Abstract

ABSTRACT Anemia during pregnancy remains a significant public health issue with a high prevalence in Indonesia. One of the primary efforts to prevent anemia is through the consumption of iron supplementation tablets (Tablet Tambah Darah/TTD). However, adherence among pregnant women to TTD consumption remains low due to insufficient monitoring and suboptimal support from healthcare services. This community engagement activity aimed to improve TTD adherence among pregnant women through the implementation of a self-monitoring card combined with a patient-centered excellent service approach by healthcare workers. The activity was conducted in Bangun Rejo Village, Tanjung Morawa Subdistrict, Deli Serdang Regency, involving 15 pregnant women in their second and third trimesters. The intervention consisted of educational sessions, distribution of TTD self-monitoring cards, and support from community health cadres. Evaluation was carried out using a pre–post test design, observation of card usage, and structured interviews. There was a significant improvement in TTD adherence, with the proportion of pregnant women consuming ≥6 tablets per week increasing from 33% to 83% after the intervention. The self-monitoring card proved effective in enhancing awareness and engagement of both mothers and their families. The implementation of excellent service also contributed to increased satisfaction with maternal health services. In conclusion, the combination of a self-monitoring card and excellent healthcare service is proven to be effective in improving TTD adherence among pregnant women. This intervention is recommended for broader implementation as part of anemia prevention efforts in maternal health programs. Abstract

Anemia in pregnancy remains a public health problem in Indonesia, with a prevalence of 48.9% (Riskesdas, 2018). Low adherence to iron and folic acid tablet (TTD) consumption is one of the main causes of high anemia rates among pregnant women. This community service activity aimed to improve TTD consumption compliance among pregnant women through interactive educational media at Sarfina Sembiring Clinic in 2025. The implementation method applied a participatory learning approach, consisting of preparation, education delivery, evaluation, and follow-up. A total of 30 pregnant women participated in the activity. Pre-test and post-test evaluations showed an increase in knowledge scores from 62.5 to 85.3 (+36.4%), while compliance with TTD consumption increased from 46.7% to 83.3% (+36.6%). Additionally, 90% of participants stated that interactive media was easier to understand compared to conventional lecture methods. These findings indicate that interactive educational media effectively improves knowledge and compliance with TTD consumption among pregnant women, making it a viable alternative for health education to support anemia prevention programs in Indonesia.

Abstract

Background: Hypertension in pregnancy is a major contributor to maternal morbidity and mortality. Pharmacological therapy is common; however, evidence on safe and affordable non-pharmacological alternatives—such as lemongrass (Cymbopogon citratus) foot soaking—remains limited among pregnant women. Lemongrass contains citral and flavonoids that promote vasodilation and relaxation, potentially reducing blood pressure.. Methods: A quasi-experimental one-group pretest–posttest design following the TREND guideline was conducted at Sarfina Primary Outpatient Clinic, Medan, Indonesia (February–April 2025). Thirty pregnant women with mild hypertension (140–159/90–99 mmHg) participated voluntarily after providing informed consent. Exclusion criteria were preeclampsia, chronic illness, or lemongrass allergy. Participants soaked their feet daily for 15 minutes in 1 L of warm lemongrass decoction (38–40 °C) for four weeks. Blood pressure was measured twice using a validated Omron HEM-7130 device, and the mean was analyzed with the Wilcoxon signed-rank test. Results: Mean systolic pressure decreased from 147.6 ± 5.8 mmHg to 132.4 ± 6.3 mmHg, and diastolic pressure from 93.8 ± 3.9 mmHg to 83.7 ± 4.2 mmHg (Z = –3.21, p < 0.001). The effect size (r = 0.59; 95% CI [–18.5, –11.3]) indicated a moderate clinical effect Conclusion: Lemongrass foot soak therapy effectively lowers mild hypertension in pregnancy through vasodilatory and relaxation mechanisms. It represents a safe, simple, and culturally appropriate complementary method that midwives can integrate into antenatal care. Larger controlled trials are recommended to confirm its long-term efficacy.
