Abstract

Perineal tissue wounds occur either spontaneously or through episiotomy after childbirth. The combination of aloe vera and betel leaf can be used as an alternative therapy. The purpose of this study was to determine the effectiveness of Lisih spray, a mixture of aloe vera and betel leaf perineal wound healing. The study design used a quasi-experimental study, with a population of 32 postpartum women on day 1 (second-degree perineal wounds). The study instrument used a REEDA Score.

Abstract

Birthball/Birthing Ball (bola kelahiran) adalah bola terapi fisik yang membantu ibu inpartu kala I ke posisi yang nyaman dalam membantu Kemajuan Persalinan Pada Kala I dan mengurangi nyeri persalinan dapat digunakan dalam berbagai gerakan salah satunya gerakan Pelvic Rocking merupakan salah satu metode untuk membantu penuruna kepala dan membantu merespon rasa sakit. Tujuan penelitian ini adalah ingin mengetahui efektivitas penerapan gerakan Pelvic Rocking dengan menggunakan Birthing Ball terhadap kemajuan persalinan dan penurunan nyeri pada kala 1 fase aktif. Rancangan pada penelitian ini adalah penelitian Quasy Eksperimen dengan poplasi adalah ibu bersalin kala I fase laten (pembukaan < 4 cm) di TMPB Ny “ K” Ngampel Mojoroto Kota Kediri dengan sampel sebanyak 32 yang dibagi menjadi dua kelompok, instrument penelitian menggunakan lembar observasi, data diuji dengan menggunakan uji Man-Whitney. Hasil penelitian pada kelompok intervensi hampir seluruhnya (93,75%) mengalami kemajuan perasalinan, sedangkan pada kelompok kontrol sebagian besar (75%) tidak mengalami kemajuan persalinan. Penilaian intensitas nyeri pada kelompok intervensi didapatkan sebagain besar (75%) skala 3 nyeri sedang dan sebagian kecil (25%) nyeri skala 2 nyeri ringan, kelompok kontrol didapatkan hampir seluruhnya (81,3%) mengalami nyeri skala 4 nyeri. Uji statistic menggunakan Man-Whitney didapatkan nilai p 0,001 < 0,05, sedangkan efektivitas penerapan gerakan pelvic rocking dengan menggunakan Birthing Ball terhadap nyeri persalinan Kala 1 Fase Aktif didapatkan nilai p 0,000 < 0,05 disimpumpulkan gerakan Pelvic Rocking dengan menggunakan Birthing Ball efektiv meningkatkan kemajuan persalinan dan menurunkan nyeri persalinan kala I fase Aktif di TMPB Ny “ K” Ngampel Mojoroto Kota Kediri. Gerakan Pelving Rocking dengan menggunakan Birthing Ball dapat dijadikan alrernatif dalam meningkatkan kemajuan persalinan dan mengurangi nyeri persalinan.

Abstract

As Indonesia’s Jaminan Kesehatan Nasional (JKN) transitions from an expansionary phase to maturity, it faces the classic iron triangle of health policy: the tension between expanding access, containing costs, and maintaining quality. While coverage rates have soared, critical questions remain regarding the equitable distribution of these benefits in a post-pandemic landscape. This study employs an Integrative Systematic Review design, synthesizing high-impact quantitative and qualitative evidence published between 2021 and 2024. Data were extracted from six primary studies utilizing large-scale national datasets (SUSENAS, IFLS) and policy reviews. The analysis moves beyond simple pooling to perform a narrative synthesis of adjusted Odds Ratios (aOR) for utilization and benefit incidence, assessing the structural determinants of effective coverage. The synthesis reveals a distinct inverse equity trade-off. While JKN ownership significantly increases the probability of inpatient utilization (aOR: 2.35), the benefits are unevenly distributed. A middle-class capture phenomenon is evident, where upper-middle-income groups experience a 41% reduction in out-of-pocket (OOP) expenditure compared to 38% for the poorest quintile. Furthermore, a quality gap persists, with non-poor populations seeing a greater reduction in unmet needs (10.4%) than the poor (7.7%), largely driven by supply-side rigidities in remote areas and administrative literacy barriers. In conclusion, JKN has successfully dismantled financial entry barriers but has not yet resolved structural inequities. The system currently functions as a regressive subsidy where the urban middle class extracts disproportionate value. Future policy must pivot from coverage expansion to supply-side equity, implementing geographic capitation differentials and targeted non-medical benefits for vulnerable populations to close the gap between legal entitlement and effective access.

Abstract

Inadequate breast milk production remains a common problem among postpartum mothers and may affect exclusive breastfeeding practices. Although moringa leaves and banana blossom have been traditionally used as natural galactagogues, evidence regarding their combined effect on breast milk production remains limited. This study aimed to analyze the effect of moringa oleifera leaves and banana blossom consumption on breast milk production among postpartum mothers in the working area of Ngasem Public Health Center, Kediri Regency, Indonesia. A pre-experimental one-group pretest–posttest design was conducted involving 15 postpartum mothers selected using purposive sampling. Breast milk production was assessed before and after the intervention using an observation checklist. Data were analyzed using the Wilcoxon Signed Rank Test with a significance level of 0.05. The findings showed that breast milk production improved after the intervention, with 86.7% of respondents demonstrating adequate milk production. Statistical analysis indicated a significant difference before and after intervention (p=0.003). Consumption of moringa leaves and banana blossom may improve breast milk production among postpartum mothers and could be considered a low-cost complementary nutritional intervention to support breastfeeding programs.
