Abstract

IntroductionLiver fibrosis (LF) results from the unregulated chronic wound healing process in liver tissue. Transforming growth factor-beta (TGF-β) is the major contributing cytokine of LF promotion through activation of quiescent hepatic stellate cells (HSCs) into myofibroblasts (MFs) and increased extracellular matrix (ECM) deposition such as collagen leading to scar tissue development. Mesenchymal stem cells (MSCs) have an immunomodulatory capability that could be used as a new treatment for repairing and regenerating LF through suppression of TGF-β. This study aimed to examine the role of MSCs in liver fibrosis animal models through suppression of TGF-β levels without scar formation particularly in the proliferation phase.MethodsIn this study, a completely randomized design was used with sample size of 24. Male Sprague Dawley rats were injected intraperitoneally (IP) with carbon tetrachloride (CCl4), twice weekly, for eight weeks to induce LF. Rats were randomly assigned to four groups: negative control, CCl4 group, and CCL4 + MSC-treated groups T1 and T2, at doses of 1 x 106 and 2x106 cells, respectively. TGF-β levels were analyzed by enzyme-linked immunosorbent assay (ELISA). One-way ANOVA and a least significant difference (LSD) was used to analyse the data. ResultsThe TGF levels of LF rat models decreased on day 7 after MSC administration. The levels of TGF-β in both MSC groups T1 and T2 decreased significantly compared with the control group (p<0.05). The TGF-β suppression capability of T2 was optimal and more significant than that of T1.ConclusionMSCs can suppress TGF levels in liver fibrosis induced rats.

Abstract

Abstrak Keluhan kulit keriput dapat terjadi karena proses penuaan yang dapat disebabkan oleh faktor intrinsik dan ekstrinsik akibat paparan ultraviolet (UV). Penelitian kedokteran membuktikan media sel punca jaringan adiposa terkondisi (conditioned medium) mempunyai kandungan beberapa growth factor dan antioksidan, diduga dapat digunakan sebagai terapi anti-aging.

Abstract

Evaluation of Objective Structured Clinical Examination (OSCE) is required to assess medical students’ skills in order to meet the Indonesian medical doctor competency standards. This study presents an innovative method for developing a mobile app-based OSCE evaluation system in Med Stud Games (MSG) and explores its condition. To evaluate MSG, we use the focus group discussion (FGD) method. We use a purposeful sampling approach, consisting of a content expert, medical education expert and medical students. The result of an experience in using MSG for two OSCE training was discussed in FGD. Data was analyzed using in vivo coding and divided into categories to advantages, problems, content, interface, and future improvements. The FGD results in 19 codes. Advantages are categorized as easy to use, fast, practical, and automation timer and scoring. Problems categorized in distraction potential of incoming calls and notifications and online only. Content results in the validity of score and checklist quality. Interface results in font size and layout. Future improvement results in saving and sharing data, open feedback and text, and user-generated content. MSG improve assessment and feedback qualities for medical students. Further observational studies are needed to assess OSCE validation to be used in exam settings

Abstract

Antisipasi dalam menghadapi ancaman EREIDs perlu dilakukan sebagai upaya pengurangan dampak buruk bagi penduduk. Sebagai tempat berkumpulnya orang banyak dalam jangka waktu lama, kapal penumpang merupakan salah satu tempat berisiko menjadi hotspot penularan EREIDs yang efektif. Simulasi kesiapsiagaan EREIDs bertujuan untuk penyajian model EREIDs dipelabuhan dan diskusi perbaikan model EREIDs. Pelaksanaan simulasi dilakukan secara hybrid mengundang Pelindo, KSOP, BKK dan operator kapal dengan materi: kesan data kesiapsiagaan EREIDs di Lembar, diskusi tupoksi masing-masing stakeholder dan menggali saran perbaikan kedepannya. Penyajian model EREIDs meliputi: 1) pengendalian teknis (1.1. fasilitas sarpras, 1.2. modifikasi sarpras dan 1.3 penyediaan instrument keselamatan), 2) pengendalian administratif (2.1. penyediaan SOP keselamatan, 2.2 Pelaksana SOP Keselamatan dan 2.3 Pengawasan pelaksanaan SOP keselamatan) dan 3) penggunaan alat pelindung diri (3.1. penyedia APD dan 3.2. Edukasi penggunaan APD). Secara kualitatif didapatkan bahwa peran utama stakeholder masing-masing: 1.1. Pelindo; 1.2. BKK; 1.3. Pelindo; 2.1. BKK; 2.2. Pelindo, Operator Kapal, KSOP; 2.3. BKK, KSOP; 3.1. Pelindo, BKK; 3.2. BKK. Adapun tantangan kesiapsiagaan EREIDs di Pelabuhan dan Kapal adalah koordinasi antar stakeholder, permintaan catatan kejadian, dan rekapitulasi insiden yang masih harus diselaraskan kembali. Dapat disimpulkan bahwa peningkatan kesiapsiagaan EREIDs dipelabuhan dan kapal merupakan penyiapan lintas sector dan lintas bidang intra dan inter stakeholder. Semua stakeholder memiliki peran krusial dalam mencegah masuknya wabah dari luar pulau dan mengirim wabah keluar pulau.

Abstract

Mesenchymal Stem Cells secrete various anti-inflammatory and regenerative SAMs-MSCs that possess immunomodulatory properties and may accelerate wound healing. As a potential agent for therapeutic, SAMs-MSCs should be stable in any condition, including under X-Ray Scanning. The previous study reveal that X-Ray Scanning may induce protein damage. However, the investigation regarding the stability of SAMs-MSCs under X-Ray Scanning is limited, thus this study aimed to investigate the stability of SAMs-MSCs after X-Ray Scanning. Mesenchymal Stem Cell medium was filtrated using the TFF method at 300 and 5 kDa filter cut off and sterilized using 0,1 um syringe. The SAMs-MSCs underwent X-ray Scanning using public Air Port X-Ray twice. The SAMs-MSCs concentration was measured using ELISA. T-test analysis was performed for the statistical analysis with P<0,05. This study revealed that X-ray scanning reduce the concentration of SAMs-MSC. A previous study found that x-ray irradiation may damage protein at 6–18 keV caused by the energy deposited by photoelectrons that are generated by the interaction of X-ray photons and the protein leading to photoelectron-induced damage.

Abstract

Penyakit Tidak Menular saat ini menjadi permasalahan yang menarik perhatian. Penyakit ini tidak dapat ditularkan melalui kontak langsung, makanan, atau air, melainkan gabungan dari faktor genetik, life style, dan lingkungan. PTM lazim terjadi dan memiliki dampak buruk bagi kesehatan masyarakat. Menurut WHO, cardiovascular disease, cancer, diabetes, dan PPOK menjadi penyebab kematian tertinggi pada PTM.  Sebagai rangkaian dari kegiatan UNRAM mengabdi, Program Studi Kedokteran Kelautan berkesempatan memberikan pelayanan kesehatan. Kegiatan ini bertujuan untuk melakukan pemeriksaan dan penapisan sebagai upaya meningkatkan derajat kesehatan masyarakat. Metode dalam kegiatan ini adalah partisipatif, meliputi perencanaan bersama desa, pemeriksaan tekanan darah dan mata, edukasi kesehatan, konsultasi, tindak lanjut melalui rujukan medis, serta pelaporan hasil, guna deteksi dini dan pencegahan hipertensi. Keberlangsungan acara ini dimulai dari proses koordinasi dengan perangkat desa, pemeriksaan kesehatan, serta rencana tindak lanjut. Peserta sebanyak 49 orang terdiri dari 32 orang laki-laki dan 17 orang perempuan. Hasil kegiatan pemeriksaan kesehatan di Balai Desa Sengkol menunjukkan partisipasi 49 orang (65,3% laki-laki, 34,7% perempuan), melebihi target 30 peserta. Ditemukan 33% prahipertensi, 21% hipertensi tahap 1, dan 4% hipertensi tahap 2. Pemeriksaan mata mendeteksi gejala awal katarak serta peningkatan kadar glukosa. Faktor risiko PTM seperti hipertensi dan riwayat kolesterol tinggi teridentifikasi, namun kontrol serta kepatuhan minum obat masih rendah. Edukasi telah diberikan, dan penapisan dini perlu digencarkan untuk mendukung pengendalian PTM secara berkelanjutan.

Abstract

Acute ischemic stroke is an acute episode of neurological dysfunction caused by focal infarction or damage that persists for ≥24 hours in the brain, spine, and retinal. Stroke is the second highest cause of death worldwide. The total prevalence of stroke continues to increase in the world and in Indonesia. However, stroke treatment is still less effective, so adjuvant therapy needs to be developed; one of them is MLC901. However, this therapy still needs to be studied further regarding its effect on acute ischemic stroke and the biomarkers used as the prognostic factors, such as red cell distribution width (RDW). This paper aims to determine the effect of MLC901 therapy on RDW values in acute ischemic stroke by analyzing various previous studies from different databases as a reference. Increased RDW values are associated with poor outcomes of acute ischemic stroke. MLC901 was shown to have neuroprotective and neurorestorative abilities in neurons that are under stress due to ischemia. MLC901 also can reduce inflammatory responses, reduce oxidative stress, and increase erythropoietin (EPO) production. So, administration of adjuvant MLC901 therapy can reduce RDW values and improve neurological outcomes in acute ischemic stroke.

Abstract

One of the conditions associated with pulmonary tuberculosis (TB) is low vitamin D levels. Vitamin D has a role in stimulating antimicrobial peptides in various immune cells to create optimal immune function. Immune function is closely related to pulmonary TB pathophysiology, especially hematology changes. This makes the monocyte-lymphocyte ratio (MLR) a novel, affordable, and easy-to-perform inflammatory marker in pulmonary TB disease. This study aimed to determine whether there is a relationship between vitamin D levels and monocyte-lymphocyte ratio in patients with pulmonary tuberculosis at West Nusa Tenggara Provincial Hospital. This study was cross- sectional. The study subjects were all pulmonary TB patients in the period 2023-2024 in the inpatient installation who met the inclusion and exclusion criteria. Statistical analysis was performed using the Spearman rho correlation test. There were 43 samples in total, the majority of which were male (53.5%), in the productive age range of 18-64 years (74.4%), and body mass index in the underweight category (46.5%). The status category of vitamin D levels in patients was divided into deficiency (<20 ng/mL) (69.8%), insufficiency (21- 29 ng/mL) (20.9%), normal (30-100 ng/mL) (9.3%), and toxicity (>100 ng/mL) (0%). The monocyte-lymphocyte ratio (MLR) level from the patient's blood test results obtained a median of 0.66 with a minimum-maximum of 0.15-6.09. The results of statistical analysis obtained a p-value of >0.05. There is no significant relationship between vitamin D levels and monocyte-lymphocyte ratio in pulmonary tuberculosis patients at West Nusa Tenggara Provincial Hospital.
