the treatment of cardiac arrest, the results of the increase in respondents' knowledge about basic life support were obtained, which was as much as 83%. In addition, in training activities, cadres are able to carry out basic life support using BHD phamptom aids and obtain a score from 0% to 75%. Therefore, it is highly expected that the village management will continue to cooperate with the government, especially the cadres in improving knowledge and other skills in preventing sudden death in the community by conducting continuous training so as to increase knowledge and skills that will have an impact on the prevention and handling of sudden death due to cardiac arrest

Abstract

According to Basic Health Research (RISKESDAS, 2018), the prevalence rate of asthma at all ages in Indonesia is 2.5%. The incidence of asthma is most common in the age group of 75 years and above and begins to decrease in the age group of 15-24 years. The role of nurses in providing nursing care to patients is by providing pharmacological therapy, non-pharmacology and providing education about bronchial asthma. In the implementation of nursing care in asthma cases, nurses are challenged or have problems with the lack of understanding of nurses about the use of literature in setting service standards such as having to use the Indonesia Nursing Diagnostic Standards (SDKI), Indonesia Nursing Output Standards (SLKI), Indonesia Nursing Intervention Standards (SIKI) and patient non-compliance in preventing asthma attacks. This case study aims to describe the nursing care of Mrs. M. E. with a medical diagnosis of bronchial asthma and analyze the gap between theory and real cases. The method used is a case study with a nursing care approach which includes review, diagnosis, planning, implementation and evaluation of nursing. The results of the case study found three nursing problems in the case, namely ineffective airway clearance related to airway hypersecretion, ineffective breathing patterns related to inhibition of breathing effort, and sleep pattern disorders related to shortness of breath. Nursing interventions and implementations are designed and implemented with dependent actions such as observation, education and interdependent such as collaborative actions. After 3 days of implementation, the problem of ineffective airway cleaning was partially resolved, and the problem of ineffective breathing patterns and sleep pattern disorders was resolved. After 3 days of implementation, the nursing problem of ineffective breathing patterns and sleep pattern disorders was resolved, while the problem of ineffective airway clearance was partially resolved and it was recommended for patients and families to follow all recommendations from health workers with the aim of preventing the onset of more severe complications
