Abstract

Hypertension, often termed a "silent killer" due to its asymptomatic nature, is a chronic condition that significantly elevates the risk of cardiovascular mortality. This cross-sectional study aimed to investigate the correlation between perceived stress levels, sleep quality, and blood pressure among medical students at Universitas Prima Indonesia in class 2021. This study used a cross-sectional observational design. A purposive sample of 56 medical students was recruited based on the predefined inclusion and exclusion criteria. Data were collected using the Perceived Stress Scale-10 (PSS-10) to assess stress levels, Pittsburgh Sleep Quality Index (PSQI) to evaluate sleep quality, and a digital sphygmomanometer to measure blood pressure. The findings revealed that the majority of respondents (75%) reported moderate stress levels, 66.1% had poor sleep quality, and 82.1% exhibited normal blood pressure. These results suggest a correlation between perceived stress, sleep quality, and blood pressure among medical students at Universitas Prima Indonesia, Class 2021.

Abstract

Hiperglikemia akibat diabetes melitus diketahui dapat menyebabkan kerusakan ginjal, sedangkan limbah kulit durian (Durio zibethinus) memiliki potensi sebagai agen antioksidan dan antidiabetik alami. Penelitian ini bertujuan untuk menilai efektivitas ekstrak etanol kulit durian Sidikalang terhadap kadar glukosa darah (KGD) dan gambaran histopatologi ginjal tikus Wistar (Rattus norvegicus) yang diinduksi aloksan. Metode penelitian menggunakan desain eksperimental dengan 25 ekor tikus jantan yang dibagi secara acak menjadi lima kelompok, yaitu kontrol negatif (CMC-Na), kontrol positif (metformin), serta tiga kelompok perlakuan dengan dosis ekstrak 125 mg/kgBB/hari, 250 mg/kgBB/hari, dan 500 mg/kgBB/hari. Induksi diabetes dilakukan menggunakan aloksan 180 mg/kg secara intraperitoneal. Hasil pengukuran KGD menunjukkan nilai rata-rata sebelum induksi sebesar 88,72 ± 11,104 mg/dL, sesudah induksi meningkat menjadi 400,08 ± 143,212 mg/dL, dan kemudian menurun secara bertahap pada hari ke-5 (385,32 ± 134,395 mg/dL), hari ke-10 (336,32 ± 140,103 mg/dL), serta hari ke-15 (254,68 ± 109,582 mg/dL). Uji normalitas menggunakan Shapiro–Wilk, sedangkan analisis perubahan dilakukan dengan uji Friedman dan Wilcoxon berpasangan disertai koreksi Bonferroni. Hasil analisis menunjukkan penurunan KGD yang bermakna antar waktu (p<0,001) pada kelompok perlakuan, dengan perbaikan signifikan mulai hari ke-5 hingga ke-15. Pemeriksaan histopatologi ginjal juga memperlihatkan perbaikan struktur jaringan dibandingkan kelompok kontrol diabetik. Dengan demikian, ekstrak kulit durian (Durio zibethinus) Sidikalang terbukti berpotensi menurunkan kadar glukosa darah dan memperbaiki kondisi histologis ginjal pada model tikus Wistar diabetes terinduksi aloksan. Penelitian lanjutan dengan variasi dosis, durasi perlakuan, serta pendekatan multikelompok yang lebih luas masih diperlukan untuk mengonfirmasi hasil ini.

Abstract

Diabetes melitus merupakan masalah kesehatan global. Ekstrak kulit durian (Durio zibethinus) kaya senyawa fenolik, berpotensi antidiabetik. Tujuan penelitian ini adalah untuk menilai efek antihiperglikemik serta proteksi histologis pankreas ekstrak kulit durian pada tikus Wistar yang diinduksi aloksan. Penelitian ini menggunakan desain eksperimental dengan kelompok kontrol dan beberapa dosis ekstrak. Kadar glukosa darah diukur pada baseline, setelah induksi, dan pada hari ke-5,10,15. Pankreas diperiksa secara histopatologi dan jumlah pulau Langerhans dihitung. Hasil penelitian menunjukan bahwa ekstrak kulit durian sidikalang menurunkan glukosa darah secara bermakna pada hari ke‑15 dibanding kontrol, dengan kecenderungan respons dosis. Kerusakan histologis acinar/insula tidak berbeda bermakna, namun jumlah pulau Langerhans kelompok ekstrak lebih tinggi signifikan dibanding kontrol (p=0,006). Kesimpulan ekstrak kulit durian menunjukkan efek antihiperglikemik dan menjaga pulau Langerhans pada model aloksan, sehingga berpotensi sebagai adjuvan fitoterapi untuk pengendalian glukosa. Penelitian lanjutan perlu menguji biomarker stres oksidatif dan uji toleransi glukosa untuk memperkuat mekanisme.

Abstract

Kulit durian (Durio zibethinus) kaya flavonoid yang berpotensi menurunkan glukosa darah dan melindungi hepar, namun bukti in vivo terstandar masih terbatas. Tujuan: Mengevaluasi efek ekstrak kulit durian terhadap kadar glukosa darah dan gambaran histopatologi hepar pada tikus Wistar yang diinduksi aloksan. Metode: Dua puluh lima tikus jantan diacak ke lima kelompok: kontrol negatif, kontrol positif (metformin 45 mg/kgBB), serta ekstrak 125, 250, dan 500 mg/kgBB (per oral). Glukosa darah diukur pra-induksi, pasca-induksi, hari ke-5, 10, dan 15; jaringan hepar dinilai semikuantitatif (nekrosis, kongesti, inflamasi) oleh penilai tersamar. Analisis menggunakan model berulang lintas waktu dan uji lanjutan yang sesuai. Hasil: Setelah induksi, terjadi hiperglikemia pada semua kelompok. Penurunan glukosa bermakna terlihat pada hari ke-10 dan 15 (p<0,05), dengan efek terbesar pada metformin, diikuti dosis ekstrak 250–500 mg/kgBB. Skor histopatologi hepar tidak berbeda bermakna antar kelompok. Kesimpulan: Ekstrak kulit durian menunjukkan aktivitas antihiperglikemik yang bergantung dosis, sementara bukti hepatoprotektif belum konklusif pada ukuran sampel ini. Diperlukan standarisasi ekstrak dan uji dengan daya (power) memadai untuk konfirmasi.

Abstract

Artritis reumatoid (AR) merupakan penyakit autoimun menahun yang ditandai peradangan sinovial berulang, sehingga berujung pada kerusakan sendi, keterbatasan fungsi, serta penurunan kualitas hidup. Tingginya aktivitas penyakit sering kali disertai masalah psikologis seperti kecemasan. Penelitian ini mengevaluasi hubungan antara tingkat aktivitas penyakit berdasarkan Disease Activity Score 28–C-reactive protein (DAS28-CRP) dengan gejala yang dinilai melalui Hospital Anxiety and Depression Scale (HADS-A) pada penderita AR di RS Royal Prima Msimpleedan menggunakan desain analitik observasional dengan pendekatan potong lintang. Sebanyak 58 pasien AR dipilih secara simple random sampling. Data dikumpulkan melalui pemeriksaan DAS28-CRP dan pengisian kuesioner HADS-A, lalu dianalisis menggunakan uji korelasi Spearman. Sebagian besar responden berjenis kelamin perempuan (91,4%) dengan aktivitas penyakit tinggi (41,4%) dan tingkat kecemasan normal (81,0%). Hasil analisis menunjukkan adanya korelasi signifikan antara aktivitas penyakit dan kecemasan (r = 0,304; p = 0,020 . Temuan ini menunjukkan bahwa semakin tinggi aktivitas penyakit, semakin besar tingkat kecemasan yang dialami pasien, sehingga aspek psikologis perlu diperhatikan dalam tatalaksana komprehensif AR. Penelitian ini memberikan kontribusi ilmiah dalam mendukung penerapan pendekatan biopsikososial sebagai strategi penting untuk meningkatkan kualitas hidup pasien melalui pengelolaan terpadu antara kondisi fisik dan psikologis. activity assessment an essential component of comprehensive care. This study aims to analyze the relationship between disease activity level based on the Disease Activity Score 28–C-Reactive Protein (DAS28-CRP) and anxiety and depression levels measured using the Hospital Anxiety and Depression Scale (HADS) among RA patients at Royal Prima Hospital Medan. The research employed an observational analytic design with a cross-sectional approach. The study sample consisted of 110 respondents who met the inclusion criteria. Data were analyzed using univariate tests to describe respondents’ characteristics, while the relationship between variables was assessed using bivariate correlation tests. The results revealed that most respondents had high disease activity, while the majority still fell into the normal category for anxiety and depression. Bivariate analysis showed a statistically significant relationship between DAS28-CRP scores and levels of anxiety and depression (p < 0.05). These findings suggest that higher disease activity is associated with an increased risk of anxiety and depression. The study concludes that a multidisciplinary approach is essential in the management of RA patients, focusing not only on physical symptom control but also on supporting patients’ mental health.

Abstract

Cystitis, often caused by Escherichia coli, triggers inflammation and tissue damage in the bladder. While the anti-adhesion properties of cranberry (Vaccinium macrocarpon) against uropathogenic bacteria are well known, its role in the subsequent healing phase, particularly through modulation of Vascular Endothelial Growth Factor (VEGF), remains poorly understood. This study aims to investigate the effect of cranberry fruit extract on VEGF levels and the degree of histopathological inflammation in a cystitis model. Fosfomycin trometamol is an effective antibiotic for treating urinary tract infections, particularly bacterial cystitis. However, the observed increase in Vascular Endothelial Growth Factor (VEGF) levels during treatment is not a direct effect of the drug but rather a biological response associated with the body’s natural healing process. In cystitis, inflammation and tissue damage in the bladder wall trigger the release of various cytokines and chemokines, including VEGF, which plays a key role in angiogenesis and tissue regeneration. Fosfomycin works by reducing bacterial load and inflammation, thereby allowing the body to enter the active healing phase. During this phase, increased VEGF levels reflect ongoing tissue repair and new blood vessel formation. Therefore, the elevation of VEGF should be interpreted not as a side effect of fosfomycin but as a biological marker of bladder tissue recovery following successful infection clearance.

Abstract

Rheumatoid arthritis (RA) poses a major health challenge due to poor medication adherence impacting disease control. This study aimed to analyze the relationship between medication adherence (MMAS-8) and disease activity (DAS28) in RA patients. A quantitative cross-sectional design was employed at RS Royal Prima Medan (May-July 2025), targeting outpatient RA patients with consecutive sampling (n=69). Data were collected using validated MMAS-8 and DAS28-CRP instruments, analyzed via Kolmogorov-Smirnov normality tests and Spearman's correlation. Results showed moderate adherence (47.8%), low disease activity predominant (31.9%), and significant moderate positive correlation (r=0.400, p<0.001). Higher adherence is linked to better disease control. Conclusion: Medication adherence moderately influences RA disease activity; multifaceted interventions are recommended.

Abstract

This study aimed to investigate the effect of cranberry extract on Vascular Endothelial Growth Factor (VEGF) levels and renal histopathological improvement in a rat model of pyelonephritis. Seven treatment groups were used, including a sham group, a negative control, a positive control (ciprofloxacin), and cranberry extract groups at doses of 100, 200, 300, and 400 mg/kgBW. Data were analyzed using the Kruskal–Wallis test followed by an all-pairwise comparison. The results showed a significant difference in VEGF levels among the groups (p = 0.046), with notably higher VEGF expression in the 300 mg/kgBW and 400 mg/kgBW cranberry groups compared to the negative control. Histopathological analysis revealed reduced inflammatory cell infiltration and improved tubular structure in the high-dose cranberry groups. These effects are associated with the bioactive compounds of cranberry, particularly proanthocyanidins and flavonoids, which exhibit anti-inflammatory, antioxidant, and angiogenic activities by enhancing VEGF expression. In conclusion, cranberry extract at doses of 300 mg/kgBW and 400 mg/kgBW demonstrates strong potential as a natural nephroprotective agent that accelerates renal tissue healing in pyelonephritis. These findings support the use of cranberry extract as an adjuvant therapy to complement conventional treatment for urinary tract infections
