Abstract

Diabetes Mellitus Type 2 is a chronic disease whose prevalence continues to increase both globally, nationally, and locally. One of the main challenges in managing this disease is medication adherence. At the Binuangeun Health Center, efforts to improve adherence have been carried out through various programs such as Prolanis, but its effectiveness has not been evenly distributed, especially in non BPJS patients. This study aims to study and explain medication adherence behavior and the factors that influence it in Type 2 Diabetes Mellitus patients at the Binuangeun Health Center, Lebak Regency in 2025. This study uses a quantitative approach with a cross-sectional design. The population is patients with type 2 diabetes mellitus who visited the Binuangeun Health Center in March 2025 totaling 106 people. The sample was taken using a simple random sampling technique with a total of 84 respondents. Data collection was carried out using a structured questionnaire from independent variables, namely determinants such as age, gender, education, occupation, duration of suffering, complications, knowledge, motivation, beliefs, attitudes, perceptions, access to services, family support and health worker support. The dependent variable, namely medication adherence, was measured by the MMAS-8 questionnaire, observation and confirmation from people closest to them. Data will be processed univariately, bivariately with chi-square and multivariate with multiple logistic regression. The results of the study showed a relationship between all independent variables with the behavior of adherence to taking type 2 diabetes mellitus medication at the Binuangeun Health Center in 2025. The most dominant variable was motivation. This study is expected to provide a comprehensive picture of the fulfillment of taking medication in patients with Type 2 Diabetes Mellitus factors and factors that influence it, as a basis for developing more effective and inclusive interventions in managing this disease

Abstract

The quality of hospital services is influenced by various factors, one of which is the performance and job satisfaction of nurses, who represent the largest group of healthcare professionals and play a direct role in delivering patient care. At Rumah Sakit Pusdikkes, East Jakarta, a decline in patient satisfaction with nursing services has been identified, which is suspected to be related to nurses’ job satisfaction and performance. This study aims to analyze the impact of job satisfaction and nurse performance on the quality of healthcare services at RS Pusdikkes East Jakarta. This study employed a quantitative research method with a cross-sectional design. The study population consisted of all 78 staff nurses at RS Pusdikkes, all of whom were included as the sample using a total sampling technique. Data were collected using a Likert scale–based questionnaire that had undergone validity and reliability testing. Data analysis was carried out through several methods, including univariate analysis, bivariate analysis using the Chi-Square test, and multivariate analysis through logistic regression. The results showed that most nurses had high levels of job satisfaction and performance. There was a significant relationship between job satisfaction and service quality (p = 0.004), as well as between nurse performance and service quality (p = 0.001). Multivariate analysis revealed that nurse performance was the most influential factor, with an odds ratio of 31.8 in affecting service quality (p = 0.000), while job satisfaction (p = 0.620) did not have a significant effect when nurse performance was taken into account. Based on these findings, it can be concluded that job satisfaction and nurse performance jointly contribute to service quality. Therefore, managerial efforts should focus on improving nurses’ well-being and systematically evaluating their performance to ensure optimal healthcare service quality

Abstract

Healthcare-associated infections (HAIs) remain a global challenge affecting patient safety, healthcare quality, and costs[1]. Global HAIs prevalence is estimated at 3.5–12%, with higher rates in rate reported in developing countries (10.1%) compared to developed countries (7.6%)[2]. In Indonesia, a Perdalin Jaya survey conductes in hospitals in Jakarta reported high infection rates, including Surgical Site Infection (SSI) at 18.9% and pneumonia at 24.5% [3]. Surveillance at Clinic X (2024) also indicated SSI (2.94%) and phlebitis (1.96%, exceeding the national standard set by the Ministry of Health (≤1.5%). This study aimed to evaluate the implementation of the Infection Prevention and Control (IPC) program at Clinic X. A concurrent embedded mixed-method design was employed, involving 60 patients for quantitative analysis and in-depth interviews with healthcare workers and management qualitative insight. The results demonstrated that medical waste and management and the availability of facilities and infrastructure were significantly associated with IPC implementation. Medical waste management emerged as the most dominant factor (p=0.044; OR=3.667), while dressing changes, intravenous catheter insertion, and injection procedures were not significantly associated. Barriers to IPC implementation include limited facilities and suboptimal compiance among healthcare workes, whereas managerial support, standars operating procedures (SOPs), and adherence to personal protective equipment (PPE) served as enabling factors. In conclusion, effective IPC implementation requires strengthning clinic policies in accordance with Ministy of Health Regulation No. 27/2017, improving medical waste management systems, and ensuring continious training and monitoring
