Abstract

Pneumonia, a serious respiratory infection, remains a significant global health challenge in the modern era. This disease not only impacts the physical health of patients, but also often triggers deep anxiety and requires intervention. One form of intervention that can be given is a spiritual approach through mindfulness meditation combined with a self-regulation theory approach to provide positive stimuli in reducing anxiety in pneumonia patients. This community service focuses on implementing a spiritual mindfulness intervention approach combined with self-regulation in reducing anxiety in pneumonia patients. This method is implemented through a series of sessions involving: 1) Introduction to the concept of mindfulness and basic techniques; 2) Integration of spiritual elements according to each patient's beliefs; 3) Self-regulation training, including breathing and relaxation techniques; 4) Daily mindfulness practice with guidance; 5) Group discussions to share experiences and support each other. The characteristics of the participants are an average age of 45.6 years and female gender (60%) is more dominant. The average duration of pneumonia is 14.3 days. Before the intervention, the average score of the patient's anxiety level was 24.8 (indicating moderate anxiety). After being given mindfulness activities, participants showed improvements in their anxiety conditions, the average score was 14.2 (indicating a significant decrease). The combination of mindfulness, spiritual approaches, and self-regulation showed very promising results in reducing anxiety levels in pneumonia patients. This holistic approach combines meditation techniques, spiritual practices, and emotion management strategies for patients.

Abstract

Introduction: Ventilator weaning is the process of transitioning a patient from ventilator support to independent breathing for more than 48 hours. A 30°-45°head-up position, where the patient's head is raised above the bed, with the body parallel to the bed and the legs straight or unflexed, is believed to influence the success of the weaning process. This study aims to analyze the relationship between a 30°-45°head-up position and the success of ventilator weaning. Methods: A retrospective design was used, with 40 patients meeting the inclusion and exclusion criteria. Total sampling was applied, taking the entire population as the sample. Data were collected from patient medical records in the ICU from May to July 2023 to evaluate the head-up position and the success of ventilator weaning. Data analysis was performed using the chi-square test to determine the significance of the relationship between the variables. Results: The study results showed a significant association between a 30°-45° head-up position and successful weaning in ventilator patients, with a p-value of 0.000 (<0.05). The majority of patients positioned with their heads up were successfully extubated and did not require further ventilator support for the next 48 hours. This head-up position is believed to help increase lung capacity and respiratory efficiency during the weaning process. Conclusions: maintaining a 30°-45° head-up position playe a crucial role in successful ventilator weaning. Therefore, careful monitoring and thorough assessment during the weaning process could be essential to ensure rapid and accurate extubation in ventilator patients.

Abstract

Integrated patient progress record (IPPR) documentation shows that it is still not 100% complete and needs improvement. The aim of the research is to develop a model of nurse behavior based on work productivity and the informational motivation behavioral skills on IPPR documentation compliance. Phase 1 research used explanatory research with a cross sectional approach on 155 nurses at Blambangan Hospital, Banyuwangi who were recruited using cluster random sampling. Independent variables: organizational characteristics, nurse characteristics, work characteristics, information skills, social motivation, documentation behavior and the dependent variable is documentation compliance. The instrument is an instrument that was modified by the researcher and has first been tested and declared valid and reliable. Analysis of research data using SEM-PLS. Phase 2 research uses a qualitative design in the preparation of modules through FGD and expert consultation. Phase 1 research shows that the outer model construct validity of all indicators is valid with loading factor values ≥ 0.5. Reliability for all indicators shows reliability with Cronbach's alpha > 0.6. Hypothesis testing showed that all were significant, only the influence of work characteristics on nurse characteristics was not significant. The results of the phase 2 research module are based on recommendations from FGDs and expert consultations. During model development, the strongest path in combining the two theories is the nurse characteristics path through the work productivity and IMBS-based documentation behavior path and towards IPPR documentation compliance. So, it is very important to emphasize that in increasing nurses' compliance in carrying out IPPR nursing documentation, it is necessary to improve nurses' self-efficacy by increasing the knowledge, abilities, attitudes, personality, self-perception, beliefs and hopes that exist within nurses.
