Abstract

Abstract: Study Literature: Patomekanisme Gangguan Kognitif Pada Pasien Hipertensi. Menurut Konsensus Penatalaksanaan Hipertensi oleh Indonesian Society of Hypertension (INaSH), hipertensi emergensi merupakan situasi hipertensi derajat 3 yang diikuti dengan hypertension-mediated organ damage (HMOD) akut. Atau dapat didefinisikan sebagai peningkatan tekanan darah sistol dan diastol yang berada di atas 180/110 mm Hg. Hipertensi masih menjadi permasalahan global dan diperkirakan akan meningkat sebanyak 29% pada tahun 2025, dengan angka mortalitas mencapai 4,6%. Sekitar 75 juta atau 32% dari orang dewasa di Amerika Serikat mengalami hipertensi dengan sebanyak 1%-3% pasien dapat mengalami krisis hipertensi. Hipertensi memiliki hubungan yang erat dengan peningkatan risiko terjadinya disfungsi kognitif terutama pada individu lanjut usia. Ketika seseorang lansia mengalami hipertensi, akan terjadi gangguan umum pada beberapa mekanisme homeostasis termasuk regulasi aliran darah ke otak dan tekanan mikrovaskular. Salah satu contoh dari gangguan kognitif yang dapat terjadi adalah alzheimer.

Abstract

Highlight: Chemotherapy induces neurotoxicity through DNA crosslink, impaired calcium homeostasis, mitochondrial damage, increased reactive oxygen species, pro-inflammatory cascade, axon degeneration, and programmed cell death. CIPN has a primary impact on the sensory neuron. CIPN is diagnosed based on the patient's history, chemotherapy history, and neurologic examination. Some pharmacological and non-pharmacological treatments are hypothesized to reduce CIPN symptoms, but only duloxetine is recommended.   ABSTRACT Chemotherapy-induced peripheral neuropathy (CIPN) is the most common and severe neurological side effect of many commonly used chemotherapy agents. It affects more than 60% of cancer patients. Approximately 30%–40% of patients have persistent symptoms five months or longer after stopping treatment. Even years after completing chemotherapy, some patients still experience CIPN symptoms. CIPN increases the annual cost of healthcare, leads to detrimental dose reduction and even cessation of treatment, and severely affects cancer survivors’ quality of life. Chemotherapy induces neurotoxicity through a variety of mechanisms that lead to neuronal cell damage or cell death. This mechanism of neurotoxicity varies depending on the specific agent. CIPN is characterized predominantly by sensory axonal peripheral neuropathy. Motor and autonomic symptoms may appear, but less frequently. To diagnose CIPN, a thorough patient's history and neurological examination are required. The current approach to CIPN management focuses on managing the symptoms of neuropathic pain and reducing or stopping the chemotherapy agent when CIPN manifests. There is no proven or advised prophylaxis therapy for CIPN. The point of this review was to talk about how some commonly used chemotherapy agents (such as platinum-based compounds, taxanes, vinca alkaloids, bortezomib, and thalidomide) cause CIPN, how to diagnose it, and the newest treatments that are available.

Abstract

Latar belakang: Decompression Sickness (DCS) adalah penyakit multisistem disebabkan oleh gelembung gas terlarut yang terbentuk dalam darah dan jaringan selama atau pasca penurunan tekanan lingkungan mendadak. Isi: Insiden DCS jarang terjadi, hanya sekitar 3 kasus per 10.000 penyelaman. Insidennya meningkat pada nelayan penyelam tradisional. Inner Ear Barotrauma (IEB) merupakan suatu kerusakan jaringan fisik yang terjadi akibat ketidakseimbangan antara tekanan udara pada ruang telinga tengah dan telinga dalam. Alternobaric vertigo (AV) merupakan suatu kondisi kejadian vertigo sementara yang biasanya akan berkembang saat naik atau melakukan manuver valsava. Faktor risiko lainya pada AV meliputi paparan kebisingan saat melakukan penyelaman. Review ini menggunakan Inner Ear Disorders in SCUBA Divers: A Review sebagai panduan dalam menyusun artikel. Penelusuran menggunakan Google Scholar, Pubmed, dan Plos One. Kesimpulan: Decompression Sickness (DCS) dapat menyebabkan Gangguan keseimbangan pada IEB, Inner Ear Decompression Sickness, dan AV.

Abstract

Background: sleep is a physiological need for every human, including medical students. Studies have shown that poor sleep quality can lead to a decline in learning and memory abilities in medical students. The dual process theory explains that certain types of memory depend on common sleep conditions. This study aims to determine the association between sleep quality and short-term memory function, as measured by the digit span test, among medical students at the University of Mataram Objective: To determine the association between sleep quality and short-term memory function as measured by the digit span test among Medical Students at the University of Mataram. Methods: Observational analytic study with a cross-sectional design. The primary data included sociodemographic characteristics, Pittsburgh Sleep Quality Index, and the Digit Span Test. Data were analyzed using chi-square test and Fisher's exact test. Results: 62 participants were observed. Participants ranged from 18 to 23 years, with 42 students (67.7%) identified as female. 71% of the participants experienced poor sleep quality, while 29% reported good sleep quality. All 62 students showed normal performance in the forward digit span test. In the backward digit span test, 79% obtained normal results, while 21% performed abnormally. There is no relationship between sleep quality and short-term memory (p=0,739). Conclusion: sleep quality doesn’t influence memory function among medical students at the University of Mataram.

Abstract

BACKGROUND: Acute ischemic stroke (AIS) is usually treated with thrombolysis, however the percentage of patients receiving this therapy is not quite low. Therefore, it is necessary to find alternative therapy using neuroprotective agent such as Moleac (MLC) 901. Heat shock proteins (HSP)-70 and matrix metalloproteinase (MMP)-9 are usually related to AIS due to the triggered stroke-induced physiological stress. However, the effect of MLC 901 on Hsp70 mRNA expression, HSP-70 and MMP-9 remains unclear. This study was conducted to determine the effect of MLC 901 on those three parameters in relation to cerebral infarction volume and functional outcomes in an AIS model.METHODS: Rats were induced with AIS using unilateral common carotid artery occlusion (UCAO) and received three different treatments: 43.2 mg/200 gBW MLC 901, 21.6 mg/200 gBW MLC 901, and sodium carboxymethyl cellulose (CMC-Na), that were administered orally for 14 days. HSP-70 and MMP-9 protein levels were assessed using enzyme-linked immunosorbent assay (ELISA), and Hsp70 mRNA expression was assessed using quantitative real-time polymerase chain reaction (qRT-PCR). Foot fault scores for evaluation functional outcome and infarction volume were assessed by ImageJ.RESULTS: AIS-induction increased HSP-70, MMP-9, and Hsp70 mRNA expression within 24-48 h. MMP-9, HSP-70 , and Hsp70 mRNA expression were reduced by MLC 901. MLC 901 at dose of 43.2 mg/200 gBW and 21.6 mg/200 gBW were effective in reducing these levels compared to the control. MLC 901 improved functional outcomes and decreased cerebral infarction volume. Moreover, a dosage of 43.2 mg/200 gBW was more effective in reducing Hsp70 mRNA expression and HSP-70, improving functional outcomes, and reducing cerebral infarction volume than a dosage of 21.6 mg/200 gBW, but not MMP-9 protein.CONCLUSION: MLC 901 effectively decreased Hsp70 mRNA expression, HSP-70 and MMP-9 protein levels, infarct volume, and functional outcomes. MLC 901 could be a potential therapeutic agent for AIS.KEYWORDS: MLC 901, HSP-70, MMP-9, acute ischemic stroke

Abstract

Epilepsi merupakan penyakit kronis yang ditandai dengan adanya bangkitan kejang yang berulang. Di Indonesia diperkirakan 1,5 juta orang menderita epilepsi. Berbagai kondisi dapat mempengaruhi terjadinya kejang antara lain gangguan keseimbangan elektrolit serta kadar gula darah yang rendah yang dapat terjadi pada kondisi puasa. Bertepatan dengan peringatan purple day, dilaksanakann kegiatan pengabdian masyarakat dengan tujuan memberikan edukasi kepada penderita dan keluarga epilepsi mengenai dampak puasa serta strategi aman dalam menjalani puasa. Kegiatan pengabdian kepada masyarakat ini dilakukan di poliklinik Neurologi Rumah Sakit Umum Daerah (RSUD) Provinsi NTB dengan metode penyuluhan interaktif menggunakan slide power point. Peserta diberikan pre test kemudian dilanjutkan dengan pemberian materi mengenai dampak puasa terhadap epilepsi dan tips berpuasa yang aman untuk penderita epilepsi, dilanjutkan dengan sesi diskusi dan diakhiri dengan pemberian post test. Sebanyak 15 peserta turut berpartisipasi dalam kegiatan ini. Kegiatan diawali dengan pelaksanaan pre-test untuk mengukur pengetahuan awal peserta mengenai pengaruh puasa terhadap epilepsi. Hasil pre-test menunjukkan nilai rata-rata sebesar 70,6. Setelah itu, peserta mendapatkan edukasi melalui sesi penyuluhan yang disampaikan secara interaktif dan informatif. Kegiatan diakhiri dengan pemberian post-test guna mengevaluasi peningkatan pemahaman peserta. Hasil post-test menunjukkan peningkatan signifikan, dengan nilai rata-rata mencapai 90,6, menandakan efektivitas penyuluhan yang telah diberikan.. Komunikasi, edukasi dan informasi yang baik merupakan solusi untuk meningkatkan pengetahuan pengaruh puasa pada epilepsi pada penderita epilepsi.

Abstract

Indonesia memiliki ribuan pulau. Hal ini berdampak pada meningkatnya kegiatan penyelaman bawah laut. Penyelaman yang tidak sesuai prosedur dapat menimbulkan gangguan kesehatan berupa penyakit dekompresi yang salah satunya mengakibatkan neuropati. Emboli gas nitrogen menimbulkan respons inflamasi sistemik yang dapat memicu kerusakan pada saraf perifer. Faktor risiko gangguan dekompresi antara lain kurang pengalaman dalam menyelam, melakukan penyelaman berulang kali dalam waktu yang sama, tidak mematuhi prinsip-prinsip penyelaman dan sering kali melakukan yo-yo diving. Gangguan saraf perifer yang dapat terjadi berupa sindroma terowongan carpal, neuropati nervus peroneus, neuropati nervus ulnaris, mononeuritis multiplex. Pemeriksaan elektrodiagnostik menunjukkan adanya demielinisasi yang ditandai dengan peningkatan kecepatan hantar saraf. Pemberian terapi hiperbarik oksigen sesuai standar Angkatan Laut Amerika (United State Navy Treatment) menjadi standar dalam pengobatan penyakit dekompresi disertai dengan pemberian terapi simptomatik dapat memperbaiki gejala gangguan saraf tepi. Abstract

Headache is a common symptom in both children and adolescents which impacts quality of life, health, and social functioning. Most headaches are primary headaches, but the presence of red flags may indicate the possibility of secondary headaches. The purpose of this education program is to increase awareness and insight among students in Mataram City regarding the red flags of headache thus they could be more aware if having one of the signs. The Education was attanded by 96 students at SMAN 1 and SMAN 3 Mataram using counseling method by presenting content using power point and followed by presenting illustrative video and flyer about signs and red flags of headache for students. Pre- and post-test were done using google form to evaluate participants' knowledge and understanding. The pre- and post-test questions consisted of 5 multiple choice questions, with an average score on the pre-test was 41.5 and the average score on the post-test was 70.6. These results show an increase in knowledge and understanding related to the red flags of headaches in students. Education is a solution to increase knowledge for detecting red flags of headache among students.

Abstract

Guillain-Barré Syndrome (GBS) is a neurological disorder of the peripheral nervous system and is the most common cause of acute paralysis worldwide. Guillain Barre Syndrome occurs in 1 to 4 patients per 100,000 population worldwide per year and the highest incidence is around 30 - 50 years of age (productive age). Prevention, early detection, and early management of guillain barre syndrome are very important things to do, especially for medical personnel. Community Service for medical personnel of Public Health Care Karang Taliwang and Tanjung Karang began with the implementation of pretest, followed by counselling, and ended with a post test session. This educational activity is aimed at increasing the understanding of cadres in the working area of the karang taliwang health centre about guillain barre syndrome. Abstract

Introduction: Smartphone use among adolescents, particularly in Mataram City, West Nusa Tenggara, is highly prevalent. Nighttime smartphone useis associated with delayed sleep onset, as it extends the time adolescents more time to engage in various activities. Several studies have shown that smartphones use can contribute to sleep disturbances, including insomnia. This study aims to investigate the relationship between nighttime smartphone use and insomnia among adolescents in Mataram City.Methods: This study employed a cross-sectional design and included 362 students from Mataram City, selected through non-probability consecutive sampling method. Participants completed questionnaires assessing nighttime smartphone use and insomnia symptoms using Insomnia Severity Index. Data were analyzed using Spearman’s rank correlation test.Result: Of the total sample (44.5% male), the majority were 16 years old (62.4%), enrolled in Grade 11 (82.6%), and most (66.9%) were in the natural science program. The majority of participants were classified as having subclinical insomnia (43.9%), while only 2.5% experienced severe insomnia. The findings revealed a positive correlation between the frequency of nighttime smartphone and insomnia among adolescents (r=0.104; p=0.049).Conclusion: There is a significant relationship between the frequency of nighttime smartphone use and insomnia among adolescents in Mataram City.
