Abstract

Introduction : Sympathetic Ophthalmia (SO) is a rare and devastating complication of ocular trauma. Incidence reached 0.26% among cohort in paediatric open globe injury. We present a case report of pediatric Sympathetic Ophthalmia. Case Illustration : An 8-year-old boy is present in an outpatient clinic with a history of corneal lacerating injury on LE 1 month ago and had surgical repair 7 days after the injury. The patient complained of redness and blurry vision in both eyes. VA RE 0.5 and LE 0.05. RE revealed ciliary injection, flare/cell (f/s) +2/+2, and vitreous cell +1. LE revealed ciliary injection, corneal scars, f/s +2/+2, and vitreous cell +2 (Fig. 1). Fundus and OCT revealed macular oedema with exudative retinal detachment RE and bilateral optic nerve head swelling (Fig. 2). Patient was diagnosed with sympathetic ophthalmia and treated with prednisolone acetate 8x per day for RLE with high-dose oral steroid methylprednisolone 1 mg/kg body weight (32 mg). One week after treatment, clinical findings were improved, and the steroid was tapered. After 2 months f/s, macular oedema, exudative retinal detachment and optic nerve head swelling were resolved with VA RE 1.0 and LE 0.4. Discussion : Although the etiology of SO is currently unknown, it is believed that hypersensitivity following trauma to the fellow eye may be the trigger. The severity of SO's clinical presentation might range from mild to severe. Conclusion : Sympathetic ophthalmia is a serious complication that rarely occurs after trauma, especially in the pediatric population. Prompt treatment of oral and topical corticosteroids may lower the inflammatory response.

Abstract

Introduction : Malposition of the intraocular lens (IOL) can be in the form of decentration, subluxation, dislocation, or luxation. Conservative management and surgery are used to treat IOL subluxation. Case Illustration : A 14-year-old boy came to clinic with chief complaint of diplopia since 1 month ago. There were histories of cataract surgery of both eyes in 2014 and 2015 at a secondary hospital, secondary IOL implantation and membranectomy of right eye in December 2017, and IOL reposition of right eye in February 2022. Ophthalmologist examination revealed BCVA of right eye is 0.5 with S+10.00 C-2.25 x180 and left eye is 1/300. The right eye's intraocular pressure was 23 mmHg and the left eye's was 16 mmHg. Anterior segment examination of the right eye showed aphakia with subluxation of the IOL to the inferotemporal (Figure 1) and PC IOL of the left eye. Patient diagnosed with aphakia ODS with IOL subluxation OD, secondary glaucoma OD, astigmatisma Hypermetropia composite OD, astigmatism Myopia Compositus OS, Hypermetropia OD, Myopia Gravior OS, Deprivative Amblyopia ODS, and Last Eye OD. Patient given monofocal spectacles, prednisolone asetat eye drop tapering dose, and timolol maleate eye drop, with observation of IOL position Discussion : Recurrent IOL subluxation without clear signs of zonular or capsular instability in this patient was managed with conservative management using spectacles and medication due to secondary glaucoma. Long-term observation needed to evaluate possibility of repositioning or explanting the IOL in the future. Conclusion : Management of IOL subluxation could temporarily undergo conservative treatment. A surgical approach is considered in cases of recurrent dislocation

Abstract

Abstract Introduction & Objectives : One of the main therapies used to treat amblyopia in the dominant eye is occlusion therapy. When used on the dominant eye during activities, eye patches can lead to quality of life issues include visual, cosmetic, and psychosocial disorders. To improve visual acuity in the suppressed eye and stop a progressive loss of visual acuity, therapy must be followed. An important factor affecting a child's compliance with occlusion therapy is parental support. Parental negativity affects children's level of support, and as a result, a decreased compliance. This research aimed to determine the relationship between quality of life and the perspectives of parents and children receiving occlusion therapy. Methods : This is a correlation study with a cross-sectional design. Fifty-one subjects were addressed in the amblyopia children's group and fifty-one subjects in the parents’ group. Every participant was directly interviewed. Correlation analysis was used to assess the relationship between the results of interviews with children and their parents. Results : There is a decrease in the quality of life in children undergoing therapy, both from the perspective of the child and their parents. The decline in the quality of life that occurs in children is not as low as parents think. The results of the questionnaires for the children and their parents in total demonstrated a significant correlation (p = 0.0001) and a moderately positive correlation (R = 0.598). Conclusion : There is a positive correlation between children and parents in assessing the quality of life due to occlusion therapy.

Abstract

Abstract Introduction & Objectives : Retinopathy of prematurity (ROP) is a vasoproliferative disorder of the retina that occurs in premature infants. Incidence of ROP is higher in middle- and lower-income countries, including Indonesia; hence information about the characteristics of the patients is essential to make strategies regarding its management. The aim of this study is to describe the characteristics and managementof patients with retinopathy of prematurity at the National Eye Center, Cicendo Eye Hospital. Methods : Data were obtained retrospectively from medical records from January 2018 until December 2020 using ICD-10 coding for retinopathy of prematurity. Variables include demographic data, clinical characteristics, and management. Results : In total, 177 patients (340 eyes) were included in this study. Demographically, 53.7% of the patients were boys; 67.8% came from West Java; 93.2% were referred; and 85.9% were health coverage users. The mean gestational age was 29.9±2.6 weeks and the mean birth weight was 1415.6±393.1 grams. Most of the patients have bilateral conditions and already have stage 5 ROP, regardless of their gestational age and birth weight. Conservative management was given to 95.5% of stage 5 ROP patients. As many as 50% of stage 4 patients underwent pars plana vitrectomy, and 85.4% stage 3 patients were managed by laser photocoagulation. Those in earlier stages were mainly observed first. Conclusion : Patients with ROP at Cicendo Eye Hospital mostly came in the advanced stage of the disease with conservative management. Early screening, timely referral, and prompt treatment are recommended in order to prevent blindness in premature infants.

Abstract

Abstract Introduction & Objectives : Strabismus causes visual impairment due to ocular misalignment, interferes cosmetic appearance, results in negative self-image and impacts quality of life. Studies in adults have shown that successful strabismus surgery improves quality of life, but awareness of strabismus surgery and its benefits are still lacking. Studies regarding the impact of conservative treatment on quality of life are limited, so further study is required. Methods : This was an observational study comparing 2 unpaired groups. The method used in this study was cross-sectional and samples were obtained purposefully according to medical records and consecutively based on outpatient visits. Results : Thirty-six subjects in group A, who had undergone strabismus surgery and 33 subjects in group B who were still managed conservatively were included in this study. Both groups had similar demographic characteristics, except parental education background (p=0.048). Comparative test of2 unpaired groups in this study showed higher quality of life scores in group A (p=0.0001). Conclusion : The quality of life in subjects who had undergone strabismus surgery was better than that in those who were still managed conservatively.

Abstract

Pendahuluan: Retinopati diabetik merupakan komplikasi mikrovaskular yang diakibatkan oleh penyakit diabetes mellitus kronis. Retinopati diabetik memerlukan skrining sebagai deteksi dini untuk mengontrol keparahannya.  Metode: Penelitian ini bertujuan untuk mengetahui karakteristik pasien retinopati diabetik yang terjaring skrining oleh Rumah Sakit Mata Cicendo pada tahun 2022 serta kelompok yang rentan yang mengalami retinopati diabetik. Karakteristik pasien dikategorikan menjadi karakteristik demografi, klinis, dan status oftalmologis. Desain penelitian ini berupa studi deskriptif potong lintang menggunakan data kuesioner skrinining pasien yang dilakukan pada tahun 2022.  Hasil: Rumah sakit mata cicendo berhasil melakukan skrining terhadap 435 pasien. Data yang terkumpul menunjukkan pasien yang mengikuti skrining mayoritas perempuan (71,49%), berusia 60-79 tahun (64,6%), memiliki riwayat hipertensi (66,44%), dan memiliki durasi diabetes 5-10 tahun (42,99%). Prevalensi DR dan VTDR dari pasien diabetes mellitus yang mengikuti skrining adalah 20,23% dan 3,68%. Mayoritas pasien berjenis kelamin perempuan (21,54%), kelompok umur 40-59  tahun (23,36%), memiliki riwayat hipertensi (20,76%), memiliki riwayat dislipidemia (22,22%) dan durasi diabetes 5-10 tahun (25,13%) mengalami retinopati diabetik.  Kesimpulan: Retinopati diabetik rentan terjadi pada orang dengan jenis kelamin perempuan, umur 40-59 tahun, riwayat hipertensi, riwayat dislipidemia, dan durasi diabetes 5-10 tahun.

Abstract

In the process of a pediatric eye exam, cycloplegic refraction is a crucial step. Since no single cycloplegic drug is ideal, a combination regimen is employed. This study compares the ocular (refractive power and pupil diameter) and systemic (blood pressure and heart rate) effects of administering a combination of 1% cyclopentolate and 2.5% phenylephrine, with or without 1% tropicamide, to children with refractive errors. This study aimed to discover a more feasible regimen for children. This was a single-blind study, a randomized clinical trial conducted from November–December 2020 in children with mild to moderate refractive errors. Group A received 1% cyclopentolate, 2.5% phenylephrine, and 1% tropicamide (SFT), whereas group B received 1% cyclopentolate and 2.5% phenylephrine (SF). The outcomes were measured using an auto refractometer and IOL Master® 700. Before and 60 minutes after medication was administered, blood pressure and heart rate were measured. There were 54 participants (108 eyes) with an average age of 12.85± 2.84 years. Although the SFT group's refractive power and pupil width were greater than those of the SF group, the differences were not statistically significant (p=0.271 and p=0.088). Heart rate (p=0.521) and blood pressure (systolic p=0.201; diastolic p=0.950) did not significantly differ between the two groups. Despite mydriasis manifested more rapidly in the SFT group, there was no discernible difference in the cycloplegic effects between those groups. SF might be a more feasible regimen for cycloplegic refraction in children with refractive errors.

Abstract

Introduction: Cataract remains as a leading cause of visual impairment in children. Surgical intervention and post-operative refractive correction remain fundamental. This study aims to describe the refractive and visual outcome of pediatric cataract surgery in a tertiary eye center in Indonesia.   Methods: This retrospective study was conducted utilizing medical records of all congenital and developmental cataracts patients undergoing cataract extraction procedures with or without primary IOL implantation between January and December 2022. Exclusion criteria included patients who did not undergo visual acuity and objective refraction examination 1-month post-cataract surgery and those with incomplete medical records data.   Results: A total of 118 eyes from 65 patients was included in this study. Most patients had bilateral cataracts (93.80%) and operated at the median age of 18 (0.96-212.64) months. Post-operatively, there were 71 (60.19%) aphakic and 47 (39.81%) pseudophakic patients with a respective refractive status of +18.00 (12.00 – 21.13) D and +0.60 (±2.37) D. Prediction error (PE) and absolute prediction error (APE) were obtained within 1.00 D. Most aphakic patients had unquantifiable visual acuity both before (91.50%) and after (83%) surgery. Among pseudophakic patients, nine (19.16%) had visual acuity of ≥ 6/12 and seven (14.89%) had visual acuity of <6/12 - 6/18 post-operatively. No patients had visual acuity ≥ 6/12 and <6/12 - 6/18 before surgery.   Conclusion: Post-operative refractive status of both aphakic and pseudophakic patients were well within correctable range. There was an improvement of vision after surgery. Limited visual potential may be attributed to the presence of amblyopia.

Abstract

Introduction: Impaired blood flow is an early sign of retinal dysfunction in diabetes. Impaired blood flow of the radial peripapillary capillaries can affect the Retinal Nerve Fiber Layer (RNFL) or ganglion cell function. Methods: This was an analytic cross-sectional study in February-April 2019 at Bandung. The subjects of this study were 41 people (79 eyes) divided into two groups, group A (healthy subjects) 19 people (37 eyes) and group B (DM type 2 without diabetic retinopathy) 22 people (42 eyes). Peripapillary retinal perfusion was assesed using OCT Angiography and RNFL thickness was assesed using OCT. Pearson or Spearman correlation statistics test was used to analyze the result. P value ≤0.05 was consiedered significant. Results: There was a decrease in peripapillary retinal perfusion density in the inferior quadrant (P = 0.003) and flux index throughout the peripapillary retinal quadrant (P = 0.0001) but an increase in RNFL thickness in the peripapillary temporal quadrant (P = 0.012) compared to control grup. Positive correlation was found between peripapillary retinal perfusion density and RNFL thickness globally (r = 0.480, P = 0.001), superior quadrant (r = 0.436, P = 0.004), and inferior quadrant (r = 0.608, P = 0.000). A positive correlation was also found between the peripapillary flux and RNFL thickness globally (r = 0.517, P = 0.000), superior quadrant (r = 0.630, P = 0.000), and inferior quadrant (r = 0.519, P = 0.000). Conclusions: There was a relationship between peripapillary retinal perfusion and RNFL thickness in DM patients without diabetic retinopathy. Abstract

Introduction: Vernal conjunctivitis is a chronic, recurrent form of allergic conjunctivitis that occurs mainly in children and is commonly found in warm and humid climates or tropical countries, including Indonesia. Although vernal conjunctivitis is a self-limiting disease, it often leads to visual impairment and significantly affects the patient's quality of life. Methods: This retrospective descriptive study was conducted in Cicendo National Eye Hospital from March to April 2023 using the total sampling method to obtain the clinical features of vernal conjunctivitis patients. Result: This study took 117 patients (234 eyes) registered in electronic medical records from January to December 2022 with an age range of 7-18 years. The mean age of the patients was 10.73 ± 3.02 years, with total of 78 patients (66.67%) being male. A total of 30 patients (25.64%) had a history of atopic allergies. All patients in this study had a bilateral condition. The palpebral type was diagnosed in 85 patients (72.64%). Red eyes were the most prominent symptom experienced in 89 eyes (76.06%). Papillary signs were seen in 203 eyes (86.75%), with an uncorrected visual acuity of 6/6 to 6/18 were found in 163 eyes (69.66%). A small proportion of patients experienced complications in 93 eyes (39.74%). Conclusion: The clinical features of vernal conjunctivitis patients in Cicendo National Eye Hospital are similar to those in other tropical countries. Most patients were male and complained of itching and red eyes. The palpebral type is the most common type. Few distinct features were noted indicating a low association with atopy.
