Abstract

Diabetes Mellitus adalah penyakit kronik yang disebabkan oleh faktor genetik, karena kesalahan pola makan, atau gaya hidup yang tidak sehat. Diet Diabetes dan Self management Diabetes menjadi poin penting pada tata laksana DM tipe 2. Persadia (Perhimpunan Diabetes Indonesia) adalah perkumpulan pasien dan keluarga penderita diabetes di Indonesia yang bertujuan untuk edukasi pada pasien DM tipe 2. Persadia bertujuan meningkatkan kualitas hidup penderita diabetes dan mencegah komplikasi diabetes pada pasien diabetes. Analisis situasi sebelumnya menunjukkan pasien diabetes anggota persadia cabang Surakarta mempunyai keterbatasan dalam pengetahuan tentang diet diabetes, self managemengt diabetes, dan tingkat stres dalam menjalakan diet cukup tinggi. Tujuan pengabdian ini adalah pengabdian untuk meningkatkan pengetahuan diet diabetes, self management diabetes, dan menurunkan tingkat stres pada pasien DM tipe 2 anggota persadia di RS UNS. Metode yang dilakukan adalah dengan pemberian ceramah, diskusi, dan video edukasi. Sebelum dan sesudah pengabdian dilakukan pemberian kuesioner pengetahuan diet diabetes, kuesioner self management diabetes, dan kuesioner tingkat stres dalam menjalankan diet. Hasil pengabdian menunjukkan peningkatan pengetahuan diet diabetes (rerata peningkatan skor pengetahuan diet diabetes adalah 24,5+6,2), self management diabetes (rerata peningkatan skor self management diabetes 8,2+ 4,34), dan penurunan tingkat stres (rerata penurunan skor 14,5+5,32) pada pasien DM tipe 2 anggota persadia di RS UNS. Secara tidak langsung kegiatan pengabdian kepada masyarakat ini akan meningkatkan kualitas hidup dan kesejahteraan pada pasien DM tipe 2 dalam lingkungan persadia RS UNS.

Abstract

N-acetylcysteine has antioxidant and anti-inflammatory activities that could potentially improve the clinical outcomes of coronavirus disease 2019 (COVID-19) patients. N-acetylcysteine potentially inhibits NLRP3 (NOD-, LRR- and pyrin domain-containing protein 3) inflammasome and results in control oxidative stress and cytokine release in COVID-19 patients. The aim of this study was to assess the effect of N-acetylcysteine in reducing the neutrophil-lymphocyte ratio (NLR) in COVID-19 patients. A randomized controlled clinical trial was conducted among severe and moderate COVID-19 patients. The treatment group received oral 1200 mg daily of N-acetylcysteine (three times a day) and the standard care for COVID-19, while the control group received standard care for COVID-19 and a placebo. The NLR was determined on the first day of admission and after the seventh day of treatment. A paired Student t-test was used to compare the NLR before and after treatment while independent Student t-test was used to compare the NLR between treatment and control groups. A total of 40 severe and moderate COVID-19 were enrolled, 20 people in each group, with a mean age was 44.68±13.24 years old. The mean NLR on the first day was 9.44 in the treatment group and 8.84 in the control group. After the seventh day, the mean NLR was 4.27 and 11.54 in the treatment group and control group, respectively. The mean changes of NLR (the pre-treatment compared to post-treatment) in the treatment and control group were reduced 4.05 and increased 3.34, respectively. The NLR in treatment group significantly decreased compared to the control group (p<0.001). In conclusion, N-acetylcysteine 1200 mg daily could reduce the NLR in severe and moderate COVID-19 patients.

Abstract

Acute exacerbation chronic obstructive pulmonary disease (AECOPD) is associated with significant poor survival. Mesenchymal stem cells (MSC) therapy has been a promising treatment for COPD; therefore, it has the potential to be an additional therapy for AECOPD. Its potential is associated with its secretome since it has anti-inflammatory and immunomodulator activities. The aim of this study was to determine the effect of the secretome as an adjuvant therapy in reducing the levels of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), procalcitonin, and the length of stay in AECOPD patients. A clinical control trial study was conducted among 28 moderate and severe AECOPD patients who were hospitalized from January to February 2023. The control group (n=14) received standard therapy of AECOPD while the treatment group (n=14) received standard therapy plus secretome 1 ml twice daily for three days. The levels of IL-6, TNF-α, and procalcitonin were measured at admission and on the fourth day of treatment. The length of stay was calculated from the time the patient was admitted until the patient was discharged from hospital. The data were compared using a paired Student t-test, chi-squared test and Mann-Whitney test as appropriate. In the treatment group, the levels of IL-6, TNF-α and procalcitonin after the treatment reduced 13.09 pg/mL, 5.00 pg/mL and 751.26 pg/mL, respectively compared to pre-treatment. In contrast, the levels of IL-6, TNF-α and procalcitonin increased 48.56 pg/mL, 44.48 pg/mL and 346.96 pg/mL, respectively after four days of treatment. There was a significant reduction of IL-6, TNF-α and procalcitonin in treatment group compared to the control group with p=0.022, p=0.009 and p=0.001, respectively. However, there was no significant reduction of the length of stay (p=0.072). In conclusion, administration of secretome to AECOPD patients could reduce the levels of IL-6, TNF-α and procalcitonin.

Abstract

Leptospirosis is an uncommon infectious illness – a spirochetal zoonosis – caused by Leptospira species and the primary cause of human leptospirosis is exposure to the urine of infected rodents. Clinical manifestations of human leptospirosis are diverse, ranging from asymptomatic infection to severe life-threatening with multiorgan dysfunction. The severe condition is known as Weil's disease, which is characterized by feverish illness with jaundice, acute kidney damage, and bleeding. The aim of this case report was to present a Weil's disease which occurred simultaneously with a community-acquired pneumonia (CAP) resulting in serious complications. A 41-year-old man with Weil's disease, as well as CAP caused by Streptococcus pneumoniae, and septic shock was presented. The patient was treated accordingly after establishing the diagnosis through history taking, physical examination, and laboratory tests. In this instance, the score for diagnosing leptospirosis based on Modified Faine's Criteria was calculated resulting possible diagnoses; and therefore, therapeutic management was initiated. Despite presenting with severe symptoms, the patient recovered completely after receiving antibiotics and supportive care. This study highlights that when a patient has Weil’s disease and a CAP infection, which could cause unfavorable consequence, a prompt diagnosis and proper treatment could result satisfied patient recovery.

Abstract

Coronavirus disease 2019 (COVID-19) causes severe acute respiratory disease in humans and has spread rapidly worldwide since its first identification in December 2019. The neutrophil-to-lymphocyte ratio (NLR) describes the balance between the severity of inflammation and the immune system to be used as an important systemic inflammatory marker. Rapid progression of clinical deterioration is characterized by severe respiratory symptoms related to high levels of pro-inflammatory cytokines, like interleukin-6 (IL-6), indicating that the occurrence of cytokine storms leads to increased mortality. This study aims to assess the correlation between IL-6 and NLR in predicting the severity of COVID-19. This prospective cohort study was conducted at the COVID-19 ward of Universitas Sebelas Maret Hospital in August–September 2021. This study involved 66 COVID-19 patients >18 years old with asymptomatic to critical degree and Charlson Comorbidity Index (CCI) value ≤3. Examination of laboratory parameters and serum IL-6 was carried out when the patient entered the Emergency Room. Statistical test with Pearson’s correlation test, significant if p<0.05. There is no significant correlation between IL-6 and NLR with p=0.56 and r=0.08, and a strong correlation between IL-6 and disease severity with p=0.000 and r=0.454. The conclusion is that IL-6 does not correlate with NLR and strongly correlates with disease severity in COVID-19 patients.

Abstract

Background: Community-acquired pneumonia (CAP) is a respiratory infectious disease caused by bacteria, viruses, or fungi. Procalcitonin (PCT) levels will rise, especially in bacterial infection. PCT examination in CAP can help to confirm the diagnosis and anticipate complications. CAP is diagnosed by symptoms, vital signs, laboratory tests, and radiographic investigations. Inflammatory biomarkers are required for predicting causative microorganisms, guiding antibiotic therapy, and determining severity. The purpose of this study is to compare NLR, WBC, and TNF-α levels in CAP patients dependent on PCT level.Methods: This was an analytical cross-sectional study involving 43 CAP patients admitted to Universitas Sebelas Maret (UNS) Hospital and Moewardi Regional General Hospital Surakarta between February and March 2023. Patients were separated into two groups based on PCT levels: those with levels ≤0.12 ng/mL and >0.12 ng/mL. All patients provided a blood sample for NLR, WBC, TNF-α, and PCT testing. The independent t-test and Mann-Whitney tests were performed for statistical analysis of two unpaired groups, and the Chi-square or Fisher exact test was utilized for ordinal categorical data. If the value of P<0.05, the result is statistically significant.Results: NLR and TNF-α levels were higher in the PCT group >0.12 ng/mL and statistically significant with a value of P=0.001. WBC levels in the PCT group >0.12 ng/mL were higher, but the difference was not statistically significant (P=0.096).Conclusion: The NLR value, WBC, and TNF-α levels were found to be higher in the group with PCT >0.12 ng/mL.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is the leading cause of morbidity and mortality worldwide. Exposure to cigarette smoke activates alveolar macrophages producing neutrophils and proteases such as MMP-9 and NE that damage the extracellular matrix triggering emphysema as well as spill over into the systemic circulation. This study aimed to determine the correlation of serum MMP-9 level and blood absolute neutrophil count with diffusion capacity and exercise capacity of stable COPD patients.Methods: A cross-sectional observational study was conducted in stable COPD patients visiting the pulmonary outpatient clinic of Universitas Sebelas Maret (UNS) Hospital in October 2022. Consecutive sampling was applied for sample collection. We assessed diffusion capacity and exercise capacity with the DLCO method and 6MWT, respectively. We examined serum MMP-9 level and blood absolute neutrophil count. All data were statistically analyzed with SPSS 22.0 and P<0.05 was considered significant.Results: Thirty stable COPD patients were included in the study comprising 19 males (63.3%), and 11 females (36.7%). With a mean age of 61.90±9.99. Serum MMP-9 level did not correlate with either diffusion capacity and blood absolute neutrophil count (P=0.898 and P=0.589, respectively). However, serum MMP-9 level had a significant correlation with exercise capacity (P=0.014). There was no correlation between blood absolute neutrophil count with diffusion capacity and exercise capacity (P=0.0281 and P=0.592, respectively). Diffusion capacity related to exercise capacity (P=0.001).Conclusion: Serum MMP-9 level is not related to diffusion capacity, but it is related to exercise capacity. Blood absolute neutrophil count is not related to diffusion capacity or exercise capacity. Serum MMP-9 level is not associated with blood absolute neutrophil count. Diffusion capacity is related to exercise capacity.

Abstract

Background: Chronic Obstructive Pulmonary Disease (COPD) is a leading cause of mortality worldwide. Acute Exacerbations of COPD (AECOPD) significantly impact patient outcomes and healthcare costs. Thyroid hormone dysregulation has been proposed as a potential systemic effect of AECOPD. However, the relationship between thyroid hormones and AECOPD clinical predictors, such as %FEV1, exacerbation severity, and hospitalization duration, remains unclear.Objective: This study aimed to investigate the relationship between thyroid hormones and key clinical predictors in patients with Acute Exacerbation of COPD (AECOPD), including %FEV1, exacerbation severity, and Length of Stay (LOS).Method: This is a cross-sectional study which included 32 patients admitted for AECOPD from November 2023 to January 2024 at Sebelas Maret University Hospital. Thyroid function was assessed by measuring serum levels of Thyroid-Stimulating Hormone (TSH), Free Triiodothyronine (FT3), and Free Thyroxine (FT4). We evaluated the association between thyroid hormone levels and Forced Expiratory Volume in 1 second (FEV1), severity of exacerbation, and Length of Stay (LOS).Results: The study found a significant positive correlation between FT3 and %FEV1 (r=0.414, p=0.019), indicating a relationship between higher FT3 and better lung function. A significant negative correlation was observed between FT3 and exacerbation severity (r=-0.506, p=0.003), then FT3 and LOS (r=-0.350, p=0.050). FT4 also negatively correlated with exacerbation severity (r=-0.367, p=0.039). The optimal cut-off for FT3 to predict severe exacerbation was <2.65 pmol/L, with a sensitivity of 80.8% and specificity of 100%.Conclusion: The study indicates a significant relationship between thyroid hormones, particularly FT3, and clinical outcomes in AECOPD patients. These findings suggest that thyroid hormone assessment could be useful in predicting the severity of acute COPD exacerbations, contributing to more targeted management strategie.

Abstract

Kejadian gawat daruat bisa terjadi pada siapa saja, kapan saja dan dimana saja. Keadaan ini menuntut kesiapan para tenaga kesehatan untuk melakukan tindakan sesuai dengan kompetensinya. Salah satu profesi kesehatan yang bertanggung jawab dalam penatalaksaan pasien gawat darurat adalah perawat. Penelitian di medan tahun 2019 menyebutkan mayoritas perawat tidak memahami cara membaca rekam jantung yang baik. Setelah berkoordinasi dengan ketua PPNI Rumah Sakit Universitas Sebelas Maret, permasalahan utama yang dialami PPNI adalah Pengetahuan mengenai cara membaca EKG pada perawat masih terbatas. Perawat tidak mengetahui gambaran rekam jantung yang berbahaya. Pelatihan membaca rekam jantung jarang ditemui di era pandemic COVID-19, dan tidak ada buku panduan khusus untuk membaca rekam jantung bagi perawat. Tahap-tahap pengabdian dilakukan dengan mengadakan workshop pelatihan membaca EKG secara daring pada minggu ketiga bulan April 2021.Tahap pengabdian selanjutnya adalah buku petunjuk EKG dan brosur EKG. Terjadi peningkatan pengetahuan perawat terhadap EKG dengan peningkatan nilai pretes 55,5 menjadi 72,5. Pelatihan EKG bagi perawat meningkatkan pengetahuan perawat sehingga meningkatkan kualitas pelayanan Kesehatan.

Abstract

Background: His WHO data for 2019 puts him third in COPD deaths, at about 3.23 million. Decreased respiratory function is one of the symptoms of COPD that causes respiratory arrest that impacts quality of life. Pulmonary rehabilitation in COPD patients is a means of controlling and reducing symptoms and optimizing functional function.Method: Experimental study with pre-test and post-test groups design. The number of study participants was 30 stable COPD outpatient in group B and E of the Respiratory Clinic of the UNS Hospital during the period of February 2023 by targeted sampling. The control group (n = 15) received standard care and the treatment group (n=15) received standard care plus additional balloon-inflation exercises for 6 weeks. Comparisons between pretest and posttest groups were made on %VEP1 score, breathlessness symptoms, CAT questionnaire, exercise capacity, and EID.Result: Increased values of %VEP1 (p=0.001), decreased Borg scale (p=0.001), decreased CAT score (p = <0.001), and physical activity performance (p = 0.001) versus his EID (p = <0.001) increase.Conclusion: Inflating the balloon affects %VEP1, Borg scale, CAT score, training capacity increase, and EID values. Balloon blowing exercise increases the %VEP1 value, lowers the Borg scale, lowers the CAT score, increases exercise capacity, and EID.Keywords: %VEP1, Borg scale, training capacity, CAT, EID
